Sacroiliac Joint Dysfunction is the most common diagnosis for sacral pain. Poor mobility, stability, or strength causes dysfunctional movement in the pelvic/ lumbar spine hip complex creating stress and compression. Recovery to activity typically occurs in weeks to months.
Yet, we can not blame Sacroiliac Joint Dysfunction for all sacral pain in the world. Sacroiliac Joint Dysfunction tends to localize into the SI joint region and lower back… so what about those who experience aches and pain in the butt crease?
We will cover that here.
In this article, I will cover all you need to know about sacrum pain. Included are causes, other possible diagnoses creating your sacral pain, and some treatments that work!

Let’s start with where your sacrum is.
Your sacrum is the prominent bone at the very bottom of your lower back. Shaped like a triangle, this large bone of the spine forms the cornerstone of your pelvis. On each side of the sacral bone are the ilium bones. Together with the ilium and the sacrum form the sacroiliac joints (SI joints).
Your lumbar spine (lower backbones) sits on top of your sacrum, below the coccyx bone (tailbone) is attached. The S1 intervertebral disc sits on the top of the broad sacral base. On top of the S1 intervertebral disc sits the lowest spinal bone called the L5 vertebral body.

Who Gets Sacrum Pain?
Pain in the sacral region starts typically around the age of 20, but you can experience it when you’re older as well. It tends to begin around the lower back/ SI joint region before it travels down the butt crease and sometimes in bony areas of the tailbone.
Note: Numbness into the region between your legs can be from another severe condition known as Cauda Equina Syndrome. You can read more about it here.
Sedentary and deconditioned people experience a deep tailbone and sacral ache more than physically active people. Sitting seems to be the most substantial associated factor to those people who develop it.
Yet, active people are not exempt from having pain in their sacrum, though.
Runners, cyclists, and weightlifters all can suffer from SI joint pain, low back stiffness, tailbone pain, and glute ache.
We will take a deep dive into common symptoms in a later section.
Causes of Sacrum Pain
Sitting seems to be the commonality with people who experience sacral and tailbone pain (aka coccydynia).
Sitting places pressure on the tailbone and lower back, tolerable in small doses, yet when sitting dominates your day, the pressure will build up.
Most of the time, your body can handle tolerant the pressure associated with sitting. Pain is your body telling you that it is being compressed and needs a break.
The most straightforward treatment is removing or modifying the triggering activity: sitting.
There are a few simple ways to modify sitting.
Sitting on the end of your seat
Replacing sitting time (going for a walk)
Adding lumbar support to your chair
Changing the seat completely
You do not need to make these lifestyle change forever, but it may be the most straightforward remedy for a few weeks.
Just like an engine that has overheated after an intense drag race, removing the trigger (redlining the engine) for a short period is enough to allow your engine to function correctly again.
The deep ache in your sacrum and tailbone is no different. The best long-term plan may be to optimize your body so sitting is not triggering (AKA optimizing your engine cooling system).
We can discuss this in later sections.
Symptoms
Symptoms around the sacrum and tailbone vary from person to person, based upon their actual diagnosis/ root cause. Here is a list of what many people experience:
Pain that improves with walking
Symptoms increase with prolonged sitting
Symptoms increase with bike riding in an arrow position, better when upright
Needle-like feeling on the sacrum
Deep ache in the tailbone
Sharp sensation on tailbone with sitting or pressure
Lower back pain or stiffness
Tight hip flexor
Hamstring tightness

    My consultant dismissed SI joints as the MRI showed nothing. I have a nerve conduction test and it came back negative from the spine. I get all the symptoms you mentioned in the video. If i place my hands on my hips and my thumb on that area right over the SI joints either side I get sore pain, tingling running into my foot and a mild sciatic shooting pain in my buttocks. It goes as soon as I release my thumb from the SI joint area. I struggle with sleeping especially laying on my back or sitting on a hard chair. I think the compression is compounded and irritating my SI joints and the l4 l5 nerves that pass by them. That’s my own diagnosis, 2 consults, 2 MRIs and 3 physios later. It’s been nearly 18 months.

    @@Unclebubba1 I had an SI joint injection which did help, but unfortunately it didn't solve any issue longterm, it just gave short term relief. Especially since I was doing all the wrong exercises when I had the si joint shot since the PT i had at the time was having me do the wrong exercises since he thought it was a disc issue, but now i'm seeing a different PT and they diagnosed me with an Si joint problem and it's crazy how much better I am!

