OB/GYN Explains Induction of Labor: Pros, Cons, and What You Need to Know

The Doctors Bjorkman, a board-certified OB/GYN and Pediatrician couple who are also new parents share what you need to know about labor induction, when it's indicated, the potential risks/benefits, and review of the ARRIVE Trial. They also share the decision they made for Sarah's pregnancy and their own baby.
We also got a lot of questions about the ARRIVE trial and risks/benefits seen in elective inductions of otherwise healthy pregnancy at 39 weeks. Because of this, we went back and added some more information about this from a prior version of this episode -- 11:30

Was supposed to be induced at 39 weeks. Go to 37 week appt and blood pressure was through the roof so I was sent to Labor and Delivery to be induced that night and was SO scared because I have a huge fear of medical procedures or whatever with the vagina. I prayed for a miracle and 5 mins after being told how I’d be induced which I wasn’t looking forward to, I started having contractions and going into labor naturally! When they checked me I was 4 cm and was able to just get a very small dose of Pitocin which is what I was hoping! Thank you God! Now I have a beautiful 1 month old baby girl!

I was induced at 38 wks due to babe having growth restriction. First pregnancy and VERY unfavorable cervix. Checked in at 5:30pm, did 4 doses of vaginal Cytotec overnight- each dose placed every 4 hrs. I got to sleep with Benadryl through the night so I rested as the cyto started to do it’s thing. By 12pm I got to 3cm and my body kicked into gear naturally- no Pitocin required 🙌. Got epidural at 7cm pitocin at level 3 for 3 hrs and out came babe at 11:34pm. It was a beautiful experience! I was a nervous wreck beforehand so I Just wanted to share my positive induction L&D experience 🎉❤!

I was induced last week and went from 1cm to 8cm in an hour and began to involuntarily push the baby out. Ended up getting the epidural so I could relax my uterus as I was having back-to-back contractions and thought baby would become distressed. Let’s just say it was way more intense than I thought it’d be. After epidural, I began pushing about 1.5 hours later. My baby girl is super healthy so I’m happy she’s here!

I had the same thing happen. I had had pitocin, and that worked fine for me. With my 3rd, I was given cervidil and my body didn't like it. I had back to back contractions that were a 10 for pain. I wanted an epidural and was told I had to be 4cm before they would give me one. I was in excruciating pain for hours I started shaking. They finally decided to take it out. I reached 4cm but in the 30 minutes it took for the epidural to arrive my body was already trying to push out baby on its own. The nurse told me to sit still and stop pushing while I got the epidural. She didn't think I was ready. She recheck me after the epidural was in, and I was a 10. Sometimes you get nurses that think they know more than you, and you are the one suffering. I am expecting my 5th baby and I have fast labors. All my babies from start of labor to delivery were 6 hours. It didn't matter if it was induction or natural.

I was induced 11 days overdue with the gel. I was 1 cm dilated, and only 25% effaced. I was told my cervix was very, very unfavorable, and not to expect anything to happen that night. After being sent home after an hour of observation, we didn't even make it the 20 mins to get home before I was moaning in excruciating pain and turned around to go back to the hospital. They had me in triage waiting for the doctor to come back from a c-section to check me again but stated it was probably the gel overstimulating my cervix and they were most likely going to flush it out. No one thought I was in active labor. I begged for pain management but said I couldn't get anything till I was checked by the doctor and in a room. Doctor was still doing the c-section so I had to wait. Well, after about 45 mins a nurse comes and takes me from triage without the doctor checking me and brings me to a delivery room. I had baby within 10 mins of being in that delivery room with the doctor showing up only 5 minutes prior to delivery. From the gel being administered to having baby, unmedicated, it was a total of 3 hrs.

Just my advice: if you start the induction process, are on Pitocin, and get to the point your OB has to break your water, I would give up on any "natural birth" ideas you ever have and get that epidural ASAP! Pitocin contractions and natural contractions ARE NOT THE SAME THING! Also, do not let them put your IV in your dominant hand- very important. If the nurse can't find a vein in your non dominant hand- tell them cool, please go find someone else better than you. You don't know how long the process will be after induction, and do you want to spend 2 days wiping yourself with your IV hand? No it's gross and annoying!

