The State of Covid & the Triple-demic: A Conversation w/ Your Local Epidemiologist Katelyn Jetelina

In this UCSF Covid Grand Rounds, we’ll hear from Katelyn Jetelina, who achieved prominence during Covid for her superb scientific communication about Covid and other infectious threats. We’ll cover a variety of topics, including the current state of Covid (as well as flu and RSV), how to weigh risk today, the new Covid variants, who is dying of Covid, and communicating about Covid in the current era.
Speakers:
Katelyn Jetelina, MPH, PhD, is an epidemiologist, data scientist, and internationally renowned scientific communicator. She is the director of Population Health Analytics at a nonprofit, nonpartisan health policy think tank. She is also a senior scientific advisor to a number of government and nonprofit agencies, including the White House, the CDC, and the Make-A-Wish Foundation. She is the author and publisher of Your Local Epidemiologist, a public health newsletter that “translates” ever-evolving science to the general public. YLE has been viewed over 160 million times and has become a key resource for those trying to keep up with the science of Covid. Dr. Jetelina has received multiple national awards for her work, including from the National Academies of Science and the United States Department of Health and Human Services.

    If hospitals are overrun EVERY winter! Maybe they are under staffed😂 when I was an ICU nurse we were FULL every winter it’s NOT abnormal

    I was the only one in my knitting group masked yesterday. One woman tested positive for covid later that day. I’m glad for any reduction in risk the mask gave me. Thanks to both of you for your advocacy and communication.

    @@joannawarrens5117 because you are crafty I will share this. I sew fleece ear straps on to my N95’s. You could even use fabric from an old sweatshirt etc. I know what you mean about the straps being uncomfortable. This helps for sure. I’m happy to hear you’re wearing a mask regardless of KN95 or N95. It is better then nothing. You would not get as high a viral load with either one. And hopefully not get it at all.

    Can I ask why we are spending so much attention on COVID when the senior population I work with is just as vulnerable to equally transmissible and dangerous viruses like viral pneumonia, Norwalk, influenza etc? If we are truly concerned, maybe we should test for all of them and not just COVID?

    viruses `thrive in moist warm environment called the nose, wearing a mask not only increases the virus affecting people but the masks itself is the best way to transfer it from room to room. Biden has already funded more gain of function viruses at labs in the heart of citys. never been a flu or covid vaccine made or taken. don't call shots vaccines. there is a reason it has always been called a flu shot instead of a vaccine. remember vaccines work.

    @@somewhatinformed1208 Thanks you. However, that is not what I was asking about. I have to be swabbed for COVID whenever I enter a long term care facility. My question was, why are we focusing on only COVID when other viruses are equally transmissible and dangerous to vulnerable populations?

    Because people aren't afraid enough of the other viruses. With COVID, this was a plandemic of hype to get people willing to take a vaccine. DARPA discussed this in 2012.

Even they inventor of the nose swab said it would not detect covid it would only detect viruses if he'd been around anyone with a virus it would show up. Somehow he died a few days later after he made it clear they do not have a way to test for covid only his test which calls everything covid. He pretty much said if you get tested for covid they're going to find something and call it covid.

    This was a great presentation. I was lucky enough to have stumbled upon Dr Jetelina very early in the pandemic. I work with the elderly and those living with dementia. I copied her newsletters whenever she posted because I felt she had her finger on the pulse of the disease and wrote in clear, understandable English AND backed up her thoughts with real science studies, which gave my clients a way to understand what was happening, In 2020 there was so much misinformation clouding the horizon and I found her to be a beacon of light! I continue to value her knowledge. Thanks!

    That's what I was wondering and troubled by. You literally know that there could be potentially long term health ramifications to getting infected and yet you choose to go unmasked? I hope she realizes how much of a hypocrite she sounds. I agree with the other guy and his behavior in terms of high quality masking when in public or indoors. No idea why she has given up despite having so much knowledge about the mechanism and the science. Keep masking everyone!

