Improving fatigue and brain fog in POTS: Could Modafinil help?

Modafinil is an FDA approved medication which is used for excessive daytime sleepiness in patients with obstructive sleep apnoea. It has also been shown to improve cognitive function and mood problems in patients with Narcolepsy. In addition, there is some evidence that it improves symptoms of depression, anxiety and irritability in patients with sleep apnoea. It does this by increasing levels of serotonin and glutamate the brain. The really interesting thing is that in 2014, a group of researchers decided to study this agent in a small group of patients with POTS (Kpayaeh et al) and found that Modafinil at a dose of 100mg did not substantially increase the heart rate or make symptoms worse in patients with POTS which therefore suggested that it could be used in this group of patients.

    Modafinil has been life changing for me. I feel a lot like my old self again (mentally). I didn’t realize HOW bad it was until I took this medication and saw significant improvement. It hasn’t negatively impacted my heart rate. It has really improved my quality of life

    Professional and helpful as always! I love your videos and I love you as a person :) you are so empathic and you are genuinely trying to help people.I know this video is not about this but you helped me out to much talking about palpitations and ectopic beats.Im a professional swimmer and I sometimes get palpitations after hard workouts(i’ve been checked out by quite a few cardiologist but they couldn’t rest my mind as you did even if every test came back fine they didnt take the time to explain to me what might be happening).Keep up the good work!

    You are so nice and gentle as always... Watching your videos is a healing expierence. I had such a bad doctor not taking care of my disease when it was just appearing (arrythmia) so I reached a very hard point. Luckily my new cardiologist worked so well I improved really a lot.

    I will bet many of these people with POTS also are people with ME/CFS. Many of us with ME/CFS also have POTS. ME/CFS scientists have found an overall problem with energy and they know it is in the plasma. For example, immune system cells are in the normal range and know what to do but they run out of energy doing it. I suspect our POTS is related to the utter system-wide exhaustion for some of us. The Workwell organization has some recent studies that show a significant CO issue when having ME/CFS patients do a two day Cardio Pulmonary Exercise Test (CPET). I hope Dr. Gupta will comment on this some time in a future video. His insights are great!

    Thanks for sharing your expertise. Could you give a lecture on how viruses can affect the heart, not just the Coronavirus which has been the center of attention lately, but the flu virus as well. Pre-existing conditions like valve disorders, arrhythmias, people who have had valve replacements etc. I can only imagine the stress these illnesses place on the body. What can we do to help ourselves and avoid major health complications. Thanks!

    What if you get a heart palpitation when taking a deep breath sometimes is that normal? I have had multiple heart tests that all came back normal so should I be worried? Also some palpitations I get cause dizziness for like 2 seconds then it goes away. Is this normal for someone with anxiety

    I've read that thiamine deficiency can cause issues with nervous system. Once I received B1 injections, my symptoms improved.

    I also take thiamine my POTS was caused by 2 antibiotics called metronidazole and amoxicillin. Was yours caused by a medication and youve found somthings that has healed your POTS entirely? Thanks for info👍

    @Vasanthi I have been able to take it all my life until I took an antibiotic called metronidazole it does something in the Liver That inhibits thiamine and it also crosses the blood brain barrier then after they gave me amoxicillin which finished the job which also passes the blood brain barrier and can affect the autonomic system thiamine is the fuel for the central nervous system

    Back is 1980s with fibromyalgia and chronic fatigue syndrome they loaded us with antidepressants and thyroid meds saying brain fog was cause by problems in HPA axis. They also gave amphetamines which made rest of you go faster but with a still slow brain. I wrote study and facilitated support groups for this for 23 years I had over 400 members with same symptoms. I hope you find something to help heart patients maybe it will help the rest of us with different diseases having similar "brain fog" issues.

    It seems many people with Cfs and fibromyalgia often have underlying mental health issues. Maybe this leads to dysregulation of nervous system like in ptsd etc

    The brain fog is the worst part of having POTS, and I was diagnosed nine years ago. I could live with all the other symptoms, but having my brain off-line makes working impossible. I have taken modafinil for my POTS, but never for more than a few days, because I cannot fall asleep at a decent hour after I take it first thing in the morning. It was originally suggested that I start taking it back in 2011 when I was first diagnosed with POTS. I'm going to do further research to see if the sleeplessness abates after I take it for a while.     Maybe you have gastritis. It can cause similar symptoms. Try taking zinc carnosine twice a day.

