Addiction is a neurological disorder that affects the reward system in the brain. In a healthy person, the reward system reinforces important behaviors that are essential for survival such as eating, drinking, sex, and social interaction. For example, the reward system ensures that you reach for food when you are hungry, because you know that after eating you will feel good. In other words, it makes the activity of eating pleasurable and memorable, so you would want to do it again and again whenever you feel hungry. Drugs of abuse hijack this system, turning the person’s natural needs into drug needs.
The brain consists of billions of neurons, or nerve cells, which communicate via chemical messages, or neurotransmitters. When a neuron is sufficiently stimulated, an electrical impulse called an action potential is generated and travels down the axon to the nerve terminal. Here, it triggers the release of a neurotransmitter into the synaptic cleft - a space between neurons. The neurotransmitter then binds to a receptor on a neighboring neuron, generating a signal in it, thereby transmitting the information to that neuron.
The major reward pathways involve transmission of the neurotransmitter dopamine from the ventral tegmental area - the VTA - of the midbrain to the limbic system and the frontal cortex. Engaging in enjoyable activities generates action potentials in dopamine-producing neurons of the VTA. This causes dopamine release from the neurons into the synaptic space. Dopamine then binds to and stimulates dopamine-receptor on the receiving neuron. This stimulation by dopamine is believed to produce the pleasurable feelings or rewarding effect. Dopamine molecules are then removed from the synaptic space and transported back in to the transmitting neuron by a special protein called dopamine-transporter.
Most drugs of abuse increase the level of dopamine in the reward pathway. Some drugs such as alcohol, heroin, and nicotine indirectly excite the dopamine-producing neurons in the VTA so that they generate more action potentials. Cocaine acts at the nerve terminal. It binds to dopamine-transporter and blocks the re-uptake of dopamine. Methamphetamine - a psychostimulant - acts similarly to cocaine in blocking dopamine removal. In addition, it can enter the neuron, into the dopamine-containing vesicles where it triggers dopamine release even in the absence of action potentials.
Different drugs act different way but the common outcome is that dopamine builds-up in the synapse to a much greater amount than normal. This causes a continuous stimulation, maybe over-stimulation of receiving neurons and is responsible for prolonged and intense euphoria experienced by drug users. Repeated exposure to dopamine surges caused by drugs eventually de-sensitizes the reward system. The system is no longer responsive to everyday stimuli; the only thing that is rewarding is the drug. That is how drugs change the person’s life priority. After some time, even the drug loses its ability to reward and higher doses are required to achieve the rewarding effect. This ultimately leads to drug overdose.

    Agreed . When you give up on fighting addiction then you give up on life and caring about family. I have friends who have gone years without relapse and it has been due to therapy and a strong relationship with God

    I am in full time recovery from cocaine addiction!! I suffered with horrendous cravings regular! understanding how addiction works with the dopamine was a key ingredient to overcoming my addiction, once we understand something like addiction and how our brains reacts, it can help us to move forward. ih hope everybody is well and at peace :)

    Overdose (especially lethal) actually occurs most often after a period of sobriety. With heroin, it's the respiratory depression that most often results in death, and tolerance to that effect develops along with a tolerance to the high. It's the decrease in tolerance that occurs during sobriety. An ensuing relapse where the user attempts to use roughly what they were prior to detox has a much more marked response. This can happen in as short a span as week or two as well.

    Thank you so much for sharing this. I was talking to, and went out with someone with whom I felt a genuine connection. We went out on a Friday. I didn't know that the day before was his 1 year anniversary of sobriety. I just found out that the following weekend, he passed due to a fentanyl overdose. The week leading up to that his communication seemed sporadic and short - completely different. The last thing he said to me was,."I definitely want to see you again soon.... Still thinking about what a great time we had." That was Wednesday and I never heard from him again. I couldn't understand why. Then I found out he'd passed the following Sunday. He had a good heart. He was planning for the future. He is in Heaven now. He knew the Lord. There is no more pain or suffering for him. But he was special.

    My best friend died 12.5 years ago from injecting heroin and drinking alcohol. The heroin on its own would unlikely of killed him and the alcohol on its own would of killed him. But the both together caused him to die. Very sad 😔. I remember it like it was yesterday. I continued to take drugs for many years after that. I finally stopped in 2020 when I went to treatment. And have stayed clean since 🙏❤️❤️❤️

