How a Brain Implant and AI Gave a Woman with Paralysis Her Voice Back

Ann is helping researchers develop new brain-computer technology (BCI) that could one day allow stroke survivors like her to communicate more naturally through a digital avatar that resembles a person. The breakthrough technology was developed by researchers from UC San Francisco and UC Berkeley and decodes Ann's brain signals, turning them into text, speech and facial expressions for a digital avatar for the first time. The research is led by UCSF's Edward Chang, MD, chair of neurological surgery.

    Locked in syndrome is the scariest and cruel ailment. You can see she is bright and totally tuned in. Some people would stop trying. Some families and caregivers would assume the patient has reduced cognitive function but that is clearly not so. She could be a great counselor.

    It is extremely scary, but this woman clearly does not have full-on locked in syndrome. She is able to move her head, so she can't speak but she still has ways to signal to others (she can signal yes-no, for example). People who are truly locked in cannot even move their eyes and have zero means of providing any kind of feedback on anything despite being fully aware and intact mentally. The worst nightmare I can imagine. However, devices like the ones shown in the video are starting to provide some means of communication also in such cases.

    I saw a short "horror" film about an elderly couple. The wife had suffered a stroke some time ago, and the husband was losing patience as her caretaker. he started to neglect her. Left her in a cold shower, but she fought and fought to crawl to him. She said "I'm....here!!!" ...

    As the parent of a 10 yr old non-verbal child who has never learnt to speak I understand that this technology is not applicable to children like mine. But as someone who has spent countless times seeing the frustration in my son because he can't express himself, this made me cry I'm so happy for Anne and all others this technology will help to feel like whole people again, and have fulfilling lives, and to be heard and understood.     ​@@LifeOdysseyMotivation unfortunately it's true, this technology wouldn't be able to help someone who never learned to speak in the first place. the video mentioned the machine works by detecting the signals sent by her brain to her mouth. basically it reads her lips straight from her brain & converts those movement signals into the right words. for someone who doesn't know how to form those mouth shapes, like someone who has never spoken before, there wouldn't be any movement signal to pick up from. but maybe one day there will be a machine that can read signals from the language part of the brain to translate thought concepts into speech without the person having to know the exact words for what theyre thinking about or the right order for their words. i'm excited to see how this technology develops!

    @@possibleproblem479The good news is even if they did, it wouldn’t really matter. AI is at a point where this level of tech isn’t that difficult to achieve in different ways (getting around specific parents, most of which are already patented and open-source). The required technology and data are mostly available to almost everybody. The difficult part is getting the hardware required, the funding, and a willing participant.

    I love to see things like this. My wife had a stroke at 32 (16 years ago). I feel like it stole our life. Seeing solutions like this, makes me happy. People don't realize what the stroke stole from my wife. I want nothing more for her than to have some quality of life back and made easier through these types of tools. Thank you!

I can't imagine everyone leaving me to rot in a bed by myself, if the roles were reversed. People don't want to stick around when the times get worse.     If I'm not mistaken, I saw Anne in a previous documentary where they showed her using the slower machine which, at that time, was the most recent technology. Absolutely astounding they have created this. JUST WOW. As a fellow scientist who works with cancer cells in a lab, I often forget our work impacts actual people bc i never get to see it

    @@lookupverazhou8599 Yes, using A.I. You do realize that A.I. is a broad term for a vast amount of systems and mechanisms, right? Not knocking you or anything, but this is quite literally a machine taking her electrical impulses and translating them using an A.I. algorithm to deduct the words, tone, pitch, emotion, all of that. And while it may not be perfect, or the most expressive, it's still an artificially intelligent algorithm. But I'm not an expert on this or anything, just an observation.

There is no artist who can create something completely new. Or did you mean that Ai takes the work of artists? This is also not true artists will always exist, and this crying of artists because of the "loss" of their work and cope about the fact that there is no "soul" in machine art is already happening for the second time in history (cameras) but as you can see artists still exist.

    Agreed my dad had 2 aneurysms one of which blew in his 40's. My mother stayed with my dad and cared for him till he passed at 76 from AML. They truly loved each other. It's fantastic what these people are doing to make their lives more full.

    I'm not sure if you know we are in the book of revelation, God tells us to not fall for the deception, not to partake in transhumanism, as you can lose your soul, God is the source of our healing not science.     As a daughter of someone who had a stroke when I was young this brings tears to my eyes 💜 Thankfully my mom still has her speech but half her body is paralyzed and I know they’re using amazing tech to restore people’s lives. AI is very scary to me in certain aspects but used in this way, it’s truly amazing! I hope one day she’ll be able to walk and move without pain and assistance. Thank you guys for all you do!

    My grandmother caught polio at 2 years old and is paralyzed from the waist down. It's fascinating this kind of technology... one of my biggest fears is paralysis or amputation and I can't imagine how hard it is for these people. I wonder a lot how technology and science will be able to restore these part's of people's lives..

