    Hate to say it but in many ways, modern medicine is highly problematic. The approach to treat the symptom, not the patient and then insurance (!) having the last say on treatment,… Med school is overrated in those areas where trial and error can help a patient understand their body so much better.

    For me the bloating and gas after meals disappeared as a nice side effect when I began to eat low carb, no sugar and no junkfood and moderate vegetables. My diet consists now of more animal foods and not more then 2 of 3 different vegetables at the same time. No beans.

    Weird that they treated my Sibo with ppl and antibiotics 😳 now they are pushing my kids with them. I’m having us all do this test ! Good way to stay ‘ thank you 😊

    I had my motility + gut lining impaired by food poisoning which caused constant reflux when I ate anything. Got put on pantoprazole (a PPI) REPEATEDLY because the issue kept coming back and developed SIBO pretty quick 😑 Wish I could get after every one of those doctors with a newspaper + that I’d found your channel earlier!

    I probably have less confidence in my GI doctor than any doctor I have ever known. Every stomach or abdominal issue I have he wants to put me on pantoprazole. He wanted me on it for life but I (on my own) found out about LINX surgery and I had it done which fixed the majority of my GERD so I only go back to my GI doctor now when I need a colonoscopy.

    I am usually a pretty jolly person but I did go back and left reviews for doctors including partially blackened blood tests for proof. Should have been treated, wasn’t. Ok then 👍

    Thank you for the WARNING ⚠️ on PPIs.. took me years to fix my digestion after taking Omeprazole for just over a year. Used Betaine HCl high dose (4-5caps) to get things going. I’m off Betaine and on my way to full recovery. Awesome video 👏🏽👏🏽👏🏽👏🏽👏🏽

    @@heartsoulholisticservices the purpose of HCL is to help you breakdown protein. But did you know it helps your stomach start reproducing gastric acid? I started with one capsule 💊 and gradually to two …up to four. When I got to four I could feel the strength of the acid and slowly reduced back to about 2. After a while I reduced to one then. By this time I was able to go off them. I do use once in a while if I overindulge but right now even on carnivore I haven’t used Betaine HCL. As for PPIs, I quit as soon as I discovered the dangers of taking them long term. I encourage everyone to get off PPIs and starting using Betaine HCL in conjunction with enzymes and Ox Bile (bile acid) to get digestion going. Won’t need to take it long before things change for good. PS: one does not need to be on any medication or supplements forever. The body is capable of getting back to normal when you help it. Change diet (drop those carbs) and give it the chance.

    @@brothercaleb Thank you for your response! I was aware and have been patiently waiting for my body to no longer need it. Unfortunately, when I do the HCL challenge I can take 6 before I eat and feel nothing. zero. I have not increased more because I think that's a lot of pills so I basically take 6 with my big meals. I tried to ween off last year and it backfired pretty fast. I am low carb and only eat whole foods so maybe I have other digestive issues I need to address? Currently treating hydrogen SIBO and I also have dysbiosis in the large intestine -just a bundle of fun over here!

    @@heartsoulholisticservices have you tried carnivore diet? Helps starve those bad microbes as you’re completely eliminating sugar. Do you have food sensitivities? Also think about oxalate poisoning, salicylates etc Also, spread the capsules as you eat… in other words, don’t take them all at one before meal. Start a meal first, then take first capsule… eat a little and add another capsule. Eat a little and add… and so on. Also, what do you mean when said “… it backfired “, (when you try to wean off)? Btw, people are known to take more caps than 6. You really want to get to where you “feel” that sensation (I don’t want to say “burning”), then backup one cap-this is YOUR limit (everyone is different). Trust me, when you get there. You won’t have to continue for months… I didn’t anyways.

