This video is about how I treat TMJD. I go over my specific modality of treatment which consists of:

    I'd be in your office next week it I were in your area - thank you so much for the info. I didn't know what my tinnitus, neck & jaw pain and vertigo was caused from 15 years ago. After seeing many other specialist I went to a TMJ doctor. Unfortunately the splints that were made kept me awake at night and tend to make me grind even more.     Hi Dr. Just want to say I think you are an angel doing God's work. Helping a problem that does not get talked about or people dont know how to treat. And providing information and taking the time to answer questions on youtube when you are already busy is so kind! Do you know long tmj episodes can last usually? As far as weeks or months?

    Great advice ! I am getting myofacial release on my jaw today from massage therapist. Also, what are you thoughts on lidocaine injections or the trigger point injections ??

    Thank you for the video. Can you please do a video on what you recommend for those living with a chronic lock meaning there’s no hope for the disc to recapture since the condyle has been remodeled?

    What do you mean by the condyle being remodeled? I think I may have this. I basically don't have that bony nob on the right side of my Jaw unlike my left side. This makes my jaw just hard to control. It feels loose and it gets tired quickly. Sometimes you try to talk and it just does dosent operate like I'm telling it to.

    @@zakvince7425 When the disc is displaced without reduction and you are bone on bone the condyle changes shape with time it can become flatter. I have never met a dentist who knows how to treat disc displacements without reduction

    I received Orthotics when i was done wearing braces but i repeatedly got them broken on accident and the waiting period between each time made my TMJ worse. It's located on my left joint mostly and a little at the righ joint. It feels like my jaw is being pushed to one side by the pressure. I've been living with this problem for over 12 years, it messes with my orientation too like i can't tell where my front middle is and i need to focus extra hard to walk straight. Balance is completely off.

    Thanks Doc for the video. My wife has suffered with TMJ for years. Unfortunately, she has dentures and the uppers most often hurt her gums (which seem to swell). Any suggestions for TMJ self treatment (or ??) for denture wearers?

    I had custom orthotics made but my jaw was never relaxed with them because my teeth wouldn’t “rest together” right. I would constantly grind on the orthotic and move my teeth around to find a good position to relax in, but couldn’t. Would you say that’s because the orthotic perhaps just wasn’t made right or the right kind, or that I would just need a much longer time wearing it for my mouth to become used to it? It was made after connecting sticky measuring devices to my face to see on a screen where my jaw seemed comfortable most. The orthotics also began shifting my bite, so my bottom teeth would close right under my top teeth, making my smile no longer aligned then. My regular dentist urged me to stop wearing it immediately (the orthotic was made by a neuromuscular dentist).

    Hi Doc thank you so much for the genuine video. I have been having tmj issue since 2017. I tried splints. And doing chiropractor as well. But lately if I wear my splint my neck stiffness won't let me live and if I don't wear my splint jaw kills me.. not sure what to do ? Do you have any suggestions? I have been searching answers my whole life and please let me know if have any advise. Thank you so much

    Would you ever suggest an arthroscopy for displaced disc with reduction? And do you have patients that the clicking/popping never goes away even once disc and pain is stabilized?

    I don't believe that an arthroscopy can help DDWR long term. Yes, I have some patients where the click/pop remains even after the pain is gone.

    So grateful for your information that you bring to us. I do have muscle pain on my right side of jaw which I believe TMJD. I wear a nightguard, but feel an orthotic would be better for me. I have had the nightguard for quite a few years, and the guard has indentations from my bites at night. Will the orthotic help with not only the muscle pain but also from grinding? I am also diagnosed with Trigeminal Neuralgia. In your opinion, is it possible that the jaw pain from TMJ could cause TN?

    I have been putting a small piece of cotton in my painful ear with lavender essential oil on it. It works and if I don’t have it in the pain comes back. I take it out at night and it doesn’t bother my hearing at work.

    It’s not a cure but it saved my life. I wear a nightguard that my dentist made every night. I have tried everything else

    Very helpful information! I had an ear infection back in December 2020 from swabbing both ears. Was given antibiotics but it didn’t work. What lead to healing the infection was from taking prednisone for a week. I had a CT scan, MRI, and a hearing test and it all came back normal. To this day, chronically I’d feel dizzy and nauseous. I have no fever or any fluids coming out of my ear. Also, my upper teeth feels achy. I have 2 questions… 1) can there be residue from the ear infection? 2) does TMJD can be traced back from the ear infection?

    It’s likely not residue from an ear infection. It may have been tmjd all along. Or tmjd that developed during or after your ear infection.

