    As a former RN of 13 years I appreciate the lengths you went through to deliver this info. I went through nearly two years of chlomid therapy for infertility treatment. After completion of that I maintained well for a while with proper diet, sleep, and exercise regimen. However, (in my new career of 4 years) my work schedule flipped back to a swing shift 7 on 7 off nights to days. I felt the nose dive, and my recent labs came back with a 157 and Free of 6.8. I had discussed with my physician the possibility of subq vs IM. Your information definitely helped to inform me on a decision I feel that’s right for me. Thanks again! Hopefully I can get back to feeling like myself again soon.

    Been doing daily SC with a 27 insulin pin the last month, after 5 years of E3.5D IM. Same dose. Feels like more energy and increased libido. Find myself waking up naturally without an alarm and not needing a nap during the day.

    @@manmedicine most certainly. It makes sense that it can flow different through your body at diff rates with the IM. That the SUB q keeps it always the same evenly distributed. A lot of the TRT clinics act like you have to do it IM. Why I don’t know 🤷‍♂️. My cyp 200 is in Grape Seed oil. Hopefully it won’t be to thick to run smaller needle on sub. Only way to know for sure is try it I guess !! I feel much better that I won’t be wasting it doing sub q. I’ll be taking the Gonadorelin .5ml 2 x a week. The test cyp is 1ml every week. I can do it in one shot or split it into 2 !!!

    Thank you for your answer. Extremely informative. I am at the beginning of a TRT path. I started with Androtop/Androgel. I was not satisfied. But I kept it for a year just to see more long term results. Now biweekly intramuscular injections of Omnadren (Sustanon). Results are pretty much incredible in all areas: sexual, mental, psychological. I am not sure if it is related, but hypotension and bradycardia that developed some few years back are gone. Once again, thank you for your video.

    Hey Doc I just wanted to thank you for this video. I just started TRT (1 week ago) and I was unsure about IM versus subcutaneous injections. Also when you talked about the angles was very helpful as well!! I still have some questions about drawing from the vial with a tiny air bubble that I can never get rid of.     Been doing daily SubQ for little over 15 years. Always steady levels. No pain at injection sites. I prefer fat pads around hip flexor area it absorbs better then belly with no lumps. No scar tissue.

    Best breakdown injection vid I have found yet. And trust me I've seen hundreds. Thanks for taking the time to put that together. My question is, if and when you first start TRT and they start you on a smaller dose to begin with, is there a chance it might lower your free T levels if it's not enough. So for your first 8 weeks for example, prior to your second blood test if the initial dose is to low is there a chance your T levels go down because your natural T is turned off and the TRT dose was not enough to bring you up past your natural T levels. Hope that makes since, thanks again.

    Excellent presentation as always. Would the body absorb that same amount of Test from a SubQ injection versus IM injection? Also, assuming I understood correctly, would SubQ injections have a more "gentle" effect on HCT given the slower absorption rate?

    Thank you for a great educational video. I currently inject testosterone cypionate twice per week using a 27 gauge needle into a muscle. This relatively narrow gauge needle is far more comfortable than larger diameter needles and may ultimately produce less long term issues such as scarring. I have also found that needles from different manufacturers can vary widely in how comfortable they are to use. Not all brands of needles necessarily have an ideal sharpness for comfortable IM injections.

    You can even warm up just above room temp using warm water, it thins out the testosterone, and I even use insulin needles!

    Use a different (larger) syringe if you’re worried about sharpness. Love using 25g 5/8” in my shoulders… fresh receptors.

    @@astarpery6260 I meant to say use a thicker gauge needle for loading the syringe of your dose. Then replace the needle with whatever gauge you like depending on location. A fresh needle goes in (pierces) easier and comes out way smoother vs the same needle to load and inject. My loader needle’s are >21 gauge. I’ve also started using insulin syringes to micro does sub q using one syringe and it’s awesome. Watch a video of how needle tips bend under a microscope just from loading meds.

    Surprised to not hear more from you regarding spikes and drops in levels typically seen with IM every 3 days or 1 times per week versus what is becoming increasingly popular with SUBQ micro dose daily. Any thoughts there Doc? Thank you

    My endocrinologist prescribed Xyosted 50mg every 5 days. My endocrinologist billed it as 50mg 2x a week for insurance purposes. I have great insurance and they approved that frequency vs every week and I pay $0 a month. I have been loving Xyosted for the ease and convenience, but I understand that it is a fancy auto-injector and a pharma money grab. My endocrinologist said that Xyosted has been relatively becoming a bit more popular, but not commonly prescribed. Amazing video - appreciate it.

    Big money grab for sure. All that cost gets spread out and adds to everyone's bill. Enanthate is inferior anyway since Cypionate lasts longer and can be done more often (and is super cheap). My guess is that they chose that ester just so they could sell more shots due to the sharper peak and trough. Big pharma is a scam.