    @@lisazammit6534 As a retired medical professional, I can tell you that cloth masks are useless in preventing anything; surgical masks are designed to protect the incision, so on the exhale, not the inhale; N 95 masks protect on inhalation, but not exhalation….sort of like a one way valve works.

    Antigen tests are not picking up positives for new variant as easily. I tested negative 4 times before Thanksgiving with symptoms and all were negative (1 a day) before I got very sick the day after Thanksgiving and tested positive. I got my partner and my mom sick- same symptoms but less severe, neither tested positive but both had been boosted with the bivalent one. I had not because I was "going to wait until after finals". I had no way to report my case. I think these are all issues in data tracking.

    I am struggling to find the right words. On the one hand I am highly critical of Katelyn Jetelina while on the other hand I do not desire to put her down especially considering the terrible personal safety experiences she is having with some nutcase critics. Why not say nothing you may ask. Well, but she is reaching a large number of people including people I know in my community who blindly accept whatever she says without critical thinking. Maybe that's the answer, I just ask some relevant questions and let you do your own critical thinking rather then take my word for it. Is Katelyn Jetelina trained in infectious disease epidemiology or is her work focused on a different subject? Are issues surrounding Covid-19 highly complex and should experts we listen to have a high degree of academic pedigree? What is her Google Scholar rating (as compared to e.g. the host, Robert Wachter)? How do you feel about her not identifying data sources on her first graph? In the United States we practice individual medicine, how do you feel about her opinion that we are too individualistic and her advocacy for China's zero covid strategy?

    I think there was a study showing in the real world the bivalent was no better than monovalent.....do not remember where I read it....I do know that recently my husband tested positiive (booster 3,4,5)....and was as sick as me...I had one booster Nov. 2021....have COPD..

    Why do epidemiologists not wonder how new variants just "pop up in various places around the world" as she put it? Isn't epidemiology the study of how diseases spread from person to person? Are we back to the Middle Ages, believing in spontaneous generation? She showed a spectacular lack of interest in how variants can just "pop up in various places", when that is actually her job.

    Epidemiologists say it’s due to the high case counts throughout the world. The more cases there are, the more mutations there are, and the better chance of a successful subvariant emerging. It’s like buying two billion tickets for Powerball. This virus is going to hit the jackpot on a consistent basis.

    Great work Katelyn! You have been my YLE for almost 2 years. Many times, I skip to the "Bottom Line", but the communication is easy to understand for the lay person and without hysterics of TV media. The viral interaction is interesting and does deserve dedicated research.

    Surgeons wear masks to prevent droplets. Also there is a scientific distinction between efficacy of intentional, specific masking and the the efficacy of population general masking. The trade-offs are also different.

    Sure. "Before COVID-19 the incidence of myocarditis was between one and 10 cases per 100,000 people per year. Rates are highest in males between 18 and 30 years old. Interestingly, most cases of myocarditis in the highest risk group are in otherwise healthy and active people. According to the U.S. Centers for Disease Control and Prevention, the risk of myocarditis after infection with COVID-19 is much higher, at 146 cases per 100,000. The risk is higher for males, older adults (ages 50+) and children under 16 years old. Soccer player Alphonso Davies, 21, of Canada’s national men’s team, was sidelined by heart inflammation after having COVID-19. Post-vaccination myocarditis Myocarditis following COVID-19 vaccination is rare and the risk is much smaller than the risks of cardiac injury linked to COVID-19 itself. Based on a study out of Israel, the risk of post-vaccine myocarditis is 2.13 cases per 100,000 vaccinated, which is within the range usually seen in the general population. This study is consistent with others in the United States and Israel which put the overall incidence of post-vaccine myocarditis between 0.3 and five cases per 100,000 people."

    Why aren’t the BQs and XBBs causing a much larger wave now? The early findings said they evade all preexisting immunity. I appreciate the attempt to explain but this is still a mystery to me.

