Replacing Silver Fillings with Biomimetic Dentistry | When, Why, and How to replace Amalgam

Deciding when and how to replace silver fillings is an important decision. This video will review the problems of silvering fillings and the different replacement options including Biomimetic Dentistry.
HISTORY & BACKGROUND
Silver fillings have been used in dentistry since 1826. They consist of mercury, silver, copper, tin, and other trace metals. Together these metals mix to form an alloy which is commonly known as a silver filling. The use of mercury in silver fillings has resulted in widespread safety concerns and a history full of controversy.

9:40 FREQUENTLY ASKED QUESTIONS
10:30 HOW MUCH DOES IT COST TO REPLACE SILVER FILLINGS?
THE PROBLEM WITH SILVER FILLINGS
In addition to health and safety concerns from mercury, silver fillings fail to adequately restore the natural properties and strength of teeth. As a result, teeth with silver fillings face damaging long-term complications such as fractures, cracks, and root canals. Many of these old fillings are treated with crowns which can be avoided by a more natural, biomimetic treatment approach.
SILVER FILLINGS RELEASE MERCURY
The fact is that mercury vapor is released from silver fillings, and scientists agree that mercury absorbed in high enough doses will cause health problems. There is strong evidence supporting the use of alternative mercury-free treatment options that can perform better and restore teeth more naturally. These alternative options include white fillings and ceramic restorations which are similar to natural tooth structure and eliminate unnecessary exposure to mercury.
STRUCTURAL INTEGRITY OF TEETH RESTORED WITH SILVER FILLINGS
Silver fillings are extremely stiff and they do not reconnect the tooth back together because there is no adhesion between the silver filling and your tooth. As a result, teeth restored with silver fillings are much weaker than intact teeth and experience more complications. Also, the placement of silver fillings requires shaving and removal of healthy portions of teeth as part of the technique and material requirements, leading to further weakening of the tooth. Teeth restored with silver fillings are likely to develop cracks and experience other complications including sensitivity, pain, gaps around the edges, cavities, root canals, fractures, and a higher chance of extraction.
SILVER TOOTH REPLACEMENT OPTIONS
- White filling (also known as composite filling)
- Ceramic inlay or Ceramic onlay
- Crown (usually the last resort)
CHANGING SILVER FILLINGS TO WHITE FILLINGS
When the existing silver filling is small, and there is a large amount of healthy remaining tooth structure (no cavity, crack, fracture), then a white filling will be the best option. The white filling is made of a composite material (also known as a composite filling) which comes in a variety of options and brands with different properties.
DO NOT REPLACE SILVER FILLINGS WITH CROWNS
White fillings can not adequately restore teeth that are more structurally compromised, but a crown is almost completely avoidable when replacing silver fillings. A crown involves removing (shaving or drilling) substantial amounts of healthy tooth structure which increases the chance of complications and increases the risk of root canals further. Ceramic Inlays and Onlays conserve significantly more tooth structure than crowns and restore the structure, function, and strength of the teeth when provided with a biomimetic approach. This combination minimizes damaging complications and maximizes long-term success compared to crowns.
How much does it cost to replace silver fillings for white?
The cost for white fillings varies greatly because there is no “standard” for the quality of the white filling. A filling is not a product, but rather a service, and the cost is largely dependent on the quality of the restoration and the expertise of the providing dentist. A small white filling can be performed very quickly with low quality and cost as low as $50. On the other hand, a large white filling can cost as high as $1500 when it is provided with optimal quality and this type of filling takes significantly longer (1-2 hours) to make.