    I initially started out doing IM with 25g*1" and then switched to shallow IM with a 27g*5/8". Quad injection. Long term A little less trauma to muscle area and less waste of T in the longer bigger G needle. 27/ 5)8" are a little harder to find most are 1/2" which I use too.

    Hey Doc, Loving the videos. Keep up the great work. I recently switched to subq after doing some research and I’m loving it so far. When I was combing the internet for info I found claim after claim that SubQ lowered hematocrit levels significantly. Some claiming as much as 41%. Is there any science to back this up? So far I haven’t seen it. Keep up the great work.

    Yes drawing into a syringe with a 27 ga or 29 ga can be quite slow. But you can get a vial adapter that allows you to draw your test out of the bottle without putting the needle into the vial using a luer lock syringe. Then just draw your test and put your luer lock 27 ga half inch needle (have never found a 29ga half inch luer lock) back on the syringe and you're ready to go. My pharmacy sends me a new vial adapter whenever I get a new order of test.

    I don't know if this may be a thing but when I was starting TRT one of my ankle had a small amount of fluid buildup, as I have progressed with TRT subquestions injections this went away, now for whatever reason I have changed for intermuscular and the fluids/swelling is back ( have to add I had a small, cuff muscle injury, popped a muscle fiber and it is painful to walk) Anyhow this Lymph fluid transfer is interesting for me I think. I am going back to SC to see if that is relevant for me . Thank you for your knowledge and explanation.

    Another great video! Thanks for the resources. I will point out Test vs. DHT vs. Estradiol levels in the transgender IM vs SQ 1000mg injection reference. The chart was extended over 12 weeks. I think it is very clear that at the 1 week mark which is when most of us inject (typically sooner/bi-weekly) there is about a 15-20% increase in DHT levels over IM route. Have you uploaded a video on DHT and it’s effects on libido yet? If not I’d love to hear your input. Thanks again for the great work and keeping it EBP!

    Thanks. I agree with you regarding the DHT bump. Due to time constraints, I didn't include it in the video, but the SC enanthate study showed near identical testosterone to DHT ratios with IM delivery. I'm not sure I believe that, but I thought it was interesting.

    I've been on for about 9 years and did delts, glutes and quads IM for about 8. I switched to sub-q when I started getting infections IM. It was strange, like my body started to reject it, and I'd get a dermatitis type of infection if I shot IM. After two rounds of antibiotics I decided to start sub-q, and it's been fine aside from what seems like more bloating in the abdomen, where I administer. I'm trying to get the right balance with small amounts of aromasin. Getting monthly bloods now to nail it down.

    My total T was lower and E2 was higher when I switch to subq. I also increased from 100mg/weekly to 120mg/weekly and increased from 1x to 2x a week. I'm trying shallow IM in delts now

    Thanks Dr Wingo, fellow doc here, been on trt x 18 months tried the subq method and noticed a fairly noticeable drop off in multiple parameters (libido, strength, focus and energy). Went back to IM method and things turned around quickly. Interestingly, as I reverted back to IM method my HGB jumped up to 18/HCT 51. Free T jumped way up. I theorize that my SHBG decreased when on subq then when switched back to IM (same dosage) then free T climbed substantially. I know anecdotal, but my colleague noticed the same type of decline when trying subq method. Thanks for your vids, greatly appreciate your time and knowledge. 💪🏻💪🏻💪🏻

    IM works fine... 3x week, four sites, rotated, 27g 19mm needle into quads, 13mm into delts. Shallow into both muscles. Zero pip. Zero infections. Swab everything twice, wash hands... I hate the oil sub cut.

    In my experience with SubQ the lumping risk goes up with the more volume you inject at once. 0.5mL I get a lump probably 50% of the time. I choice subQ because it showed a lower E2 and higher total test in the trough compared to IM from the studies I had seen 2-3 years ago. Presumably due to the slower absorption. Plus it just put my mind at ease, don’t have to worry about injecting into veins etc.

    I was doing intramuscular TRT for a number of years. I hated doing it each week. However, my doctor wanted me to start subcutaneous injections. I've been doing this for about 4 weeks now. I no longer stress doing my injections. The benefits are so much better. Less issues, less pain, and I still get good results. I cannot call this a downside, but same may. The medicine/TRT sometimes gives a feeling of heat afterwards. This passes within an hour. Thank you for this video. Taking the time to explain this in detail.

    No lumps after injections since I moved to pinching skin around my belly. No pain, no stress. I do it in other places around the belly each week. I use a 25-gauge, 1-inch needle to inject. Although, I don't use the full inch. Just enough to get under the skin and go slow. I do this around a 90-degree angle.