    Hi Dr! I just got diagnosed with TMD disc displacement with reduction on both sides. The doctor wants to use an orthotic for 1 year used all the time except for eating. He wants to re center the condyle and then allow the tissue to form a new pseudo disc in which to properly function. After a year the goal is to then wean to nighttime orthotic only. Does this creation of a pseudo disk sound right to you? He said it’s a 99% success rate if the patient is compliant. Also I’ve had a few burning teeth pains in the past and he suspects trigeminal neuralgia. Could the TN be due to my TMJD? Coils treating the TMd with orthotic fix the TN tooth pain?

    Hello Dr. Mistry, How do I know if I need an orthotic or just myofacial release? I don't have clicking or popping, just tight, sore masseter and temporalis muscles. They give me a nonstop "tension headache" affecting the forehead, temples and sometimes down to my left upper jaw and molars. Thx for any guidance you have. G from Ontario Canada

    Dr. Priya - Blessings to you! A TMJ-specific MRI detected I have disc displacement and an atrophic disc. I believe this is due to a dental injury I sustained in 2013 in which my jaw was open too wide, for too long during a procedure. Is it possible to conservatively treat disc-atrophy and atrophy of the ligaments, etc. within the joint capsule? If the disc has shrunk, essentially, can much be done? Thanks! J

    It's dependent on so many factors - the size and shape of the condyles, ligament laxity in the joint, the extent of atrophy of the disc, and more. I would still say it's worth trying to recapture the disc.

    I suffered tmj after having my wisdom teeth removed. That's when my left jaw gave me ALOT of pain for 16 months. I was at my wits end. No support from dentist.What procedure did you have? I too feel my mouth was opened too wide and too long. Splint in situ, mayby botox..migraines intense.

    Hi dr! Would it be possible for my masseter muscle to shrink back to its original size if i treat my TMD? I had jaw clicking on my left side for 5 years and I realised my left masseter is way bigger than my right.

    Hi Priya. I have had jaw problems ever since I was young and I used to get a lot of pain. The last couple years the pain has basically gone away, but I believe this is because it has actually gotten worse and I am passed the pain stage. I am now 18 and I have difficulty controlling the right side of my Jaw when talking, eating, etc. When I put my fingers on the condyle(bone that moves down when you open your jaw I believe) the right side is almost non existent. You can feel it move slightly, but it's nothing like the left side where you really feel the knob. Is this tmj without reduction or could it be a partial jaw dislocation that I have been living with for a few years? The second option doesent seem likely to me, but I don't have any other explanation as to why the actual bone/condyle isn't as prominent on the right side? Any help would be greatly appreciated thankyou.

    No I haven’t. I made an appointment to see one and she blew me off. And next day, she emailed me to ask me would I be coming the following day and I said NO! Anyone who blows off a meeting /office with w/o talking to me directly is not worth seeing. I already don’t trust her. Not Good! If I have to, I will fly there to see a doc. Tomorrow I’m going to chiropractor who wks with TMJ pax. So we will see. I really want to see you. You are very good and has demonstrated concern about your patients. I like that. I can pay to talk to via the phone or if you know of a doctor who deals with TMJD.

    Yes I have seen this a lot with my patients but unfortunately tinnitus is the one symptom that does not respond predictably to treatment.

    Thanks so much for your videos! I was wondering what kind of imaging you like to have on your patients? I've been told MRI is the gold standard for determining disc position and morphology, while others say a CBCT is sufficient. I'm a little confused about the importance of the MRI, because the TMD specialists who swear by it can't actually read the scan images as they are not radiologists, they only read a radiologist's description. So it would seem the TMD treatment isn't really benefiting from the precise information the MRI provides.

    Do night guards help? I have one from the dentist but no one takes insurance for TMJ in CO and I have spent so much just with tmj consolations. I get headaches dizziness and popping and clicking.

    It's hard to say what caused it. There are typically many factors involved. I wouldn't recommend doing jaw exercises as this can make things worse.

    No, they don't typically need surgery because it's usually the muscles that we just cannot calm down. I'm getting trained in using TekScan (also called T Scan) and I'm hopeful that that will be the solution for at least some of those 10%!

    Um hey Dr. so like im currently wearing braces right now. And i heard that one of the symptoms of TMJ is clicking and popping sounds ( i dont feel it when i put my finger tips in front of my ears), i dont get these but i do get these wierd sounds, i'll describe it as grains of sand falling on the ground.     if it's a regular nightguard, it likely won't help with pain. if it's a tmj orthotic that's made correctly, it will. the term "splint" can be used to describe a nightguard or an orthotic, so it's hard for me to answer your question.

