When you switch from the left side of the algorithm to the right do you give epi immediately even if there hasn’t been 3 to5 mins between doses? Also, thank you fir your videos, they are such a big help!!!

From my experience working in the icu, the doc will ask the recorder how much time has elapsed from the last epi dose and will make a decision from there. But I think generally they will wait at least 3 minutes.

Great question Robert O'Connell 💜 If the patient is in asystole or PEA, the Epinephrine is given as soon as possible based on the most up to date guidelines. Some providers have started to switch to the latest evidence based practice guidelines while others are still taking a little bit to get on board. I can tell you based on what I have seen from personal practice, the providers have taken the lead to implement the correct practices. It is ultimately our responsibility to speak up and advise the Epinephrine ASAP if no one is taking the initiative to give it when Asystole or PEA is noted on the monitor 💜☺️

hey i'm confused because that v.fib looks like Torsades de pointes? Is there a difference?

  • @NurseCheung

Great question.V-Fib appears as a completely erratic and unorganized rhythm with no clear waves or complexes, while Torsades de Pointes has a more organized, abnormal, pattern of QRS complexes that seem to twist around the baseline. I hope that helps.