    Modafanil is the best medication I've ever taken. It was prescribed for hypersomnia but has caused the pleasant side effects of lifting brain fog and mood, helping with pots and I've had fewer migraines. The only downside has been sometimes over exerting myself and then having extra sore joints from hEDS.

    Melanie, I think people like us with hyper mobility spectrum disorders/EDS have been very much ignored. Only now it seems to be changing with experts like Dr/Mr Gupta, I asked about PITS about 4 years ago, no it’s rare I was told (as with everything else) I blacked out twice in 5 days, once coming down stairs, a bonus was it did something to my shoulder and elbow that was troubling me for a couple of years. Of course nothing showed on x rays 🙄 then I blacked out crossing a road, smashing my face against the road, my face was a mess, I hurt my wrists and dislocated my thumb too, Damn think wouldn’t stay in place. I realised by afternoon I needed to go to a & e, the dr was shocked, he was disgusted that I’d been ignored after telling drs for years. He couldn’t believe I hadn’t broken any facial bones or my wrists. He began to examine me, he took a finger then said ‘it’s silly seeing how your fingers are moving because you already told me you’re hypermobile’ I decided I liked him, he listened. He triggered referrals to neurology and cardiology. That was auto 2019, I was sent 3 appointments from cardiology, they cancelled all 3. Chasing it up I found they’d discharged me! You’ll know how much energy it takes to chase things up. Years ago I was diagnosed with bjhs, now it doesn’t exist. I was told I’d be fine by 40 because I’d have stiffened up, it wouldn’t affect anything else….he was wrong! Now the rheumatologist I’m supposed to see was trained by him…she’s pointless too

    I have POTS w severe brain fog and fatigue. It’s truly debilitating and yet; we don’t qualify for disability. My last hope is corlanor. I’ve lost my career for over a decade with this illness. I was a young mother and already, I’m in my early 40s.

    what do you think about the keto diet an monitoring your blood glucose level....i done this and my high bp went normal....yet in the uk...my dr just said...oh ur bp is high...boom il stick you on doxadosin.....u must have narrowing of the arterys...so i went for a cc scam wich i paid myself in nutfield hospital in manchester400 quid...an hey ho..score 0......so what the hell is my local gp for...a guess in my health??

    I first got brain fog when I was 15 and it never really went away. Last week I found out I have POTS. I am 22 now :( Modafinil sounds like a dangerous drug, one of my friends who took it told me he gets chest pains with it and if he is healthy I imageine I would end up in the ER with cardiac arrest if I took it.

    I have had POTS since the 2nd of January 2013. I am very happy to see that you are so interested in the subject as a cardiologist told me years ago in Yeovil, Somerset that I was wasting his time. Fortunately I do have medical knowledge and I had 1 GP that listened to me so I was able to get to ULCH and get a official diagnosis of POTS along with hyper mobility which is also common for the condition as well as chronic fatigue syndrome. The only medication that has worked for me for all of these years has been prednisolone. I was taking 5mg daily and now living further north I take 7.5mg daily. There are so many comorbidities that affect most systems in the body simply because it the autonomic system causing the body to act in shock everytime I move and sometimes even sitting still. This has had a direct impact on my heart due to such a fast sinus rhythm that I now have more in depth heart problems. Certain medications that work for some people do not work for me. The prednisolone works for fatigue due to the body's misuse of my adrenal gland. When it comes to quality vs. quantity and knowing 7.5mg of prednisolone is the highest dose that should have the least long term effect on bone degradation, this has been a life saver for me. I do wish you would contact me privately to be able to discuss all I have learned about POTS that is still not being talked about. I am only in Durham and would be happy to travel by train to be a Guinea pig if it helps. Keep up the amazing videos and I love the piano background.

    Strange to hear this about another disease. I and millions suffer chronic fatigue syndrome and fibromyalgia. If I stand correct we as group back as far back as 1988 coined this term, "brain fog" because its a familiar symptom along with debilitating fatigue. I and others have tried many things to help this one symptom of ours. I as lay person conducted my own study published in Journal of Musculoskeletal Pain in 2000 looking for answers for this and of course a cure.. Getting old still waiting for a cause and cure for all symptoms. So good luck. Interesting to hear term again.

