    Hello Dr Bhatti, I love the detailed insights you provide in these videos. I am Norwood 2 with some thinning around my temples. I am considering a HT (hopefully at your clinic) in April '21. My donor area is quite good and I do not have hair loss in the crown area. I recently started taking finasteride for receding hairline. What would you recommend - Stopping finasteride right away or one month prior to the HT? Thanks!

    Hi Dr, I'm wondering if there is a benefit in taking Finasteride indefinitely post hair transplantation. Would the benefits outweigh the risk? Patient is a young male in his 20s.

    Firstly, 20 is not eligible for a hair transplant. For others getting a hair transplant and have continued progressive baldness, it is a good idea to continue Finasteride for as long as it takes for the baldness gene to switch off.

    Im planning to get a hair transplant next year and I’ve been on finasteride for 2 years. Should I stop taking finasteride a year before my procedure and start taking it again after my transplant? How long exactly does it take for the effects of finasteride to wear off? I saw in your video that you said only stop one month prior to the procedure. However, I’ve discontinued use for 3 weeks before and I didn’t notice any change in my hair.

    One month is sufficient to make out anagen grafts under magnification Stopping for too long may undermine the benefits you may have achieved with it's use since many years.

    Thank you for the very usuful information. I have never taken finasteride and consider a hair transplant. However, do you think that it is useful to start taking finasteride AFTER the hair transplant to maintain the transplanted hair and for how long (or it should be taken lifelong after the transplant)?     Thanks. Just 6 months after hair transplant. Lifelong for thinning hair. At times when transplanted hair begins to thin if hair are not from permanent zone, you may need to restart it.

    Hallo Dr, firstly thank you for the very informative video. I am planning to have a hair transplant in the coming months and have been taking a combination of finisteride and minoxidil for about a year. The hair loss has definitely stabilized and the density at the top has improved. There was never a problem in the donor area.. always tones of hair there. I’ve been thinking that stopping the medication would give me a better picture of where hairs should be transplanted... how long before the treatment should I stop, if necessary? I was hoping to have the treatment at the end of December.

    @@kevelliot2928 hey, thanks. Yeah I stopped both about 2 months before surgery. I was recommended to slowly ween off the minoxidil and stopped the Finasterid immediately. I definitely noticed that my hair became less dense over the two months. I am very happy with the transplanted area and which that I had maybe done a bit on the crown, which actually didn’t bother me earlier. I am back in both medications. I think I will stick to both and am now applying minoxidil only once a day to my crown. Hope this helps.

    I was sceptical to take finasteride but today I am so happy to take it and avoid the transplant I have more hairs than when was in my 20's , so far no any side affects at all

    All hairs go through all of the phases of growth and loss, anagen, catagen, telegoen. So I don't know what you mean by preferring only transplanting anagen hairs.Whatever hair you transplant, provided you're transplanting a healthy terminal hair from the 'safe donor' zone, then that hair will go through all 3 phases of hair growth anyway. Please clarify if you can Dr. I love your videos and you give great explanations. This is the only video where I'm confused with your explanations.

    That is a pertinent question and very helpful. Hair that is not in anagen will be more susceptible to handling, dessication and transfer. It is less likely to survive. Also high magnification will be needed to identify it, which most centers do not use. If FUE technique is being used, hair other than anagen has a much higher rate of transection than the under 2% chance for anagen hair. I hope that is what you wished to know.     Hello doctor, I’m someone who used prophecia in the past (for about 10 years) then quit for a few month (because I mistakenly thought I had some side effects) then went back on it again (for about 4 months) and now considering a surgery. When do you recommend is the best time to have a surgery? Should I wait for few more months until the full results of prophecia show?

    SO you suggests that a guy who is on finasteride for a long time (years ) and now wants to do hair transplent . shoud stop finasteride a month before doing the procedure and going back after hair transplent is done . is a month looks like enough time ? because as i know the hair cycles are 3-4 month . maybe it should be longer ?

    A month is enough to see thinning (miniaturisation) under the 5x magnification we use.

