Oh my gosh! I had always been puzzled as to why you would actually want fat grafting on the lower lids. Thank you, Dr. Prasad for you honest opinion. Having had a few upper eyelid revisions, I appreciate doctors who want the best for their patients and not treat them as guinea pigs or just to line up their pockets.

You're welcome, Brat Pack! I also scratch my head as to why doctors still recommend fat grafting under the eyes when it's so highly susceptible to complications. Doctors who perform fat grafting under the eyes usually don't have to the experience or expertise to correct the complications, so eyelid specialists like me who don't recommend the procedure to begin with are often the ones doing the revision and restoration work. Hi, I have posted some images as indicated as 1 week after surgery, but generally they are taken 3-6 months after surgery. Since there is no external incision, and no skin removed, transconjunctival blepharoplasty usually has a faster healing time, and quicker resolving of temporary swelling than other types of eyelid surgery with an incision in the skin, and skin removal. I hope this clarifies things for you.

Would you recommend any of your colleagues that live in California that are just as qualified as you are? I'm interested but can't afford to travel to the east coast and have the procedure done.

Hi Megan Brown, I'm not aware of specific eyelid surgeons in California, but look for a surgeon who does the transconjunctival blepharoplasty technique where no skin is removed, and no external incision is made. Insist on it by name and with its details as its a more specialized technique not used by most general plastic or cosmetic surgeons. I hope this clarifies things for you.

Hi Doctor ,thanks a lot for this video. My mother has very heavy bags under her eyes. As this severely impacts her looks, she has already asked several doctors. Each one of them have told her her bags can't be removed, because it's water retention rather than fat. She has tried everything: sleeping on her back, drinking enough water (she stops at least 4 hours before going to sleep), avoiding sodium, balanced diet and lowered salt consumption. Is there any hope or are some people condemned for life?

I have been able to treat many challenging cases in the eye area as an oculoplastic surgeon, as I'm trained as an ophthalmologist as well as a cosmetic eyelid specialist. You recommend that not inject fat under eyes at all? Then what do you do to treay lower eyelid hollowness . Do you reposition fat bulges to depressed area? . I had a lot of problem with this kind of procedure too like fat resorption and ectropion . What do you do when there is an abundant skin excess and skin folding in lower eyelid . Have you ever done skin only flap ( without muscle elevation ) to treat deep wrinkles and folds ) .

Hi Pristin V, While under normal circumstances you may be able to fly out the same day if there are no sutures used for the surgery, current travel and quarantine restrictions puts it out of my hands. Normally, I recommend flying out the same day as surgery dependent on your general health condition. You are the best wish u we’re close to me I just had a face lift and blepharoplasty and now when I look left it is dubble vision, also the other eye doesnt close all the way or blink the same as the other one. The bad dubble vision one also the bottom lid hangs out which I’ve been Pushing up on it and it seems better the other I’m pulling down, helped a little too. Did he ruin my muscle is that why I’m seeing dubble?. Wish u would make a pod cast on this subject. Will I get better? He says the muscle is just swollen and it is 3 months the swelling is just going down but they look different, worse and one is smaller. I had 2 other eye blepharplastys before with good results, he said that’s why it is like this and he was to only take out fat not skin,which he said he did, they look baggy and now I have a tear through. Can anything be done for me?

I was left hollow and thin after lower Blepharoplasty in California many years ago. I wish repositioning the under eye fat was offered to me. I'm now having to do fat grafting under the eyes every few years when fat absorbs away. It's So costly and I have broken capillaries under eyes/darkness if I don't have fat grafting. Many surgeons do not so well unless they are specialized under the eyes. Most of them target the cheeks and not the under eyes. I found one doc who was amazing but he costs an arm and a leg now. Lol 😭💀

Is a transcunjunctival approach possible with skin pinch only? I'm relatively young (mid 20s) but was recommended a skin pinch because I have this "roll" on my lower eyelids. My biggest fear is increased scleral show/ectropion

Hi Baton Rouge, It is possible that you have redundant skin that requires a skin pinch blepharoplasty, but I would need to perform an examination to confirm it before I suggest the procedure. If done by eyelid specialist/ oculoplastic/ oculofacial surgeon like myself, there will be no scleral show, ectropion, eyelid retraction, or a skin shortage. I hope this clarifies things for you.

Hi Michael Hightower, Your follow up appointments can be done over Skype or FaceTime, or even email pictures of your progress. For lower eyelid surgery, our patients usually return to work a week after surgery, and often a laser is used to close the internal incision, so there are no sutures that need to be removed a week after surgery. There is also less temporary swelling involved in lower eyelid surgery as compared to upper eyelid surgery as there is no skin removal or external incision on the eyelid skin. Hi Alvaro Cifuentes, The transconjunctival approach to lower blepharoplasty, which is my preferred approach, doesn't involve an external incision on the skin as the incision is made on the inside of the lower eyelid. Hi Natalia KP, Lower blepharoplasty is for treating puffy under eye bags, and is not the right procedure for treating hollowing, unless the height of puffy eye bags are making the area above and below appear hollow in comparison. hey doc, im 16 and have had dark circle eye bags since I was very little. I have very hollow dark circle eye bags. Also they're very purple looking. As my bags have a purple tint does that mean that there is not enough blood circulation? what treatment would help reduce this?