    History of spondylolisthesis L5-S1 discovered at age 19 in 1973. At 20 had difficult childbirth, was told I "broke my tailbone" and it was removed 11 months later. Have had severe sitting burning pain since the childbirth as well as following surgery which has continued to this day. Currently experiencing sciatica or SI joint or piriformis syndrome or hip problem; no one has made a definitive diagnosis, but PT flared it so bad on two separate occasions, I won't go there again. Any thoughts?

    I love your videos. Can you make a video on an issue my friend is going through? She had an ankle sprain years ago. But it is mild swollen at all times, plus when she does strenuous activities it radiates up to the legs, thighs and hips. Also she get burning sensation. What could be the reason to that?? Is it peroneul tendonitis or trigger points in fibularis muscles and glute minimus?

    Hello! I’d love to make a video about that. However, it may be a long time before it releases. We are scheduled out with two falling videos, and three shorts a week until next December. If she needs any help in the meantime, have her reach out to us, as we do virtual sessions as well. We have seen some cases like that. But when it gets to the point of burning, sometimes you need to take the whole limb into consideration, as you start to rehabilitate it. Simple ankle turns tend to be a local tissue issue but as things to get a little bit older in chronicity, and we need to consider limb as well.

    Can you tell me what it would be if it’s nerve based also? I have had an mri and it’s shows no nerves being compressed directly from the spine; however I do have L3 and L4 disc degeneration also inflammation of facets around that area but the nerve sensation comes and goes for a few weeks. The soreness is around my si joint, buttocks and I also get nerve pain numbness in my outer thigh and sensation in both feet not at sane time.

    Ive had pain and fatigue in my low back I assume SI area into my groin. Mostly on the left side, but sometimes on the right. I’ve put up with this for years. Ide love to get rid of it.

    I have both pain near groin and numbness and shooting pain to my feet, my mri said l5 s1 disc degeneration but im not sure if that’s causing both problems

    Hello, if the pain is caused by nerve root does that mean I should feel pain in my vertbrae area? Because I have pain that radiate down to my foot and most of my pain is felt in the sacrum area before radiating downwards

    Thank you. Your video is really helpful. I am a 57 yo normally active, healthy male. Until recently I was still playing soccer & tennis but I have been suffering severe chronic hip, leg, foot pain & numbness for several months. I cannot stand or walk for more than 30 seconds in the afternoon/evenings. This includes tingling of the instep, calf & toes. Your video has convinced me that my problem is not solely down to my mildly arthritic si joint.

    @Worldsbiggestlonerunfortunately not. I have have a 2nd L5 nerve block which was useful diagnostically (as it temporarily shot off my symptoms). This treatment also included steroids. I’m currently undergoing regular physiotherapy. I’m going to give myself until March 2024 and if I’m not healed by them I’m going to have surgery…

    I am 55, was did Pilates 4 times a week walked an hour daily and then fell a year ago. I have experienced low back and radiating leg pain. I’m having difficulty walking now Chiropractor said SI joint issue, Osteo Dr said sciatica. You mentioned nerve treatment in this case. What does that mean?

    Taking pressure off a nerve is the name of the game and then slowly subjecting it to loads so it can become more tolerant (if it is intolerant in the first place). There are simple tests that a skilled practitioner can perform to see what your nerve needs and to suggest a non-surgical program. Why the nerve has pressure in the first place may vary.     How far up in the front can it go? Anywhere near the belly button? I have pain in the low back, near the side and into buttocks, all pain stops right at the knee, but I also have little twinges of pain around the pelvic area in the front. Can this be SI?

    About a year a go i went to physicsl therapy for SI Joint problem. This year I had an MRI and the doctor said that he didnt think the joint looked bad. Sent me to a pain clinic and that doctor said I had an SI Joint problem and gave meloxicam and sent me to a spine clinic for adjustments and it helped for 2 weeks. Now its really awful and i dont know how to fix it. I have an appointment for a cortisone shot in 2 weeks. Its all on the right side and its hard to work all day. Im on a 3 hour work restriction. Pain is in the front of my knee now and bottom of my foot too. Ugh, im so tired of this.

    @@BarbTodd Yes, I tried that plus I've lost some weight. Still taking Meloxicam. I'm doing better. I just can't sleep in the positions I used to. After a long day, when I lay down, my whole leg just throbs until I fall asleep. I wakeup and it's fine.
