The night of the hair transplant procedure | Care during the first 12 hours after the procedure

The first 12 hours after a hair transplant are crucial. This video by Dr Bhatti of Darling Buds hair transplant clinic explains the Dos & Donts and precautions what to do and what not to do on the first night after a hair transplant. How to avoid swelling after hair transplant and what to do so there is no bleeding from grafts after the procedure. Dr Bhatti expalins all in a comprehensive manner.
5 OBJECTIVES OF CARE IN FIRST 12 HOURS:
1. Avoiding trauma.
2. Preventing swelling or edema.
3. Keeping grafts well hydrated
4. Prevention of accumullation of oozing serum around grafts.
5. Keeping infection at bay.
AVOIDING TRAUMA:
1. Do not wear a shirt that is not front open after the hair transplant. A vest is a big NO-NO. These can displace grafts in hundreds and therefore we advise all our patients to get a front open shirt. If you did not heed the advice, please go home in clinic dress.
2. Do not bend forwards. Slip on shoes, or just slippers are ideal for the day of the procedure. This is because bending forwards increases blood flow and can dislodge grafts. Moreover, excessive bending forwards can cause swelling around the face.
3. Do not adjust the bandana or cap time and again once you have worn it. Do not pat down the bandana. It is meant to not touch your recipient area.
4. While entering the car and especially if you are tall, make sure you do not hit your head. This is a common accident and is disastrous for the procedure. The grafts that come out get infected and hence cannt be placed back in. Grafts are just sitting in the recipient sites, they have no hold since the blood clot has not yet formed. Moreover dislodged grafts can cause brisk bleeding.
5. Buckle up when in the car. Sitting alongside the driver in the front seat is advisable since this seat can be reclined for safety. Back seats have less space between your head and the roof.
6. Stay away from children and pets for 3 days.
ON REACHING YOUR ROOM:
1. Take off the bandana from behind forward as has been explained. Inspect the planted area in the mirror.
2. Start to spray immediately and keep doing this every 20 minutes till you go to bed.
3. If staying with relatives and friends excuse yourself from excessive mingling. This becomes awkward when you are meeting your relatives or friends after a long time.
4. Switch on the AC if it is summer time.
WHY IS 20 MINUTE INTERVAL SPRAYING ESSENTIAL?
1. To hydrate the grafts since the graft is partly sticking out. Grafts are not placed flush skin to skin to avoid cobble-stoning. We do not want this portion that is abutting above the skin to dessicate.
2. To wash out the serum that is oozing out from along the perimeter of the graft as the clot forms. Accumulation of dried serum causes increased crust formation and is also home to bacteria since the recipient site is left open.
PREVENTING SWELLING :
1. Hotels may provide a reclining chair or couch on request. If you are more comfortable on the bed, prop yourself up against the bead-stead with 3 hard pillows or 6 soft pillows. If crown work has been done, use a neck pillow to support the recipient area so the crown stays off the pillow and sheets. A rolled up towel also works well.
2. The posture you need to place your head in is propped up in bed, leaning back 60 degrees.
3. Do not animate facial muscles like too much talking, laughing.
4. Avoid unnecessary coughing and sneezing as much as possible.
5. Keep your phone away in silent mode.
6. If you have to, you can be on whatsapp texting people but the phone should be lifted up not the head to read messages.
Once you get up from sleep, and it is the crack of dawn, you are much safer than what you were when you went to sleep.
Congratulations, half the battle is won!

    Greetings,Dr.Bhatti, Thank you so much for posting such helpful content.Much appreciated. I had my first head wash today,6 days after my surgery. The method that my clinic used included the use of a foreceps like manual device to scrub and pick out any superficial dead/dried skin.Though they were able to get rid of most of the scabs,I feel they were a bit too hard. I am scared thinking they might have dislodged the placed grafts however there was no blood. I have been reading and watching all these post operative videos focused on headwash and most of them advise to be as gentle and careful as possible in the first 10 days. From your experience,could you share how to tell if there’s been any damage caused to the placed graft?Anything I should be worried about?

    Though I too exercise caution in this regard and allow scabs/crusts to fall off on their own by 14th day, the grafts are one with the body fully consolidated by 6th day. You should not worry too much.

    I had my FUE 4 days ago, and bumped my recipient area on the metal ridge of the shower this morning. There was no bleeding. Will everything be fine? Thanks

    If there is no bleeding garfts have not been damaged. Only there may be shock loss but hair will grow.

    You are likely taking oral Finasteride and so there is no probem with intermittent fasting.

    Sir, I recently underwent a hair transplant procedure. After 48 hours, my doctor performed the first head wash. During this, he massaged the donor area but used only saline water on the frontal side. The same procedure was repeated after 72 hours. However, after 96 hours, when the doctor wasn't available, one of the clinic staff performed the head wash, massaged the scalp, and removed all the scabs. I'm feeling anxious and worried that the grafts may have been affected or dislodged due to the massage or scab removal after 96 hours. Could you please advise me on this matter?

    Tahnks for the question. The grafts in the first 12 hours should be in airconditioned cold environment. After this the grafts are maintained by the body temperature which is the optimal tempretarure for their survival. Hot or cold weather ceases to be of significance. Only they should be kept safe from penetrating UVA rays.

    FUE done in the best clinics isalways micro in nature. Any 'micro' less than 0.7-0.9 mm diameter punch size and slit size will destroy the follicles. Anything more will cause untold misery. Rest is all marketing.

    Initial shedding lasts 4-6 weeks and then the results appear in 3-6 months

    Thanks so much for the info. Less than 2 hours after the procedure i noticed blood trickling down. Is that definitely a sign of lost grafts or is some bleeding normal? Because I heard bleeding on day of the operation or after is normal and after day 3-4 is it dangerous? What is your take?

    Bleeding the evening after surgery may mean a graft lost or just plain bleeding around a graft. Bleeding the next day onwards definitely means a graft was lost. Can you share a picture?

    Hi doctor, After how long after a hair transplant can I have my first haircut? I had a hair transplant in Turkey and my doctor told me to wait 1 year before my first hair cut in order not to dislodge the hair grafts. Do I really need to wait 1 year?

    Grafts are firm after 7 days. They can't get dislodged. You can safely cut, trim shave after 6 weeks.

    The acne should be quiescent. Acne of adolescence and earl adulthood is difficut to treat.

    @@hairtransplantinindia Where do I begin? I could write a book..two books. First surgery 1988, last 2020. I’ve seen and read, heard and researched a lot, but one thing I’ll say people is this. If you have worked really hard on yourself and on correcting procedures from the past, don’t ever think you are ‘out of the woods’ so to speak. It’s easy to relax, and a slip in judgment can undo so much, and then there is no going back.

    I'm a deep sleeper. I slept on bed on 1st night.. n God knows what happened next. In morning, there was swelling on forehead which lateron spread on whole face. It took almost 2 weeks to go completely. This point is very important. Though, I was instructedto sleep upright but I took it lightly N thereby faced consequences too.
