Teeth whitening is a popular procedure and it can be extremely safe and effective. Unfortunately, most teeth whitening techniques are ineffective and unsafe with misleading claims. For instance, the science proves that the application of light does not enhance the results and it is a gimmick! In this video, I will explain the best method for safely bleaching your teeth with the custom-designed teeth whitening trays (also known as bleaching trays).

WHAT IS THE BEST TEETH WHITENING GEL?
The best and safest whitening gel on the market is 10% carbamide peroxide. Higher concentrations do not translate to better results, but they do guarantee higher sensitivity for most patients. 10% carbamide peroxide has been extensively studied by Dr. Van Haywood and shown to have safe and effective. The lower concentration is made more effective by increasing the application time.
THE BEST TEETH WHITENING METHOD
The best and safest teeth whitening is achieved with properly designed and fitted custom whitening trays. This method of whitening is also known as Nightguard vital bleaching among dental professionals, but most people refer to this as custom tray whitening. The use of light does not enhance or activate the gel and it is a gimmick. Always avoid any methods that involve abrasive ingredients or acid as they will damage your enamel and they are completely unnecessary.
DESIGN FEATURES FOR BEST RESULTS WITH CUSTOM WHITENING TRAYS
A lot of custom whitening trays have poor designs that are ineffective and waste expensive gel material. The goal of the whitening tray is to form an extremely precise and tight gingival seal or gumline seal to prevent leakage of whitening material out of the tray and to prevent saliva from entering between the tray. The best tray material is slightly thicker and rubbery to make this tight seal. Most trays are scalloped to follow the gumline but this is a poor design for seal and make the trays flimsy. Your dental models need to be customized prior to making the trays so that they slightly fit under the gumline to form this super tight seal. Lastly, no reservoir is recommended to hold the whitening gel because studies show that it only wastes gel without improving results.
HOW TO PROPERLY USE CUSTOM WHITENING TRAYS
When custom whitening trays are designed as described above, they are easy to use and maintain. When placing the gel, only place a small amount on the front (facial) surface of each tooth. When placing your tray, use your finger to adapt the tray around each tooth and then wipe any excess gel that comes out of the tray.
HOW LONG TO WEAR CUSTOM WHITENING TRAYS
You can safely whiten teeth with 10% carbamide peroxide for 10+ hours. The gel is active this entire time, but there are certain conditions that may not allow for this extended wear. This includes gum recession, cavities, defective fillings, periodontal or gum disease, or areas of dentin exposure. Your dentist must advise you of any limitations. These limitations can be addressed prior to whitening to allow for a more comfortable experience. When none of these conditions are present, most patients are able to wear their trays overnight. I recommend 2 nights on, one night off for the best results.
HOW TO CLEAN AND CARE FOR CUSTOM WHITENING TRAYS
- After removing your trays, a cotton swab (Q-tips) is the best for removing most of the gel residue.
- Do not use hot water as it may cause your tray to lose its shape.
- Before using the tray again, use one more cotton swab. The dried gel is easier to use.

    Hi Dr. Matt, thank you for this informative video. I got 35% carbamide peroxide, is it safe to use and can it damage teeth enamel, and for how long would you recommend using it if it’s safe. And the whitening trays do not cover all of my teeth, there’s a bit at the top that’s not covered by it, can that cause a problem or can i use it still?

    You can go continuously with the gels I recommend but i recommend starting slow to see if you develop sensitivity. If you don’t and you are using a lower gel concentration, then it’s okay 👍🏼

    You can use that overnight but only if you have done 10% overnight and tolerated it well. If you don't have 10% try 3-4 hours and if you tolerate it well, then add a few more hours overnight and see how it goes. Some people can tolerate 20% carbamide peroxide overnight but I don't recommend long-term use with 20%.

    Hi Dr. Nejad, Should you brush with toothpaste before putting in trays? I've read that it's better to brush/floss with just water before whitening (and brush with toothpaste after whitening).     Hi, Great video! I ibought the white dental care novon teeth whitening product. The active ingredient is 10% carbamide peroxide. I was wondering if this product is safe to use after the expiration date? It expired at the start of this year? If I use it it won't damage the surface of my teeth would it? It is more likely that it will not be effective in whitening my teeth? The product in the syringes is still clear and has been stored properly since purchase.

    I’ve just completed the 2 weeks 2 days ago, going in tomorrow for the in house session. I’ve noticed once I get my trays out in morning, teeth look whiter, but as the day goes by they turn back to the yellow shade. And now with the 2 days break my teeth look as before the 2 weeks. Does this mean it’s not working on my teeth? I followed instructions as per dentist recomandation. Any advices please?

    Hi Matt very informative thank you. I’ve got custom trays using 6% peroxide hour at a time. I’ve read elsewhere that once I remove the trays I can clean them with a toothbrush and water (no toothpaste)? I notice that’s different to your advice in this video though

    There are different options. I try to avoid brushing them but that is ok. Be gentle on the tray and avoid scrubbing hard.

    Not really. Bondings do not whiten unless it’s surface stain and the gel can’t penetrate through the bondings to your tooth structure effectively.

