Key death data from EXCEL trial of stents and heart surgery held back - BBC Newsnight

Doctors working on a clinical trial for treatment of heart disease held back key data, Newsnight has been told. Subscribe to our channel here: goo.gl/31Q53F
The Excel trial tested whether stents were as effective as open heart surgery at treating patients with a heart problem called left main disease.
The data suggested more people fitted with stents were dying after three years.
It was eventually published - but only after treatment guidelines that partly relied on the trial had been written.
These guidelines recommend both stents and heart surgery for certain patients with left main disease.
The authors of the trial said it was carried out rigorously and to accepted academic standards.

    It would be great if the animation of the usage of a stent would be realistic. Usually a stent opens the diameter of the blood vessel by pushing plague aside and not down the blood vessel.

    Peer reviewed medical journal articles more often than not do not reveal the raw data to the reviewers. This is common place in the US. It needs to change.

    This doesn’t explain anything weather patients changed lifestyle or if stents failed it just says stents are bad

    You have to identify the two different stents as in bare metal and drug eluding stents. The latter cause more inflammation.

    Big Money Incentives for Stents: In my friend's case, angina, not a heart attack brought him to the hospital. The doc took a video x-ray of his heart called coronary angiography. He was shown a graphic of the closed artery with just enough pixel definition and clarity to see the main artery and not new tiny blood vessels bypassing the semi-closed vessel. Since he cycled 16 miles without discomfort the day before this graphic was made, he likely didn't need the stent because his body made tiny blood vessels around the blockage creating a natural bypass. In his case the informed consent form was given to him on the table just before the stent insertion procedure when he didn't have his glasses. The fact that he was exposed to prolonged x-ray twice one for the angiography and one for the stenting procedure was never explained. Never again. We no longer trust allopathic cardiologists. Most of them are in the dark ages insisting eating high cholesterol foods cause heart vessel blockages from high cholesterol in the body. This is an example of non evidence based medical ARTS being applied instead of real statistics driven science. We are told half truths. This is unacceptable and something needs to be done to make it right.

    Somehow a petition must be made to inform the patient of the heavy load of medications required for a long time after stenting. The doctors avoid giving the information before the procedure and then load it on the patient after the stenting when it is too late. My otherwise healthy brother who I feel was actually suffering from hiatal hernia acid reflux that induces heart pain or discomfort was lured by the doctor via angiogram reading claim of arterial blockage. A leading Indian heart specialist has come out with the information that our bodies create by passes on their own and neither bypass nor stenting is required for blocked arteries. He says heart attacks are due to distress in the mind, anger and other negative emotions the person cannot handle and no case really exist of death from a blocked artery. Within a month, my otherwise healthy brother with tons of energy has been turned into a frail thin wasted creature via 8 deadly medications, which include blood thinners, immunity depressors, statins and what not and he cannot even remove that stupid unneeded stent. Where are these evil money worshipping killer allopaths think they will be going in their next lifetime. Obviously they will end up as slaves in some CRUEL civilisation in some weird planet out there in our Universe. There is little we the public can do for the present as ads for their paraphernalia are food for the media who also are not on the side of the public but lean over willy nilly to big business and Pharmacies.

    When I was accepted for PhD's with a full scholarship, I was told not long after the Tories got in that they got rid of all funding for all research across the UK. A professor told me to ask for private companies to sponsor me for the other half of the funding I couldn't pay for as a single mum. I said no because these companies would have only wanted me to do what helps them sell their products and I would be seen as a joke in the community therefore ruining my career. So now these researchers are jokes and have their careers ruined. We need funding for unbiased research so those of us who love research can actually improve the lives of others!

    There are serious concerns about the presentation and interpretation of these clinical trials. The best they can do is to release to the public all trial data for an independent analysis.

    I’ve had 5 for 7 years. Still running my 5k’s and weight training. I may drop down dead…. but I would have if I didn’t have them and there are LOTS of people out there walking around with plaque undiagnosed….

    This video is about the integrity of trial reports. Not about Stent or By-pass surgery. From the video it seems By-pass surgery gives a better outcome than stents. Both are acceptable treatment for blocked heart arteries. Each patient has to decide for themselves, between major invasive surgery or a minor day surgery. The attendant risk are as varied as the patients.

    There is no such thing as key data. All data is vital and must be applied in a person centred manner. Data interpretation can differ depending on each individual biological system. Correlated data is a guide, use your brain. Extended patient monitoring is a minefield. Results become less relevant as time progresses. Were the people with stents continuing an unhealthy lifestyle that led to the procedure in the first place?

    What responsibility will be taken by the people involved in hiding this data? Why has it taken this long to come to light? No one involved had enough concerns to have blown the whistle on this trail and consequent guidelines?? Disgusting!!! This raises far more questions than it answers. Most significantly how will this change the practices used in heart care going forward?

    I feel the love for your brother and the desperation in your tone. I hope he got past the shock and started putting on weight again. I had a HA 7 years ago at age 42, I had 5 stents inserted. I wouldn’t be here without them. As a very fit half marathoner, footballer and guy who is on good shape ; I do agree that stress is a a big part of it (working too hard and also had a young family - so no sleep!), genetics too and when I was younger I smoked a bit (stopped before 30)and the usual social life. After my HA - the shock made me lose weight; I went from 12 and half stone to just over 10; I have now put it back on. I run my 5k’s and weight train , walk miles, eat well- no pain. I take Statins, aspirin and blood pressure tablets- the standard. I also supplement K2 and D. I think your comment are naturally biased- I think the truth is somewhere in the middle. I hope your brother has now returned to full health.
