    About 2 weeks ago I had tear trough implants and one lower eyelid has retracted. My surgeon has me massaging and using steroid tear drops. In a few days he’s going to try steroid injection. I’m so nervous. This was my first ever procedure and I’m sad that Im experiencing complications. Im considering requesting a consultation with you for a second opinion since you’re a specialist on the eyes. My surgeon hasn’t seen this issue prior to me, so that makes me nervous.

    Hello Dr. Prasad. I had a lower blepharoplasty performed by a Double Board Certified Plastic Surgeon (mistake #1) 6 months ago. I also had a deep plane facelift. Back in 2012 I had basil cell removal surgery (MOHS) under my left eye right where the lower lashes are in the corner closest to the nose. Anyway, I had listened to your videos BEFORE I had this surgery and made sure this doctor performed the transconjunctival procedure of which he said yes. He knew I had that surgery back in 2012 and when on the surgical table before my surgery, he reminded me of that basil cell surgery and I said that I would prefer if we do not do the eyes of which he told me he was going to do the eyes in a way that would be no problem so I agreed with him and said okay you are the surgeon (mistake #2). I am not sure which way he ended up doing it however now I have lower lid retraction and entropion in my left eye, dry eye syndrome, lashes scratching my eye, unclear vision and a rounded eye (losing my beautiful almond shape eye). The right is not near as bad however after seeing ophthalmologist did say one eye lash was going in my right eye too. I doubt I would need extensive work on my right eye as I do not feel the eye is round but what I can say is both my eyes have hallow underneath now and make them appear as "set in" ... It's awful. I am so depressed and am beside myself with what he did to me. I also have scar tissue built up on my sides of eyes where the stitches were from the face surgery. So needless to say I am a hot mess! I am 64 years old. It's not only cosmetic with me and know that I will need to have this revision surgery performed on one or both eyes. My questions would be 1) do you think you could help me? 2) what is the benefit of using your own skin (from roof of mouth) over Enduragen? (I ask because now I have dry mouth syndrome which I never had before-- so the surgery caused this.-- not sure if you could use the roof of my mouth? 3) I know you said you use hydro fillers -- I have never used a filler in my life -- nor was I planning to -- so with that does that mean you would have to keep the fillers up year after year? I live in Chicago and would not be able to fly to New York or Virginia every year to have that done? Okay .. please respond. I know I have a lot of questions but this would be helpful for considering you as my surgeon. I want one of the best and I think highly of you. I appreciate your time and videos!

    Hi @pobox244, Sorry to hear about your problems. To answer your questions: 1) yes, I can probably help with your revision surgery; 2) skin grafts are usually a last resort as grafted skin may appear differently than the original skin in the area, so I would normally opt for lower eyelid retraction repair with Enduragen first.     What your thoughts on SOOF lifts? Could you make video on this? I recently had a consultation and was recommended to have a transconjunctival blepharoplasty with SOOF lift

    Hi Lucky8, I have performed SOOF lifts, and it is an option for mid-facial rejuvenation. However, an examination would be needed before I would recommend it as there are other options to rejuvenate the cheeks that may be more effective, such as as addressing upper and mid cheek volume loss, and more extensive facelift procedures not performed by other oculofacial/ oculoplastic surgeons. The next video I'll be publishing tomorrow is about mid face rejuvenation, but SOOF lifts are not covered in the video as they don't address volume loss in the mid cheek.     I read retraction can also happen with transconjunctival if you are the type to develop scar tissue, so this is a risk, even that scarless blepharoplasty could still cause it due to scar tissue pulling down the eyelid plus fat no longer being there giving support to the lower eyelid

    Hi @MegaCosmoVision, While it is possible to have scar tissue form with the transconjunctival method, there are steps taken so scar tissue does not affect results, and does not pull down the lower eyelid. After decades performing lower eyelid surgery with the transconjunctival method, I have not had issues with lower eyelid retraction in my patients who have their primary lower eyelid surgery performed by me.     Hi @donamathew2538, Lower blepharoplasty is an art in that a little fat should be sculpted precisely, but left behind for a continuous contour from the lower eyelid, into the cheek, and to prevent a hollow appearance that can be just as bad as having eye bags. The inferior oblique muscle is identified as part of the surgery to differentiate between the central, and medial fat pad that causes eye bags, but the muscle itself is not treated, or compromised with an incision with the transconjunctival blepharoplasty technique, as this surgery is done from behind the muscle.     Hi Doc. Nice and clear explanation. May i ask your advice please? I have chemosis for 1 year after lower blepharoplasty with various of oral treatments ( eye drops such as Hialid / Hyalub and various type of steroids including steroid tablets ) with 2x cutting the blister skin to get the liquid out, but the chemosis still stay the same. Most of the times the colour turns red, but it will be gone everytime i drop Oculenta. I hope you can give me some ideas what is actually might has happened to my eye ( only 1 eye got chemosis ). I really appreciate for your feedback and help. Thanks.