    @@brothercaleb I have not gone full carnivore but I did experiment with a more keto stlye diet years ago but the fat was too high for me and protein is harder for me to digest so I am weary to try that again. I have also heard htose diets can rally mess up the microbiome since they feed off fibers and I digest fibers and fruits and veggies fine. I don't have any food sensitivities that I physically feeeel but my testing showed eggs, avo, and salmon, cabbage, and broccoli so I've been avoiding some fo the most healthy fats which has been hard. I don't need to do low FODMAP or anything. My digestion is just sluggish and wondering if its related to my vagus nerve? I had food poisoning in 2019 and then I had a lot fo stress in 2020 and again this year. I was taking the caps throughout the meal but then got confused because I heard people were taking them all before -so hard to know what is the right thing, isn't it!

I can help guide the conversation with other providers for my one on one patients, but I don't feel comfortable doing a video on thunderstand.     Dear Dr Nicole, i am an MD in Vietnam, i really appreciate your video, and i also have the same opinion on PPI. There is an Systematic review last year " Association Between Psychosocial Disorders and Gastroesophageal Reflux Disease: A Systematic Review and Meta-analysis". They also seperate GERD into erosive GERD and Nonerosive GERD. If nonerosive GERD, we should not give our patient PPI, because nonerosive GERD has different pathogenic to erosive GERD. Erosive GERD is usually has a lot of acid while, nonerosive GERD is usually related to Stress which has less Acid, Shorten of Acid and keep using PPI is really a problem. As my experience, if a patient has Non erosive GERD we should help them release stress, reduce inflammatory. I try the Curcumin in Tumerric for myself and it work, but i believe most of the inflammatory drugs will help, or lemon balm which reduce the cortisol also help.

    As a practioner myself one must also evaluate Bile in this scenario. The last thing one wants is all that gastric acid/chyme hitting the duodenum unopposed by more alkaline media. Enter....Duodenal Ulcer! 😮

    PPI therapy 100% caused my SIBO. I suddenly got bloated eating my normal diet of beans. I smelled the difference in my BMs. 💯

    For me, it wasn’t how many HCLs I took a meal that caused symptoms… it was how many I took in a DAY. If I take any more than 3-4 HCLs a day, I get a burning in my esophagus from over production of acid. So for me I can’t take any more than 1 HCL per meal. My chronic reflux has improved soooo much as a result!

    YES….I agree PPI’s are terrible! I Was diagnosed with gastritis (triggered by stress & coupled with back to back rounds of antibiotic/prednisone for virus). Three months of PPI’s caused my digestive system to get MUCH WORSE. Food sensitivities got even more restrictive and bloating/burping escalated exponentially. I’ll never take a PPI again.

    Very helpful video for me but looking for one more note of advice on this challenge. My naturopath wants me to do the HCL challenge for 6 months, seems rather long right? She makes it sound like the HCL challenge is a cure, because it will teach my body to actually produce more acid on its own once I am done with the challenge. Is this true? I have not been able to find any research to verify that. Is there any danger in doing this challenge without knowing weather your stomach lining is robust or weak? Thanks!

    Hi Dr. DiNezza I'm a 35Y old male who was recently prescribed Dexilant from an ENT specialist where he noticed that my throat was dry and my vocal cords we're irritated. However, I have NO heartburn or acid reflux, I do however have belching or what feels like stomach gurgling or pressure after I eat which is then followed by a burning in my throat as well quite a lot of gas.This occurs on a daily basis. Could this be low stomach acid or simply stress or anything else for that matter? Thanks you and love your videos

    Thanks for addressing this Dr DiNezza! I’ve long suspected that I have low stomach acid but due to gastritis I have never tolerated HCL supplementation. Pretty sure I don’t have any stomach mucous lining so figuring out how to work on that first.

    I had the same problem. Try taking 5 grams of L-glutamine powder in water on an empty stomach every day. I took it in the middle of the night. A couple of months of that should fix your stomach lining and get the gastritis under control. Then I would add 500 mgs. of vitamin C in the ascorbic acid form with a meal once a day to kill bacteria, so gastritis doesn't come back. Then once that is stable, you can lower the L-glutamine to 2 grams per night, keep up the vit. C, and maybe try the HCL and go slowly. You may have to tinker with it, but that is what has worked for me for a decade now. Good luck!