With my first baby I had her natural also with no epidural or anything to help with pain and for me the pain of the contractions was the same. I was able to tolerate both.

I thought I’d share my story as someone who had an unfavorable cervix but had to be induced at 38+2 due to gestational hypertension. I started at 1cm and after 6 doses of Cytotec in 24 hours, I had only progressed to a 1.5cm with a still very firm and high cervix. I was having regular contractions the entire 24 hours. OB gave me the option to go home if I wanted too and try again the following week but we would have to be monitoring my BP every couple of hours and running Pre-E labs every other day or I could stay and keep trying but if I continued to not progress, there was a possibly I would end up having a cesarean as it could put too much stress on the baby. I decided to stay but asked if we could try a different medication. They inserted Cervidil and I instantly felt a shift in my contractions. At the 6 hour mark, I had my bloody show and felt very optimistic. At the 12 hour mark, they did a cervical check and I was 5cm dilated. They broke my water and I decided to get an epidural to allow my body to relax and progress as I had been in labor for 36 hours at this point. Pitocin was started shortly after. Within 2 hours, I went from 5cm to 10cm. Did one practice push and had to labor down because my doctor was still on his way and nurses were scared I’d push him out before he arrived. Baby’s heart rate began to drop and before I knew it, baby was out in 3 pushes. Had a 1st degree tear that required only 2 stitches. Although it was not the labor I imagined at all, I am so glad it did not end in a cesarean and I was able to deliver vaginally.

I was induced, started on Pitocin, did nothing. Then had the folley ball, complete nightmare, did nothing. My doc broke my water at about 7am in the morning, and had my daughter at 8:02pm. After they broke my water they cranked up the Pitocin and the contractions were coming every 25-30 seconds, and they were intense. I had no prior knowledge or warning by nurses/doc that Pitocin contractions and natural contractions ARE NOT THE SAME so I was still that idiot trying to labor "naturally" without pain med. I was so exhausted and terrified me/baby would go into distress and need a C section, so I told them I wanted the epidural. Luckily, I had a fantastic experience with the epidural and could still feel my contractions happening (just minus the pain) and I knew when to push with my body.

I know this isn’t your full time job and we’re lucky you post anything at all. Anyway, thank you again. I appreciate you guys so much.

I don’t know your OB of course, and I’m not at al trained in midwifery , tho I was a RN for ten years. I think the reason sometimes medical people seem to dismiss some of the fears is because they have seen the other side / people who refused indiction or whatever it was that was recommended and had a terrible outcome. It’s not meant as an excuse , but staff are human and it’s often so frustrating when you see things from a medical point of view , and I guess in terms of statistics of how many people experience negative effects, and also the severity of those effects. Maybe because I have made the mistake in the past after seeing a patient go against medical advice and have a very poor outcome, I was quite short and abrupt with the next patient who also refused. I wanted to scream at them don’t be so stupid do you know what can happen ?? Yes you have some fears but put it in perspective with the risks !! Of course I didn’t say that, and I’m not saying it’s an excuse. But that dr has seen many patients some end very sadly, and sometimes it’s hard to be patient and try give information and advice and have resistance. I have to say I am lucky my Ob S and midwives were very open to talking about my fears etc , but they did all strongly recommend indiction and explain the multiple reasons in my case , and the risks. I wasn’t against it either, and maybe being a RN gives me a kind of view of trusting the staff more so than I would if I had never been on the other side. Before the ob even finished I told her I’m on board whatever she feels needed for a healthy baby / but she said it’s important I have a day and understand the reasons and risks and what may or may not happen. Ideally the dr will listen and address fears etc, but I guess I see why sometimes they can’t , they are human too. I admit I had heard a lot about c section and induction being pushed on people , yet when I asked about c section the Ob and mid wives both said there’s no reason to advise one based on current circumstance. That something may happen along the way and it may be needed , but it’s not needed to start there. And that they would let me know if it came to a point it was recommended. I found that to be comforting too. I have to say I’m not thrilled about indiction , it’s not ideal nor would I choose it if there weren’t reasons my medical team felt if needed. And yes I have some fears and concerns too. But end of the day i understand the medical reasons why they are recommending it , and when compared to the risk of not getting induced I’m totally behind the medical opinion. I found a lot of pressure from people telling me it’s a mistake to get induced or even give birth in hospital , and that my body knows what to do etc. and that’s great for people that works out for I am very happy they have that experience. But personally my medical team have recommended not just hospital birth but to travel to the next town over where there is NICU just in case as there are a few little risk factors. And they strongly recommended induction again for more than one reason, so it was really frustrating to me getting so much criticism from those who had been able to do a natural or home birth. I feel like the medical staff too get frustrated too trying to recommend and explain risks etc and yet patients can be so reluctant to listen / either because they have a fixed idea of how birth will go or should go , or they have been pressured as I was to ignore medical advice and just naturally let it all happen.

