    This was a very nice review; I especially appreciate your contextualizing it so that practitioners can think beyond the algorithms and plan for the patient's prompt transfer to OR. As a footnote, in practice I would ask an RN to go to the pyxis (some stock it within the crash cart) and get 6-18 mg (ready for first and second round) of adenosine while I wheeled the cart over and attached the pads. In practice, we both know how sometimes there are delays in either getting the defibrillator ready or in having the meds ready with a working IV. In other words, I would flame in the adenosine if it was ready before the defibrillator was, and vice versa. After all, if we are even considering a little etomidate or midazolam, if you have time to retrieve those sedatives, you have time for a trial of adenosine. It's about the patient's clinical acuity AND the readiness of your interventions. The cardizem (drips) mentioned elsewhere are useful, of course, but your video scenario is not where I would order that until the more immediate interventions of adenosine / cardioversion have been tried first, given the pleurevac full of blood and a hypotensive patient. Again, your entire series of clinical review videos are fantastic.

    Thank you so much for this case study. Understanding that the MVA and the Signs and Symptoms presents instability and the REASON to cardiovert. Presently studying for my RN boards and I want to understand this just in case.

    On my TELE step down unit, when you call the Dr for a tachycardia they are always going to say either give diltiazem or start an amiodarone drip. We don't do the syncronized cardioversion, at least I haven't seen it...in which case if they are unstable (LOw BP), they need to go to the ICU, asap. ......GOOD LESSON!

    Synchronized Cardioversion is typically saved for those in new onset arrhythmias if medication does not work ☺️ I have seen it done about two times in my career. It definitely is not the first time of treatment but can be used when all else fails.

    It is not the exact exam on the ACLS Megacode; however, it is very similar and the concepts are the same ☺️💜