    I made a mess of my first day, not enough gel as I didn't watch a video like this, the gel is just sitting in the indentations. Gel is not spread at all, I took one off and topped it up, but it was moist and now watered down. Lesson learned: Day 2 will be better. I just hope I don't wake up with white dots on my teeth 😅 Thanks for the tutorial. Gave you a sub good sir.

    I appreciate you are still checking and answering questions even after 2years. As you know h2o2is very reactive, I have been told by other dentists the h2o2 is done reacting, has done all the stain removal / whitening that it will ever do after about 15/20 min. Another way of saying it is, 90% of the work is done in 15/20min. Empirically I have put a drop of various %'s of h2oh2 on various hard surfaces and it does stop bubbling after 15/20min. Your thoughts? This might explain why the in-office procedure is to replace the gel atthe 15min mark on 30 min whitenings and at 20min intervals on the 60min whitenings. ?? I don' tthink the "gel" medium plays a roll in any efficacy, just easier to apply, liquid h2o2 is easier to spill and larger liability at 45% per se.

    Carbamide peroxide reacts to form Hydrogen peroxide slowly. It is active for up to 10 hours. Its a slow release mechanism of H2O2 and that is what I use and recommend.

    Thank you for the informative video! The upper tray i received doesnt quite reach the gumline of a lot of the teeth. Will this affect anything apart from having less of a seal? Am I correct in saying that the solution doesnt need to touch the entire tooth but instead gets absorbed into the tooth? I dont have any issue with the gel not staying in the tray, just curious if this will affect results

    Usually should seal at the gumline, there are rare exceptions such as when you have significant recession with cervical root areas exposed. Best- MN

    You can try but generally they do not form a good seal around the gums to keep the gel in place. I would expect the results to be less effective and it could irritate the gums. The retainers have a different purpose than whitening trays.

    Ive just started on custom trays with the 10% gel, and have been told to wear for 1 hour per day for 14 days. I havent experienced any sensitivity. Would you recommend upping the wear time? Is 1 hour enough?

    It depends on the material and design. Its definitely not the most effective and you have to avoid irritating the gums.

    The syringes I use have a little cap. I would place the cap on so it doesn't leak out or at least put it in a bag until the syringe is done.

    I've been using a 10% gel for quite some time now. I saw a significant change but I still want my teeth to be whiter. So I was wondering if there was a certain amount of time that if I exceeded, it would damage my teeth.

    If you are using 10% CP, it is safe to use for prolonged periods. Sometimes with patients that have tetracycline staining, they go over 6 months and there are case studies on that. I routinely have had patients use it for a few months. Best- MN

    No. You need to keep saliva off the tooth and prevent ingesting the material. Also bad for the gums and leads to irritation

    What gel? How good are trays? The seal is the deal. If its not a watertight seal, its not going to be too effective. The material is important too.

    I usually do this overnight. There is no waiting period required if you did it during the day. Its really a myth that waiting will make the results better/longer lasting. I would just rinse out with water and go about my day.

    Hi, i have a question. I have a stain line (not cavity, I asked the dentist) in one of my molars that I would like to lighten. Could I use some 6% hydrogen peroxide in that section of the tray to lighten it? I know you're only supposed to put the gel at the front but the stain really bothers me.

    It could could work, but depends on what kind of stain. Are you sure its not the margin of a crown or restoration? It might also be abfraction.

    You can try. The seal isnt great on invisalign and its likely to irritate your gums so it is not advised.

    Your orthodontist doesn't know what he/she is talking about. The gel is not supposed to be swallowed, its meant to only touch teeth. The tray should keep the gel on the teeth and keep saliva away from the gel. Invisalign doesn't do any of it. Feel free to do whatever you want, but don't assume its safe to do it. Its worth knowing that peroxide makes free radicals and thats not something you want to have chronic exposure to systemtically. With the right tray, that is controlled.

    Depends on why and its impossible to be sure so you should discuss with your dentist, but my guess is you are either using the wrong gel or have a poorly fitting tray. Best- MN

    Well, I have no way to know what quality the specific dentist does. In principle, a quality made tray should have the best fit, and best seal. Drugstore kits are not comparable at all to this because they are not custom, or they involve some very basic customization options compared to trays that are specifically made for you and customized for best results. With that said, some dentist made trays are garbage so it really depends on the dentist. In terms of the actual whitening gel, there is a big difference because the ones a dentist can provide are prescription only. Sometimes you see them on amazon but they are gray market and not authorized for sale by the manufacturer. Over the counter or drugstore kits with lights are a gimmick. Drugstore whitening gels are not too effective. White strips kind of work but don't do a great job in between the teeth where you floss.

    No. It needs to be in contact for an extended period and it irritates your soft tissue like gums and cheeks. Not effective or safe.

    Sounds like your tray isn't forming a good seal and the gel is leaking out. The quality of your trays is critical, otherwise it is pointless to wear them overnight.

    My dentist instructed me for 9 days only and 1hour a day. Why such a low amount ? Would it be risky to do it overnight as suggested ? Thanks Dr!

    Hi Nicolas- well it depends. There are different materials, difference concentrations, etc. I would not go against what he says, but what I have described has been extensively studied and documented in the dental literature (Van Haywood).
