SOAP notes can be incredibly confusing to chart, know what information to put where, and how to do it in an organized fashion. In today’s video I will walk you through step by step of how I write notes as a nurse practitioner. Using subjective, objective, assessment, and treatment as the guideline for the notes, I’ll explain how to make note writing as a healthcare provider easier.

    Im still a student (doing RNs) but I actually love writing progress notes. Maybe it's because I'm a student and it really helps enforce my learning going through a case methodically that way. Thanks for the video, very thorough and educational!

    I’m a scribe for an oncology nurse practitioner. :) get one of us and then you can do slightly less charting haha (unless you have a bad scribe. But we won’t talk about that).

    How long can I hold on to soap note before I put it into the electronic system? 24hr max? Week? Month? Can’t find an answer to this on google or even the board.
