The hex head screw should have been replaced with a slotted set screw of an appropriate length. As an alternative, a cylindrical shaft threaded at one end and slotted at the other to fit the thread in the wrench handle and the interior diameter of the worm gear would be even better. The screw should never extend beyond the body of the wrench head. It should be flush or slightly recessed when tightened down.
