    Exactly, I'm a phsio and I give these exercises to my OA patients. I have had read the literature and it clearly states that quad strength decreases knee pain for people with OA.

    Ya, I'm confused. Last week I was told by my orthopedic surgeon to strengthen my quads and glutes for my knee arthritis. He gave me a referral for PT and a cortisone injection in my knee. Now I'm not sure who is right. 🤦‍♀️

    I’m with you on this one, Alfred He’s the only one saying, don’t strengthen your quads. But I’ve noticed anybody who’s asking questions he does not respond. He only responds to people who agree with him I still think he has a lot of great contact, but I’m kind of lost on the quad thing I have articular cartilage damage on my knees and I’m trying to find stretches and exercises….anyways very confusing

    @@matterafact what he taught in this video is correct, you only have to study biomechanics to realize it. most therapists are just copy and paste their exercises without much thinking. you need to glutes to kick in to support the quads. think of barbell squat, try to do it with knees rotate in vs out to notice the difference in your quads.

    My orthopedic surgeon told me the same thing as far as the first he said about using the machine where you're pushing against resistance. My Dr said no one should be doing that one. But he didn't say anything about not strengthening your quads. I sure wish this guy would respond as to why he thinks this is bad. Makes me nervous when they choose not to respond.

    @@Chanatxmy doc even specifically told me even if leg extensions hurt you should still get maximum extension. I suffer from an osteochondral defect of 2mm on my lateral femoral condyle and shit gets painful under load.

    I will say from my personal experience. The exercisew that were recommended to me from my physiotherapist are not working for me. They keep telling doing these exercisee will help but it is not helping. I am going to try what he is saying.

    These are all the exercises I've been told to do for years!!! And yes they have made me worse. Glut strengthening are the best medicine!!!

    @Valerie Becker you can strengthen your gluts by just sitting and squeezing your butt muscles. I do it while I'm sitting watching TV, in the car or anytime I'm sitting. You can ulternate cheeks. I found that the side I have more weakness my butt muscle is definitely weaker. Good luck and keep watching these videos! He has helped me so much!

    Open kinetic chain (leg extensions) and closed kinetic chain exercises [pistol or isometric squats) are not universally bad to the knees What makes those exercises bad for the knees is not the exercises for themselves but doing them not having a well balanced, coordinated and integrated kinetic chain, where quads are way stronger than hamstrings and glute amnesia on top of that You have to go from corrective and therapeutic exercise to create sound and good motor patterns to then progressively and slowly load them in order to make the tissues more resilient Stability and isolation goes first and then mobility and integration

    So we should just allow our quads to waste away so we become pathetic and frail? No way. The only way to stay healthy as we age is to maintain and increase strength, especially in the large muscle groups like the quads and glutes. Got knee osteoarthritis? You have to keep working these muscles to stay as strong as possible. If you’ve had knee replacement you have to work back to doing all these exercises to avoid becoming frail, weakly and disease prone.

    Avoiding quad work may help with knee pain, but the atrophy and muscle imbalance consequences will be regretted.

    I absolutely hated the ones where you had to lift your legs and hold it while laying down on your back. Between my back muscles and my thigh muscles I didn't know which one was worse!!

    My post-knee surgery is so much better after I discovered your channel for knee rehab! Within days of stopping the cookie cutter version of PT ghersoy exercises and doing yours…I have seen improvement of range of motion, swelling and reduction in pain! Thank you, Sir!!!!

    Pistol squats are the ones I’ve been told to do here in uk by my physiotherapist 🇬🇧. I have bone on bone osteoarthritis and a meniscus tear! I wondered why it wasn’t improving and the exercise was so painful 😖 So glad I’ve found your channel ❤

    I was getting shots for my knees. I saw a physical therapist on you tube stand feet 8” apart we will call neutral. tap your toe forward, n tap sideways,n tap backwards and repeat I haven’t had a shot in two years. good luck ❤

    I'm coming up on hip replacements and I've had knee surgery on one knee. Torn meniscus in both but the other one doesn't bother me anymore. It's felt the same as the surgical knee for a couple of years. I do your glute exercises but I have been doing extensions too. Wouldn't extensions be okay as long as you kept the reps high and the weight low and stretched daily? I do some yoga stretches, the ones my hips will allow anyway. 😂

    Don’t listen to this guy.. start learning your lowerbody muscles/parts and work on ankle/hip mobility.. thats main cause for knee pain. Most people are not flexible and have weak muscles from inactivity

    I lost full extension, which I think resulted in my having to use my quads to stand & walk. After TKR, the Fellow who visited me the morning after surgery pushed my leg down…essentially forcing an extension. Thankful that he did. It hurt! I continued to work on extension by placing my heel on a pillow and letting gravity pull my leg down. Thankful because now my leg is now straight and now I can follow your advice to work on glutes and no longer have to rely on my quads to stand or walk.

