Retrograde alopecia & hair transplant | Have I LOST the Hair Loss Battle? | Darling Buds Clinic

Retrograde alopecia (reverse balding) has no mention in the traditional Norwood balding scale for androgenetic alopecia.
Retrograde alopecia was never a concern for FUT since donor was confined to the central zone. But with FUE coming into vogue and a larger area being chosen as the donor, concerns about entities like retrograde alopecia that restrict the safe permanent zone became a matter of avid discussion while discussing the harvesting zone.
So, what is Retrograde Alopecia? Well, this is miniaturisation of hair that starts from the nape of the neck or the nuchal region and around the ears, and travels upwards, is progressive and continues to gradually efface the permanent donor zone.
However nuchal alopecia in those with Sikh pattern traction alopecia does not fall in this category.
This nuchal zone has to be assessed for retrograde alopecia while considering a patient for FUE hair transplant.
So is it that if you have retrograde alopecia, you have lost the battle? Only in severe forms of retrograde alopecia this statement is true. The hair in the nape of the neck are usually not harvested in FUE but they are of great utility to render naturalness to the hairline and for creating softness in restored temple points. They are also used for eyebrow restoration. With retrograde alopecia this can be a hindrance. And this is the reason why in males we would rather prefer to isolate singles under the magnification for the hairline and the temple points rather than rely on nape of neck hair. We relied heavily on nape of neck hair for many years for softening the hairline but have over a period of time realised that even in those not suffering from visible retrograde alopecia, these hairs fall off with time since everyone has some degree of subclinical retrograde alopecia that occurs with time as one ages.Retrograde alopecia may be the first indicator that a person is heading towards extensive baldness. This has to be kept in mind while planning a hair transplant in a young person who also has positive family history of extensive baldness.
Can we correct retrograde alopecia? Yes you can with Minoxidil and Finasteride but corrected reverse balding is drug dependant and when we stop medication it will soon return. Hair transplant in retrograde alopecia areas is not a modality to be considered since with shortage of grafts bald zones take priority.
So retrograde alopecia has clinical significance since:
1. It constricts the balding zone
2. Rules out softening of hairline with nape of neck hairs
3. Is an indicator of shape of things to come- a sign of extensive baldness with meagre donor supply
4. Severe retrograde alopecia may preclude a hair transplant procedure

    What will be the aim. Moreover, you would be wasting precious grafts which cannot be harvested again if you run out of grafts later in life. Moreover, you will always be chasing the margin of retrograde alopecia since it will continue to advance.If it is a concern, wear you hair at the back of the head short, or get SMP done.

    Hello, I'm 27 and I believe I have retrograde alopecia for the past 5 years but the thing is the top of my head is completely fine. I would be classified as a Norwood 0 or 1 (whatever is the lowest) and have a thick crown. During these past few years the only hair that has gotten this is on the sides exactly where retrograde alopecia effects. Is it possible that male pattern baldness won't effect the top of my head or am I destined to go bald? Thanks

    I had a small thinning above my ears and lower nape.. I asked this with my doc he said it is not retrograde alopecia.. Most patients have a small thinning above ears and nape coz they have mpb hairs on sides and nape prone to hair thinnng coz of mpb.

    Yes he is right. Ad that is the reason nape of neck is not preferred for softening of hairlines as was practised a decade before. nape of neck hair vanish over time.

    I have retrograde alopecia.. i am nw2.. i have been taking finasteride which has stopped the progression of balding.. my donor area is thinned due to retrograde alopecia but non donor area is still thick.. is it possible to take hair from on top of scalp and transplant it to the hairline?

    No that will be most improper. The top will soon begin to thin too. Hair transplant is most likely not for you. If you wish to retain hair please consult doctor and he may most likely sk you to be on Finasteride in the long term after evaluation.

    Retrograde alopecia if treated with a DHT blocker will always need the medication in future if grafts have been transplanted from this area to balding zones on the head.

