We have completed an extensive, evidence-based professional practice redesign of our care coordination approach as part of the Care Coordination Program of Work.
The focus of this project is to enable our care coordination teams to better use their clinical expertise and judgement to assess and address patient and family needs consistently, in collaboration with all those in the circle of care.
Every patient is unique, but our new approach to care coordination is devoid of variability; it’s consistent for all patients.
What this means is that regardless of where a patient lives in our region, they will receive the same care coordination approach, the same questions from their care coordinator, the same quality of care and the same compassion.