    The clinic I went to asked me if I took any drugs, I said no. However, I informed them my donor area was solid and thick full of hair before my hair transplant. I have recieved 3,000 hair transplanted grafts, so far I love it and I'm happy with the progress so far on top of my diet and vitamins.

    Do you mean don't use it just before one, or for a long time before? I've heard of not using it just before, but I've told to use it 6 months-1 year before my HT so I'm confused

    If you are committed to the medication, you need not stop before the procedure. If you are unsure, it is important to stop so the doctor knows which hairs not to harvest.

    So the purpose of not taking finasteride is because of the donor area and not the Temples and scalp?? Finasteride also increase density in the donor areas too?? I thought those areas are less dht sensitive

    The purpose is to help the surgeon during harvesting to pick up anagen phase grafts and not those that look like them due to Finasteride

    Hello Doctor Bhatti, great content! I am not a good candidate for FUE. Does this logic also applies when the technique used is FUT? Since a strip os skin is taken, I believe all the hair from that strip is transplant regardless of its phase?

    If you are not a candidate for FUE, you will not be one for FUT. The telogen hairs and the catagen hairs are not usually transplantable. They comprise 20%.

My doctor I wasn't a good candidate for FUE because of low density in the donor are, but that around 1500 grafts could extracted using FUT to restore the hairline. So I guess that the logic of not taking finasteride before a hair transplant also applies to FUT?

    Does not have a role for androgenetic alopecia. Result is fleeting and you need to expose yourself to syringes again and again. It s ineffective, costly and cumbersome.

    Yes Minoxidil once at night is sufficient for hair loss and even after hair transplant. The efficacy of Minoxidil does not stop after some time- it is only that balding continues unabated with age if you are genetically so determined.

    So, you only recommend taking finasteride after six months after a HT? Every clinic I know has recommended finasteride for life, and, even dutasteride if finasteride stops working.

    However my question is ...stopping finasteride may cause hair loss in crown area !!?? ...and also combining this fact with poor growth of transplanted hair in crown area....isn't that a risk of losing a good looking crown area with bald crown area ??

    Only permanent hair should be transplanted. Hair transplant in crown is as successful as it is elsewhere, only density given is lesser since we do not have as many grafts in a vast majority of patients.

    👋🏻 Doc, Can U recommend Doctors or clinics worldwide to consider for an HT? I know in India there’s Eugenix, what others can U recommend for me to consider? Be$t bang for the money without sacrificing quality.. I will only consider FUE for I believe that FUT is kinda barbaric in this day & age.. Your thoughts 💭? Also some but not all use grafts from the lower beard area for mid scalp & crown are.. Your thongs there too... Thank U in advance..

    I would recommend doctors that do the harvesting and slit making themselves and so I would recommend Dr Bauman in USA, Dr Lorenzo in Europe.

    Reflex HyperAndrogenety. Dr. Can you please talk about this. Is it common? Can Finasteride cause it? Im ok with a " normal shed ". But a hardcore shed that can accelerate your hair loss its too much for me. Im thinking in jumping on Finasteride but im really apreensive because of this.

    Mr Rsihav, all my patients take Tab Finasteride 1 mg a day for 6 months after evaluation for fitness. Your doctor may not have advised you since he might have though you are not suitable for OF.

    Stemoxydine has not been around for long enough to understand it clinically and hence I do not prescribe it to my patients.

    What if you're a 37-year-old individual with Norwood 2/3 hair loss and a thinner-than-average donor area, experiencing some hair miniaturization in donor area? This person is already using Finasteride. Would you recommend them to continue with Finasteride and possibly begin using Minoxidil before considering a hair transplant?

    Not Minoxidil, but Oral Finasteride for 4-6 months before the procedure will help in stabilising baldness and building up the donor hair. But keep in mind that the medication will need to be continued. When you stop the hair will go back to the present condition.