    These hairs are thin due to the onslaught of DHT as a component of androgenetic alopecia. Only a DHT blocker will help

    Sir. I have a question. I got a Hair Trans. a year ago, i didnt get a good result. 5 mounts ago i started to thin in my donor area and i know ist that i started to thin in the top of my ears i guss is retro grade alopecia. This happened becouse of the HT? That started the progress?? Am taking finasteride 1mg a day for 8 mounths and minoxidil for a year twice a day. But i can stable my alopecia. What can i do? Take dutas.. ?? Or while another 4 mounts on finasteride to see results?

    How old are you? If you are above 25, it is likely that your scalp donor was involved in miniaturisation. You may not have been the right candiate for a hair transplant.Presently you should in consultation with your doctor seriously consider being on a combination of DHT blockerswith dose pertinent to your level and speed of thinning.     Am on finasteride por 8 Mounths now, 1mg a day but i haven't seen results and the hair loss is stil progressing. Your video is very interesting. Should i wait another 4 months to see any results. Or combined dutasteride.

    It seems you have a strong family genetic history of baldness. You can seek consultation locally and see if you can start Finasteride. Otherwise even if you are heading towards extensive baldness, SMP is quiet fashionable these days. You do not have to worry.

    Stress is one of the factors that increases balding bt the underlying most important factor is genetics. Hair cloning once it comes will be a boon for those with meager donor supply.

    I smoke marijuana in raw form is that going to trigger my alopecia more r increase DHT.. ( I'm 31 and my present condition is I have 20 per hair left on my scalp and the rest part is doing great)

    No, there is no evidence that marijuana will aggravate baldness. I think it would on the contrary decrease stress.

    1. Thank you for this video! - I have been looking for information on this subject. 2. Question - I always had VERY thick, coarse, black hair growing up until my early 20's. My hair started to thin a little then, but I had so much hair, it was barely noticeable. In my mid 20's I started to have a few hairs on my sides around my sideburns/ears start to turn white. They did not turn grey at all, just PURE WHITE. They were still very thick/coarse. That progressed pretty quickly from 5-10 hairs on each side to dozens, to too many to count by my late 20s. Being in my late 20's with that much white hair was annoying especially since all the rest of my hair was naturally jet black, so I began dying my hair, mostly just on the sides and back since that was the only areas with white hair. That seemed like a good solution and I continued to do that for the next 10+ years (I am 43 years old now). Around age 38 my hairloss was progressing significantly on top/crown, my scalp was becoming visible. My front hairline was receding, but I still had great coverage in front, sides, and back. I was worried about starting propecia, so I put that off. I tried minoxidil, but that did not seem to help, and over time, I gave up on it. Around age 40 the hair loss on top/crown got very bad, very fast, to the point of being almost completely bald there, however the front, sides, and back were still pretty dense. It looked very odd, I didn't have a "bald spot" on back, it was nearly the entire top of my head and crown being almost completely bald. That was when I decided to finally try Propecia. Family Info: My father and his 5 brothers are all 70-80% bald. My father actually has the most hair out of all of them, but is still very thin on top and completely bald at crown. My mother's father died at 80 with a completely FULL, thick head of hair. Age 41 - I started Propecia and using Minoxidil again. I did not have high expectations, but my hope was to prevent it from getting worse and I was hoping for some improvement. GOOD NEWS - within 6 months there was DRAMATIC improvement. Far better than anything I expected. I was no where near back to a full head of hair, but I went from 95% bald on top/crown to seeing hair growth/density basically start to move from the front, sides, and lower back up/in toward the top. The top was still pretty bare, but instead of having a bare area that was about 5 inches wide and 7 inches front to back, that space is now about 3 inches wide and 4 inches front to back and the area on top continues to grow hair little by little. It has been almost 2 years now (about 22 months) and the hair on top continues to make VERY slow improvement. The first 6 months was the most dramatic improvement, since then it has been very small improvement, HOWEVER... here is the new problem. Around the time I started Propecia and got back on Minoxidil, the hair on the lower sides and nape of my neck were not only almost all white, but they were getting VERY thin. Both thin in density and thin hairs. That began before Propecia/Minoxidil, but progressed rapidly since. I was worried that dying that hair might have been causing damage and the hair to stop growing. I kept that hair VERY short, so the dye was in direct contact with my skin. I don't know if the dye did that, or if Propecia/Minoxidil have contributed. It is VERY confusing, while the hair on the top/crown of my head has improved over the last 2 years, the hair on the lower sides and nape of my neck are almost bald now. I grow almost no hair there now and have have resorted to just shaving those areas and doing a fade up into the higher sides/higher back area, but I honestly hate that look. So... My Question (finally) Is... - Does this sound like retrograde alopecia? - Or could it be damage from hair dyes? - If it could be damage from hair dyes, is that reversible if I stop dying all together? - Could it be from Propecia/Minoxidil? - Could it be be from weight gain? I have gained about 30-40 pounds and I notice it a LOT in my face/neck. - Could it be a sign of some other issue? - Thyroid? - A nutritional deficiency? There is SOOOO much information online about "normal" male pattern baldness, but I have had a hard time finding any info on this subject. I appreciate your video and appreciate any thoughts/info you may have. Thank you.

