Stages of Chronic Kidney Disease [CKD]

Over 30 million people in the United States have a diagnosis of chronic kidney disease and 33% are at risk for CKD. That's one in three. That means when you are driving down the highway, one out of every three drivers that you pass is at risk for chronic kidney disease. And many of them don't even know it and that's because kidney disease is largely a silent disease. Today, I'm going to talk to you about the stages of CKD and what they actually mean for you. I'm Dr. Frita. I'm a medical doctor who has been triple board certified in nephrology, internal medicine, and pediatrics. I am the founder of Midtown Atlanta Nephrology and a medical director for a nonprofit dialysis unit. Chronic kidney disease, CKD. Now, if you have been diagnosed with this, I'm sure that your doctor has talked to you about what stage you're in. You're in stage III, you're in stage IV, you're in stage V. What does that mean? Well, if you have CKD, that means that you have either kidney damage or a loss of kidney function that has been occurring for three or more months. The kidneys aren't working the way they should, and they haven't been for three or more months.
What exactly are the kidneys supposed to do? What are the kidneys? Where are the kidneys? Well, the kidneys are your two organs that are located in your lower back, in your flanks. They're located where the love handles are. And each kidney is about 9 to 11 centimeters long, and they are literally shaped like kidney beans, two kidney beans. The function of the kidneys is to filter out the blood and to get rid of excess poisons, excess toxins, excess salt, excess water. When the kidneys aren't filtering the way that they should, then you have some kidney damage. And if this kidney damage lasts for three or more months, you have CKD.
So as a nephrologist, what tests do I do to see if you actually have CKD? Well, one thing is that I test your urine and I test to see if you have blood in your urine or if you have protein albumin in your urine. So blood and albumin, this protein, are actually very large molecules. And when your kidneys are filtering properly, they should not allow blood and this protein to filter out and to get into your urine. So if I see that you have protein or blood in your urine that has come from the kidneys or through the kidneys, then I know that there is some damage to the kidneys. So when you hear your doctor say, "Oh, you have some protein in your urine," that could be an early sign that you have kidney damage, even if your lab work is normal.
Another early test to screen for CKD is in your blood work, the creatinine. So what is the creatinine? Creatinine is a waste product from muscle metabolism. And the way that creatinine is excreted from your body, the way you get rid of creatinine from your body is through your urine. So when the creatinine level in your blood is too high, we know that the kidneys are not functioning properly because they're not getting rid of the creatinine the way that they're supposed to.
There are five stages of chronic kidney disease.

CKD Stage 1 Kidney Disease is the stage when the kidneys are at their best function and 5 is when the kidneys are functioning at their lowest capacity and where one may need a kidney transplant or dialysis. Stage 1 of CKD is when the GFR is 90 milliliters per minute or greater.
Stage 2 CKD is when the GFR is between 60 milliliters per minute and 89 milliliters per minute.
The next stage is stage 3 Chronic Kidney Disease [CKD], stage 3 chronic kidney disease. Now, this is actually the most common stage of kidney disease. And in stage 3, patients can actually start to develop some symptoms or some lab abnormalities to signify the effects of chronic kidney disease.
For stage 4 chronic kidney disease, the GFR is between 15 milliliters per minute and 29 milliliters per minute.
Finally, the last stage of CKD is stage 5, and you are in stage 5 CKD when your GFR is less than 15 milliliters per minute.
These are the five stages of chronic kidney disease.

    @@wesleylicorish779 Yes it is possible. I also was at stage 3 with a GFR of 49. Went on a plant based diet and exercised every day. 10 weeks later I have a GFR of 70 and am now in stage 2.

    I scrolled to find an understandable explanation of GFR numbers and Creatinine numbers. You are amazing! I understood every word, as a non-medical person. My family has a long history of kidney failure, diabetes, high blood pressure, heart failure and a host of of other rare and ultra rare diseases such as, rare bone disease, pancreatitis and Ahus. Trust me that is not all. I am the youngest of 7 siblings at 62. I have fought to be proactive, with my health. I have HBP, and arthritis. I have been trying to get someone to help me do medically preventive care. Along with my own research. You have helped me so much THANK YOU!

