    Really helpful information. It is more like an educational program. Well calculated, judgmental analysis of two approaches or techniques of surgery and how cheap cost could badly affect the results and could increase the upcoming cost to correct it. But even costly surgeons don't care about the technique they adopted and patient do not only suffer physically but financially also. Big thanks you care about your patients.

    Hi Talat Alishah, You can start by asking for the transconjunctival blepharoplasty procedure by name, and insist on not having any external incision and removal of any lower eyelid skin before agreeing to any procedure. If the doctor cannot perform the procedure, or insists that skin removal is necessary (it isn't in most cases, unless the skin folds over as shown in the video for skin pinch blepharoplasty) then look for another surgeon.     I'm glad you found the video informative, TR_NW. You should know that the transconjunctival approach from the inside of the eyelid is not practiced by most surgeons, so some may downplay the approach since they can't perform it. A surgeon who performs the transconjunctival approach is more specialized in lower eyelid surgery, so I suggest you find a specialist if your doctor is not trained with this technique.     6 days ago I had a lower eyelid correction with fat transfer in which the eye sphincter muscle was tightened. I still have bandages and stitches that will be removed on Monday. There is still some transparent, moist 'mucus' coming out of my left eye in particular. My eye is no longer red. Is that normal, and will it eventually go away? Could that still be wound fluid?

    I have had a lot of fillers over the years and it looked great for a long time however, now that Im getting older. Just underneath my eyes are blueing tint all around the tear trough and it's puffy and has little dent all around the lower side. Looks terrible. I don't even want to go anywhere now. I have appointment with Dr Prasad next month. Looking forward to that appointment.

    Hi M Hask, Thanks for watching! While I may know surgeons in the Georgia area, we all have different aesthetic styles and practice focuses. I suggest you ask for transconjunctival blepharoplasty by name which involves no external incision in the lower eyelid for visible scarring and potential damage to lower eyelid support structures; and insist on no lower eyelid skin removal to treat wrinkles, as this can cause skin shortage that will pull down the lower eyelids. Under eye wrinkles can be treated non-surgically to improve skin quality. I hope this helps you.

    I did my lower eyelid surgery with one of the most famous Beverly Hills plastic surgeons and not only he disfigured my eyes but also damaged my nerve that supports the skin under my eye. I spent a fortune to get disfigured. Now even the specialists are trying to talk me out of skin graft because they say it would drop because of the damaged nerve.

    Hi angelecara35, Puffy eye bags visible all the time are caused by orbital fat that has pushed forward, so transferring fat to an area with excessive fat isn't the right procedure for treatment. Fat transfer is done by some doctors to treat hollowing, but it is highly prone to complications, so I strongly don't advise it.     Do you have any recommendations for a great, artistic, oculofacial plastic surgeon in the San Francisco Bay Area, or the Sacramento area? I saw a plastic surgeon out here the other day, who wanted to do a transcutaneous approach on me. He actually did my dad's eyes about 20 years ago though, and they still look good (no ectropion, entropion, rounding, etc). I'm just not sure where to turn, as this physician was the only "word of mouth" plastic surgeon I knew of out here, since moving. Thanks in advance. :)

    Hi Doctor Darcy, While I do know other oculoplastic surgeons from around the country, we all have different styles and practice focuses. Not all oculoplastic surgeons focus on cosmetic eyelid surgery. I have seen well done transcutaneous blepharoplasties, but I would rather minimize risks further with the transconjunctival approach. I suggest you look at prospective doctors' before and after pictures, and ask them if they can perform the transconjunctival approach as that usually indicates that they have more experience and expertise with cosmetic lower eyelid surgery.     Hi Dawn Martyne Riley, I preserve the lower eyelid rim by not cutting into the lower eyelid support structures by performing transconjunctival blepharoplasty from the inside of the eyelid, so there is no external incision that can compromise support anatomy. I usually perform a canthopexy to help patients who lost lower eyelid support with the external incision of a transcutaneous blepharoplasty, and/or unnecessary lower eyelid skin removal performed by another surgeon.     Hi Rafael Gomez, In addition to local anesthesia, we also use LITE IV sedation to calm and relax our patients during their procedure, so they are in a twilight state of consciousness rather than fully awake during the procedure. I hope this clarifies things for you.