    Thanks for the comment after watching my video. In reply- Yes this does sound like reverse balding that is effacing the scalp permanent zone If you send me pictures I will let you know for sure. Hair dye does not cause this sort of thinning. Nor does Finasteride or Minoxidil cause thinning of hair in any body area. Weight gain does not cause hair loss. Sudden weight loss does. The thinning is due to androgenetic alopecia, in all likelihood. But to be certain you could get these lab tests done- thyroid profile, hemoglobin and serum ferritin. Take good care!

Since seeing your video I have been doing more research on this topic. Are there any treatments for this? Topical? Oral? Micro-needling? Or is this hair just done?

    Hello sir , i had an autoimmune disorder duodenal ulcer 2 years back then i got scalp psoriasis a year back and last year i treated that autoimmune disorder but with scalp psoriasis i had tellogen effluvium as well i lost a lot of hair in no time. So after treating duodenal ulcer last year just after 5 months i got my thick hair back again with great density but because i was having scalp infection at the same time i went to a dermatologist he said its sebborheic dermatitis now as i was having lot of flakes Suddenly i lost my hairline probably hair all over the scalp got thin i m very disappointed and stressed right now because this all happened at the very young age i m just 21 years old Am i suffering from dupa 😢 ?     I had thinning above my ears and from the bottom of my kneck when I was a teenage, but then I didn't know what was happening to me because I was a kid. Now I am 32 years old with a diffuse thinning and a norwood 2 or 3 in the temples. Also some crown. Is too late for me because so much time has happened?

    Sir my father is not bald but my mama both ar bald age 47/45 but they have no retrograde alopicia so sir have i any strong family history and i have any changd to retrograde alopicia plzzzzzzzzzzzzz rrplyyyy

    My father have no baldness but my two mama they have baldness trier age Between 47/45years so i have any change to be bald on future and sir iff i use finestaride then my retrograde alopicia will stop totally or hair fallen down slowly i want totally stop hair loss its possible

    @@hairtransplantinindia he was trying to tell that his dad is not at all balding and both uncles from mother side went by age of 45 and 47 ,,,he suspect that he have retrograde alopecia ,,,he was asking whether finasteride will work for retrograde alopecia or not ,and regrow hair where he lost ,,And atleast he want to stop hairloss ,then save what he have...seems like he was mentally stressed i guess , feeling sorry for this man..

    The best age is when your baldness is stable, you yourself feel the need for a hairline and you still have grafts left in the permanent donor zone. This may vary greatly.

    @@hairtransplantinindia I'm norwood 2.5 now, I have not lost most of my hair. So I dunno, but most of my family members have norwood 1-2 at 40-60 years old of age. I'm 22 😭. Only few males on my dad and mom sides are balding, most of them are not balding at all but I'm ill fated.
