PART 3: Neighbourhood-Based Care - Care Coordination Program of Work

In a regional study of Mississauga Halton residents’ care needs, we found that social, cultural and economic statuses - sometimes called SEC statuses - varied significantly between neighbourhoods across our region. SEC indicators such as race, income and education level have a significant impact on people's health and wellbeing.
We used those insights as evidence to realign our care coordination teams and service providers to neighbourhoods across the region. This supported the creation of connected, consistent, integrated care teams who are experts in resources available to patients, based on where they live.
This new, refreshed approach to community-based health care focuses on connecting patients, like Betty, to the services and supports they need to recover more quickly and closer to home, with the help of a consistent care team. This approach also takes into account the social determinants of health that impact our ability to coordinate care for patients in our diverse community.
