Sacroiliitis is inflammation of the sacroiliac joint. The sacroiliac joint is one of the joints connecting the low back and hips. There are several different causes of sacroiliitis, from pregnancy-related sacroiliitis, to autoimmune-related sacroiliitis from arthritis (various types). Sacroiliac Joint Dysfunction is the most common diagnosis for sacral pain. Poor mobility, stability, or strength causes dysfunctional movement in the pelvic/ lumbar spine hip complex creating stress and compression. Recovery to activity typically occurs in weeks to months.
Yet, we can not blame Sacroiliac Joint Dysfunction for all sacral pain in the world. Sacroiliac Joint Dysfunction tends to localize into the SI joint region and lower back… so what about those who experience aches and pain in the gluteal crease?
We will cover that here.
In this article, I will cover all you need to know about sacrum pain. Included are causes, other possible diagnoses creating your sacral pain, and some treatments that work!

Let’s start with where your sacrum is.
Your sacrum is the prominent bone at the very bottom of your lower back. Shaped like a triangle, this large bone of the spine forms the cornerstone of your pelvis. On each side of the sacral bone are the ilium bones. Together with the ilium and the sacrum form the sacroiliac joints (SI joints).
Your lumbar spine (lower backbones) sits on top of your sacrum, below the coccyx bone (tailbone) is attached. The S1 intervertebral disc sits on the top of the broad sacral base. On top of the S1 intervertebral disc sits the lowest spinal bone called the L5 vertebral body.

Who Gets Sacrum Pain?
Pain in the sacral region starts typically around the age of 20, but you can experience it when you’re older as well. It tends to begin around the lower back/ SI joint region before it travels down the gluteal crease and sometimes in bony areas of the tailbone.
Note: Numbness into the region between your legs can be from another severe condition known as Cauda Equina Syndrome. You can read more about it here.
Sedentary and deconditioned people experience a deep tailbone and sacral ache more than physically active people. Sitting seems to be the most substantial associated factor to those people who develop it.
Yet, active people are not exempt from having pain in their sacrum, though.
Runners, cyclists, and weightlifters all can suffer from SI joint pain, low back stiffness, tailbone pain, and glute ache.
We will take a deep dive into common symptoms in a later section.
Causes of Sacrum Pain
Sitting seems to be the commonality with people who experience sacral and tailbone pain (aka coccydynia).
Sitting places pressure on the tailbone and lower back, tolerable in small doses, yet when sitting dominates your day, the pressure will build up.
Most of the time, your body can handle tolerant the pressure associated with sitting. Pain is your body telling you that it is being compressed and needs a break.
The most straightforward treatment is removing or modifying the triggering activity: sitting.
There are a few simple ways to modify sitting.
Sitting on the end of your seat
Replacing sitting time (going for a walk)
Adding lumbar support to your chair
Changing the seat completely
You do not need to make these lifestyle change forever, but it may be the most straightforward remedy for a few weeks.
Just like an engine that has overheated after an intense drag race, removing the trigger (redlining the engine) for a short period is enough to allow your engine to function correctly again.
The deep ache in your sacrum and tailbone is no different. The best long-term plan may be to optimize your body so sitting is not triggering (AKA optimizing your engine cooling system).

    Over the years ive had many injuries, a few neck bulging discs, torn meniscus both knees and they've all resolved. But this Si pain is the worst. It has gradually gotten worse. My pain switches sides a lot. Im going 11 years like this. I dont know if i can last a few more years like this. Sometimes i feel like quitting on life. This pain is a living hell.

    This SI pain is horrible.Its certainly a different pain that the 2 herniated neck discs I had replaced recently.This is a scorching type a ache.Mine is also undulating pain.

    @@jenkemjones68 sir I recommend you go to the chiro for the Si Pain. After 5 chiro visits and adjustments my pain is almost completely gone. About 95% better. I haven't gone back in 4 months and I feel great. I'm sorry your going through this. Stay strong man. I sympathize with anyone who has pain.