    @@nidhixiiixii - High dose Vit. C can kill many types of bad bacteria, but do not use the slow release forms. You want it all in the stomach so the cheap ascorbic acid works best for this purpose. Then L-glutamine can help heal your gut lining, especially if you take it on an empty stomach. Imo, they work best as a team; just don't take them at the same time of day. Don't take Vitamin C when you have an active ulcer. L-glutamine can help with that also.

    Hi Dr. I am taking almost 10 capsule of betain hcl with food but no burning sensation or any kind of discomfort. I even tried to take 1 capsule of betain hcl on empty stomach still no burning or discomfort at all. Can you explain whats happening? You may have answered it in the video but please share your thoughts. Btw my baking soda test also produces no burping at all, even after doing the test multiple time.

    I'm confused.....at 2:49 in someone with plenty of acid naturally occurring, they take one tablet and acid gets pushed up the esophagus and causes symptoms to be felt. However, then at 4:57, where someone may have an outrageous amount of acid, but has a good mucous lining, you said they will not feel symptoms from the extra acid put in by the tablets. But wouldn't it still get pushed up the esophagus and so therefore WOULD be felt? It doesn't look like the mucous lining would prevent acid being pushed up the esophagus. maybe I am missing something, but that has me confused.

    I’ve been on lansoprazole for 13 years..I tried betaine HCL I took the recommended daily dose of 2 capsules which where 480mg i experienced severe headaches from it also I tried ACV 2 table spoons in a glass of water and that also gave me a headache and jaw pain..I need to get off the PPI’S

    My doctor automatically assumed that I had a high acid level because of epigastric pain. She did not examine me but prescribed PPI because she said the stomach acid enters the esophagus and causes pain. I suffer from constant bloating and stomach pain despite having given up smoking and minimizing coffee and spicy food. Now I understand that PPI suppresses stomach acid; I took the baking soda test and did not burp at all within 3 or 5 or even 10 minutes. If I would try the HCL test, what type or brand would I use? Can you recommend?

    I often theorized I have low acid.....I have some gastro problems & some tolerable discomfort, but very often, I get easy food poisoning when eating out. I always eat cooked food & nothing uncooked. Seems like I'm sensitive to the least amount of bacteria, & I get a stomach ach for a week. I was on Zantac as a kid in 1985-1992 & donatel liquid then also..

    Hi Nicole, very informative video, thank you. I took 1 hcl table after a high protein meal and got a warm feeling in my stomach, however around 3 hours after I took it and lasted thru the day. Does that mean I have sufficient stomach acid?

    It either means that you have sufficient stomach acid, gastritis, or a thin mucus barrier. I would look to other symptoms and history to determine which is most likely. For example, if you have indigestion, difficulty digesting meat, excessive belching and/or feeling of excess fullness after you eat, then I would still suspect low HCl.

    Hang on low stomach acid means to high a ph level not volume. I took 16 pills a meal before I got warming but it stopped the reflux and digestive issues. Stopped due to cost. Reflux is poor diet plus high pH levels. Take 1/4 TSP of bicarb of soda in 1/2 glass of water, if your not burping within a couple of minutes your stomach pH is to high.

    Should you take hcl if you have a possible h pylori infection or presence? I have heard this before but not sure if I understand the connection or the science of it. My gi map stool test came back positive for h pylori but I don’t have any of the usual symptoms of h pylori

    Apparently H pylori can cause stomach acid to decline. So it is recommended to take HCL with food while you’re getting treated for the bacteria. Otherwise it will come back after the treatment because you’re stomach acid wont be strong enough to protect from these pathogens.

    Dr I have gastritis and I think leaky gut issues besides suffering from high histamine and I know my stomach acid is low because of my I take aloe Vera juice and the supplements like enzymes and DLG and something called gut healing that help me too. I stopped taking omeprazol few years ago I never took for long time and I am taking the least medication for all my overall health and trying to get rid of the those medication as soon as I can

    I’ve mostly cured my heartburn with ACV and lemon water, but still dealing with bloating, constipation slow bowel movement. I’m Taking oregano oil and garlic extract as of now. Going to see what happens.