So happy to have come across your video. I just got an amalgam on my two back top teeth. I felt little bits go down my neck as he was filling it because he didn’t use a dam, and on top of that, he did not inform me that he was using silver at all! I was surprised/upset to see it in the mirror after I got home. Anyway, I now have an appointment with a more trustworthy dentist to see if he will replace them with white ones. I know it is not recommended to replace with composites before you need to, but I’m worried about the possible expansion of the silver with hot and cold causing my tooth to crack. I’m very upset because I feel like there is no right answer here. Anyway, thanks for your informative video! Glad to see there are dentists who genuinely care to inform. :)

I have had 20 years of chronic and systemic pain, progressive adrenal fatigue, and disabling brain fog. I encountered an ad on KZread yesterday describing these symptoms as potentially being attributable to mercury amalgam fillings. I had two fillings many years ago and I’ve been struggling with trying to figure out what was causing my pain and fatigue. It seems to me that mercury is a most obvious culprit and I appreciate your video because I am researching what it’s going to take to replace these fillings. Thank you, Doctor!

@@BiomimeticDentistry I have a question: I have an inlay done in Aug 2019. However, the tooth now smells every time I use Waterpik to floss the area surrounding the tooth. Can I remove the inlay and replace it with a normal filling? I never had this issue with a normal filling. I know the cavity is deep but it hasn't reached the nerve yet. Even if it reaches the nerve, I will have a root canal treatment with a filling (if it is possible to have one instead of an inlay).

I’m interested in getting my one and only silver filling removed and replaced with a white filling. I have a really severe gag reflex and the dentist who placed the silver filling in my tooth said it was my only option. I wasn’t happy with him after that and I switched to a dentist who offered iv sedation to fill the other teeth that had cavities with white filling. I had the silver filling since 2009. The tooth with the silver filling is starting to feel a little sensitive l, but not when I chew or bite down. I think it’s starting to chip or crack. Thank you for the video. It helped me to decide to ask the dentist I go to now if he would be willing to remove and replace the silver filling. I go to a dentist who offers iv sedation and his office is Mercury free. I just couldn’t get used to the silver filling. But know more about silver fillings and the other filling options, it wouldn’t hurt to ask my dentist his opinion and if the silver filling can be removed and replaced successfully on that molar.

I had a gap between the central and lateral incisor teeth both. Which made food always get stuck between them everytime I eat. The doctor filled that gap but he made a gap between the lateral incisor and canine eye tooth. And now I'm having the same problem all over again but from the other side when I actually didn't have that problem there. There was no gap. Is that normal or is that not okay? And what would you suggest me to do?

ive had like 10 amalgam fillings for the past 15 years and ive been having metallic taste in my mouth for over a year and many many many other neurological symptoms and gastrointestinal problems and no doctor says that it could be possible that the amalgam fillings could be at fault (maybe they dont know how the amalgam can decay or what substances lead to mobilization? and also methylmercury?) do you think i should get the fillings removed asap? and how much money do you think i should save? insurance only covers removal if someone is allergic to the amalgam 1 month ago, one filling got loose and fell out even im really mad that this was done to me as a child, and ive always been very sensitive to environmental toxins since last year, i was even diagnosed with autism spectrum disorder, and i never had those symptoms in that severity (i had symptoms before i had those fillings but its only gotten worse ever since)

Hi Alex, sorry to hear about this. Regarding the cost- It depends on the size of the fillings. Some may need inlay or onlay restorations, some might need crowns, and some could do with fillings of different sizes. If the teeth are cracked, its more likely that an inlay or onlay restoration would be recommended. Anyhow, I can't really estimate the cost because it depends on the treatment plan. Regarding my recommendation- I usually evaluate the restoration in reference to the actual current condition + the patient's desire. It is impossible to predict if removal with result in resolution of your symptoms, but if the restorations are already problematic or defective then there is nothing to lose. If the restorations are perfectly intact, then I would say it is up to you because there is no definitive way to determine if this will help without trying, and thats a risk you would need to take if you determine that you have exhausted other options.