    Hi Doc . Just to let you know I’m injecting test Cyp and a small dose of Deca micro dosing every week subconsciously and it’s awesome. Better then intramuscular. I use the small diabetic syringe and put the small bottles in my pocket to warm them up so it’s easier to draw through a very small needle. I have no scar tissue issues and it’s less painful to inject . I normally inject at two places to split the doses up . I have no aromatase issues at all and found out it almost simulate your own natural slow testosterone release in to your body . I’m 65 of age and put on some nice lean muscle and train just as hard as the younger guys in the gym . Mind you I have been training since I was 16 years old and have competed in bodybuilding competitions in the 90s so I am familiar with the drug side of the sport but on small doses. But I find now I feel the best I have ever been. ☺️👍

    I started using Xyosted a few months ago. Definitely more convenient to use, vs. IM. I've only had a few issues where, about after a week, it felt like it created a nodule at the injection site with some minor pain when I would touch it. Another time, I must have hit a vein because it created a bruise where you can see black & blue marks underneath the skin.

    Xyosted ends up being a huge cash grab for the doctor, pharmacy & drug company that gets spread across insurance and costs everyone a ton of money. It's test E put into an autoinjector - they've created nothing new... Test cypionate is better (longer lasting more stable injections) - it costs me $15 for 20 weeks supply at 100mg per week.

    U can back load an insulin syringe with a regular syringe. Pull the plunger out, fill the syringe and slowly, carefully put the plunger back in.

    Have you seen cases where less injection frequency is better? I'm seeing a move among people who tried frequent injections to go back to less frequency, citing the extended time it takes to break down a larger depo, ester half life, psychology of not thinking about TRT every day, and overall compliance.

    Why even draw up the solution with the insulin needle? I simply carefully takeoff the plunger of the insulin needle. I take a regular syringe with a 22 gauge needle and fill it and load up my insulin needle as needed. Then I put carefully back the plunger, being careful not to push it through the needle, it does not come out if you tap it and put the plunger on and turn it the right side up. This works great and you do not have a dull injection needle. An insulin needle will hold roughly one milliliter. So you can fill it up halfway that would be a half and then inject another part of the body with the other half. It's real quick efficient and easy not as long either to do it this way.

    I was told to just use 18 Gauge to withdraw and a 27 1/2 inch needle to inject into the delts and glutes? Is that considered subq in the glutes or am I wasting the testosterone with such a small needle into glutes? I think you answered my question at the end, Not a fat ass but sounds like with a small needle it will be treated like a subq regardless where you inject if too shallow! By the way I did hit my sciatic nerve the first week IM with a larger needle and it sucked... I was ready to quit the whole journey!

    I had nodules and PIP almost everytime in any muscles from IM, switched to SubQ micro dosing and wow, don't feel a thing its so easy! I draw with a 21g so its very quick then switch to 27g 1/2" works like a charm! Waiting for bloodwork for my levels but im not worried about it. Very informative, great video!👌🏻

    Hey Doc, I have been prescribed XYOSTED and well, let me share my experience and labs. While its very convinient by just opening the pen and injecting yourself, its extremly expensive. I also found that my red blood cells went up while on it. Something that didnt happen with test cypo. I do use insulin syringes to give my weekly dose of test cypo and like it alot better than the intramuscular as I had to go to docs office myself because i was to chicken to do it myself...but with insulin syringes, much better. My levels I find to be more stable and dont experiences highs and lows. Oh, blood pressure also went up with xyosted for some reason. Perhaps the ethanate, you would know better than me. Anyone reading this, i would not pay for the xyosted, too expensive, and mostly a gimmick. Doesnt do anything different than the regular way except convience. I would like ask, as far as insulin syringes go, which is the best for injecting and whats the most you can inject sub q of any substance?

    I am doing great so far on Xyostead. Cyp gave me very high blood pressure 155/100 and my doctor switched me to Xyostead. I am on week number 4 and feeling great. I haven't seen a significant increase in my blood pressure and no noticeable water retention. With cyp IM injections, my face looked puffy and round. Xyostead is not on the list of preferred medications by my insurance, but they still cover most of it, and I pay $60 out of pocket. Xyostead also has a plan now for people whose their insurance doesn't cover it m. I believe it is around $150 per month.

    yes, usually. The purpose of pulling back when you do an IM injection is so you don't inject directly into a large vein or arteriole. You don't really have those right under your skin in most cases.

