ABOUT THIS VIDEO:
Another patient came to me with a routine carotid ultrasound. I had to break the bad news that this study, although by far the most common, is rarely needed. And it doesn't help us much. These are "flow studies", measuring the flow in the carotid arteries.They don't help us understand heart attack & stroke risk.
In this video, we go deeper into why CIMT is not done so often: it's hard to do right. Why? Perspective matters - just like photography. .

ABOUT DR. BREWER
Dr. Brewer started as an Emergency Doctor. After seeing too many preventable heart attacks, he went to Johns Hopkins to learn Preventive Medicine. While there, he went on the run the post-graduate training program (residency) in Preventive Medicine. From there, he made a career of practicing and managing preventive medicine and primary care clinics. His later role in this area was Chief Medical Officer for Premise, which has close to 1,000 primary care/prevention clinics. He was also the Chief Medical Officer for MDLIVE, the second largest telemedicine company. More recently, he founded PrevMed, a heart attack, and stroke prevention clinic.
At PrevMed, we focus on heart attack, stroke, and cognitive decline. We serve patients who have already experienced an event as well as those who have not developed a diagnosis or event. Dr. Brewer provides services via telemedicine or in person if you're in the Lexington, KY area. We find a lot of undiagnosed prediabetes or insulin resistance. Treating unrecognized risk factors like prediabetes allows reduction of risk and prevention of disease.

I think that this is one of, if not the most important presentation series on KZread. It deals will a very serious health problem and ways to prevent and manage it. Your slow and methodic approach digging into the causation and various testing methods will educate people. And it’s with this knowledge people can take their health into their own hands. I thank you Doctor. Your work, I’m sure, is saving lives and debilitating events.

Morning Doctor B. I let you know this trick to get around Elsevier etc on your berberine video but just in case you don’t see it there, here’s how you can find journal articles even though there’s a paywall. Just copy the title off pinned abstract and paste in Google Scholar. If there’s a PDF anywhere in the world, it’ll show up. I found the article you could only get the abstract of that way. One of the imaging centers rang me back regarding CIMT. Yes they do it, need a Drs referral. $350AUD but will get $180AUD back from medicare. So that isn't too bad. I also know these guys at the Private Hospital, they are good.
The only self-referred imaging place near me doesn't do the CIMT scan, just the regular ultrasound scan. And they don't even currently offer a heart CT scan for a CAC score.

Down here in Australia,I have sent of two requests to local imaging centers, to see if they provide this service, and the cost.

Thanks. I have multiple patients in Europe, the Middle East, Australia, New Zealand, etc. I look forward to a time when we could provide/train providers in these areas. So far, most of these global patients have started with us when they were traveling in the US.
We don't have CIMT in the Philippines. We only have Carotid Duplex Test.
Glad I found a place 1 1/2 hrs away to get the CIMT and relatively inexpensive too. Out of curiosity, is that the way natives of Louisville pronounce the name of their city?

hard to understand why insurance companies don't pay for such a powerful diagnostic tool. The cost seems comparable to the carotid ultrasound at about $200.

It's far better than a carotid ultrasound. I get it that there are challenges with tech skill set & repeatability. But far better than carotid u/s, stress tests, CACS, etc.
Cedars Sinai Hospital in Los Angeles offers CIMT. How might a consumer develop a confidence level in the quality of a CIMT provided by various providers?

It's a good question. I've seen that on the web. I've not used them. So I'm not clear on their skills. The specific question you bring up is a very good one. Unfortunately, I don't have a great answer at this point. I need to think more about this. Obviously, if I were able to get some patients and review the services & reports. Speaking with the techs & their supervisors would be critical.

I just checked Todd's website. It states it is proprietary.
Interesting video Doc! Question: Given that plaque builds up between the intima & media layers of the arterial wall, what keeps the intimal lining from rupturing & still contain the plaque? Also, I'm located in CT. Do you know any lab in the New Haven area that can perform the CIMT testing procedure? Thanks for your videos..............

I forgot to tell you. Call Heidi or others at CardioRisk. They have a tech in New Jersey routinely. They may have someone that can take the train up.