    @@hairtransplantinindia thank you. As I said I use oral finasteride. I have been on oral finasteride for 2 years. I have no intentions of stopping it. I just had a hair transplant for my hairlone and was surprised when the doctor said that there is signs of miniaturization at my donor area. I was left shocked because I'm only a Norwood2/3. Plus I have been taking over oral finasteride for 2 years before hairtransppant. The donor area doesn't even seem that thin... I made a video of it and sent it to my doctor. Could I send the video to you and get your impressions of it? I would be soooo grateful! I highly respect you and your valuable knowledge on this matter.

    Sir, post hair transplant we can use minoxidil and finasteride only for 6 months ? Post that we should not use ? and post hair transplant how frequently we use finasteride and minoxidil ? Daily or weekly once or twice, etc ?

    Many people continue to use them for a long period beyond mandatory 6 months and dose can be halved. In fact the patient will realise if the decreased dose is beneficial or he has to revert to the original dosage.

    Pleses stop 7-10 days before the procedure since the increasesd blood supply due to its vasodilator action can cause more than usual bleeding making the procedure difficult.

    Hi doc, i wanted to ask what is the worsr that can happend if i use minoxidil and finateride years before hair transplant. Im 22 and i dont have money right now to do the hair transplant but my hair is falling out, my hair has fallen in the front and i still have a lot of hair in crown area.     AT 22 you are not eligible for a hair transplant as it is. So your plan to wait till 3 years is best. Use Tab Finasteride 0.5 mg on alternate days after getting total testosterone tested.

    Shouldn’t someone take finasteride long term after the transplant anyways, so that the non transplanted hair are not attacked by dht? So what is the point of not taking it before the hair transplant…? Another question is: how can you tell that non Anagen hair in the donor are the one prone to MPB and they are not just in a different cycle phase?

    At the time of the procedure it is essential to know that we are not transplanting miniaturising hairs that have become robust by the use of a DHT blocker. hence I always have the patient stop the medication 30days before. For your second question: Yes there is no way to tell. Hence only good quality hairs are taken. We do not harvest all the hairs.

    Interesting, I started it three months before the transplant and I was thinking the same thing but it's too late now. I have some questions though as it seems no surgeon in the world is absolutely sure regarding hair transplants, I believe we are in a somehow experimentation phase, next generations probably will be better off. 1) Since finasteride is for long term it makes sense that it wouldn't make difference three months before the procedure, I would assume that we are told to start it before so it will have an effect after three months which would be after the procedure. 2) I expect that I will take finasteride for ever... or at least for the years to come, I am 42 and I plan to take finasteride for the next 10 years maybe..? Not for 6 months only, what's the point of that? Besides we should also consider how each person perceives life regarding to age. What is short term and what is long term? For me for example as I am 42 I want to have hair until 55 more or less, that is mid term for me. I will stop finasteride then and I will loose whatever is to be lost. And then I will die... Let's not forget that not only we die but we become old before that. My point is maybe short term or mid term is better depending the age. I want to live well (with hair) now, after 55 years old I would be considered old and life will be pretty depressing anyway.

    What you think short term may turn out in the long term in the end. I have many patients who come to me and say- "Doctor, I am just 25 and I want hair till 35. After this I will be married and at that age it will not matter!". This is so wrong since perspective about yourself keeps changing as we age. At every stage of life you wish to remain youthful. So, I have 60+ year olds as well coming to me for a hair transplant! Finasteride should be taken till the gene of baldness is in full swing. Once the gene switches off (and this is not sudden like a switch being turned off!) one can consider tapering off the dose and relying on medications like Minoxidil, etc. Otherwise Finasteride is crucial since it deals with the prime cause of androgenic alopecia- DHT.

Maybe people laughed at me, I admit 55 years old maybe has been too pessimistic. I just got a panic attack, what if I live up to 90? Still 35 years after 55...

    I wish i watched this video before my hair implant i was in finastride 2 half month's before surgery?. I never used finastride before that i am 41 and was told that my donar area was very good and they said i need 4200 to 4300 grafts for coverage of my balding . But i had good side and donor..

