Intracranial Pressure is the pressure within the intracranial cavity also known as the skull. It is determined by the volume of the three contents: brain tissue, cerebrospinal fluid and blood. Intracranial Pressure is not always the same, and changes due to several different factors. For example, the intracranial pressure will be different when lying than it would be when walking.
It is measured in millimeters of mercury (mmHg) and the normal level of ICP in a healthy adult that is in a resting state is 7-10mmHg, mmHg when upright.
Since the cranium is like a closed, fixed box, none of these are compressible or expandabe
Pathophysiology
Small increases in brain volume do not lead to immediate increase in ICP because of the ability of the CSF to be displaced into the spinal canal. However, once the ICP has reached around 25 mmHg, small increases in brain volume can lead to marked elevations in ICP.
There are different stages of Intra Cranial Pressure:
Stage 1 is defined by Minimal increases in ICP due to compensatory mechanisms are known as intracranial hypertension.
Stage 2 is defined as: any change in volume greater than 100-120 mL would mean a drastic increase in ICP.
Characteristics of stage 2 of intracranial hypertension include compromise of neuronal oxygenation and systemic arteriolar vasoconstriction to increase Mean Arterial Pressure which is referred to as: (MAP) and Cerebral perfusion pressure which you will see abbreviated as (CPP), This is the pressure of blood flowing to the brain.

    Hello! I am a recent new graduate nurse that will be starting in the surgical ICU next month. I was browsing through inspirational youtube videos to help me focus on the positive tips that would help me with my new career when I stumbled upon your channel. WOW!!!! I cannot thank you enough for sharing all of your experiences and advice. You have taken some of my greatest fears about the upcoming challenges I lose sleep over every night. I have to admit that I was getting pretty intimidated after reading post after post on my allnurses webpage about how new graduates aren't cut out for ICU etc.. I feel like your encouragement and genuine interest in helping others be successful is very much a talent, moreover what probably makes you such a phenomenal nurse. I appreciate your videos and I just had to say how much you have inspired the confidence in me to move forward with my endeavors as a critical care nurse. THANK YOU THANK YOU!!!!!!!

    I suffered from this for years causing daily headaches and major migraines. Doctors treated me for migraines using various medications. None worked. Finally a doctor tested my spinal fluid pressure and bingo. He did this on a whim cause it is so rare in males. He put me on an anti-seizure (retired from its traditional use) for a month and a half. I got better and no longer have to take the meds. He said if it comes back we can treat again. It's been over 2 years and finally know what not having a headache is like. People need to realize it can affect men.

    Hi Caroline. I am grateful that I found you and your videos. I am in an e-line program that has very little instruction from a professor. We are basically teaching ourselves everything. We do have live clinicals but even there we are expected to know everything the face-to-face students know. I say all of that because your videos have helped with knowing how to study and just hearing the medical terms and medications is so helpful. Thank you.

    Last year I experienced this icp not because of anything, improper intake of food...my hemoglobin level decreased to 7 and severe headache that led to double vision..my brain liquid level increased up to 25 mmhg..and finally doctors removed that excess liquid by injection in near by area of spinal cord..one of the worst days in my life..after that I Changed my life style in a healthy way..now I am perfectly alright 😃

    I was looking for a movie post duty now and I saw your vid. Today I had a pediatric patient who's having intracranial bleeding (unknown cause maybe an aneurysm) however, the child's status started to deteriorate from bad Glasgow scale to active seizures (tonic clonic) then into dilated fixed pupils, (received mannitol, decadron, phenytoin, depakine, Nacl3%) we had him into urgent ct scan where the bleeding was squeezing the brain causing herniation, however an OR done to evacuate the blood then a craniatomy, I had him post opp intubated with a ventriculastomy for icp monitoring (risk for infection), the patient stablized however... he's a critical patient still seizing and glasgow of 4. I wanted to share this since it's our topic, You learn so much at PICU I love working their !

    Hi Caroline! Perfect timing ICP is on my next exam. Thank you! Video request- Holistic nursing care One of the reasons I love your videos is your positivity! I enjoy reading books by authors like Wayne Dyer, Jack Canfield, & Abraham Hicks. I practice visualization, meditation & even EFT when things get really stressful. I know that having these resources ( your motivational videos included) have helped me be successful in nursing school. They've also helped me realize how important mental well being is and how it impacts our health. Topics like visualization for pain, music therapy, etc. are covered in my textbooks but I don't really see them in incorporated during clinical. I'm wondering if you could share some ideas on how to incorporate some of these concepts into practice. Thank you again for all you tips & tricks but most importantly for being so honest. As a nursing student who struggled in school as a child, I really appreciate the stories you've shared.

    Thanks for all the videos you do! I am switching my major to nursing and have found them very helpful. I was wondering if you could make a video discussing violence towards nurses by patients; how common you think it is, what units have the most violent patients and the least, how to handle it, if you've had any experience with this issue, etc. I've seen a few articles recently of patients attacking nurses so it's making me anxious and I'd like more information.

    More info for you if you want to add and it's pretty key: 1. Empty Sella is flattening of the Pituitary gland. It is a key finding on MRI with high ICP. Along with that, low Endcrine function, especially Testosterone for men and Estrogen for women, along with Empty Sella would warrant an LP if symptomatic. 2. Pulsatile Tinnitus is super common with high ICP and when infection has been ruled out, high ICP should be suspected. Blurred vision which can be intermittent is also very common as can be Ptosis or drooping of a unilateral eyelid. 3. I have Pseudotumor personally and it took me eight years to get a diagnosis. My opening pressure was 47, which is nuts. I didn't play around with medications and went straight for an LP Shunt. I thought I had a Chiari Malformation b/c the symptoms are similar. Rapid decompression can cause a Chiari, but if you have low lying cerebellar tonsils, you might want to try a shunt before being decompressed. My research is leading me to believe that extremely high ICP causes Chiari and not Vice Versa. Finally, be aware that pressure on the spinal cord is very common with high ICP,so symptoms are not restricted to the cranium. Postural Tachycardia and other Autonomic Dysfunction can occur, but so can conditions such as Anxiety disorders and depression as a symptom rather than a situational problem. Body aches that resemble Fibromyalgia can be from high ICP alone. Basically, if there is a spinal nerve attached to the affected area, don't rule out high ICP. This is ground breaking stuff and much more common than you think. On a cadaver, you get one shot if that to look at whatever. Once the area is decompressed, you won't be able to keep your study going unless you find a way to induce high ICP on a dead person. I live people, it takes a minimum of 2 days to get ICP back up as we only make about 7 ml of CSF per day. Most LP remove about 15ml on average, so do the math. My own symptom list is a mile long and was progressive over time. My cause was from Trauma, but no one every put 2 and 2 together, but in the end, I did and I got my answers. My shunt is now in and I'm a new man and 100% symptom free when I thought I was headed for death, so write me if you have questions. Gary ~~~~~~~~~~~

I forgot to ad that I thought I had chronic Lyme for years b/c it was the only thing that made sense. I was wrong and wasted a lot of time and money on IV Antibiotics, but the good side is that I wasn't dead wrong :-)

    So let me get this straight.. Once ICP reaches MAP or above 75/80mmhg . The cerebral blood volume and blood flow increases, cerebral perfusion pressure falls because CPP is = to MAP - ICP .. resulting in systemic bp increase , resulting in cerebral Vaso dilation .. Resulting in increased ICP. Then repeating the process until cushions triad and acidosis of the Brain and coning is started .
