    Thanks. Just here for a quick refresh. Once examined a retired neurology professor in his 90s. He told me, in his soft, clipped RP, that he was “having some extrapyramidal signs...” *Gulp*. Anyway, I did a full neuro exam, and when I got to Hoffman’s test, he said, “Ah, I see you’ve been well taught.” Thanks, Prof 😊

    A positive Hoffmann’s reflex and finger jerks suggest hypertonia, but can occur in healthy individuals, and are not useful signs in isolation. In cerebellar diseases, the reflexes may be pendular, and muscle contraction and relaxation tend to be slow, but these are not sensitive or specific to cerebellar signs

    I have hoffman sign in both hand, i have it for as long as i can remember , don't know it was pathological, I don't have any muscle wasting or anything serious, i'm 26

    About a small percentage of the population have positive Hoffman sign without any neurological issues. Conditions like multiple sclerosis and hyperthyroidism can also give a positive sign. Anxiety also results in a positive Hoffman sign

    I have had a neck pain for 3 months now with weird feeling in my left arm and leg with headaches. Anyone has an idea?

    I have this both sides although the left side is worse (have a pronator drift on that side). Three consecutive flicks of the middle finger and my thumb and index finger touch each other. If I move my neck and do this test my thumb and index finger touch each other in one flick.

    One flick or 5 flicks I would have this checked out especially if there is neck involvement.

    Would this make sense if someone has a bulging disc at C5/C6 that's pressing on the spinal cord? Please say yes. 😣

    Reason being, my mom has ALS. I have the bulging disc and am awaiting my neurosurgeon referral. Two neuros told me they don't think I have ALS. But things are so bizarre in my world. My whole dang hand jumps sometimes doing this. I have Relapsing Polychondritis as well. All of this, every last thing, was discovered during the pandemic.

    If the Hoffman sign is +ve regardless of the cause there will be flexion of the thumb and the fingers come together a little bit, right?

    What exactly does the Hoffman test tell you? Stenosis ? Cord compression? What is motor lesions or what kind of neurological conditions can a positive test give a clue to?

    A positive Hoffman's sign is indicative of an upper motor neurone lesion (UMNL) i.e. a lesion in a part of the nervous system above the anterior horn of the spine (eg. spinal cord, brain).

    ello , im worried about the checks my docs have been doing , im having a lot of problems with arm pain BOTH arms , its been going on for around 9 weeks . its driving me nuts , i feel like ive done 10 rounds of boxing where ive punched my self out and my arms are heavy , tired , weak sore and burning , its got to the stage i cant lift my 1 and a half year old daughter out of her cot has any one any ideas , i also noticed my blood pressure and pules has went up over this time although the gps have never check my blood pressure ive my own machine , they say tennis elbow but im worried its not as inm getting weaker and weaker , and reply welcome expect trolls who think they are funny but they are just being A$$holes and need to get a life lol

    Hi Pat. Your symptoms sound exactly like how my cervical spondylosis started. Ask for a cervical MRI to see if this could be your problem. My doctor didn't take it seriously and mine turned into cervical spondylotic myelopathy (CSM) making my arms and legs feel heavy as it progressed. Ask your doctor for a cervical MRI before you have worse symptoms. Some of my damage is permanent from waiting so long. Also consider seeing a neurologist. God bless!

    @@roseo322 even if im male lol ? i seem to have came out the other side ( i hope ) but never got the root due to this covid-19 so i might never know , i watched a program called somethings killing me and it sounds like i had mild rat lung worm or something from a rat weil's disease as i make organic composts and we had a few around ,i couldnt lift my arms for near 3 months or even open a peanut at its worst thanks for your reply and stay safe > Paddy

    I have bilateral hoffman sign, hypereflexia of both knee, but I don't have clonus , I don't any issue with my strenght, i'm totally normal , i'm 22 ,should I worry ?

    Hi, Thank you for the video. If when making the Hoffman sign there is a slight movement in the index, but not in the thumb, can it be considered positive? I have reviewed his great videos and there are very clear Hoffman tests, but I do not know if it can be considered positive when there is a flex only in the index. Thaks you.

    Hi Ignacio, even if only the index finger moves, you should then check the other side and check deep tendon reflexes to see if they are hyperreflexive. I would also check for clonus in the ankle. It's not uncommon for some people who do not have neurological issues to have a positive Hoffman's sign. You need to worry when they have a combination of all the above. Don't forget to add the Babinski test to that. More to come and please share. Thanks!