One thing I feel I have questions about that I have yet to see in my binge watch of your videos, in the few week since I found out I am pregnant, is cost. I know cost varies widely based on where you are, your insurance, and your provider; however, there must be data on the average cost of inductions vs waiting which probably runs somewhat related to the length of your hospital stay but I would like to see that at least made a quick side note when you guys are able to gather any cost related data on what options pregnant women have. Thanks!! Love your vids and your expertise!!

I am equal parts terrified of labor being harder/taking longer, and slightly relieved that I can have a "plan." Good luck! I was induced 8 weeks ago and wanted to go without the pain medication but ended up getting intravenous pain medication while I waited for the epidural! I had cytotec and cervidil before the pitocin and my contractions were very unpredictable (some were short and others were longer, some were mild and others were very intense) What really got me was the time between contractions was not regular at all, so I had no way to mentally prepare myself for the next one and ultimately why I decided for the epidural!

@viriacosta6321 unfortunately it was pretty rough. I went in Sunday at 3pm to induce and my contractions were 5 mins apart they started me on Cytotek to open up, but it took a very long time. Monday my water broke and I was finally good to start pushing at 10cm and after pushing really hard for a very long time of making little progress we found out he was transverse (turned crooked and coming down at an angle and pretty big. They tried to turn him (and during that time my epidural ran out and the anesthesiologist was in back to back c sections for about an hour). So I was not having the best time. Finally we decided to do a c-section. so I have very sore abs from the pushing extreme ab workout, and a sore incision area from the c section too. But everything was worth all the struggle because I have the most amazing son that I can't get enough of and I would do it all over again to see that sweet face. 🥰 He was 7 pounds 10 ounces 3:57pm on Monday.

But truthfully for me it was easy. I came to the hospital 2.5cm dialated and when being induced after 2 hours I was 6cm dialated. The progress for fast for me. Contractions do get stronger, it went from feeling pain 2-3 to a 7-8 fast and there are no breaks between, I had like 30 seconds break. I wanted an epidural so I got it. I was more scared to get an epidural but it was easy. The labor was fast for me. Everything from going to the hospital and delivering my baby was 9hours. This was my first baby.

I had 4 inductions. First was at 39w elective with cytotec. Baby had horrible reactions, low heart rate/oxygen issues. Had an epidural and couldn’t feel my contractions or pushing. Baby was out in 30 mins and I never felt it. Total process 14hrs from start to finish. Babies 2-4 all were induced with pitocin only at 37w for hypertension. Very VERY painful active labor contractions but painless “developing” contractions. I would progress from 3-10cm within less than 2 hours. My last baby I progressed in just under 1 hour from when my water was broken. Highly recommend pitocin over cytotec as my child developed intellectual disabilities and my other children were perfectly healthy. Just get the epidural to help mitigate pain with active pitocin labor and it’ll be fine. One birth had no time for epidural and it was traumatizing.