    Saw someone mentioned the pellets as a future topic and itd be great to see you cover bio identical cream as an option too. I apply daily and saw my total T level soar above 'top end' range here in UK but with no alleviation of symptoms, which for me are mostly mental eg brain fog, rollercoaster emotions. Doc advised injections but as its only been 3 months im going to give it the same amount of time again to see if my body adjusts. Lastly i bought your lifestyle/workout book a few months back and wow the HIT workouts are killer...first session I had a chest and back pump like never before and i have been lifting decades. That was 20 mins in the gym and at that point i was done. Im more ecto/meso as per the guidelines and so rotating between a fat loss and bulk period. The book is awesome and your authenticity really comes across well. Cheers

    12:38 that graph and other graphs on testosterone enanthate (like on the Xyosted website) lead me to believe the typical halflife is probably around 10 days, not the 4-5 days I often hear people say.

    Hey doc, my question is can you draw and inject with the same insulin needle to do SC (perhaps dulling the needle for injection) or do you initially use a traditional syringe and needle say 21 inch to withdraw then switch out toa 27 g 1/2 inch for the SC into the stomach?

    With SubQ you can break up your dose into several jabs per week. No nodules with smaller dose…massage the site a little and all good. More steady levels as well

    if anyone is wondering if its ok to subq sustanon, i can tell you it absorbs the same way like any other ester and I personally observe zero difference in terms of pip/serum levels.

    Unfortunately after injecting for years the inflammation caused is just like food and toxin intake so every injectiin causes a inflammatory response as in calcuim build up in the arteries. When injecting sub into glute with slin pen my calcuim score has come all the way down to single figures. I find prop Ed the best wished I had done years ago. If u feel lumping then I use a muscle pulsator straight after inj to stop this 👍

    Idk how to dose the subcutanous stuff. If I'm doing 500mgs of test e a week in two 250mgs 1ml IM shots. do I just convert that into the same two 1ml shots twice a week subcutaneously?

    What i know is that you can maximum inject 0.3 ml per subq sitting. If you do 2 ml a week (500mg) you can do daily 0.28 to reach same levels. Good luck !

    Wow some of that you must be talking to other docs because some of it's going right over my head. It's like pulling teeth with my doctor to get generic Viagra and a mile of testosterone they give me like a amount every two weeks on both medicines and that's supposed to make me happy.. I wish you were my Doc. I just started doing Im injections on on myself and there's barely enough there to get in the it's just real difficult when seeing guys all over the internet injecting all this testosterone and getting Viagra when they're.. I just don't I'm going to be 65 December 31st and I just feel like I have a spark of life in me now that I'm getting testosterone and Viagra generic..

    I just started the xyosted today my levels were 231 my urologist basically gave me a choice as to which route I wanted to go being a recovering heroin addict I didn't want to be loading up syringes and having flashbacks lol medicade is covering it so y not great job explaining

    Where is the best place to do a subq injection? I ask because I’ve tried it in my belly fat and after 5 weeks my levels had dropped 350 points. Same exact protocol as I was doing intramuscular.

    I had a bad experience injecting in my thigh, it twitched hard. Now I can't frigign inject. Even trying subQ I cant work up the nerve -- so I've just ordered an autoinjector.... Will see how that goes.

    I spoke with a physician's assistantat at a large trt clinic today. He said that, based on the blood work he's seen, subq injections will not spike hematocrit levels as much as IM injections, all else equal. Any truth to this?

    Debatable. The difference isnt big if there is one. Made zero difference for me or most patients. Once you reach steady state the levels are pretty much the same and so the HCT response is as well. Personal results may vary...

    One inch needles are way too long for my thigh. I almost always hit nerves. I switched to 1cc syringes, and it's much better. No lumps under the skin either. Testosterone was 220 in December it is up to 1100 as of April 3rd

    I've been going subq once weekly around my love handle for a while now. I swapped ever since an uncomfortable experience I had while injecting into my quad. I stuck the needle in and the muscle raised up really high and peaked into a mountain shape around the needle. It didn't hurt much worse that normal but it felt very strange and I got a bit nauseated when it happened. I suppose it was the muscle spasming. I've never seen anything like it.

    @@Joe_Friday Yes dude same here. I looked down and my leg muscle looked like it had worms! I had to massage them down. I just ordered 27 1/2" and 29g 5/8" for subQ injections. They appear to be the way to go. IM TRT Looks like its kind of out dated. But also possibly that guys are injection "other stuff too" that has to go IM, which is why they still do it. I've seen so many comments about crazy shit happening from IM injections 2-6 years out. IM seems to be.... bad for you.

    My urologist prescribed me the zyosted 75mg and my insurance wouldn't cover it. I called his office 5 times and made sure he switched me to a generic. The auto injector was nice but for $150 a month it definitely wasn't worth it.

    Ok thanks, it’s ok but when I had my bloods checked they were like a roller coaster and some would get bad pip from injection that was hit and miss to , it’s the most common one in the uk to start you off with and the cheapest

    I've switched to Sustanon from Cypionate 4 months ago cos it's just so much cheaper. I've been doing subq (every other day) and there is zero pip or any difference to serum levels.
