I'm not picking every note Shaw, the pattern I`m using throughout is: up, up, down and then pull-off.

And about your second question: I don't know how hard it looks to you but I don't have big hands so for me keeping the stretches on the left hand for so long was a challenge; and tryng to keep a constant 16th note rhythm with economy picking and legato has always been another challenge.