    Hi Xime Fdez, Positioning, and preventing migration of under eye fillers have a lot to do with the placement technique of the doctor performing the procedure. Also, amount of filler is a factor in migration, but very little filler material is used in the lower eyelids.     Hi Melanie Roofe, Googling for specific surgical techniques might help if their websites do feature transconjunctival blepharoplasty, as well as PRP and laser treatments. However, as many websites may not be as detailed, you may have to contact prospective offices asking if the surgeon performs transconjunctival blepharoplasty by name. Also, expect surgeons' offices that don't offer transconjunctival blepharoplasty to be little defensive about not being trained in this technique. I hope this helps you.

    I just had Transconjuctival Blepharoppasty with laser resurfacing 10 days ago. I’m a 41 year old male and I see what appears to be volume loss or a raised wedge on my cheek bones. Is this normal or could it be swelling from the surgery? Also under my eyes are still red from the laser, how long before that starts to fade away thank you.

    Hi cornbluff1, I would have to examine your cheek area to determine what the issue is. Post-surgical swelling is possible, but also a difference in volume between your lower eyelids, and the upper cheek area called the eyelid-cheek junction. Sometimes volume enhancement is needed in the upper cheek after lower eyelid surgery to better blend this junction.     @9:50 and also @10:20 I see patients that have eye bags and these eye bags cause a darkness in the skin. I also have this darkness of skin for my right eye bag. May I know what is the actual cause of this darkness of skin? Does removal of the fats from the bags automatically mean that the area will become lighter after surgery? Thanks.

    I had lower bleph surgery leaving a L shape indentation under my eye a year later I had a revision which caused my lower lid to turn out and even worse I have a large lump of scar tissue under the skin on my cheekbone! I’ve lived with it for 3 years and I would like to have the lid repaired and scar tissue removed via inside the lid. Will scar tissue reappear? Is this a risky procedure?

    Dr Prasad, about the fat pads, how do you actually decide if you do repositioning, or if you remove all three? How is repositioning actually done? I am really apprehensive about the surgery. I haven’t decided on it yet.

    Hi, I don't perform fat repositioning because the pedicle of fat used in repositioning is not enough to provide volume in the lower eyelid/ upper cheek area, so I would use a filler instead if there is adjacent hollowing. I hope this clarifies things for you.

    Hi there! I commented on ur last video in regards to the surgeon wanting to remove skin from my moms upper eyelid to CORRECT her lower blepharoplasty complications which is ectropion in both eyes and retraction. He wants to use the upper eyelid skin for the skin graft. In ur last comment on ur previous video.....were u meaning that there isn’t much skin in the upper eyelid to be efficient enough for the skin graft?? Also she can run into the same problem (ectropion/retraction) with removing the skin from the upper eyelids correct? If he pulls to much skin from the upper lid? Im very nervous for her to do this corrective surgery 😩

    Hi JL123, I said that there is a possibility of doing a graft from the upper eyelid skin, but often there isn't much redundant skin in the upper eyelid. I indicated other grafting possibilities that are not from the upper eyelid, as well as graft materials like Enduragen in your previous questions in the other video. I hope this helps you.

    Hi Doctor. I have deep set eyes- w hollowness. It was corrected adequately w fillers for yrs. Now, I think I have too much filler. Even had some dissolved. Still see two discrete round deposits of what I think is old filler. Can that be imaged w an MRI? I was told recently by a facial plastic surgeon that I’m not a good candidate for under eye blepharoplasty because I have festoons!?!! Is this so?