    We heard the same about BA.5 last late spring. It was supposed to show a high level of immunity escape / evasion. Hence everyone who had contracted BA1 or BA.2, would get omikron again. All scientific fear mongering, because in the end BA.5 was a dud. All these early scientific studies on BA.5 were untrue. In reality (that is: not in science), a BA.1 or BA.2 infection gave an excellent protection against BA.5. Crappy science.

    Thank you. Esp discussing long covid, re-infections adding to it, vaccines slowing long covid, masks. Less worried about getting sick, far more worried about life-long issues. Why doesn't Katelyn wear a mask more often if long covid scares her? (And yeah, I respect the heck out of both Katelyn and Bob.) Brain scans, dementia, heart failure all discussed. If the swiss cheese method is critical, why aren't we talking about masking, or at least indoor air quality? Or vaccine mandates? I even the experts have given up.

    Another IMHO more extremely important question to ask would be for each and every citizen of every state to find out EXACTLY how the tax payers funded trillions of dollars for " air quality " for schools ( hvac) and school room extensions are being spent and where EXACTLY is it is in your states . That was a HUGE money laundering, grease the palms of our supporters , transfer of wealth that we Americans have ever seen . And will ever pay for . I bring it up because you mention air quality . And I'm sure you also mean for our children and teachers . Find the money and you will find the corruption and contempt for the average American.

    The population most likely to develop long covid is females between 20-60 years old. I would think she falls into this group? Why isn't she worried?

    Perhaps she doesn't have any of the additional contributing issues for LC, including pre-existing mental health conditions and pre-existing autoimmune diagnoses. We see the same demographic you mentioned heavily represented in rheumatology with RA, Lupus, MS, etc.

    I cannot stand with these kind of people. Saying “long COVID” is bad. Saying “COVID” is serious. And on the other hand themselves don’t mask up. This kind of doctors and experts are stupid. “There are so many unknowns about this new virus. We don’t know how to treat it. All the medicine and treatments are still in experimenting phase. Let’s all join this wild/open field/unknown medicines human body trial. Let’s don’t do it in a controllable lab. Let’s just do it in the real world. Just let everybody get infected so we can have a lot of animal body to check the result of the experiment after few years later. Oh! By the way, maybe that’s how doctors keep their jobs? Let people sicker.” I don’t go into any public indoor places. I only do curbside pickup, drive through windows and no-contact delivery. I’m a software engineer. I work from home. I wear N95 if I have to. This year I had to deal with Department of Motor Vehicles and I cannot find a way to fix the problem online with them so that was the only time I had to step into public indoor place. Who knows what will happen few years later this new virus will do to your brain. I cannot imagine if I cannot write/read computer code, watch sci-fi movies, play video games and enjoy skiing on the slopes. I am going to wait until either we have the cure, a proof no body getting COVID brain, or everyone on earth is infected… If everybody all get COVID brain, then I feel ok “let’s have brain fog all together” and make every software buggy~. I’ll join the party then. 🎉❤

    I'm not sure why some are so readily accepting this as the "new normal." The new normal is dying kids? Dying elders? Disabling people with long covid? We need to have a loooong conversation about propaganda and the function of society, ie to care for people (not to go to restaurants, buy stuff, etc).

    Made it to 15th minute. I wonder what makes you think I need sport commentators? You can say I do not know, you can find pandemics interesting but I would advise against saying it aloud. Stop the pandemic, if possible flu too or walk off, I have no time for your games and am not interested in your research, I simply want to stay healthy. I will do quarantines, I will do masks and self isolation, but I do not want sport commentators. If you wish to claim tile of health worker, work on health, I do not need your report on how well viruses are spreading.

    I had COVID twice and 5 shots. Should I keep getting shots and COVID or just the latest COVID variant? It seems all the MDs, DOs, and phDs on youtube aren't happy with the lack of evidence for further booster efficacy and some are out right hostile(probably for views).

    While not quite all the way through, I would say this is one of the least beneficial of the UCSF Grand Rounds. Seemed like some of the homework and background was less rigorous than normal for UCSF. For example, living in Ohio, I follow multiple medical professionals globally. There was an assertion that Texas does not sample wastewater, but I know that Dr. Klotman at Baylor College of Medicine frequently reports wastewater sample summaries on his weekly podcast. Perhaps there should be more complete and rigorous homework performed by the guest.