    @@indigenousamerican3148 I haven't been to my chiro in a while as she couldn't do much for my 2 herniated discs a few months ago.But she tried.The neck is great now after surgery.I will revisit my chiropractor for this though.I can't afford another surgery as I have to work for my fam.Chronic pain is a depressing imprisonment.Thank you for the quick reply.I hope the best for you and your family also.

    Check out Dr Brooke Goldner. Her protocol eliminated this for me after being housebound for almost 2 years. It only took 9 days and it was just gone. Results are very typical.

    I've been injured for 5 months now, I'm a soccer player and I don't want it to ruin my hopes of making it, I've been stretching but the pain seems to be worse afterwards. It doesn't seem to be going away, I got injured in the same place a few times while trying to play with with injury, I got diagnosed with it almost a month ago.

    Hello, walking, sitting, general exercising is usually pain free for SI joint. I have sharp pains in SI when laying on back and getting up. Laying on my side is usually ok, but laying on my back is the problem, particularly on both sides of the tail bone.

    I overstretched my back 6 months ago for 2 days doing some work bending down. Then next day, my back was sore and swollen I couldn't move too much. Then I decided to go to my chiropractor on the right side he tey to adjust once and it didn't popped out then he tried one more time harder that time and oh my! Next day I couldn't breath because all the spasms on my right side, since then I can't recover and I've been in physical therapies for 4 months now. And the pain comes and go. When it comes just by twisting or even doing laundry it keeps me a whole.month on bed. They tried dry needling at the therapies but I had a bad reaction so they won't do it again on me then cupping but doesn't work to much. I'm getting depressed thinking that this might be permanent, I'm thinking going to my orthopedic and see what's going on, 😔 this changed my life completely. My left side is amazing and strong and I'm so scared to go to the chiropractor again, what can I do?

    I'm not sure if that's what I have. I have a dull pain/sensation. Right above my glutes and my knees hurt a bit too. I workout a often; weightlifting. I notice I feel it when I do a barbell RDL. I tried today to really focus on keeping my lumbar in the right position, but maybe I'm doing something incorrectly. Again, it's not the lower back, but I feel it right at the top of my glutes. The lower back probably has something to do with it, because when this dull pain went away months ago, so did my knee pain. So I'm trying to figure this out again. Should i look more into the SI joint? Piriformis?

    Can sacroiliac joint pain be caused by a fall? In 2020, I was going down our stairs, which are hardwood and I was wearing socks…well, I slipped and my left side where my lower back and butt meet squarely hit the lip of the stair. I have a bulge there now that I can move around and it’s painful. I’ve seen countless doctors that won’t give me the time of day about it. Best they’ve done was an L5-S1 x-ray. But the way you described undulating seems to fit me best. I have an appt with an ortho next week and I’m gonna beg for an MRI.

    I have a SI tear labral acetabulum upper tear flattened right troc head thickened?widening it my lower femur bone above the knee. Yeah I had a MRI with terrible contrast. I was in a head on car accident at a stop sign. The guy's dog jumped on his lap he had no stop sign it was a weird intersection. My hip hurt immediately but the xray showed the ER nothing broken I just kept blaming the seat belt anchor and kept on it for 9 months until I couldn't lift my foot it hurt terribly bending groin walking just no movement or laying on it. In PT I started to get worse inflammation my leg was bigger than the other so big I went a size up so I can get my leg in. My PT guy told me I need to go to the walk in I said no...so I got my MRI and today I got a shot in labral tear. The doctor keeps bringing up surgery I pushed for another round of PT. The guy is really tough he blew me off when my pain before I was diagnosised and said it was in my head that I just had a bursa on my IT band. He was really wrong and 2 months later the inflammation is still there. I don't trust him. Do you have suggestions?

