It was actually not unforced and big error, he was on very, very worn wet tyres and it was almost impossible to turn properly at normal or even slightly lower speed. As they explain at 11:05 that "two unforced errors almost cost" Hakkinens Championship I was sure they build up for the moment where Hamilton threw away his Championship by beaching in the pits in China 2007. The Coulthard incident at Adelaide isn't all it seems, I think it was one of the Saubers had dropped a load of oil on the pit entrance and left it very slippy. If memory serves correctly Roberto Moreno span and hit the wall and Herbert completely missed the entrance too but obviously because of the high profile nature of being race leader it's the one that gets remembered. Still a pretty shocking error though seeing as the majority of the drivers negotiated it without issue

Same as the one in Monza. The carnage on the first lap took place in the same corner that DC lost it, which I think supports what he was saying about there being oil on the track. In the ‘95 season DC was scintillating; 5 pole positions, better at race starts than Damon, and lost victories in Argentina, Belgium, Italy and Australia all through no fault of his own.

I think Mansell's unintended switching off the car while heading towards a victory on the last lap should be included here. He was absoultely sure he was going to win and started waving at the crowds before crossing the line. When moving his hand out of the cockpit he hit a switch in te car and the engine died. In combination with other mishaps that year, it cost him the title.