    Honestly, we don't know for sure. But theoretically, it will work best if taken toward the beginning of the meal or shortly prior to the meal.

    Hi Dr DiNezza I stumbled across this video. I’m helping My 17 yr old daughter start this challenge. She has suffered for many years with swallowing discomfort. We’ve been on and off of omeprazol several times, had every scope, scan, and test you can think of under the care of 2 different pedi gastroenterologists. I’m at a complete loss. So we are going to see if perhaps her issue is too LOW stomach acid. It’s her swallowing that bothers her so much, not really acid reflux feeling.

    There's a lady I work with and she also had troubles swallowing, she would often throw up while trying to eat. They did a barium test and finally did a scope and there was so much scarring from reflux that her esophagus was narrowed, she also never felt the pain of heartburn or anything, maybe they missed something with your daughter's tests. But don't knock the stomach acidification, acid reflux happens when the stomach isn't acidic enough. There's also this thing generally geared for babies, to stretch the neck backward to loosen those throat muscles to help swallowing. Maybe that could help? What about tonsil stones?

    @@teril.3485 Thank you for your reply!! We are back on omeprazole for a while. She cycles beteween that and pepcid, then off, then repeat. She has also tried the flip side... taking HCL. I dont know...we are hoping to see a good adult gastro when she turns 18 in January

    I’ve got the same as well ! Had h-pylori when I got food poisoning from Indian food and caught h-pylori. You need cleanse your whole stomach, eat Whole Foods vegetables and fruits drink celery juice every morning.

    Hi Nicole, My name is Bennie , well I get straight into it. I just started with symptoms of gerd is what they tell me. It all started with me not being able to eat due to the fact of having heartburn, nausea,upset stomach and constipation. Couple of doctors put me on sacrulfate and famotidine. Seems like it made worse. I've stop taking them. I've been able to eat and heartburn and acid reflux is a bit better. Granted I'm eating very little and when I eat a bit more it seems like it's hits my stomach quickly and doesn't move. I feel very bloated and constipated. I have started talking Metamucil and stool softner. Still dealing with constipation and bloatingness and some heartburn, burping, and upset stomach when while eating or a bit afterwards. Any advice/ Ideas... Thanks

    Try a betaine HCl challenge and see if it's stomach. I think it's part of the issue for me.

    If you want to get rid of constipation , take a teaspoon of coconut oil twice daily. You will see results quickly. You can reduce to one teaspoon once you are having regular bowel movements.

    Agree on the mucus lining masking stomach acid effects but regardless of that, wouldn't the real test be to repeat the baking soda and water test until you begin to burp within 1-3 minutes as the indicator that you've reached the optimal number of bHCL capsules? Or is the bHCL intake/response local to the meal you take it with and doesn't evolve into a permanent fix? Further, before one starts screwing around with one's stomach acid pH, shouldn't one first assess/fix any bile thickness/sludge/stones/issues with the gallbladder before the bHCL-enhanced acidic load from the stomach comes marching down to the duodenum so it can be neutralized by the bile beforehand?

    Thank you for this great information! Now, what if you have chronic gastritis? No ulcers. Can you take HCI? I had this functional test done and it showed mild/moderate low stomach acid.

    I believe you have to heal the gastritis first then raise the stomach acid otherwise it will cause discomfort, I have the same problem and am trying to find a solution, mastic gum might be a good alternative and Dgl licorice has studies , look up and see what you think , I’m going to try it , also it might be a bacterial overgrowth causing the problem or possibly bile acid backing up

    @@debracarson5330 Thank you, I have been taking DGL and my gallbladder was removed some years ago. Had Gastritis basically all my adult life, sometimes is inactive and other times is gets bad but is like a lifetime problem. Maybe my best option is to just take the pills and see how it goes.

