This is a patient example of bilateral proximal biceps tendon rupture. This condition is rare. The majority of biceps tendon ruptures involve the long head of the biceps and occurs proximally.
When the biceps tendon ruptures, the biceps muscle bunches up in the distal portion of the arm. Rupture causes a “popeye muscle” appearance.
Both proximal heads of the biceps muscle arise from the scapula. The short head of the biceps originates from the coracoid process. The long head of the biceps originates from the supraglenoid tubercle just above the shoulder joint.
Rupture of the long head of the biceps tendon occurs at the bicipital groove and the muscle then moves towards the elbow (popeye). The short head of the biceps remains attached to the coracoid.
Usually there is less disability with proximal biceps tendon rupture than distal biceps tendon rupture.
The patient may experience cramping, pain and cosmetic problems with proximal biceps tendon rupture. The physician may need to perform a surgical procedure called tenodesis of the biceps tendon especially if there is an associated rotator cuff pathology.

    Once I had an aged patient like this. When I explained to his wife what Popeye deformity is, she replied “If my husband is like Popeye, am I Olive?”.

    i had 2 prior labrum repairs. An anchor came out from the 1st one and an anchor was inserted too aggressivelly on the second. I somehow moved it deeper through some motion/action. I found a surgeon that specializes in shoulder reserfacing. He identified severe glenohumeral artheritis and we decided to go foward with the TSR. I've had bicep pain since surgery #1 but was told the bicep tendon was just strained from the labrum. My new doctor identified a bicep tendon tear "at the groove". That led me here. i diddn't know what the groove was. there was a list of problems noted during the surgery. anyway, your video is extemely well done

    I have a partial proximal tear of the long head. Will the pain ever go away? Its been 5 months. I do PT to try to strengthen the other shoulder muscles. I want to be able to throw a ball or a Frisbee again without pain. I am 42.

    Mine ruptured in April, 2 months after rotator cuff surgery on the same arm. When I attempt to workout, it cramps and hurts. How long am I supposed to wait for it to heal before I can lift again?

    Mine was on 20 February. It's July and I can almost lift the arm almost as far as the other. It hurts but the pain isn't as debilitating.

    Thanks Dr Nabil. I learn alot from tour videos. Is it possible that you could do a tutorial on the bicep tendon in its groove post tenotomy. My orthopedic surgeon cut my bicep tendon in my arthroscopic acromioplasty and bursectomy surgery. I still dont get it as all my imaging reports said my bicep tendon was unremarkable. Does the long head bicep tendon stay in its groove without problems. I would appreciate your knowledge and expertise.

    Perhaps a compression sleeve along with resting the arm? Seems it would be an unpleasant experience.

    @@jstarkillah6502 tendons don't repair, when there is a full rupture, and you should check up with a doctor as soon as possible, the longer you wait the shorter the tendon gets and more scar tissue forms