Circulation can be a cause of dark circles, and both circulation and discoloration can be improved with platelet-rich plasma (PRP), which I usually administer to treat dark circles. Hi Twisa Bekrang, Sorry to hear about your problem. It's still not clear if your eye bags are present all the time, which is likely caused by prolapsed fat; or eye bags are intermittent, which can be caused by allergies, salt intake, sinus issues, lack of sleep, etc. An examination is needed to determine what's best for you. As an oculoplastic and cosmetic surgeon, I specialize in treating eye bags that are caused by fat, with my approach to this featured in this video, so watch it closely if your eye bags are present all the time. Hi Prasad , one of my eyes has more fat than the other and when I smile the smile bulge looks different. When I get my surgery , one thing that I wonder is if I will be able to keep the smile bulge that I like on the eye that doesn’t have a lot of fat , and reposition the fat in the other eye so when I smile it looks the same? I’m scared the eye with less fat will look hallow if it’s worked on

Hi ESOC, The bulge you see when you smile is caused by the orbicularis oculi muscle, while the puffiness in eye bags is caused by prolapsed orbital fat. @@AmiyaPrasadMD I just had fat repositioning done under eye. Looks good, natural. Unfortunately, I have a small scar on one side of my eye where the dissolvable stitiches were. Not sure what to do to improve. More noticeable to me then anyone else. Would CO2 be a good option to make less noticeable.

Hi@@minkycat66, I would have to examine your scar before making any specific treatment recommendations for you. I perform lower eyelid surgery without an external incision, and often closing the internal incision with a laser, so a scar, and sutures are completely avoided.

Hi Zoltan Attila Elekman, Sorry, but the exact opposite is true because each individual has unique facial characteristics, so their surgical needs are different. This is why we don't have the same cost for every surgery. A doctor who charges less for surgery makes up the difference by seeing more patients, and will have to perform surgery faster. Eye bag surgery is very prone to complications when not done by a specialist, such as eyelids being pulled down due to skin removal and an external incision, which I know because I don't do both these techniques, and I am constantly repairing eye bag surgery done by other doctors. Having cheap surgery and a revision surgery is far more expensive than getting it right the first time. I hope this clarifies things for you.

What happens when the eye ball are big, it still a chance that can eyes become hollow after upper and lower blepharoplasty?

Hi Queen Bee, I would have to examine your eye area before giving an answer, as eyeball position can differ between the eyes due to bone structure, or something else. Generally, prolapsed orbital fat should be artistically sculpted, with a little fat left behind for nice, continuous contour, and to prevent hollowing. I hope this helps you.

Hi heena harmukh, Sorry, I'm not familiar with doctors in India. I suggest you ask for transconjunctival blepharoplasty by name which involves no external incision in the lower eyelid for visible scarring and potential damage to lower eyelid support structures; and insist on no lower eyelid skin removal to treat wrinkles, as this can cause skin shortage that will pull down the lower eyelids. Under eye wrinkles can be treated non-surgically to improve skin quality. I hope this helps you.

Hello doctor, I have not seen anywhere that it is possible that fat grafting in the area under the eyelid could cause lower eyelid retraction by damaging the obicularis oculi muscle as you say at 5:50 in the video, are you sure about this information? No surgeon that i know (nor any source to my knowledge) talks about it, moreover, we should find this problem with hyaluronic injections in under eye area if we follow this reasoning, which is not an information communicated by the surgeons and doctors. Hi @user-ly2vv2ig4d, I'm an oculoplastic/oculofacial plastic surgeon, meaning I'm a specialist in eyelid procedures, as was an ophthalmologist or eye surgeon. Although I don't recommend or perform fat grafting under thin eyelid skin, I often perform revision surgery to correct complications for fat grafting in the lower eyelids, so what I say in my videos come from clinical, and specialist experience. The surgeons who recommend and perform fat grafting in the lower eyelids can't remove grafted fat when complications arise, so their patients end up becoming my patients. Unlike grafted fat, hyaluronic acid fillers do not require a blood supply to survive, and their shape remains predictable, and malleable to fit the treated area. Grafted fat cannot hold its original shape once removed from its native blood supply, so results are less predictable, and complications occur when the inconsistent texture of grafted fat is seen through thin lower eyelid skin. I hope this clarifies things for you.

I have no doubt about the unpredictability of the results of fat grafting, I know that there can be lumps and irregularities on the surface of the skin which is very thin in this area, also asymmetries and resorption. @@AmiyaPrasadMD Thanks for the reply. I actually saw that video after I posted this. I'm 36 and starting to see the hereditary bags people get on both sides of my family. I'm doing retinol now, and am trying to find prp treatments in my country. But I'm considering surgery later on down the line.

@@AmiyaPrasadMD I don't think I have enough fat popping out to warrant surgery yet, so I'm hoping the retinol and prp helps to keep the skin firm and to reduce the purple appearce I've had since childhood. So far retinol, caffine and massage have reduced the puffiness a lot, but I seem to have one small pocket on my right side that's not going down. I'm wondering if my meibomian glands may be clogged? I have a cyst that sometimes forms in the same place on the left side.

Hi Ruchika Kc, I'm not familiar with doctors in India, but I suggest you ask for transconjunctival blepharoplasty by name, which involves no external incision in the lower eyelid for visible scarring and potential damage to lower eyelid support structures; and insist on no lower eyelid skin removal to treat wrinkles, as this can cause skin shortage that will pull down the lower eyelids. Under eye wrinkles can be treated non-surgically to improve skin quality. I hope this helps you.

Hi Bhawesh aswal, Yes, eye bags that are present all the time are caused by fat, which is sculpted and removed with lower blepharoplasty. I hope this clarifies things for you.

Hi @annaneufeld8571, Sorry to hear about your problem. You do need to have this properly diagnosed by your surgeon as the muscles that lift the eyebrows are not directly involved in lower eyelid blepharoplasty. I hope this helps you.