    Why do doctors wait until things are so bad before doing anything so often??? More money? It seems anytime I try to find ways of PREVENTING things they arent helpful. Who wants to go to dialysis 3 times a week for the rest of their lives??

    I have pulp in my kidney GFR has been at 60 , 2021 I had 600 protein in my urine. My Endocronologist put me on Piglitazone… NEPHROLOGIST WAS NEVER MENTIONED.. the med did help. I was exhausted all of the time, though doing my job as house cleaner, burning headaches both got better last lab test 7 months ago went down significantly on the protein. He still diagnosed me as a Type 2 diabetic though my A1C was 5.7

    I HEAR you!!!! I had CKD for 12 years before l was diagnosed with it… my PC knew it and didn’t bother to tell me! It wasn’t until l needed surgery to have a breast lift that my WONDERFUL Cosmetic Surgery did all the tests before surgery and told me l needed to see a Kidney Doctor Before he would proceed with my surgery Saw a kidney Doctor a female who was so kind and wondering why l hadn’t been told about my Stage 3 kidney disease I told her what the labs had said from 12 yrs back and she was in shock and l asked why a Doctor wouldn’t have told me about it and she shook her head and said that most doctors don’t do anything until you need dialysis!!!!!!!!! WTF it’s all about the money 💰 I am so thankful l know and am able to watch my diet and l only drink water 2 liters everyday and my kidneys have stay around 45 to 50 % l am 67 yrs old and want to live as long as l can but l do tell everyone l know or talk with to ask for their Kidney levels to be checked so they can make good decisions about their health 🙏 Proactive not reactive

I’ve just been informed that I have been coded as having CKD for the last 13 years with no discussion of stages or what I can do to improve my kidney function. I’ve found lots of information online about stages but yours is the first to explain why a “normal” GFR is called Stage 1. It has baffled me but I now understand!     Thank you Dr. Frita. This helps so much to understand CKD. My mom of 76yrs. has hypothyroidism, Cholesterol, and last been diagnosed with BP just this year and among other things. She has taken lots of medication. Ibuprofen, Naproxen, Antibiotics, and is taking 325mg of Aspirin daily. She has been on Hydroxyurea for over 2 years now due to the Essential Thrombocythemia. She has been suffering from UTI'S for the last 2 years. Lots of antibiotics!!! They recently told her she had protein in her urine but not to worry about it!? With what I just heard I believe she is ready for a visit to the Nephrologist!!! My mom is a ticking bomb! 😬

    My Gfr has been 56 for the last 4 yrs however my doctor tells me it is nothing to worry about....Im 67yrs and have had high blood since age 50...I am not over weight, no diabetes...active...Im glad I found your channel....Wish I lived in your area, id visit you however I live in Iowa....P.S. my level was 54 on my last blood work in 2019....white female..

    Good morning, is there a way I can get some advice from you. I have had many of the issues you spoke about since 2017. And I was always given meds for uti. I've been in and out hospital here in GA. Now my symptoms are worse alone with new symptoms. I was in so much pain when I came across your videos.     Thank you for letting me understand m condition CKD. For several years I had been told by I have high creatinine but the doctors nobody told I had CKD and neither did they explain GFR that was on my regular quarterly blood work.

    Hi Dr Frieda. I am so grateful for ur Videos. I am stage (3b). I found out I had this disease when I was(74) years Old. I am now (81) and started off with GFR 59 now it is 44. I have a Nephrologist My concern now is that I am experiencing a rash & lots of itching. I am very concerned & scared At the same time.

    I also have PKD and I’ve started getting a rash on my wrist and two of my fingers and it itches and I’m wondering if it’s connected to the PKD.