    I felt shock in my left SI joint last February (2020) while lifting a 15 & 17.5kg dumbbells (Bending at my hips).. I took a break from gym for 45 days and was back with exercising at home after that. In May, I felt sharp pain in my lower back while skipping rope. I took a break from workouts and started walking. I used to get severe groin and buttock pain along with lower back pain. I continued walking and gradually started jogging. Everything was fine. But on October 28, 2020 I moved out of city and during shifting process, strained my back. Since then my back pain has not reduced. I visited doctor, he said I have minor slip disc. After a month, he said it is sacroiliitis. I got Xray done, it shows everything to be normal. I get pain in lower back only when I bend to lift weights. I also get pain in left iliac crest when I touch it. What is the problem with me? Please help! :( It is very traumatizing.

    Hello! Wondering if could help me out real quick. I have been having pretty bad SI joint pain recently. It seems to come in waves. It will be really bad for a week or so then be ok for a month or more. It always starts by waking me in the middle of the night, which can be excruciating. It gets better with activity like walking, and is always worst in the middle of the night. I have seen a Rheumatologist who did an xray. Recent blood work has been done for ESR, CRP, and other inflammatory markers that were negative. Negative test for HLA-b27. Just did an MRI when I was having no pain for a while, and it came back negative. It just started acting up again. I have been in physical therapy for it for a few weeks now. What do you think could be the issue? Could it still be inflammatory, such as ankylosing spondylitis, and just not show up on any tests yet? I am concerned because of the morning pain.

    I just got diagnosed with this but it just happened over night! I don’t have arthritis at all (I’m 29F) but I just woke up and went to ER and he gave me Percocet Valium and an anti-inflammatory. I hope this goes away. What brings it on?? He said it is possibly viral??

    Can you get this without having an auto immune. I had it so bad. It lasted 5 weeks. I laid on the bed on my stomach with my feet on the floor for the first couple of weeks and was able to get on the bed on the right side only. It was on the left side. My sister doesn't have an autoimmune, I have RA. She said that she had had it 3 times and she was able to sit and lay as normal. I've never had something that hurt so bad. The first day that I had it, I threw up all night because the pain was so bad. I can't take anti-inflammatory. It it it a long time to go away. Now I have learned that it will keep coming. I am making myself tea using turmeric roots and go get and honey. I am hoping that it will keep that inflammation away. I am so afraid of this returning. I do battle RA and I try to keep it from flaring to bad. I do go to pain management. I got a shot on each side of my back to help treat the sciatic nerve. In the meantime because of the terrible pain, my blood pressure went up high. I had a terrible time during this first episode. My pain management told me that because of RA that it will continue to come. And so did a couple of subscriber's on KZread. That is terrible news. So my question is can you get this without an autoimmune. Can you get it with is to arthritis

    Aswell as acetabulum impingement i had accident 29 years ago despite telling drs over over again i finally had to pay privately out of my benefits.mine is dulating just walking up slight slope put it out after physio.

    Hello, my name is Maria. I’ve had back pain for many years now, but, a year ago, I fell off the ledge of my bathtub , no sacroiliac pain right away, but, about six months later, started having severe pain, while getting up from a chair, so, I saw pain management, pointed exactly to where my pain was, and, he said it was sacroiliac issue, and, I had a cortisone injection done, it helped, but, a few months later, I picked something up off the floor, with my right foot( yes, my toes) and, bringing my foot up toward my left side, I reached and slightly twisted, and, felt the jolt of pain, IMMEDIATELY, and, it puts me back to where I started( can’t sit for long, or stand, lots if tenderness and tightness) don’t want another cortisone infection ( have osteoporosis) what do I do?!!!

    Hi. I’m 22. 6 months ago I had an injury while doing a deadlift. Didn’t know what it was. I kept on doing everything normally. As a college student I spent 6-7 hours on the computer chair with some bad posture. I also went on motorcycle rides which made everything worse. 2 days ago during a football ⚽️ match it got extremely bad and I had to stop playing. When I came back home I couldn’t even move my lower back. I went to a doctor the next day, it was a lot better but still painful and hard to walk. He diagnosed me with Sacroiliitis after an xray and gave me some muscle relaxants, pain killers and anti inflammatory gel. Now I feel good after a nights sleep on the ground but the moment I do something like tying shoes, car rides etc. the pain becomes extremely intense and I have to lie down again. How do I get rid of this pain. Thank you

