    Awesome video doc! Do you know if GI or RMGIs are still best practice for lining direct restorations? I have seen images showing how the GIs form a super hard/well bonded hybrid layer with dentin and am wondering if you know how that bond compares to immediate dentin sealing?

    Im not using flowable for IDS, its for resin coating. The APX is for your restorative foundation

    Hi Dr., Can any adhesive system be used for IDS no matter if single or multi components? And don't you loose the temporary in cases like the shown prep because not enough wrapping or undercuts present? I have such experience...:/ Best regards!!

    Dr Nejad. What if after IDS n composite build up: i decided I had put too much composite for undercuts( i will try not to) and would like to refine the shape n angles in prep wall again. What would be my protocol? Thanks

    Good question. Summary: For the tooth, Air abrasion, etch, adhesive, cement with warmed composite. For the restoration: etch, clean, silane, adhesive

    Dear Dr, on your delivery day, how does your luting resin cement bond to the fully cured adhesive and flowable composites? Because as far as I know, in direct restoration, composite bonds to the oxygen inhibited layer of previously layered composites. And now with no oxygen inhibited layer, how does the luting cement works in the dentine part. Of course enamel part we still etch and bond on delivery day.

    Amazing For crowns that not transmit light through it like zirconia , how to ensure that the bond on the tooth is fully cured without disturping the fitting of the crown when we bond zironia with mpd and resin cement ?

    I have questions! Is the resin coating step absolutely necessary? Is there still an air-inhibited layer if we just seal with adhesive? Is resin coating possible for a full coverage crown prep?

    There are different techniques for veneers and full coverage crowns that I cover in my courses. Its possible to resin coat some crown preps but not all depending on the reduction/ restorative space.     I just realized there is also modulus of resilience, but apparently flowable has a higher modulus of resilience compared to conventional 'packable'.

    shrinkage stress of a material is essentially equal to the modulus of elasticity x strain (shrinkage volume). Lowering the modulus lowers the shrinkage stress. Hope that helps. Best-MN

    You need to know your bonding system. This bonding system has no solvent in the adhesive (6th generation) but many do. There is no solvent to evaporate. The best bonding systems do not have a solvent in the adhesive but in the first component (primer or self-etching primer).     Yes you can! The emphasis is to protect the tooth from needing a root canal, but yes, even root canaled teeth are treated with the biomimetic approach and it is just as important. Often times, this is the important intervention point that can prevent loss of the tooth over the next 5-20 years. Best- MN

    Amazing I read that vaseline will affecting the future bonding of the ceramic restoration as it is not easily remove , so usage of glycerin as separating medium under temporaty is more safe , right ?

    With the bonding agent shown, no but it depends on what you use for sure. In general, the ones I recommend do not air thin or dry because that would make the results ineffective but with some systems you must evaporate solvent in the bond.

    Sort of. The restoration is bonded to your composite, which is bonded to your dentin. It is a laminated structure, just like the DEJ adheres enamel to dentin. Of course, the enamel as well.

    There are a lot of advantages. Large direct restorations do not recover the stiffness and strength of teeth fully. Polymerization stress, stiffness, modulus of elasticity, and durability long-term will be limited. Cuspal coverage direct restorations can be acceptable but are by no means optimal.

    Yes but the technique varies and the performance of 5th gen is not optimal compared to 4th and 6th.

    Thanks Dr Nejad for this information post. I apologise for my question as this topic is new to me. On some links I see other people doing IDS only after they have done block out of undercuts and deep areas. Is that the same in effectiveness?? Assuming if I have done IDS but decided to retouch the prep floors n walls ( eg too much build up of cr on deep areas or too much comp resin on undercuts areas , should I redo ids again??? Thanks

    If I use the fourth or fifth generation of Bond, do I have to etch with 37% phosphoric acid? Is it for enamel and dentin and how long does it last? Before applying the bond

    I only recommend 4th and 6th. If you use 4th generation (total-etch) then of course you would need to etch otherwise it wouldn't work.
