Tennis Elbow Surgery - How Do You Know When It's Time? (Lateral Epicondylitis)

Surgery for Tennis Elbow: How do you know when it's time? Detailed article: goo.gl/yHxms4
When you have an extremely stubborn, chronic case of Lateral Epicondylitis or Medial Epicondylitis (Tennis Elbow or Golfer's Elbow)
And you've been struggling with for it months - Or maybe even for years!
► When should you realistically begin to consider surgery as a treatment option?
► How do you know if you're a good candidate for a surgical procedure or not?
► And when is it time to abandon more conservative treatment options, because you've tried hard enough and long enough?
(These are not easy thoughts to face or questions to deal with for anyone who considers surgery a last resort!)
I hope this episode helps you in your decision making process, if you find yourself starting to wonder!
Here's my full, detailed post on this difficult question if you'd like to look at a few other considerations:

Ive had 3 surgeries. ( golfers and tennis elbows left and right ). Torn the tendons on both arms, 1 on the left which has recovered well nearly 1 year, but the right arm i have done twice and will need to do a third. Its a terrible condition. had multiple steriod injections before. Will never be the same but, being pain free in years is great.

This is a fantastic rundown! I have been struggling with medial epicondylitis for 18 months. I'm 51 and play a ton of golf. I went through the hoops of PT (which I knew wasn't going to help), tried a cortisone injection (got a few weeks relief from it), then after an MRI they found a 7mm tear. (I haven't weight trained or played golf since Feb, and then I couldn't barely swing a club). I then opted for a PRP injection (which I suspected wasn't going to help and actually made the pain worse than before); I think the physician may have botched it. My sports med physician gave me the choice between PRP and surgery, so I figured I'd try the least invasive option first. Anyhow, I just got surgery a few days ago. They did tendon release and debridement. My surgeon told me that this was my last resort due to the state of those Tendons. I'm hoping I get good results.

It's been three weeks since your comment and surgery (thank you for the praise!) How are you doing now? Hopefully your postop acute symptoms are way down and you're doing well. Another 3 weeks and I imagine you'll really be noticing an improvement if all goes as expected. I'm not anti surgery. When you really need it you need it and Tennis Elbow surgery has a really high success rate, as I understand it.

Hey, KZread - Channel subscribers and visitors. I'm sorry it was such a long time between my last video and this one. I really want to do more full "talking head style" videos but, as you can see, this is another podcast / audio only one... (I find it takes so much more time, preparation and energy to make those visual videos that I rarely get to it.) I think it's better to at least share my podcasts here on KZread rather than not posting anything at all - What do you think? I hope you find this video on the question of when to start considering surgery helpful - and let me know if you have any thoughts about the format! And here's the link to the accompanying article, which goes into a little more detail: tenniselbowclassroom.com/tennis-elbow-treatments/tennis-elbow-surgery-when-is-it-time/

How severe does a tendon tear have to be before you need surgery? It's a tough question and here's my latest post on it: tenniselbowclassroom.com/treatments/healing-tendon-tears/ (It's a pretty extensive article. 700+ words. I don't know if there's going to be a video yet) - but I hope it helps you get some sense of what your options are, if you have a tear (especially if you have Golfer's or Tennis Elbow.) In some cases you may be able to heal and recover through a minimally-invasive procedure like PRP, Prolotherapy, Stem Cell Injection Treatments - OR one of the newer procedures like Tenex or TenJet. (And I do believe some milder tears can be healed and rehabilitated without any medical measures.) ALSO... I have an article and video (which I haven't gotten around to uploading here yet) on the question of "When do you need an MRI when you have Golfer's Elbow or Tennis Elbow?" tenniselbowclassroom.com/what-is-te/mri/

I have bilateral epicondylitis in which an MRI revealed advanced tendinopathy and low-grade intrasubstance tearing of the origin of the common extensor tendon in the left elbow and an x-ray revealed calcification at the insertion of the triceps tendon on the dorsal aspect of the olecranon in both elbows. The pain is obviously worse on the left arm. Would you recommend removing the calcium to help with the healing?

I'm sorry. I can't make these kinds of direct, personal recommendations. What does your surgeon recommend? Has the option of using Shockwave Therapy to break up the calicification come up? (I don't know whether it's a realistic option or not - but it might be worth asking about and looking into.)

Would that be better than doin surgery?

I currently have tennis elbow (I am 46 years old, 6.5 '', 230 lbs) which was most probably caused from pull ups or TRX rows. I have it 2 months now and since then I have stopped exercising my upper body. Only legs, abs and lower back. I have done 2 weeks of physio therapies with electricity and ultrasound (unfortunately combined with ice...) with no success. Also tried some exercises, but they seem to make things worse (practically holding a dumbell, put the wrist in the upmost position using the other hand and leaving it to go down slowly). Have tried also some massage, either on the joint that feels pain, or in the muscles above and below the elbow, but to be honest is not consistent and of course I don't know how to do it exactly. I will try your methods and upload my feedback. Just a question for the future, when and if I start doing again some exercises. When doing pull ups is it better for the tendons to hold the bar with your palm instead of the fingers? And is it better to do push ups making a punch with your hands and not the typical twisted palm position? Thank you! PS: In the past I had a similar problem in my wrist and the physiotherapist I was going to (in Athens-Greece) was using some wood sticks (with a small ball in the edge) to remove the scar tissues. There was a hell of a pain in the process but in a couple of weeks the pain disappeared and never came back! And another time, when I had problem with the shoulder, I went to a chiropractician who massaged all my hand from the elbow to the wrist. Again pain disappeared after only 2 sessions! Unfortunately I don't have access to any of those guys now. But this makes me thing, out of my personal experience, that removing the scar tissue is the most important process for healing.

Do you have the "classic" kind of bone spurs: Osteophytes, (which form inside the joint) Or do you have Enthesophytes, which form where tendons attach to bones? There's a big difference.

But i also had raidial nerve surgery in firearm and they lengthened my tendon after scraping it clean. It was so tight it was also causing my rad nerve problem. My nerve area hurts way worse than my tendon. My nerve was squeeezed flat dr moorey said so it will hurt more as the nerve is free n mad from surgery. I had pain meds all day yest in hosp. Im missing my legs soo they kept me obver night. And i had some up to 1pm and none since. Ill take b4 bed for sure. Therapy starts in 2 weeks. Stitches gotta come out b4 he said. Anyway its not that bad. Ive been thru way worse.

I had an mri and I have high grade partial tear of the lateral tendon. I’ve done 2 prp shots. I even had a cast put on so that I would rest it. I’ve been off work for 2months. It’s looking like I need surgery, because when the therapist pushes on the tendon it still hurts. 😬😁

Now it's just bad, no inflammation but there is a tear mild the doctor said. He said the tendon quality didn't look good for my age either (im 21).i know exactly what he meant. There is now scartissue and a tear so i booked a surgery in a month. HOW TO HEAL FASTER? Use bpc157 it did help me noticeably but i damaged it again 🙄.