    I am type 2 diabetic and have osteoarthritis. I found I was feeling very tired and had a lot more pain than usual. My blood sugar is very well controlled. My Doc sent me for blood tests. It appears that I have a coxsackie virus still in my system from a previous sore throat infection. This apparently can cause chronic fatigue and pain symptoms I am told. To boost your immune system eat healthily but get a lot of sleep. Good sleep patterns make the kind of blood cells that fight sickness apparently. So don't fight the need to sleep unless of course you are at work.

    jewellryaddict, you might like to read The Polio Paradox by Dr Richard Bruno about post polio syndrome as this touches on CFS and fibromyalgia. Post polio has many similar symptoms but is not mentioned often as we polio survivors are all dying out now. Also many thousands diagnosed with CFS and fibro may actually have had non-paralytic polio, undiagnosed, and may now be unwittingly suffering pps. Just a thought particularly if anyone close to you had polio.

    You probably don't answer personal questions, but I'll try. I had a cardiac ablation December 16th 2019. Two months later I am still having episodes which include faintness, nausea, and a racing heart lasting up to a couple of minutes. Admittedly this is better than before the ablation when episodes lasted up to 4 hours and I passed out more than once. I wore a Holter monitor after the procedure and the doctor said the analysis was normal. It doesn't feel normal. Will this clear up completely at some point?

    I recently had Open-Heart Surgery to repair a Dissecting, Descending Aorta and developed Orthoststic Hypotention to go along with my High Blood Pressure . . . an Interesting combination. I left the Hospital after a 10 Day stay and a 50 lb weight loss and four different medications. As I had difficulty with the side-effects of the medication, (and because I wanted to heal naturally), I stopped taking any medications at all, however, The biggest problem is that I cannot fully manage on my own is the Standing Low blood pressure and thought that this might be a good thing to try. I was also diagnosed with OSA about 20 years ago and was unable to tolerate CPAP so it might help with that as well. Any suggestions from Viewers?

    @@dickiedollop Yes and No. While I was in Recovery, the Doctors were doing everything they could to Force my Body to a Blood Pressure level that I couldn't maintain and when standing,I would feel faint and had to sit or Lie down. I'm 76 years old and, my BP was Ideal and at 60 years old, my BP was almost always 120/80 and it appeared that I was one the few in my Family's History to have escaped an early Death. I had realized that a Higher BP would increase my chances of further problems, but, I have been active all my Life and the Quality rather than the Length of My Life is as Important to me. I tried going back on a basic BP Medication, (Amlodipine, 2.5 MG, each day for one week and my BP dropped below Normal, so I took BP readings regularly and thought to take them when it showed any significant Increase. That was over a week ago, and my BP is remaining Lower than Wanted. My Pulse Pressure is almost always higher or lower than the 30 points considered as 'Normal', and indicates some remaining issues. Wise? Probably not, But as that Noted Sage always said, "I am what I am, and that's all what I am!".

    Johnny B I was on Amlodipine and Losartan then I lost 30kg in weight and was able to come off them as well as type 2 diabetic medication. I started to become more active with swimming, running, cycling mainly I used to check my BP randomly and despite all the weight loss and being fit able to do 10k runs swim 2000 metres off the bat I noticed my BP creeping up so after a 12 hr monitor I was recommenced on Losartan 50mg and was fine for awhile until I started to get light headed when standing up and dizzy spells, so halved the dose to 25mg and run by my Gp who was happy, but funnily since doing this I’m starting to get orthostatic hypotension symptoms again so may end up coming off altogether again which being 54 and having been medication since my early 40’s I assumed I’d be on anti hypertension medication for life. It’s true about weight and exercise being good for you, but I never thought I’d be off the diabetic meds but am so glad I am as they were awful for side effects ! Good luck for the future, you are clearly doing well. 👍🏻

    @@dickiedollop I'm not surprised that there are others, like me, who didn't respond well to standard medications as they are designed to deal with a "remedy" for a problem that will generally work for a number of people. I've lived long enough that I have seen a lot of Friends, Family and Acquaintances Pass, and most of them had suffered from the ;Remedies' associated with their Individual Heal Issues. One of the Problems seems to be our Individuality as Blanket solutions can't cover that. Added to that is the Poor quality of the so-called 'Food; that we are supposed to maintain Life with. Overly Processed bits of Garbage, stripped of Most of their natural Nutrients that barely provide enough to do what they should. It sound's as if you've gone through the same BS as most of us have and that have, for the most part, failed us. When I was still a Kid, (early 20's), I realized that there were a lot of things that I would have Little or no Control over, however, I did have a small measure of Choice as to whether I would React or Respond to it. A person who can only React to circumstances is like a Leaf in the Wind . . . they go whenever and wherever the Wind takes them. A person who can train themselves to Respond, however, is like a Bird in the Wind . . . they learn to use the Power of the WInd to take them where they want to go. Emotion, like a Strong Wind, can be almost impossible to control, but, with effort and practice, it can be used to move a person where they want or need to be. As with most things in Life, our outcomes are usually a result of the choices we make from moment to moment. I simply Chose to be a Bird rather than a Leaf, as I suspect you have.     My fatigue has been so bad 3 yrs now. At my age .it has severely deconditioned my heart and body. I feel chest pain at night, hard to breath. Tacky... But i also take ativan at night. So not sure if that's makin it or my heart us just getting so weak. Like my muscles.... I'm so scared.... My dysautonomia is mostly hypotention.