I'm not sure if you still look at the comments for this video, but I just want to thank you for this video. After years of neglecting my dental health I ended up having to get all of my back teeth filled with amalgams last year. But despite me taking this step in restoring my dental health, my mouth honestly feels worse than it did before the restoration. My bite constantly feels off and my teeth still feel sensitive despite this taking place almost a year ago. And now finding out that my fillings could potentially be poisoning my body too makes me feel even worse. I definitely do want to get my amalgam fillings replaced with a composite, but I feel like the damage has been done already and trying to get them replaced would be too costly. I am honestly not sure what the next step should be for me.

If I was you, I'd use this time to save up on more money to be able to replace them. What's done is done and having a tooth with metal on it is better than being toothless. If you're not having any medical problems relating to what silver fillings could cause then I wouldn't rush on getting them removed as they're guaranteed to last around a decade

  • @BiomimeticDentistry

The damage is not done, it continues with cracks that develop. See the new tooth cycle video I posted. If your bite is off and you are uncomfortable, you should replace them. However, please do your research. Replacing them does not mean it will get better for sure and the quality of the service is the most important aspect. A white filling can be terrible or excellent, but its the provider that makes that difference not the filling alone.

Better than non-bonded amalgam but nothing like tooth substance. Composite is similar to dentin. Metal has a high modulus of elasticity that is incompatible with how an intact tooth is designed. Well remember that the most important thing is the quality of the restoration. A white filling can be more prone to problems when it is done poorly, or it can be better than a silver filling when it is done well. I would rather have the best restoration possible, but that is not to say that any dentist will produce the same results. Some may provide really poor results with composite fillings so you want to ask them what they recommend. I want people to know that it is possible to get the absolute best results with composite (white) fillings when everything is done optimally, but that doesn't mean it will always be the best option, especially if it is done quickly or poorly without the proper steps to make it succeed.

I read a lot of questions on Quora about dentistry, and it turns out many old dentists in America (say graduated in the 1970s) still believe composite restorations cannot and should not be performed on posterior teeth, and perhaps some of them rarely done composite fillings at all. They say composite fillings on posterior teeth (uniformly) last no longer than 5 years. Do you think this is still the case with today's advancement of dental material?

No, most recent articles show composite outperforms amalgam restorations even when placed in average conditions (Opdam, et.al). In prime conditions, far superior but people love to cling onto the past and act like nothing can improve over 50 years...

My advice is not to change a restoration unless there are signs of problems or defects. Some patients want them removed due to fear or concern of the metal exposure, but that is not substantiated and I personally believe some patients are more sensitive to it than others. It's a personal choice. Do you eat organic everything? I don't think pesticides are good for you, but people can make their own choices.

With no regard to physical properties? In other words, is the priority everything you listed regardless of how well it performs from a strength, durability, and structural integrity standpoint? The topic is very complicated and the best filling material is your own tooth structure and minimizing the need for replacement, but some materials require more tooth removal in order to restore with them.

Hard question to answer. Usually the best option is based on the actual clinical circumstances, so sometimes a filling is best and other times an inlay or onlay. Whichever is best is expected to last longest. 3D printing will revolutionize treatment options with 3D printed fillings that are stronger than fillings and cheaper than inlays! its currently very tricky to to them but its going to get better!

I understand the Mercury controversy but when Mercury is bound with other metals you don't get Mercury leakage, it changes it to a completely different type of metal. One thing I have noticed is dentists bulk fill with composite instead of doing a layering fill. Most of them cure from the top which causes the material to shrink and pull away from the walls causing more micro leakage. So our biomedical fillings just a fancy way of saying you should always have proper technique? Because bulk filling is very poor technique.

What about govt transparency? NIH did a grant study on dental white fillings. Guess what study was referring to? Increasing estrogen in body from these white fillings. Shouldn't MALES have a choice before these dental white fillings are used? This study determined whether placement of composite (white) dental restorations (fillings) & increasing the concentrations of bisphenol A (BPA) had estrogenic-like properties that raised concerns about safety abiut . amounts of the chemicals leached out of dental sealants and detected in saliva soon after the sealants were applied. Dental composites also contained some of these compounds? Why wasn't the public made aware of this and shouldn't it had been transparent?