    Hi jjowhal, It may be possible to locate encapsulated filler with an MRI, or it can be diagnosed by the doctor without an MRI. Lower blepharoplasty is not recommended for treating hollowing, unless the hollowing is result of eye bags making the adjacent area appear hollow.     @@AmiyaPrasadMD - thanks doctor. I’m going for the MRI in a few weeks. My eyes looked a lot better w filler for years. When I went overboard it looked bad. What about festoons? Is that contraindicated for lower blepharoplasty? I’ve been turned away by 2 doctors. Despite my eyes looking quite horrific

    Hi@@jjowhal, Lower blepharoplasty can be done if you have festoons, but festoons are not treated with lower blepharoplasty. An examination is needed to determine if your festoons are best camouflaged with filler, or need a separate surgical removal. I hope this clarifies things for you.

    Hi Dr. I have a question….. I had lower eyelid blepharoplasty going on 4 weeks now. My right eye has been tearing up like a week or 2 after my surgery. It’s just my right eye. I have been using eye drip lubricants during the day but that doesn’t really seem to help. Do you happen to know why that is happening or what I can do to stop the watering on my right eye? I would appreciate any suggestions or recommendations. Thank you soo much in advance. 🙏🏻🙏🏻. Oh my lower bleph was done on the inside of my eyes and not through the outside of eyes. Not sure if that makes a difference. Thank You again!!

    Hi Aflashofreality, Sorry to hear about your problem, but you should first follow up with your surgeon who can examine the problem physically for a proper diagnosis, and knows details of your surgery. It's possible for dry eyes to affecting only one eye, and make sure your vision is normal to rule out any possible eye problems. If your surgeon can't identify the cause, then I suggest you see an ophthalmologist to check if there are any issues with the eye itself. I hope this helps you.

    Dr Prasad I had a lower blep done 5 weeks ago by an oculoplastic surgeon. I have lower eyelid retraction and still have some numbness in the lower eyelids. Also movement in my lower eyelid is limited( I cannot squint)How much time do you recommend giving before deciding a revision is needed?

    Hi E S, Sorry to hear about your problem. I normally recommend that at least 6 months is needed before a revision surgery can be done due to temporary swelling obscuring results, and affecting the accuracy of a revision. I would have to examine your eyelids, and determine if there was any skin removal, or an incision through the lower eyelid skin and muscle in your previous surgery that could have caused lower eyelid retraction.     @@AmiyaPrasadMD thank you so much for responding. I only had fat pads removed. The doctor is saying that once the internal wound heals the the retraction should go away. Oh and this was transconjunctival too.

    Hi Samia Tourabi, The risks for ectropion and lower eyelid retraction are greatly reduced since transconjunctival blepharoplasty is designed to keep lower eyelid support structures intact to preserve eye shape. However, you must still insist against lower eyelid skin removal to treat wrinkles as this is a major cause of lower eyelid retraction, and ectropion.     Hi Doctor, thanks for your video. I had a lower eyelid transconjunctival blepharoplasty 4 months ago, in addition I had fat grafting under the eyes. Now, under one eye I have a little lump and my eyes looks slightly uneven ((When I touch this lump I feel a something hard ,obviously some fat that has not survived . My doctor says to wait it might go down but I don't think it will resolve. If I go for a second operation to remove this fat, can I have transconjunctival blepharoplasty? Do you think Ultherapy can help to melt that fat ??

    Hi Madina Carter, 4 months after your previous surgery may be too early for a revision, especially if you still have post-operative swelling. I don't recommend or perform fat grafting under thin lower eyelid skin due to the high risk of complications like lumpiness of grafted fat, but I do perform surgical removal of grafted fat as I do perform revision surgeries to correct work performed by other doctors. A thermal device used to melt the fat will injure thin eyelid skin before it even reaches the fat as it lies under the muscle. I hope this clarifies things for you.