    Today is my first time. I saw this video. Im really concerned. My mother passed away from kidney and heart failure. She was on dialysis. As a child she had rheumatic fever. My mother aortic valve replacement done. She passed at the age 61. Twin sisters had heart failure as well. I have been having pain over a year on my left side. I was in the hospital a few time cause of UTI infections. I didn't have. Insurance last year. I qualified for medicade and Medicare. I have primary Dr. Which I just changed. Prior pendamic. If was sick. I couldn't get an appointment. Her assistant he would tell me to go to urgent care I have done that. Button line while I was in hospital. I was told I had protein in my urine. I told the Dr this. I never had any test. I just want to feel better. Im 52 Pre menapause. I have gain 30lbs in one month. My body is retaining fluid. I feel it. My left leg as soon I start to move around it blows up. I don't have the energy like a year ago. I was wondering if I should go to the hospital. The pain in the last 3 months is getting so bad. I can't stand to wash dishes. I walk around corner Im out of breath. I have given birth 2 children both 9.9lb babies so I had my baddler lift. Best operation I had done. I was able to exercise run 5miles a Day kickbox rollerblade. After my bunion surgergy. That all went down hill. Im in the process of waiting for a new insurance card. With new Dr name on it. Unfortunately I get and there was mistake. Soon the new card should be here with the next 3days. Im really nervous cause this has been going for over a year. The pain and swelling getting worse. Im glad that I seen this video. I will ask if Im able to get these test done. Oh even my left ovary area is hurting as well. I can't sleep on left side. I do appreciate that as a Dr you were able to make a video. Just explain certain things people who are not a Dr or just have a fear of going to a Dr. This video was encouraging. Thank you. 😄

    i have a question if Dr. Frita can reply, is it possible to improve egfr as i have been told in jan 2021 that i have stage three ckd with egfr 30 and creatinine 2.3 i am 33 but now it has improved by taking hypertensive pill to 52 with creatinine 1.5 and then goes to 48 with creatinine 1.6. is there any chance to jump into satge 2?

    Dr Frita, may I pls ask and give some info on my health? Had kidney damage 2 years ago after an accident. One count the specialist talked about was 900 and dialisis was mentioned. After a couple of weeks in hospital count came down. I didn't get dialisis, it improved. After 6 months gfr was 70. For 2 years I felt healthy. 4 weeks ago I started getting nauseous and losing my appetite. I started itching on my arms, and face. Mild edema on my foot. 4th degree burn wound on my foot sole after accident that covered my whole foot sole. 2 years later the heel is still raw wound messuring about 2cmx 4cm. Blood pressure this week, 150/80. Blood in urine now and kidney infection. Feeling tired. Could it be the wound that causes progression to stage 3?

    I had this test but my Dr seem to miss that i have kidney problems with recurring infection my epithelial cells was 89, red blood cells 100 x 10 e6/l and white blood cells 100., i have constant urinating and dehidration.

    My husband is a heart transplant recipient. He will be ten years post Tx in Dec 2023. The immune suppression drugs he takes to prevent organ rejection cause kidney toxicity. His creatinine is now 2.48 and GFR 28. This was a sudden decline. I thought the GFR was stage 4 KD but his nephrologist has indicated stage 3 KD. When would it be advisable to seek an opinion Re: evaluation/kidney transplant listing? His nephrologist says there’s a lot can be done before dialysis or Tx listing. Since the waits are lengthy for kidney’s, is it wise to begin now?

    Dr. Frita I've had many of the symptoms of kidney disease. I will be getting checked out soon as per your advice. Quite concerned after hearing all the symptoms. Just wanted to thank you very much for all you do. Not many Doctors are willing to take the time to give sound medical advice to counter misinformation. My symptoms could be anything really, so I'm hoping for the best, I'm a pretty healthy, fit young man, but I don't drink enough water and I've got some bad habits I carried through the years. I will share this with my friends to support your channel, because I believe in it and I want them to become aware as well! Take care! 😃

    My right kidney is shrunk up. I am going to be 69 in February but didn’t find out about my kidney until I was 64. Dr said I was stage 3-4 kidney disease. So what does this mean?
