How to treat ACNE PIGMENTATION | PIH
PIH also known as post inflammatory hyperpigmentation is most commonly associated with past - present acne lesions. PIH is also known as ‘acne marks’ or grade 1 #acnescars
.
.
PIH can refer to brown or red acne marks. I prefer to separate red acne marks in the category of macular erythema, or PIE - post inflammatory erythema.
.
.
Facts - PIH is more common in darker skinned patients. In 50% of cases PIH can last up to one year. In 25% of cases PIH can last up to 5 years. 80% of PIH cases are seen in MILD to MODERATE acne. PIH is more common in patients who pick, additionally pigment is related to UV exposure
.
.
How to treat? Prevention is the key. Treat acne early AND effectively. Sunscreen helps. Pigment faders such as #hydroquinone #kojicacid #meladerm #arbutin #licorice #AHAs & #ascorbicacid can be effective. Low strength peels such as AHAs or BHA salicylic acid - TCA can fade pigment. Lasers including nano-pico-fractional lasers can be useful as adjunctive treatment, as can vascular modulators.

Great video as always. I ended up trying most of the OTC products you mentioned for hyperpigmentation, not necessarily acne scars, all within a two month period and am now still trying to get my skin to recover, lol. I think it was the 23% Vit C that threw it in a tailspin. Now over a month later, even though I stopped all actives, everything still irritates my skin. Lesson learned, i think we tend to be too impatient wanting to see results with actives!!

The lady told me it’s normal and they go down within a week and told me to put on some aloe Vera gel for the inflammation. But it didn’t go down at all, I got little marks everywhere on my face. They’re slightly bumpy in some places and they don’t hurt anymore but most of them are just acne scars. I’m trying so hard to get rid of them before school starts at the end of October.

Hi Dr. Lim, great video! I have east asian III-IV skin type with deep PIH marks from cystic pimples 9 month ago. About 3 spots/patches on my chin that haven't seemed to fade at all over 9 month. I feel this might be permanent. Is laser an option for me? ie. benefit outweigh the risk? Thanks!

As u have told I tried so many expensive products but there’s no changes.i went to dermatologist she put me on benzoine peroxide cream for 3 months and azealic acid cream but azealic acid cream irritate my skin a lot and stopped using it. She recommended me to go for IPL Treatment as well. Finally on Roaccutane for 4 months, still my PIH scars is left out. I don’t know what to do please suggest me? Thanks

My PIH faded TREMENDOUSLY within 3 years, sad part is I started getting breakouts again recently AND I’ve been picking them (I suffer from OCD and lately it’s been HARD for me NOT to PICK my skin...) so now I’m back at square one. I don’t remember what helped me fade them so nicely 😭 I’m trying to find a routine that works again but it’s TOUGH.

  • @aubreybondoc

I use tretinoin, azelaic acid and niacinamide. The PIH were fading but it is so slow. Although my Australian Gold Tinted Sunscreen is enough to cover them. I might add ascorbic acid soon. After this, I'll try the laser.

I place my 1st order of the ordinary HA, niacinamide, ascorbic acid8%with arbutin and the AHA toner. I wanted you suggestion on which facial wash I can use before apply these products. I have 8% benzoyl peroxide and Cerave normal to oily cleanser. Also can I use a cerave BHA cleanser or the La roche posay BHa cleanser with any of these products? I read that sometime benzoyl peroxide facial wash cannot be use with retinol or glycolic acid toner, HA or any product that I mention IS THIS TRUE?

Can we use skin care that has retinoid acid such as granactive retinoid 2% in squalane, or azelaic acid, alpha Arbutin, lactic acid 10%+HA during doing VPL laser for reducing acne scar? Or rather we should take it away for while until we quit VPL laser

I’ve used alpha arbutin before, it didn’t do anything for my skin and I used an entire bottle from the ordinary. When I looked at more reviews when I realized it wasn’t working, I noticed that many people who said it didn’t work for them had brown skin. So if you’re brown or Black I wouldn’t recommend it

@@kimdahyun2893 yes you are right. she’s not a dermatologist so doesn’t do laser etc . She advises the effects certain treatments and skin care can have on POC and what you should avoid. She does have a clinic that specialises on Hyperpigmentation for POC. Davin Lim is a dermatologist but doesn’t specialises in POC. I don’t think he has as much knowledge on POC as Dr. V. He, like most dermatologist recommended Hydroquinone on POC. Which is a big no.

So should I schedule a dermatologist visit or go to a medical spa? Med spas seem to have the patient pick the procedure without diagnosing the problem and having a professional select treatment options beforehand, and telling patients what those options are