    I had a lower eye blepharoplasty done Nov 29. 7 days later I had a follow up appointment with my surgeon and mentioned I had some bleeding from the eye. She said it was probably an internal stitch. 3 days later I had an eruption of a vein or artery in my eye and had to have emergency surgery to stop the bleeding. INCISION AND DRAINAGE, HEMATOMA, SEROMA, OR FLUID COLLECTION. How common does this occur? I feel like I now have the eyelid retraction in that eye. I am worried about future problems as I get older. Is there any? Im guessing, I will need surgery again to fix the retraction but how about long term?

    Amiya sir , Just few question I need to ask , Is eyes shape changes thought our lifetime , I mean I have seen certain peoples eyes shape changes as they grow older , as sign of maturity. And does it represent the maturity or fitness the more fit you become the eyes shapes become more small and beautiful. And what make a face beautiful, is it 1 ) indication of fitness , changes in jawline , muscle on face become noticable, glow on face , cheeks become bigger , etc 2 ) some people face contains emotion , because our face is indication of emotion and some people face is natural build around a certain emotion . 3 ) Or its just a that Golden ratio plays a huge role in beautiful face . Or all that combined .

    Hi mukesh kushwaha, I will answer what I can: The skin of the upper eyelids sags or stretches with aging, causing hooding over the eyes. In some people the eyes droop as the muscle that lifts the eyelids can weaken with aging. Fitness can help you feel younger, but some age-related changes to the body can't be addressed with fitness alone. You want to have your natural facial expressions, as these can be affected with cosmetic procedures that done excessively. The Golden Ratio exists throughout nature, so we are intrinsically drawn to that proportion of beauty. I hope this clarifies things for you.

    Can fat grafting still be done by using you surgery method from inside the lower eyelid? I was told by another plastic surgeon that he could only do fat transfer by doing the surgery on the outside below the lower lash line. I do not have eye bags, but deep hollows under my eyes which makes me look tired.

    Dr. Prasad, 6 wks ago I had an Board Cert Octoplastic Surgeon well known, perform a full bleph and fat transfer. All of the FT has reabsorbed and my lower bleph bags NEVER went away. He made incisions on both lower lids both R. and L. sides, but now trans incision (under the lash line) was performed. I paid exhorbant amount of money and the bags weren't addressed. It was done subcutaneously. I have very tight skin around my eyes and I dont understand why he couldnt do it conjuntivally. Is there anything I can do at this point, as very little was successful. He is asian and does not speak well English.

    Hi @ahmedkohil4374 , I use the transconjunctival approach in practically all cases of lower eyelid fat prolapse, and rarely perform a skin pinch as excess lower eyelid skin actually occurs very rarely, unlike what others surgeons when they remove skin, and unknowingly cause complications. I perform all my eyelid surgeries, and my other surgeries including facelifts under local anesthesia.     Hello doctor! I have a deviated septum on left side by which i can't breath properly from that side and i have nasal polyps growth on tight side.     Hi @saumyamali5619, It is possible for breathing problems, and sinus issues to cause temporary puffiness under the eye due to fluid collection, which should resolve on its own. However, nasal or sinus issues will not cause lower eyelid retraction. That said, it is possible for lower eyelid retraction due to injury of repeated and aggressive rubbing of the eyes due to the irritation of your condition. I hope this clarifies things for you.

You should ask your surgeon if they performed the transcutaneous blepharoplasty technique, and also removed lower eyelid skin that possibly led to a skin shortage. Although these types of complications are avoidable, they are very common with these surgical techniques.     Hi @mastermindmasterpiece7120, Both procedures are rarely needed after transconjunctival blepharoplasty, but there are occasions when they are needed. Transconjunctival blepharoplasty is performed to preserve lower eyelid support anatomy, so canthopexy is rarely needed to restore support. Under eye wrinkles can be treated the majority of the time non-surgically, without the need of a skin pinch in almost all cases. I hope this clarifies things for you.

    Hello Dr Prasad I had upper and lower done fourteen months ago. I have what you are describing. The surgeon did a small nip & stitch three months and it is still very droopy. Then just a second revision two weeks ago and still the same problem. My eye is always red and very irritated. She mentioned to tape and eye massages daily. It is only a temporary fix for the day. What is my third option?

    Hello Dr Prasad I did review you videos before & after my surgery. It will be 24 months this Oct since. I did a several follow-ups and taping, massages as previous months indicated by my surgeon. I am still not happy with the rounded eyes. The surgeon did the third revision four weeks ago and still I have drooping. Today I went in for another follow-up and surgeon mentioned that the final & 4th revision will be cutting my lower eyelid to keep it from drooping. I really don't want this because it will not be semantically pleasing. What should I do?

    Hi@@sharonchavarria6586, In all my lower eyelid surgery videos, I advise not to have lower eyelid skin removal because it's a major factor in lower eyelid retraction, and rounding of the eyes. I suggest you see an eyelid specialist oculoplastic/ oculofacial surgeon like myself who can restore lost tissue, and resuspend the lower eyelids with specialized procedures featured in the videos I sent you. You don't want to have skin cut if the problem you have was caused by unnecessary skin removal.     My advice: STOP GOING BACK TO THAT HACK & continuing down the path! You need a proper specialist to correct the issue your current surgeon clearly lacks the skill to do! Dr Prasad is being nice but trust me this is what he wants to tell you. Best to you!