    I can't tell if this is sarcasm. Strong quads are absolutely crucial to prevent knee and arthritic pain. Of course putting excessive pressure on the knee is harmful but spreading false information is even worse. No wonder you have a microscopic following. Go figure

    Traditional practice emphasize on quad muscle is very wrong. I'm the victim of deep squat and lunges. Now I only focus on glutes.

    Could you cite your sources? I agree that glute strenght exercises for OA, because most OA patients also cope with weaker hips and/ or lack of hip extension (sometimes because of the decrease in knee flexion, common symptome of OA). Every Physiotherapy guideline/ protocol informs that you that you also engage knee extension exercises, because especially in knee OA the quads gets weaker and stiffer which results that people walk with bended knees and cant fully extend the knees in swing phase + loading response (which actually increases the impact of the knee joint). So why would knee extensions be a bad thing if its helping with the swing phase of knee OA patiënts? And how would you say biomechicanically say otherwise? Not an attack/ critisism. i’m seriously curious in your explination, maybe theres a new study im not aware of.

    I understand what you’re saying, but my quads need worked on. What do I do then I don’t know if I have arthritis I have more of a meniscus problem.

    Hey Gaylalnn! You may be correct that your quads need work as well, however, the more important muscle to concentrate on is usually the glutes. Once the glutes are active and strong, then incorporating quad exercises into your normal routine is perfectly fine!     Hey Rae! Not necessarily! Your quadriceps pull on your patellar tendon, which could impact your arthritis condition if there is not a good strength balance between your glutes and knee extensors.     Hey PepperMD, thank you for your question! We understand that the advice about limiting hamstring and quad exercises may seem counterintuitive, but let's think of it as finding a balance, just like a seesaw. When it comes to knee arthritis and muscle imbalances, the hamstrings and quads tend to be more dominant, like the heavier side of the seesaw. This can put excess pressure on the knee joint, leading to discomfort and potential issues. Our suggestion is to focus on strengthening the glutes and engaging the core muscles, which act as the other side of the seesaw, helping to restore balance and reduce strain on the knee. You can go back to working the hamstrings and quads as soon as you are confident the glutes are stronger. You will know when the pain subsides in your knees but you will need to make sure that the strength in the glutes, hamstrings, and quads are balanced. Of course, we acknowledge that everyone's situation is unique, and what works for one person may not work for another. We encourage you to listen to your body and if you don't feel like focusing on the glutes is going to work for you then do what feels best.

    No disrespect but this dudes a quack… soooo don’t exercise your quads AT ALL and let them waste away, thus putting MORE pressure on thr knee from lack of quad strength?? No thank you… first time I EVER heard an orthopedic specialist or physical therapist say this…

    @@epmanualphysicaltherapy Thanks for the playlist. But the leg extensors being overworked? How? The big majority of people don't do enough exercises to even stay just somewhat healthy. As for me, I need to strengthen all my leg muscles, including the extensors as my upper body is too strong and heavy relatively to the base. This gives me lots of problems with my back, which is weakened due to old injuries and degenerative damage to the spine. I train Tai Chi and Qi gong and also some Kung fu and came to realise that my legs and my core muscles are too weak. I had the knee arthritis under control with turmeric and exercises until I increased the intensity of my training. Now it's bloomed up again, but I cannot ignore the need of more strength in my legs for the sake of my lower back.

    ​@@romedbucher2854 How are you doing now? Want to know what exercises have you been doing to fix the issue. Lower region of the body stores trauma. You practice Qigong so you must know the whole pelvic area can store trauma. Blocked energies. Which blocks movement- thus resulting in tight hip flexors or pelvic floor dysfunction. Target to fix the mechanics of the hip and pelvic joints. I suggest you go for pilates as they focus on balance, which targets the main roots and causes of the body's imbalance and improper movement. Bad posture and bad habits are mostly the reason of the body's imbalance and thus causes so much problems. If you are aware and gain clarity on the subject, of learning the proper and correct method to use our body's mechanics(the right exercises, good flow of movements, good posture, healing blockage of energies, healing trauma, discarding bad habits for your health, balancing of the mind, body & soul, always having a peace of mind and gratitude) all these underlying issues will be solved. You have to understand and be clear on what a certain exercise is targeting and how if it’s helping you strengthen your muscles or are you straining it. Be conscious of how your body is feeling and going through so you can target and address them correctly and can avoid unnecessary troubles. Don't blindly follow what anyone's teaching you to do. Try the exercises with an open mind, do your research, and note if these exercises are improving your body's health state or your feeling some sort of discomfort. Remember, you're recovery journey relies on your clarity on the issues, and always be in a confident state that you have healed and you're healthy and strong and much better than before. Always be positive and stay strong.

    I have No pain but my knee will not bend enough to go down stairs properly,told by one doc I have small tear in cartilage told by spealist I have a lot of arthritis full knee replacement me thinks Not will keep researching ,anyone have similar problem with No pain just refuses to bend

    literally the worst advice ever. Do you want everyone’s quads to atrophy? Research has proven there is a direct relationship between quad weakness and KOA. None of these exercises are bad if progressed APPROPRIATELY and safely. Horrible advice.