    Thank you! How do you keep your patients' spirits up while they go through treatment. It took me several months to realize this is TMD pain and now it will be two more weeks before I get my orthotic splint. Sometimes I don't know how I can keep it together pain wise and I keep worrying what if this treatment doesn't work for me?

    it can be hard but if my patients are having severe pain I usually see them weekly for TENS treatment and craniofacial therapy until they feel better.

    Dr. Mistry. Are you having the bite registration for the orthotic first in the best-relaxed position of the face and neck muscles (by a tens device) ? Or having the bite registration on the centric occlusion and then by time making adjustments and letting the neck muscles conform to that adjusted orthotic and occlusion(by physical therapy)? Which one comes first?

    It’s not about the name or type of splint, it’s about the prescription built into it that is unique to each individual. Kind of like all prescription eyeglasses looking similar but it’s the prescription built into it that makes the difference. 😊🙏🏼

    Is it possible for TMJ to cause a tightness in the jaw on the affected side? I have always had clicking, but somehow I really flared myself up and there is pretty minimal pain, it just feels like my entire jaw area is extremely tight, mostly on the affected side.     Hi Dr, I developed sever tmj pain and issues after my orthodontic braces treatment as my bite has been really compromised and upper 2 premolars extracted. I visited a tmj specialist ortho who also focuses on aireay function but since I am moving abroad in 2 months he advised me that he would give me an orthotic to wear for day and night and then slowly use it for just night time but unlike you I wont be recieiving any frequent checkups with him. My treatment basically just consists of the orthotic so I got skeptical since like u said my jaw would be changing so the orthotic should also be changed. He expects me to use just one orthotic for 8months. Should I go ahead with his treatment or do u think this wouldnt be as effective and could cause further problems The orthotic is quite difficult for me to afford as well so I dont want my treatment to be a waste of time

    I wouldn't say it's the most common sign of TMD, but it can happen yes. I'd definitely recommend seeing a doctor to rule out infection!

    Hallo, i saw your videos about myofunctional therapy and i would like to ask some questions.First, is it good to have correct tongue posture while i am in braces?Second when i try the correct tongue posture i feel like my tongue hurts my uvula in th back of the mouth.Also, should i chew hard gum to use my jaw muscles more or it will make my cheeks bigger and rounder? i am asking all of these considering i have braces with a rubber band on one side and i am gonna get rid of them in 5 mounths(already having them a year)thank you❤️

    I wouldn't try to recommend changing anything about tongue posture without first consulting with someone who provides myofunctional therapy, especially if doing so causes pain. I don't recommend chewing gum ever. :)

    Will orthotics help with arthritis? I went to orofacial pain clinic and they think that’s my cause . My face feels tense around cheek area, neck pain, headaches on forehead . Already schedule an appt with tmj specialist to see if this is the cause or not . Do you have any recommendations in Southern California ? Im between orange and Los Angeles county .     I’m struggling to get used to my night guard because I am autistic it’s fits fine I don’t get on with the texture of the night guard it feels like I’m Creating to silva when wearing it and not enough when I’m not: I think along with my doctor that I have got a glossoperyngeal neuralgia.

    You may want to talk to the dentist that fitted it. It may need to be adjusted. When I was fitted for mine, it took a little time for my jaw to adjust at first. Don’t give up hope. I have been wearing my daytime orthotic for 3 months and am almost pain free, and I was in a LOT of pain to start with. Praying the same outcome for you!

    I don’t have any pain. My jaw situation relates to an oral appliance I wore at night to treat sleep apnea. But it changed my bite. Can TMJ treatment help reset my bite and bring my jaw further back closer to where it was?

This is the most difficult condition to treat and takes 4-6 months of treatment with biweekly appointments.     I've had this for 5 years😭😭😭and it's just not going away. I think it's gotten worse because I get the worst migraines on the side of the tmj and I have started to get motion sickness as well. Please tell me what to do. I don't want to to get surgery. I'm scared

    my doctor suggesting phase 1 and phase 2 treatment. 1) phase 1 orthodontics alignment slipnt= 6 months 2) braces treatment= 1 year Total cost 2 Lakhs Can I belive on that

    Did you recaptured disc with reduction with these treatments on your patients ? Will the disc remain in recaptured position after the treatment for years?can one live normal life after disk is recaptured.? hope you will answer

    Yes, I recapture the disc with these treatments - sometimes with reduction and sometimes it's fully reduced.