    An overdose is typically caused by a central nervous system depression or over stimulation. The brain stem, the part of the brain responsible for giving signals to do things we do without thinking (breathing, blinking of eyes, etc) is depressed so much bc of the over saturation of the receptors that the brain cannot overcome it, it cannot monitor the carbon dioxide in the blood stream which would make one take a deep breath... the part of the brain needed for basic functions to sustain life become numb, incapacitated and a person sadly stops breathing bc the part of the brain that tells it to breathe is "unconscious too." So my entire point of responding to your comment specifically is to clarify that it isn't the dopamine receptors that cause the overdose, it is the nervous system depression due to the lack of brain activity/function: what causes a person to stop breathing... if that makes sense, yes its the catalyst of causing the surge of chemicals that depresses the brain, but it is the symptoms that are caused by that oversaturation of chemicals that causes the death, the overdose... the ability to control or be aware of ones utter and absolute lack of oxygen and need to breath without outside intervention. which are the consequences of too much, 99% of the time done accidently I should add. And typically, after a period of sobriety as @RespectThis said, the user thinks they can use the same amount, but it overwhelms the body and the person typically stops breathing due to lack of tolerance and also not being told/being fooled/poisoned some may say by what drug they are taking bc the pills/fentanyl out there is in the form of pharmaceutical pills,... you can look them up on a pill identifier website online and it will tell you it's something else. We are being lied to as to what is in the drugs, thinking it's one thing and being told it is, but its counterfeit, they are not real opiates from a licensed pharmacy, the pills contain solely fentanyl now a days. Fentanyl is 100x time more potent than heroin... People think they're doing heroin, but are given fentanyl unknowingly, and a very small amount will cause an overdose unintentionally... is so sad and so scary. I just wanted to clarify what causes the overdose and am not hating on you or correcting you or judging you. Thanks for trying to understand for real!

    I just want to get this off my chest. My dad is a homeless drug addict. He smokes crystal meth and fantanyl. He sleeps on my moms patio even though she has a new boyfriend. We all feel bad for my dad but there is nothing we can do for him, he made his choice, its past the point of no return. But its sooo painful. It hurts my heart and makes me cry. It hurts so bad.

    You can take him to a rehabilitation centre incase you have them in your country. He is suffering from substance use disorder. In fact you can take him to a mental health unit because it's what we do here in my country Uganda 🇺🇬

    Quite a few years ago there was a BBC lunchtime news feature on a trial to cure chemical addiction. It was reported to be +90% effective in the trials so far. It worked by measuring the frequency of the drug and then passing electrical current of the same frequency through 2 electrodes the patient held in their hands. This caused the body to quickly start to excrete the substance out through the skins pores. The clinic room even had an en-suite shower to wash it all off. I forgot all about it for years and now can find no mention of it on the net. I wish I had recorded it but I had missed the very start of the feature and thought I’ll just record the evening news and get the whole thing. It wasn’t on the evening news however.

    Pain is as regular as addiction. Taking pain mediation reduces (but not removes entirely) pain. When the pain subsides, like when one is sitting quietly, there is no urge to take pain medication because one is not prompted to do so, by the "usual" pain. Only when pain starts up again (from moving the body or being mobile (of joints) ) does the desire to take pain medication restart. It is not prompted by anything other than the "pain returning". - in a way, pain is an addictive event, always starting-up when activity happens again. - Have A Nice Day.

    I have severe chronic pain anf I do take medication for pain. I only take a very small dose per day so that I am not bedridden and can take care of my son and do a bit of work around my home and have some quality of life. I wait till pain is extremely bad before I need to collapse to take it. I am prescribed more but don't use it. I wish I didn't need it but the pain is so bad I always think of taking my life due to such poor quality of life. It's been 41 years and after a while any Human would have thrown in the towel with 24/7 pain and no sleep. I have the feeling but I'm alive and I stayed to take care of my boys when their mother ran off with her boyfriend. I am lucky I'm alive and I am grateful to all the pain specialist who try to help me.

    Pain medicine was created and intended to help people manage pain. Creating more laws and regulations that prevent doctors from legitimately prescribing necessary medication is doing nothing but creating a chronic pain crisis. Ppl who do this illegally manufacture it themselves. They sell it themselves They do not obey laws - obviously. Putting doctors in prison (YES, this is happening) and limiting the amount of pills a pharmacy can carry (which results in patients being unable to fill legitimate prescriptions), enabling pharmacists to undermine doctors, has resulted in ppl committing suicide because they can't deal with the pain day after day. They have little to no quality of life. THAT is the result of the "opioid crisis."

You are so incredibly strong even though you feel weak :) No one who has never known how much strength and mental resilience it takes to stay day after day and fight the baffling disorientation, noise and obsession that wanting to die causes...and how isolated and weak one feels as they fight the voices and tsunamis of lies alone , to fight mental illness alone but instead of buying into others fears and hateful opinions, the greatest gift we can give ourselves is trying to start recognizing the strength it takes to stay solely for the well-being of someone else. How much guts, balls, gall it takes, the amount of strength it takes a person who wants to die every day to instead stay solely out of concern for how someone else will be affected by their absence... That is courage my dear. That is unconditional, selfless love actually :) I see you! Brave heart, in my humble opinion and experience, I think you should bless those that know and love you and commit to sticking around a little while longer honey. It gets better (Cliche I know), it really does. Life is so overwhelming and suffocating until one day it's just not... and life and kids and animals; anything that you fought through the unbearable pain for: everything that you stayed for truly becomes the natural high of life and the obsession that you once were a slave too and which required you to seek the necessary relief for... abated pain if only for one-minute would be ecstasy, yes? ... It's going to happen for you, just dont give up. The world needs your courage and love and light. Please stay awhile and remind yourself what an incredibly brave and bad ass person you are for still showing up in life despite it all :) I see you and I think you're pretty amazing. the kids you spoke about are SO damn lucky to have you and will benefit from your triumphant wins that you're destined for.