    Gosh I hope not! As long as you are doing the other stuff too (mindful eating, chewing adequately, tending to your gut-brain axis) you should be able to come off within weeks or months:)

    Hi, excellent information! My mother 89 years old, is always feeling full, bloating and worst of all, digestion problems, *CONSTIPATION.* -I think she is low in stomach acidity, so I have been giving her, for 2 days: -Before each meal: 6 pills Betaine HCL (I start with 3 pills, then 4....) -And after each meal:1 pill -Ox Bile Salts (adding to it some Nutritional Yeast Flakes). -None of this two meds had help her. But..... I have a problem with the HCL: -She says she feels too full to eat, after taking 6 pills HCL. Last time we wait more than 30-45 minutes to start eating. -Is that ok to wait so long to start eating, after taking HCL pills????? -I don't know how to help her with her constipation. (I Imagine, if it is hard, she taking the 6 HCL pills, how is she going to take 10 or 14?????) -Also want to know if it is ok to give her: 1 pill Ox Bile Salts, after each meal?????     Look up PPHG (special fiber supplement) and her video on prokinetics. Stewed (google) prunes or prune juice daily (morn and or night) will help (1-2 Tbsp) and magnesium oxide.

    Constipation may relate to Magnesium citrate, I think you try it with a 300mg dosage.

    Just taking four capsules even if they were empty, I gives me acid reflux. Also when I finally get it to calm down the last thing I want to do is experiment to see how many capsules it takes to flare me up. SMH. I’ve tried this test. Too bad it doesn’t come in a liquid?

    This is great info! I have a special situation that needs a modified approach, I fear. I believe my son has low stomach acid (he was diagnosed with GERD and spent long periods of time on anti-acids). He is a special-needs child and he doesn’t necessarily complain if something hurts or if he feels uncomfortable, but we know he has reflux and it has caused him esophagitis. I’m trying to find the best way to test his level of stomach acid and figure out how much Betaine he would need. He does have a G-tube, so I could draw out a bit of the contents of his stomach with a syringe and test it. I’m thinking this might be the best way? Anyone know of any ways of directly testing the stomach juices or contents for level of acidity?

    I think he needs some zinc and thiamine support besides HCL. Zinc L-Carnosine is great for GI. Additionally give hım zinc picolinate for whole body.

    Dear Dr. Nicole, after a bleeding ulcer and hplori negative the actual sore via endoscopy shows heal, but the manifestation of symptoms left behind. Cat scan shows gall bladder is fine. Doctor says its dyspepsia. I never had GI symptoms prior to this ulcer due to ibuprofen. Tried HCL but it makes it worse. Perhaps the IV PPI's in the hospital and afterwards caused an imbalance?

    I have no idea what's going on. I can't take hcl or most antthing else. When I say reactions i mean I have extreme head pressure with hcl or lots of foods, drink, even water. Also chemical and other intolerances. 😢

    Avoid any food or liquid that irritates your gut/GI system. First if you talk with a medical Dr and confirm diagnosis with upper endo scope and biopsies. If not sure how just search online to see how where you live if you don’t have a reg Med doctor. Can’t self dx bad mucus lining or low/high acid. After your assessed google how to heal your stomach lining. This channel has lots of videos too.

    I have SIBO and i feel like being on a low carb high protein diet for over 10 yrs really messed up my tummy!! Now i am about 80% plant based because i have issues digesting animal proteins

    I used to wake up burping and tasting last night dinner and all of that has gone away except for occasional bloat and constipation. I was put on a probiotic and stopped taking it.....

    They are 2 different things. Digestive enzymes are synthetic forms of enzymes your pancreas makes normally. Betaine HCL is a synthetic form of the stomach acid your stomach makes normally. As far as which one will help you, it would depend on the condition you have.

    Digestive bitters are not the same thing as digestive enzymes. Digestive bitters are bitter herbs that are supposed to stimulate improved digestion.