    She said "Dallas" specifically does not have wastewater surveillance. She didn't say this about the whole state of Texas.

    Listen closely, she does say ‘Texas’ first, but then perhaps qualifies it to Dallas. I suspect she may not have done her homework well (been in contact with the necessary front line people)or she is speaking imprecisely. ‘Sampling’ is likely happening in Dallas if it is happening in Houston and other parts of Texas. However the ‘results’ of the samples may not be fed to CDC nor ‘published’ beyond local elected/appointed health officials.

    @@markgivens3225 she was correcting herself by being specific about Dallas. I am aware from reading her work that she knows of wastewater surveillance in other parts of Texas. Her point was well taken that it's hard to assess national trends when there is so much spotty coverage of even very large areas such as Dallas.

    I live alone, work remote and shop rarely at off hours. My only exposure is the gas pump handle and the post office box. Then I got Covid Alpha on the wind, fought for 5 weeks, recovered and haven't had a sniffle since. *Long Covid is the Jab.*

    Bottom line, the “2 weeks” to flatten the curve was nearly 3 years ago. Curve FLAT. Beds are empty of C19 patients. Now address an effective RSV vaccine! Get an angry trauma surgeon on to advocate for bigger ERs! We neeed to move on!! Women have delayed metastatic breast cancer dx!! Have an oncologist on to advocate for expedited screening to save real lives!!! You can’t monitor 500 variants it’s a total waste of money when the worst symptoms are headache and sore throat. Children are dying of flu in CA TODAY. Masks are useless unless they are N95 with a trained wearer and only on a sick person. So stay home if you have c19 or anything else!!

    Annals of Internal Medicine published a study from Israel recently that disagrees with you. Their peer-reviewed study found that surgical masks were not statistically less effective than N95 masks in preventing infection in unvaccinated healthcare workers. Deaths from COVID in the US have not dropped below 300 PER DAY this calendar year. That should matter, but because 90% of those are people over 65, it doesn't worry the average non-provider.

    @@Cathy-xi8cb A peer reviewed study? Meaning they all have the same amount of schooling and experience. Those are the ones conducting studies and verifying their validity. From Israel? And it disagrees with anyone who disagrees with it? I wrote a piece awhile back about Medical Journals. There’s definitely legit medical journals out there. But I found an Indian medical journal that will publish any supposed discoveries for a nominal fee. Obviously they don’t review the articles for authenticity. Because one guy spoofed them by sending in a supposed article about birds. Just birds in general. The crux of the article was all about how weird birds are. The author goes on to say he’d studied them real hard to catch their weirdness. Squinting his eyes to get a better look. He called his study ‘what’s with birds’. Provided a graph to delineate the crossing of normal birds to weird ones. No specific species were mentioned. It was a comedic article that they published for a couple grand. That is funny. Another thing that I wouldn’t trust are numbers regarding mask usage efficacy. No one, myself included, knew how to use them properly. When you don’t use something as intended, especially when it’s potentially life saving, you’re on your own. Like tryna use a spoon for a fork’s job. Using a hammer for a screwdriver. Nope huh? Don’t blame masks cuz we’re still feeling the pandemic blues. And don’t believe peer reviewed studies from countries other than the U.S. if that’s your country of origin.

    At TWiV Nr 659 at min29 virologist Christian Drosten indicate very low tissue temperature better to replicate SARS COV2. That explain why mask wearing was protective against SARS COV2 infection,by overheating upper respiratory tract.

    At the same time as saying that she is disappointed in how individual-focused we are being. She masks for a couple of weeks before interacting with ppl she knows are at high risk, but how about the ppl she’s around who are high risk, but she isn’t privy to that information? And she knows that we need layers of protection, yet she doesn’t want to be one of those layers for ppl at risk whom she doesn’t happen to know? She obviously is making important contributions, but, yes, her take on wearing masks is disappointing.