    I am an addict and I literally feel like I can't stop, it's making me feel suicidal, as an addict this really is a disease which I feel is no cure for, NA and AA does not work, I've tried everything and nothing can get rid of my addiction

    5 years off of heroin and i have np emotions at all nothing is worth doing. I feel like a robot for 5 years with no enjoyment in eating or achieving or in socializing and no sex drive or interest in anything at all. I believe it permanently altered my brain by using IV heroin for 10 years. Nothing could compare to the high it did give me

    It's crazy the effects drugs have on a person. I injected amphetamine/meth for 11 years and 5 years before I took a lot of cocaine. Also the last 2 years a lot of crack cocaine as well as amphetamine. I've been abstinent from all drugs for 2.5 years. But I still have strange twitches and tics that I think are from my drug use. But it doesn't bother me that much I'm the happiest I've ever been. The only times when I really struggle is when the paranoia and anxiety hit me. But its not as often anymore. If your struggling with addiction. There is hope. Trust me if I can stop so can you 💯 🙏❤️

    Been clean of heroin for 2 months now I’m glad I’m happy I still smoke weed but stopped smoking and now I’m smoking cbd cigs to stop smoking weed, it’s just the drinking rn is kinda tough

    The webs we create around ourselves need to be unwound. If you have ever untangled a ball of string, you know that yanking & pulling only makes it worse. You need to very gently & patiently unravel the knots. Be gentle & patient with yourself as you untangle your own mental knots. Get help if you need it. Above all, love yourself in the process. The willingness to let go of the old is the key. That is the secret.

    It was really awesome presentation of drug addiction treatment. Rapid opiate detox is a relatively new method of treating opiate addiction.     As an addict I can watch this, is still does not register in my brain that this is bad, addiction overalls everything, this is why the government need to do more or use me as a guinea pig to come up with some results to help addicts like me and others out there.

    If you use drugs and the reward system desensitizes, but if you stop doing drugs will the reward system be permanently desensitized or can it be like normal again?

    Do those prone to addiction have lower natural levels of dopamine or dopamine receptors? I struggled off and on with addiction. Since I was a child I've had a flat mood and seldom smile or feel contentment. It was always noticeable to others long before my addiction. Try as I might I have never felt the reward the video talks about. People would even get angry with me for not feeling happy. When I was a toddler my older brother used to humiliate me and hit me whenever I looked happy. He would snuff out my happy feeling. It went on from age 3 until I was 10. The psychologist told me I missed the developmental phase needed and that what my brother did was psychological torture common in psychopaths.

    YES!! I was put on psychotropic medication at age 5: Ritalin. So my brain chemistry, my brain function, my tiny brain that doesn't mature until age 21 per research, was inundated with stimulants... I was essentially destined to be a drug addict, conditioned... add in the childhood and adolescent trauma and unfit, neglectful parents... Viola! An addict! I was failed... I didn't have a chance sadly. But I won't be anyone's victim or a statistic. I understand how you feel, and we just have to fight that much harder... it's awful... it's hard... I feel so alone too... i constantly struggle with "why can't I be happy like everyone else?" "What's wrong with me?" "I don't understand, what am I missing, what does everyone else have that i don't?" but I'm here to say that it's not your fault. Look into childhood PTSD and think you might find the answers you seek there my friend. Keep on keeping on friend. P.S. "People who don't believe in magic will never find it"... look around you and I promise you will find what you seek if you truly let go and be present and stop judging/expecting/comparing yourself to others... find yourself and love yourself... ask someone else how to... participate in life and stop expecting something out of life; stop waiting for happiness to clock you in the head... it won't, but there is magic everywhere, just not where we expect to find it.

    Do the chemicals in the cigarette also play a role in how nicotine affects the brain? What about vaping e-liquid that has 3mg nicotine in it? It's not as bad as smoke in the lungs, but does it have the same effects as cigarettes with nicotine even though it doesn't have all of the other chemicals?

    Not sure if there are conclusive studies about other chemicals in the cigarettes. However, as long as you consume nicotine, it has the same effect on dependence. The dose may be lower and no smoke is better than smoke, but long-term studies would be needed to access the effect of e-cigarettes.

    Cigarette nicotine is free base due to tobacco being treated with some gnarly additives to make it a better buzz, vape liquid I've seen usually uses nicotine salts.

    I have osteoarthritis in my hip. I have been not been able to function for years. I have been prescribed cocodomol. I seriously can’t get out the house wothout it. Am I classed as addicted? I am supposed to have the replacement by December this year.
