Why did they have to crawl through that hole at all? Seems obvious to me that he could have just as easily reached through and unlock the door and just walk through.

This is what I was thinking. It isn't hard to get through a door with an Axe. He probably just cut a hole to unlock it from the inside.

So entertaining the theory that the axeman was biding his time. How would not notice that something was up? Like if the door was tampered with or something is broken? My theory is this was a work of a mafia ring at the time which spiralled as a serial killer case. The way they got in was with a form of master key that worked for any lock around the city. After that, it was just a case of opening the bedroom door.

A master key? Do you have any idea how many completely different shapes and styles of key there would be in a city? There's no such thing dude. Lockpicking is remarkably quiet and easy for someone who's skilled, and door locks take seconds which is a much more likely explanation