    Hi. I hope I will find time for the consultation with you in York. Currently I live in Leeds. Please make a video about other POTS symptoms. My heart rate and blood pressure is now absolutely normal coz I take bisoprolol, but I still have occasionally shortness of breath (dyspnoea) , dizziness, sleeping disturbances, post exertional malaise, strenuous exercice intolerance ( squatting, push ups). Intolerance not because of the heart symptoms, but because of my nervous system hypersensitivity, even though I am not stressful and not worried. So dizziness, fatique, tinnitus, head blood pressure/circulation, reddish eyes, blurry vision, eye floaters may appear after strenuous exercices (if 15 push ups or 15 squatting I may call strenuous?) . I am also become easily overwhelmed and feeling over stimulated in busy environments, where a lot of hustle and bustle. Also my other symptoms, but now they are rare, may include: brain fog, light-headedness, blurry vision, eyes floaters, red eyes, pinching feelings in my eyes, unability to walk in straight line, coordination problem, forgetfulness, obsessive thoughts, irritable bowel syndrome, diarrhea, appetite loss, very acute feelings (odors, notices, lights), tinnitus, pulsations in my head, vibration in my head, clicking noise in my head, insomnia, tremor (hands), derealization, depersonification, feeling of the lump or stone in the throat, excessive sweating, coldness, numbness, burning feelings on my skin (nettle-like) and many others. I even had very strange symptoms like pain in my hairs. Also some smells on the streets may make me dizzy or cause dyspnoea ( gasoline,paint,nail polish removers, bleach, maricuana). So I have some kind of hypersensitivity symptoms, especially in case of stress/bad sleep/sun heat/exercices/watching some movies (horror for example). I have complete alcohol and caffeine intolerance. Even few sips make me symptomatic. I also have very bad head pressure and blood circulation in the head when I am flying in the plane. Also tinnitus and pain in the ears when I am flying. So my heart pressure is now sorted by bisoprolol, but is there any drugs for the head pressure and head related symptoms? My MRI is good. Maybe I should make CT as well? And maybe neck CT/ultrasound?

    @@jackx7382 No. Doctors didn't find anything on the tests, so they didn't want to prescribe me anything other than calming supplements...

    @@aleshwaramnaveen the only thing I found that helps me is running/jogging and swimming pool. I started very slowly, had to listen to my body and if things were getting worse I had to slow down or even stop for a couple of days. I no longer go to the swimming pool, but I am running every day 30 minutes (in the evening time). But when I started I was only able to run 2 minutes. My health improved significantly because of the running, but I know if I stop, everything will come back.

    @@tarviky I’m so sorry that you’re still dealing with this. I had it bad a few years ago, just mild to moderate now. I’ve just come across modafinil so will ask my Dr. but I’m anticipating they’ll say no.

    The blue ring around your iris is most likely a corneal arcus-a cholesterol deposit in the eye. ... Corneal arci are considered benign, and usually do not require treatment. This condition should not affect or damage your vision, and while it is common in older patients, it is rare in those under age 40

    @@GJRnoobs It's a drug that improves cardiovascular function and endurance. It is used by people with CHF, angina, palpitations, or even athletes. I finally can walk without feeling dizzy. I no longer feel dizzy when getting up from bed or leaning over.

    Modafinil did absolutely nothing for my POTS, even at the highest dose. Slow sodium tablets, plenty of water and a low-carb high-fat low-histamine diet have had the best results, including brain fog. Is only a management, though.

    There is a drug called Mildronats that helps me. Along with high doses of vitamin B1. Thiamine deficiency can mimic POTS.

    Pots= are nutrient def. But i do not remember whitch one. I think it is B1, but not shure.

    Ah…the classic “throw medications at the problem” approached. How about you heal the brain and gut and cut out heavily-marketed foods and sleep when you’re tired before we throw medicine at people? Everyone wants a quick fix - fact of the matter is you’ve been harming your body for years before autoimmune issues arise.