    I understand the logic in this video. However, isn’t there a risk of getting a hair transplant first and then being a nonresponder to Finasteride? If so then you would go bald in areas that weren’t transplanted. As someone who took Fin for a year with zero results I would have been pisses if a doctor convinced me to get a transplant first

    No doctor should ever convince a patient to get a cosmtic surgery procedure, least of all a hair transplant. In fact the purpose of these videos is for you to make an informed decision. Though a vast majority get good results, one odd will always be unhappy since the response of the body as far as healing and medications are concerned ican vary.

I must have had a bad doctors office because they definitely tried to convince me to get a surgery at the same time as getting on Finasteride. Looking back on it I’m glad I didn’t because I ended up losing more hair during the 10 months I was on Finasteride than I ever did before. I’m now switching to Dutasteride. One last question, is it possible to get heart palpitations while taking topical minoxidil? My doctor said it was not possible but I had them during the time I tried minoxidil and never had them before and haven’t had them since I stopped using it. I’m convinced I may need to switch doctors

    Finasteride shedding does not last more than 8-10 weeks. Even with Finasteride hair loss will continue. Finasteride only considerably slows it down.

    @@hairtransplantinindia Is it possible that i am facing thinning hair due to selenium sulphide shampoo that I have been using? I've been using it since 3 years now, when i had a major dandruff problem. Before that i did not have thinning hair. That shampoo sure has helped in my dandruff problem, but every time I stop using it I get same dandruff problem again. Like scabs on my crown and temples. Doctor, Do you have a replacement option for selenium sulphide/pyrithion zinc shampoo?

    Hi thank you for the video, just a few questions 1) if I am taking finasteride at the age of 26 and would like to have a transplant in 5 years time (currently Norwood 2/3), finasteride is currently stabilising my hair loss and slowly regrowing some hair. My q is; the hairs that I have gained through finasteride if I stop the drug prior to a transplant consultation wouldn’t all these hairs then fall out? And then we would need to use more grafts to replace these? 2) is 1month sufficient time for the non ‘anagen’ hairs to go back to the wilted phase? As some reports say that it can take up to a year to lose all ground from finasteride 3) could you please make a video on scalp health, dry/ oily scalp and how to optimise the health of your scalp prior to using topicals/transplantation Many thanks

    Thanks for watching the video. If you stop oral Finasteride for 60 days and more all the hair that have grown due to it would be lost mostly permanently. For shorter durations they will be fine. Anagen hair wilt to an amount that can be detected microscopically even after 30 days.

    The most ideal situation if the alding pattern allows. Best suited are those suffering from DUPA (Diffuse Unpatterned Alopeecia). Otherwise Finasteride is required since baldness is progressive, even with Finasteride, though to a much lesser degree. Finaseride only blocks 73% DHT after all.

    There exist so many patients who have been on it for more than 10 years I know since they are my patients- and they have not reported any problem. Occasionally patients report soft erection but then it get fine after stopping it. I feel its purely psychological. I have not had a patient who has had permanent loss of erection.It is a myth propounded by some patients to extract money from the drug company Merck.

    Yes, break into 4 equal pieces. Each piece will slightly vary in dose since impossible to split tablet into equal pieces. But it's ok.

    Minoxidil should be stopped 7 days before the procedure. For different reasons- it can cause increased bleeding if not stopped.

    Ok, but would you recommend to have a hair transplant done, without trying finasteride before the procedure? The thing I am worry about are physical sides, and if I am not gonna try it before a HT, then miniaturization of my existing hair will continue. I am ok with having another procedure in couple of years, but the donor area is limited and I do not want to end up looking funny.

    @@alteregomickiego1251 I think surgical hair restoration will not work if one is sceptical about Finasteride. It may give a good result now but you will never be able to keep pace with your baldness if you are young at this moment.

    Hi thanks for the comment. Yes Finasteride in the dose 1 mg a day and Minoxidil foam once at night together with Tab. Biotin 5 mg a day and 2% Nizoral shampoo once a week will be a good comprehensive treatment. This amount of vitamin is very safe.

    Yes there is no problem. As long as you understand that the hair are being maintained by Finasteride and that otherwise they were not fit enough for relocation through a hair transplant.