    Hey dr plz help I’ve had lower back pain for over 10 years ,X-rays show nerve impingement of s1 s2 degenerate end plate a narrowing of a disc but recently the pain rotates from left to right I’m not able to walk stand or sit the pain only slightly goes when I lay down ,I’m constantly urinating then I’m paralysed for almost a week the pain feels like it’s to the left n right of the spine not in the middle that’s what made me think it’s the sacro joints I dnt know what to do I feel like ending my life that’s how bad the pain is plz help ❤️❤️❤️

    look I have a very strange case so far. I'm 20 years old and I have a herniated lumbar disc that caused me sciatica. But at some point in my trajectory the pain changed and my pain focused on my sacrum, I thought this was normal, but when I went to the doctor they did an MRI showing a high degree of sacroilitis but after blood tests the possibility of rheumatic causes was ruled out . What can it be?

    You are young and I think the disc herniation was a result of your sacroiliitis due of sciatica that is also one result of the same problem we don't understand the source of our pain until it gets worse.. Only then we realize it's sacroiliitis.

    I have pain at the top of my leg. Painful when bending over and cannot lift leg to out on sock or pants. Pain is worse with sitting/lying down but much better when up walking and moving. I just finished therapy for “tight hip flexors”. My leg is stronger but pain in hip, low back and top of thigh/groin WITHOUT improvement. Also pain around my knee and pain that extends down both side of my leg to my foot! Any ideas about what is going on???

    I'm an 18 year old female who has had this SI joint inflammation for almost a year now. I've been on physical therapy for almost a year now for it too and I just can't see it ending cuz every shot, stretch, and less stress applied to the joint has been only temporary........I almost want to start crying thinking about it because it has been so painful. I don't know if it will be permanent or not..

    Hey Jimmy. That’s a good question, but without knowing the specifics of your situation, I couldn’t tell you what a good answer would be. I can tell you from my experience a lot of people with SI joint pain have lots of different nonsurgical remedies that tend to work very well. In our office, we typically use stretches and a series of exercises to help improve stabilization, which helps out to also improve pain levels.

    I have the same disease, and same problem. Sciatica is horrible. It's ok if I'm just standing, but the second I lift my foot off the ground to take a step, and all the weight is off my leg and it's just hanging by itself, it like someone is stabbing me DEEP with a hot knife right in the top of my buttcheek. It switches sides from time to time, but has never been both sides at once. (Thank God). It comes and goes... I can be fine for 3 weeks, then all of a sudden, BAM! Out of nowhere it strikes again.... Sometimes it gets so bad that I'll be bed ridden for a week. I've gotten somewhat used to the pains that A.S. brings, everything but the Sciatica and the Uveitis that comes along with this disease... 🤷🏻

    Hi, I injured my SI joint lifting and twisting my body putting an air conditioner in the trunk of my car, I didn't feel any pain till 3 days later, it's my left side. I'm 57 year old male. I'm on Meloxicam 15 mg. one a day, I ice and heat throughout the day. It hurts mostly when I first get out of bed once I'm up and about it's subsides a little. I've been out of work for 3 days, I go back tomorrow, I work in assembly, light lifting and twisting. Am I on the right track, Meloxicam, ice and heat, should I stay off work longer? Could I do more damage going back to work? Thanks for all your help.

    Hi Mike.We have the same story.I carried an 85 lb bulky AC up a flight of stairs then 2 days latter I'm in bad pain from left SI joint.Its been hurting for 10 days now.I will see a Dr this monday.Did your condition ever improve?I'm very worried because I am out of work and I have family to take care of.Hope you can reply soon.Thanks

    Pl advise how to treat the pain, I'm a triathlete and pain increases after the Run since three months. I took antiinflammatory did core strength exercises and stopped running for two months but after next Run it bacome crazy 😭😭😭

    Thank you so much for your informative information at least now I know where I stand, mine came from sitting in a very low old chair for around eight days in the same position for 7 to 8 hours a day even though I walk a little it was still an old chair and a very odd position . I woke up one morning and I had pain in my sacrum that went around to my groin my hip area that pain went away but now 3 days later I get it when I lay down in bed on my back only my side is fine when I roll out of bed when I get out of bed and walk around it goes away so I have to find the right treatment to get that pain or get that position pop back into place so I can live my life again. The pain in my groin has one away the pain in my hips are going away now it's just at the top of my buttocks and my sacrum bone alone.