  • @BiomimeticDentistry

    @BiomimeticDentistry

    3 жыл бұрын

Hi Jess. Doesn't everyone have a choice? As far as I am aware, providers provide informed consent. There is no perfect material in existence, but you are referring to research that began with dental sealants which are the most likely to break down and leech into saliva. Some fillings contain Bis-GMA but there are also alternatives. There is no solid evidence that Bis-GMA composites do produce significant or detectable amounts of BPA with most studies concluding little or no detectable level. Regardless there are alternatives, but be aware that the use of Bis-GMA is not for simplicity or ease or conspiracy. It's the most mechanically beneficial component of composite fillings that gives good physical properties. I always try to explain that the best thing you can do is avoid needing any dentistry, but once you need it, the next best is to do treatments that are likely to last and minimize additional complications. What material you use and how much it can leech is very significantly associated with how well it is done, and how well it is sealed and adhered to the tooth, so it's hard to make any generalizations.

Smaller fillings do better than larger ones because having more tooth remaining minimizes the problems and preserves more of the strength. Best- MN

Interesting concept. I don’t think thats the case but that is not to be confused with me thinking its a great restoration. If you want to keep it really simple, remember that we had no other options in the early 1900s. Ceramic was failing rapidly because of adhesion, gold was too expensive most people (time consuming) and tooth colored fillings and crowns came later. First tooth colored fillings were terrible due to adhesion issues. So it really helps to remember it was the best option we had for the time but now we have much better options though it is still more expensive and time consuming.

Hi Steve- This is a long standing debate, but the science indicates that if there is no structural defect or failure of the filling, then the filling is in service and the biological aspects are not solid. Logically speaking, I would rather not have any in my mouth, but I wouldn't confidently claim that removing them is necessary when the science does not indicate that is a fact. I think some things are best left to patients to decide, and it's hard for anyone to make all the decisions inline with your own beliefs and values. Structurally speaking, a silver filling is also not optimal but we don't replace every filling unless there is clear signs of failure, and its a bad policy to systematically over-treat because this can lead to problems as well. 2 жыл бұрын

Well, disagreement is not an issue for me. With low quality dentistry, silver fillings are easier and will last longer. You say no problem but, this is really narrows down to pain. Over 30 years, amalgam treated teeth have an extremely high chance of cracks, symptoms, and fracture. When this occurs, the failure is considered catastrophic, as a result of many years of damage leading to a bad failure. Now, if you are saying your while fillings fell out after 6 months, all you are really saying is your dentist in India did an absolutely lousy job, and I do believe that. This entire video discusses exactly this problem, that a filling is not a product, it is a service. You can have excellent results, or terrible results depending on your dentist. Its also very likely that some dentists will not be as good at white fillings or will not put in the time. However, that doesn't change that composite fillings and bonded restorations are superior. Even in studies, composite fillings now have a greater long term survival (Opdam, et. al). Regardless, I wish you the best and you are welcome to disagree. I too disagree with you =) Best- MN

Come on now. Eating tuna or sea fish is completely up to you. Some people don't have tuna for this reason but its up to you, if you want to limit or remove it, I am sure you can. If you watched any of this video, I talk about replacing silver fillings when there is a problem or defect, not systematically replacing them just to replace them. As a matter of correction to your statement, I have not prescribed you remove or replace ANY of your silver fillings unless there is such a problem or defect, so you are literally speaking nonsense about depleting your savings. Get your facts straight. This makes it clear you didn't watch anything but felt compelled to make a commotion. Some patients do want al their silver fillings removed. That is their choice and I don't think that's a bad decision if the restorations are high quality. Unfortunately, there are times when silver fillings are replaced with really crappy composite fillings so the problems get worse (cavities, sensitivity, etc). The quality of the treatment process and the skill of the provider are without a doubt key components. It is not standardized and there is a wide range outcomes depending on how it is done.

