Yes it's spontaneous! Not everyone in Ukraine dances like this though. Most likely, these dancers are part of an ensemble and the girls in particular are probably doing snippets of the choreography they do in their group's dances. When and who goes into the center isn't choreographed though, they just ask eachother who wants to go and briefly explain what they want to do, and whoever wants to join will join. At least that's how it is every time I've done one of these. Every dancer in an ensemble usually works on a few solos for their hopak dance (basically this, but with actual choreography instead of all improvisation) which is useful for kolomekas because you get to show your solos off more. The guy with the white shirt bowtie and suspenders for example, is clearly an ensemble dancer and I'd wager is doing all of his hopak solos in the video Kolomekas are definitely one of the most fun part of Ukrainian culture and definitely one of the biggest reasons I stayed in Ukrainian dance
