I sourced it on eBay. There are several types available. If you have the tonearm rest make sure you get one with the 2 pre-drilled holes. The plates seem to be coming from Germany and eastern Europe. Cost about $100 + shipping.

Is it necessary to use glue for the new faceplate? There are also 4 screws aren't there,

the original was glued down. If you don't glue it down it will flop around, which I don't think is advisable. So the aswer would be yes

It would seem obvious that I know how to remove a counterweight. Not sure what your point is here. I simply forgot to mention that it should be removed when doing this