    @@hairtransplantinindia I got approved for a hair transplant already and want to decrease the shedding phase and increase my chances of a successful transplant. I will start Finasteride 4 weeks before my transplant and continue for years to come.

    @@AuguryOmen ofcourse you cannot take higher doses. You will need to see a local doctor and consult with him and he will follow up to see which combination gives you a result and is also safe. Do not experiment yourself.

    @@arkhafi8850 10mg of finasteride would turn a piece of stone to a woman. Your doctor must be ashamed for recommending such stupid dose of finasteride to his patients.

    While I agree that any gains that you might make would be lost if you stop taking finasteride or minoxidil, I think it's wrong to not give a patient medical therapy before deciding on whether a transplant is necessary. For people suffering from hair loss, it always makes sense to try medical therapy first. If there is only a small degree of loss that might be recovered with medical therapy, they might not need a transplant at all. Further, while it might make your job easier to decide which hairs to transplant, asking a patient to stop taking medical therapy 3 months before a transplant might also put other hairs in jeopardy. It's unclear that 3 months is enough time to shed any weak hairs prior to transplant. Informing a patient that their results might be negatively affected by stopping finasteride is I believe all that is necessary. Lastly, I don't believe that anagen hairs are necessarily always the best hairs, since follicles that are not affected by AGA still go through a normal hair cycle, and might not be in anagen at the time of extraction.

    3 points I would like to make here- 1. baldness cannot recover with medical therapy and so it is not an alternative to a hair transplant. Most people come seeking hairline restoration. 2.. I ask a person to stop Finasteride one month before procedure. This is enough time to differentiate anagen from catagen hairs and at the same time the hair that deteriorate do not do so significantly- that they will not return back to their original condition. 3. Yes there will be thin hairs which can be hairs that are entering the telogen phase but then there are hairs that are so miniaturised that this was their last cycle. Catagen hairs will not be able to withstand the process of transfer since they are extremely sensitive to coring and transfer.

    @@hairtransplantinindia thank you for your very informative answer. As far as whether medical therapy can prevent the need for a transplant, I believe there's evidence to show that. I myself have had very good results using medical therapy. My forelock and mid scalp have benefited from using medical therapy. Not to the extent as to make a hair transplant unnecessary, but it did reduce the need for some volume of grafts. I'm glad that you only say a person should stay off of medical therapy for a month instead of 3

    @@hairtransplantinindia No doctor , it was for the hair loss. I had a transplant couple of years back but stopped taking it. I have restarted it to control the hair loss. While I was on finestaride I decided to get a hair transplant done .

    If your donor area was normal, it does not matter. If it improved with Finasteride before the transplant, you can consider to continue it for longer just to be careful. When you wean off, closely observe if planted hair are shedding in huge amounts. Do this only after 9 months of procedure.

    Platelets would not have DHT. Plasma would. But it is normal DHT already circulating in your blood- nothing to be upset about.

    I disagree. Im not a doctor but the finasteride i’ve been taking since February is restoring my thinning hair. And I know plenty of people who’ve gotten most of their hair back.

    @@hairtransplantinindia I disagree with you doctor. Finasteride is meant to enhance a hair transplant. So coming off it would defeat the object as you will only lose more hair anyway post transplant if you don't take it.

    Sir if planning for child ..then it should be stop at least 2 month before as I listened...but sir in 2 month we lost lot of native hair... So to stop loss of these native hair can be use other alternative like minoxidil or topical finasteride...plz ans sir ...???

    I too recommend this if the patient is prepared to take it for 10-15 years to maintain and sustain the weak grafts that have been relocated alongwith anagen grafts.

    - using finasteride before transplant helps slowdown/prevent hairloss/miniaturization - using finasteride longterm after transplant puts your donor hair at risk of falling out - hair transplants aren't permanent, they last up to maybe 20yrs - transplanted hair will slowly pickup traits from nearby DHT sensitive hair follicles (if any are left) - donor hair will became DHT sensitive and being the miniaturization process - you want your DHT sensitive hair to fall out and only have the donor hair remain I believe this is what the DR is talking about