    I have A.S., and S.I. pain can really get severe at times.... I dealt with it for years before it finally basically crippled me, and I went to a rheumatologist and found out I had advanced A.S. I WISH like hell I had went to a Dr. long before I did, because the advance of A.S. could have been greatly slowed down before it got as far as it did for me. The damage done by A.S. is permanent, and there's no way to fix it with surgery or medicine once the damage is done. If you have ongoing pain in your hips, ribs, back, S.I. joints, DON'T just keep dealing with it and putting off a trip to your doctor. Be firm with your doc that these aren't just normal, average, pains and get something done about it before it's too late. Now, at age 50, my bottom 5 disks in my back are fused, all of my ribs are fused and I have zero chest expansion, my hips and S.I. joints are very painful, and I'm very limited in what I can physically do anymore. If it's caught early enough, A.S. can be greatly slowed down in it's progression, and you'll live a much more pain free, and less physically limited life. DON'T WAIT to go in if you're experiencing these issues for longer than several months of constant pain. I waited over 10 years, and I regret that decision GREATLY!

    I have bilateral asymmetric sacroiliitis, I've had it since I was 16 I'm 50 now. 8 years ago a board of rheumatologists said there was no inflammation and that my condition may have been congenital. I have grade 4 on one side and grade 2 on the other. I consider my condition to be chronic pain. It seems pretty permanent to me

    I have pain in my lower back, buttock and groin area when walking, sitting or lying down. The pain sometimes goes down my leg to an aching pain at the back of my knee.     When you talk about undulating how long are we typically talking about the burn because my pain comes randomly lying down and it kicks in? The burn out of the pain how long we talking a couple of hours days weeks? My pain seems to come at random once a day and it’ll be a deep dull achy feeling, my pain goes away after 30 minutes every time would you say that is a good sign? Also to mention im self diagnosing myself here my physio told me a I have a herniated disc that’s mimicking si joint pain is this true? I’m not very familiar with that as I’ve read online that finger Fortin test is accurate reeding of showing where the pain is, I get knee pain when standing with flexion and extension I get tingling in my foot sometimes. 1 last important thing I’d like to say is my physiotherapist did provocation test at least 4 different version and all came out negative had no pain at all very lost here.

    Yes running is too bad for sacroillitis ,me too my sciatica-like symptoms started after 18 months of running regularly . I wish I didn't run. At least the inflammation could maybe progress slower .

    I've had Sacroiliitis since I was 12 years old I'm now 34 and also suffer with slipped disc with some impingement. I now also suffer from Complex Regional Pain Syndrome in left ankle for over 7 years now. Had injections put in my SI joints and still no relief.

    I have had severe pain in my buttocks, hips, legs and I’ve been to numerous doctors. I’ve been told I have numerous things and they have all said pain will subside. It has now been OVER a YEAR. I’m 19 years old, use to be very active and now have this debilitating ache. I am taking ibuprofen every 4-6 hours and pain wakes me up from sleep and is NOT easy to live with. This whole experience has been so traumatizing and I need help. I’ve done PT, tried numerous stretches and have tried resting. Seems that heat is the only that that helps reduce pain. Please help me. Please. I can’t do this much longer. It has been a year of Advil and no more than 5 hours of sleep every night. Please please please help me.

    I thought i might have Sacroiliitis, But maybe i’m wrong? I spend many hours sitting per day (8-12 maybe?). When i sit down, it hurts quite a bit around the tailbone/butt region, and same when i get up. This started a few months ago and i think its gotten a bit worse. Even in cushioned chairs, it happens. When i lay in my bed though, im fine. Does anybody know what it is?