    Hi, i am 2wks PO & my surgeon performed a transcutaneous procedure on lowers here in the UK. My left eye has some retraction & swelling the other eye ok. What can i do to help resolve this? I was advised taping lid to upper & massaging with bio oil or vitamin E oil. Should this right itself without a revision? Is there anything else i can do to help it go back? Thank-you

I suggest you follow up with your surgeon as they know details of your surgery, and can examine you for any issues.     Hello. Mine the same. I had lower bleph done 5 and 1/2 weeks ago and my left eye has some retraction. I know not what to do. My surgeon keeps telling me it will go down once the swelling goes down. WHAT? Anyway, just checking in to see how you are doing? Thank you.

    I am going thru this right now. Eyelids are extroverted. Don’t close all the way esp when I am asleep. Have had 3 surgeries on eyelids. Eye pressures are very high 33 & 48. Eyes are extremely dry HELP PLEASE

    Hi@@anum3702, We use a treatment called Skin Booster in our practice, but it's not an injection. Since I don't know what the Skin Booster injection you're referring to is, and I have not examined your facial lines, I can't say with certainty that it will help you.

    Hi I had a lower eye blepharoplasty 4 weeks ago in Malaysia, I like in Australia. I has this done by an oculoplastic surgeon. I feel my under eye lids may have what you are describing. My eyes water all the time and now my bottom lashes touch my skin. Wil this get better with time?

    Hi, Sorry to say that it won't likely get better with time, especially if there was lower eyelid skin removal leading to a skin shortage, and compromise of the lower eyelid support structures.     Hi Brayden, I hope your eyes have healed by now. How long did you experiencing lid retraction? I had blepharoplasty done in Malaysia 2 months ago and I’m suffering from lid retraction at the moment. It is very worrying. Did it correct itself over time? Thank you in advance.

    @@littlelollipops me too. I had it in the states 5 and half weeks ago and my eyes have been not the same. I have lumps but I believe I have this lower eyelid retraction in my left eye. It's so sad. I am so depressed about it. I am seeing an oculoplastic surgeon on 8/3 for a second opinion. My plastic surgeon though board certified was NOT an oculoplastic surgeon. How are you doing now? My has not progressed at all. Thank you.

    Hi@@pobox244, Sorry to hear about your problem. If your oculoplastic surgeon specializes in revision eyelid surgery, they can likely help you condition. Lumps can occur with fat grafting in the lower eyelids, if that procedure was also included with your surgery.     @@AmiyaPrasadMD thank YOU Dr. Prasad. I appreciate your response. I did not have a fat transfer to my lower bleph though I did have a deep plane and neck lift. The tightness is pulling on the eyes. I listen to many of your videos and these lower eye retractions have been eye openers. I would like your opinion through a photo. I will message you. I should have gone to you if I was doing my eyes. My surgeon did not have the ocular background. I did not do my homework well enough but I did say to him that my eyes are my best feature and I would prefer not doing them if you believe in any way they would be compromised. He said they would be fine!

    Had lower eyelid blepharoplasty performed two days ago. I see one eye is normal, the other one shows evidence of orbital prolapse of lower lid. I'm so upset. Now what do I do? How can it be resolved?

    Hi @regina15ful99, Two days after surgery is too early to assess final results due to post-operative swelling affecting the appearance. You need to wait for swelling to resolve before considering a revision because operating on swollen tissue makes results inaccurate. I hope this clarifies things for you.

Had I known he was going to do the incision on the outside, I would have reconsidered my choice in surgeons. But, then again, nothing is certain. Thank you again. The transcutaneous approach can still have great results if the incision on the skin heals well, the inner anatomy is not compromised by the incision, and skin wasn't removed that could possibly cause a skin shortage.     Sir m from India.due to fat loss. i had to take far injection..Bcz per yr fillers are financial frndly for me N i have dry eye.. Now I feel like..That graft is laying in between under eyebrow..n between brw bone n lid..I feel like it cause eyelids are rubbing against my eyes..n make red veins n pains..I am at lose of hoep

    Hi @jayfasf6828, Sorry, but restoring the lower eyelids, especially if it's due to a previous surgery where skin was removed, and lower eyelid support was compromised, is an extensive procedure performed by eyelid specialists, so it will be costly. The way to avoid the cost of revision is to have had lower eyelid surgery that avoids common complications.     Dr i had done blepharoplasty and fat removal but my one eye is looking different than other and it is sunken now 😢this is killing me now and m in depressed now i thought of getting fat grafting done To my upper eyelid but i heard that it led to ptosis … is that true ? But i have no option left please advice me

    Hi Kajal Bisht, Sorry to hear about your problem. Fat grafting to the upper eyelid does not cause ptosis unless there are complications from surgery. Also, filler can be applied instead to the upper eyelid to treat hollowing, so there is another option that doesn't require surgery.     Hi@@kajalbisht8174, There is no permanent treatment for this issue as even grafted fat gets absorbed soon after surgery, and remaining fat can diminish with aging. I hope this clarifies things for you.
