Dangers of Ultra High Density of plantation in hair transplant | What is optimal density?

Dr Bhatti discusses what are the limitations of high density and ultra high density while doing a hair transplant. So how much density can we logically and rationally deliver? While both maintaining the illusion and preserving the scalp donor for future use? It is common knowledge that the most a successful hair transplant can deliver, is an almost perfect illusion, but can never rival the original density Nature gave us.
Hair transplant surgeons do not perform magic. Your available grafts are finite. Ebay, Amazon and Flipkart do not sell grafts. So the limited grafts from your own door area need to be placed strategically and this is why the hairline takes many more grafts than the rest of the areas. As we go back to the crown , the number of grafts per sq cm steadily decreases.
With the donor area smaller than the size of the bald scalp in grades 5 and 6 patterns, even if we use all the scalp donor, natural density cannot be given.
We actually cannot even harvest 50% scalp donor, for fear of making it patchy due to shock loss. Each session only harvests 20% of the scalp donor till 50% of the density is depleted- after which skin would be visible even after 2-3 procedures have been done on the same person.
Hair transplants is not sorcery or black magic that the wildest wishes will come true. It is merely an illusion that is sought to be created with limited resources available. Also we need to keep in mind that future progression of baldness will need more grafts.
And when there is miniaturization of the scalp donor, it becomes more challenging.
So how much density can we logically and rationally deliver? While both maintaining the illusion and preserving the scalp donor for future use? 45-55 is the density that is optimally feasible in non-diabetics for the hairline and lesser for other areas.Try anything more and the principle called “All or none phenomenon” steps in.
Grafts subsist on blood supply. Naturally 90-110 FUs/sq cm is what nature gives us. When we injure skin by piercing it, making innumerable slits, we cut off blood supply cause scarring and shock the tissue. In this environment any huge density if given stresses the grafts which even otherwise are fighting for survival. So if we plant more than 45-55 density, the metabolic requirement will completely outstrip available limited blood supply. Most will eventually die out. Precious grafts may be lost due to cavalier attitude since biological tissues have limitations and often times, you cannot play the numbers game with them.
Occasionally, the body does give us a pleasant surprise and they do survive too. But then since grafts are limited, are we ready to take this chance?
Haste makes waste, it is said!
It is also not just the number of grafts but their viability, the invasive nature of the procedure - how much trauma it has caused, how much the incisions have injured the underlying vessels and nerves that counts towards an uneventful recovery and a great result. And an immediate postop picture speaks more than a 1000 words describing the clinic on the website’s ‘about us’ page!
If the surgery fails, the same patient who wanted high density is not going to leave any stone unturned in blaming the clinic for it.
So if you need more density, do get it if you have the resources for it in the donor area, but get it in

    So doctor what is the ideal no. of grafts needed for grade 6/7? north of 6000? and do you think that number can be achieved using scalp hair, beard and chest ? given an average scalp doner has 4000-4500 grafts that can be used . Also when you say baldness progresses , does it happen in grade 6/7? because that is the last NW scale stage.

    Scenario: a diffuse thin with say full coverage, yet entirely dispersed see theough norwood 5 ontop…Would there be a lot of probable transections in second procedure destroying pre-existing follicles?

    Great explanation, very informative. A couple of questions if you have the time. How many grafts minimum, of normal thickness hair, are needed to create a great illusion of fullness on a crown? Also assuming that a doctor is planning 45-55 grafts/cm² for some areas of the scalp, what survival percentage should I expect? (assuming he does everything right)

    Qs 1- The crown will need more grafts for a 'great illusion' than hair line would due to reasons in the following video. However less is done due to reasons mentioned in the same video.     Ofcourse the device cannot do anything without the doctor. Hair transplant does not only involve harvesting (which the device does). There are so many factors that make a successful hair transplant. Experience counts.

    2 sittings give a much higher density if done after 6 months without any danger of damage to previously implanted grafts.

    If you are young, there is no point blocking 95% DHT. DHT has other purposes for which it is made.

    @@prasadpund4176 Hair fall will continue and hair will continue to grow due to the hair cycle. You have mostly covered progression of baldness by being on OF and so you need not worry. You are doing the best you can.

    Beard hair never go into the line of sight along leading rows of the hairline since they are coarse, thick and wiry. Whenever used they are mixed with scalp grafts in the midscalp area alone.

    One just cannot rival God's creation. Anythinf less than that is doable. Say 55-70 per square cm. Also this will significantly deplete your donor area which is limited.

    Dr I had a hair transplant of around 3800 grafts to cover my front area and a bit crown in some clinic.. I have had decent growth by now but I see some hairs have grown while others are still baby hairs which can be seen under bright light or when pouring water on it n when I touch I can feel too.. the thing that makes me feel better is that they have already popped out but still baby what's the reason behind it ? Is it because of hairfall n the hair goes under new cycle or prp is needed to boast growth.. bdw I take minoxidil n finasteride
