Trigger Finger & Trigger Thumb - Everything You Need To Know - Dr. Nabil Ebraheim

Dr. Ebraheim’s educational animated video describes the conditions of trigger finger and trigger thumb.
Trigger finger is a condition where the involved finger will catch as it bends.
The finger is straightened with a pop or a snap. The flexor tendon becomes thick and irritated as it slides through the tendon pulley causing a thickened nodule and triggering of the tendon. The discrepancy between the diameter of the tendon and the size of the pulley opening causes the triggering.
Symptoms
•Pain may be located in the palm at the base of the finger.
•Tenderness at the A1 pulley.
Causes
•Occurs more in women •More in diabetics •Rheumatoid arthritis
•Repetitive injuries •Occupational.
Locking can occur when the tendon is unable to pass through the sheath. As the tendon becomes inflammaed, it gets caught.
Treatment
•Rest •Splinting •Injection into the sheath not the tendon •Injections can be repeated •Surgical release of A1 pulley.
When releasing the A1 pulley, the release should be done at the level of the MP joint.
The A2 and A4 pulleys are important and should not be touched, excised or released.
A1 pulley is cut and the flexor tendon is released.
Technique of injection
•Injection into the flexor sheath may provide relief for triggering of the finger.
•After the site is marked, use a 25-gauge needle.
•Advance the needle through the midline down to the bone.
•Pull the needle back slightly and inject the medication freely.
How do you surgically release the A1 pulley?
•The incision is marked on the MP flexion crease.
•The incision is marked out in the palmar crease for surgical division of the A1 pulley.
•The A1 pulley is exposed and released.
•The tendon is released.
•Patient is asked to move the fingers to confirm the success of the operation.
•Incision for trigger thumb release placed in MP flexion crease.
•When releasing the flexor pollicis longus tendon, watch out for the digital radial nerve in the field of surgery.
Sagittal view ultrasound showing the flexor tendon going through the A1 pulley of the finger.
Transverse view ultrasound showing the flexor tendon going through the A1 pulley of the finger.

I had a very painful trigger finger on my middle finger, I went to a local clinic, the Doctor gave one shot of steroid injection 💉, after a week the pain was still there, I went over to the Clinic again, and the Doctor 😷 gave another steroid injection 💉...i forced my hand to force and push a cabinet 🗄, I felt something cracked inside my hand 🖐️....I went and checked with a specialist Doctor, he said my tendon on the middle finger is cracked due to the frequent steroid injection 💉, it has soften my tendon and he said not safe to take the steroid injection frequently....now I have to do a surgery for the broken tendon and my trigger finger pain as well...i can't bend the end of my middle finger, it's a very disappointed experience for me 😥...pls be careful before taking the steroid injection 💉....better get a specialist opinion...just sharing my experience with the trigger finger effects

I have had several surgical procedures for this problem years ago. Injections did not help me at all. After surgery, the pain was gone. I have a new area acting up on my ring finger left hand. I am trying massage this time. I’d like to avoid surgery although it was not bad at all.

High risks also include hand scrubbing and cleaning. Including, but not limited to floors, walls, dishes & toilets. Typing and working with constant fingers being used for strength and injuries where the fingers were stretched out too far. I didn't do a study but that's my problem with finger and thumb.

I had trigger finger for around almost 2 years, never saw a doctor, never did anything about it, it came overnight and also disappeared over night all on its own. It was difficult to work and tie my shoe laces while I had it. Now gone and it never came back again. That is my story !

I told my doctor I had trigger finger. He examined it and agreed. I treated it myself with daily massages in the painful areas. When I went back I showed my doctor my trigger finger was cured. He smiled and looked at it and said how'd you do that? I cheated because I had worked for a doctor who treated and cured trigger finger repeatedly with massage. No expensive surgery needed.

Never felt pain during or after. I Don't want any foreign unnatural injections in my body. I had a shot in my index knuckle it hurt for hours and hours. I couldn't hear that again. The surgery did not hurt so that's my solution. Just fix the problem and I can move on .... I hope your issues are resolved soon and relief you of your discomfort.

I had surgury on my should twice in the last 2 years but if it was associated with arthritis i would have imagined it would have come on sooner. So hopefully this passes

I never even knew this is what it was in my two fingers. And then to know that there's a surgery that could fix this? Or injections? God I just wish this pain would stop but I'm a house cleaner so I don't even know how I would take time off for a surgery let alone recovery. But at this time my hands are in so much pain that I have no choice but to take the time off. It's like I'm stuck because I have to make money to pay my bills. I went on KZread to look up how to open up your fingers in the morning cuz I try and I put pressure underneath my fingers where it hurts so I can open them and it takes forever to do and it's so painful to unpop them. Thanks for your comment, I found it helpful. I hope you're doing good still

@@wendyk6875 injection of steroids by trained doctors is possible, it will help to reduce the inflammation at the affected area. Usually some light physio therapy , topical cream and a finger guard to use may also be necessary. The finger guard has a special fit to help keep the finger straight ( extended). But being able to let the affected finger rest will help. This is my personal experience, surgery was not an option because of other underlying conditions. Hope you are well by now.

I worked on an assembly line for many years putting fruit cup in snack boxes used both hands ; it wasn't until after retired my fingers on both hands would lock up and I would just reset them. They didn't hurt just weird they did that. Started crocheting and playing key board and piano and it stopped happening. I have noticed drinking sodas makes my body hurt more so slacking on that . Yes ty for video.

My trigger fingers and thumbs healed with rest from typing and localised self massage and no drugs or surgery.

How long will trigger thumb last. Does it require full rest? No typing?

@@user-mu6zn6zm1hI still had to do some typing, but did extra stretches and exercises afterwards. It took my hands about a year to fully recover; so it can be a slow process but they are fully healed now.

I am on trigger finger #4. These are my experiences. The injection lasts about 6-8 weeks then I was right back where I started. Surgery works well but can reduce range of motion slightly when closing hand into a fist. I did the shot on all of them first. But on #4 I got the shot and scheduled the surgery at the same time. There is a long wait to get surgery scheduled. I don't even notice any adverse side effects after the surgery. The noise it makes when they make the cut is creepy.