Sure, you can do a composite filling that is absolutely lousy and guaranteed to fail, but you can also do the opposite if you cared and tried. We don't need people making pointless claims like this. Study, practice, do better.

Calling it a "silver" filling is so misleading. I had 13 of these fillings put in my teeth when I was 13 years old. The level of depression I experienced during my teenage years was so severe, I can't even begin to make you understand how poor the quality of my life became. Now, I am 43. I have so many "minor" ailments, it's ridiculous. People think I imagine them. Skin problems, attention problems, back pain, neck pain, gastrointestinal problems, kidney stones, problems in my toenails and fingernails, malformations in certain parts of my body, difficulty creating and retaining muscle mass, tinnitus. My parents are deceased (they both died young), and I have no medical or dental records from my youth. But the installation of these fillings was done in Berwyn, Il.

Sorry, that's what people call it. Silver is referring to the color, not the metal. The correct name is dental Amalgam, but google searches show that most patients are searching for silver filling. I am sorry for all these problems you are experiencing.

@@BiomimeticDentistry For me, it's a part of life. Through diet and supplements, I actually feel better today than I did 10 years ago. I didn't mean to imply that you are misleading people. I just think the name "silver amalgam" is misleading. If they called it "mercury amalgam," however, I feel somehow that would not have worked out from a marketing standpoint. Keep up the good work on your videos!

Angela - thats not true and I am sorry you feel that way. From my perspective, it is frustrating to hear you think money is the driving force. Im sure it makes sense that you will get a better product if someone spends the time to do it to the best level possible. Dentistry that lasts longer than the minimum standards is not magic, its just time and effort. It usually takes 2-4 times longer than doing it quickly. Just think about how that impacts the cost, including staff, rent, assistants, time in the day, etc. Not to mention the lab costs, equipment, etc. The real problem is that many patients think that that all dentistry needs to be the highest level, but don't value the cost involved, therefore most practices and insurance models are focused on providing the minimum quality. If you spend more time treating one patient at a time vs running an assembly line, it is going to be more expensive. It also costs me more. I only see 1-2 patients a day, but my fixed costs are the same or more than other practices that run at high volume. The cost is just the result, but it is not the reason. The most successful dentists in the world do cheaper dentistry with high volume practices. The also own multiple practices. With this approach, Its no wonder that most dentistry only has a life expectancy of 2-5 years under insurance reimbursements. Most patients come to realize that this is not the best treatment available, but you have to understand that better quality will require more cost because of the time, energy, effort, and expense involved in providing the better treatment. Extra cost is the result, not the reason.

Who came up with what? Silver fillings? When they were created, there was no other option. They were revolutionary, but we are way past that now.

I know someone who had 4 white fillings done at the same time and he had to change them to the silver because he said they caused him so much sensitivity that it was painful. Has this happened to anyone? I just had a white filling done myself a couple of months ago and I have a lot of sensitivity from very hot or cold drink/food so I've been thinking of going back to the dentist to get it changed back to the silver. But now I'm learning that they are somewhat toxic. I get a lot of migraines the older I get and I have had two silver ones done about 15 years ago. I don't know what to do I'm confused. (all the fillings are small btw) I hope the Dr. or anyone out there can give me some advice. Thank u!

This is what Im saying though. Absolute matter of fact is that the problem is the quality and the provider. Not the filling. Its hard to do a good composite (white) filling and it takes more time, but it also has tremendous advantages over silver fillings. The problem you describe could definitely be avoided, but most business models don’t prioritize the quality and time needed to do it well. The most important thing to do is have excellent quality dentistry, my advice is to find someone (like myself). Any other advice regarding what procedure to get is missing the big picture. I hope this helps and its what I was describing in the video.