Never EVER having another induction, experience was traumatizing. (Iugr Diagnosed, baby wasn't growing) Almost had a c-section because the baby's heart dropped a flat line a couple of times. Found out I finally dilated to 10, and the birthing process was very rushed in under an hour. Had a second-degree tear with a failed epidural. But my baby boy came out healthy, born on August 25th, 2023, at 6:18pm. weighing 5 pounds and 10 ozs 47 cm.

1000% YES! Love that documentary. The Arrive Trial is crap and inductions INCREASE the risk of csections significantly

What an amazing and educationally insightful session. I watched this weeks prior to my spouses induction. I noticed that other than pitocin her OB Doc used cytotec a precursor to pitocin are there any preferences on what is safer or mostly used? Thank you 🙏🏿 for giving us more incite.

I was induced at 40w3d with cervidil. I had asked my OB to induce if I went past my 41w, and he told me to get to the hospital the next day 🤦🏾‍♀️. Wasnt even done with fixing everything in my house. But, I made it thru with 2 days.

Very informative video! Currenty, almost 35 weeks and will talk to my OB at my 36 week appointment about induction. I live in a maternity desert where the hospital is 40 minutes away from my house and over an hour away from my job. First time mom so I have no idea if I'll even know what contractions feel like. 😅

I'm expecting my 3rd. My first two were terrifying births and I'm incredibly scared of this one. My midwife got irritated with my list of questions at my last appointment because I "did this before." I tried to tell her that was 10 years ago and I was on my own with Hyperemesis and in hiding from the biological father while navigating court for protection orders, so regular prenatal care couldn't happen. She said "don't gain another pound and get some counseling." I'm 28 weeks and gained 16 pounds. I've been in intensive PTSD treatment for years. My therapist and I made the list of questions together. I've fought so hard to keep my kids safe and to be healthy. I lost 95 pounds prior to getting pregnant to get to a healthy weight range. None of it mattered; I was just a paranoid, chubby weirdo to her. I left my last appointment embarrassed, ashamed and dreading having my baby near that lady. I wish I could just hide in a hole and have her in there or have her at home where I feel safe.

It sounds like your midwife is not empathetic to your situation at all. Ditch her asap.

Get a new midwife! I have a friend who changed doctors 3 days before giving birth because of something like this, and it was absolutely worth it.

I’m 20 weeks, right now my birth plan is to give baby boy until his due date and then plan the induction to be a day or two after his due date. As long as it’s safe for the both of us.

I had a induction at 39 weeks. My doctor thought I had pre-clamp Sia.( I didn’t have high blood pressure untill the induction process, but did have protein in my urine.) I ended up having a emergency c-section my baby got stuck and my blood pressure was through the roof.

Had all 3 with no pain management and induction with 2...2nd baby was 40 and 3 days but not really moving and showing early signs of distress. No other complications, he was 9lb10 but the pitocin gel was a much worse labor for me than my first baby who was also bigger (I'm tall, no GD). Much more intense. My 3rd baby I was induced at 39weeks and 2 because my BP was spiking with no previous preE diagnosis and because I lived in the middle of nowhere we were scared I'd go into labor wheb my partner was at work, with high BP alone or not be able to drive myself to the hospital. I was on a very slow pitocon drip steadily increasing then cutting off once labor was well under way, better than the gel, still worse than first natural labor. I hope to have my 4th without induction...it was much nicer even being my first. I chose not to have pain management but they did say it was was option. For me, having labor induced was bad enough and the IV lol I'm squeamish for needles on myself (but I've literally performed thousands of venipunctures lol) but will do the labor part just fine haha. Give me labor over someone messing with a needle in my back any day lol

It irks me that they act like pitcon doesn't make contractions worse😂. Just be honest about it already. Mommas who can compare natural birth without can vouch.

I'm about to have my third baby, my water broke naturally with both my pregnancies and my body is on track to do the same this time. But my doctor wants me to have an induction with no reason. I have no health issues and neither does my baby, and I had a very positive pregnancy experience.

I've had 5 children and 3 were induced- one with Cervadil, and 2 with membrane rupture and then Pitocin. I had labour augmented with one delivery as well after I went into labour on my own. I can't imagine choosing to have an election induction at 39 weeks- that's definitely not for me!