    I watched as he had that convo with her, and when she joked, he never smiled. He was just stoic. I would be so happy to be able to communicate more quickly and actually have a convo instead of using those glasses. But, while talking with her, not once did he smile?

    This is so awesome! I don’t know anybody who has these conditions, but as a retired nurse, I saw many who could not speak for themselves mostly children being a retired, pediatric nurse! Happy tears this is wonderful, and will be a much-needed breakthrough for the lives of these people!!

    This literally brought tears to my eyes. My grandfather had a stroke that robbed him of 15 years of his speech, spark, mobility, and ultimately his life. Having something like this would have been a miracle and would have allowed my children to get to know him the way I knew him.

    My father had a stroke 8 months ago. His speech is what got impacted. He still can’t speak but he is cognitively all there, thank god. Seeing engineering like this is truly remarkable. My heart goes out to everyone who is a caregiver and to everyone who has suffered a stroke.

    GUYS LISTEN! my mom is doing the same treatment and it seemed as nice as the video until i started to notice the look my moms face when the AI spoke after a while. I decided to use a visual alphabet so my mom could tell me what was wrong. The AI was speaking for itself and my mom felt trapped because it wasnt her talking. This is truly so evil

    As an artist and someone just breaking into work life I am often so frightened by AI, but this is a beautiful reminder that even as it brings its many pitfalls to the table in other fields it has such potential to do good.     I understand A.I can be scary as an artist. The best tip I can really say, as a developer is that A.I Art cannot be made without pieces to be trained on, or understand nuance & themes. A.I Art is Art without the soul. You will never be replaced.

    As someone who was unable to communicate with my sick paternal grandmother whenever I would visit her in Bosnia (mind you I only got to see them every other year, for a total of 5 times) because she had suffered a series of strokes in the early 2000s I NEVER got a chance to hear her beautiful voice. Whenever she saw me she would break down and as a kid it would sadden me a lot seeing my sick grandmother cry, but I was also scared and confused. It was only until I was a teenager that I realized she was crying because of how much it hurt her to see me but not be able to communicate with me. She eventually had her leg amputated too but lived on to her mid 70s - she passed over 10 years ago. I don’t expect people to read this but if this technology was around 15 years ago it would have made all the difference. I would do anything to see her again - even just one more time - to have a conversation with her and see her smile instead of cry😞 I love you grandma 🫶🏼

    Artists aren’t losing their jobs though. I get where you are coming from, and in many places in the creative process, certain types of people are getting cut out, but people still value “art” because it is human expression, and people will still pay attention to artists. If you would be so bold as to be optimistic with me, there is a possible future where people doing menial parts of the creative process are freed up to work on more meaningful things instead, like brainstorming. Plus, the creative process can be enjoyable, and I don’t think there are going to be musicians that just say “ai make a song with this melody” and be done with it, they enjoy making music because it is a challenge and a rewarding process, and ultimately that ties back into human expression and what makes art special

    There's a simpler method to communicate when someone has lost their ability to talk. No implants needed. Its called Eye Gaze, and its a system that has given thousands a way to communicate. There are videos on KZread.

    I had cancer. Getting back the ability to drink water was great. I dropped too 110 pounds. Unable to eat at all. We are very thankful for over looked things. Her doctors gave her a lot. Many doctors are amazingly kind today. Great story.

My doctor thought it was a mosquito bit. It had no bite mark. About 1 inch squared. But round shape. After 2 months i had 4 growing lumps. Traveled down too my upper chest. Fast moving lucky it did not go up. But did reach my tonsil AND TUNG. HORRIFIC CANCER

    Can you blame people though for abandoning though it can be hard to love a person who can never speak. Just know it isn't easy to take love that far.     With how AI is able to replicate voices so well these days, they can gather every footage with her voice from years ago and potentially make an AI voice that feels more like its actually her. It would really help uplift their home's morale a bit more, I think using this tech with the AI voice they have now might get to them as time goes on.

    AI voices are already surprisingly good, so I'm sure it could reach a point now with Metahuman style avatars and high-quality AI voice generation where a person's likeness in their prime could be pretty convincingly replicated.

    That's actually where I figured this video was going to go before I actually got to the part where they explained the tech, and yeah, I would hope that in the future they may be able to do that, even when the speaker themselves isn't actually talking, but rather just attempting to talk by sending those signals to her muscles.

    This brought tears to my eyes. Too often we see people with conditions like Ann and we forget that there is a person in that body that still feels and thinks. If they have the devices available to help them communicate, it helps them retain some of their independence. Seeing this technology at work and the smile on Ann's face, is definite confirmation of the positive impact it can bring to people.

    As someone with a disability that effects my ability to speak, this is amazing. I'm so thankful that I can usually move enough to use a conventional AAC and a little sign language but I am aware that could change and some days is already very hard. One of my great fears is loosing the use of my arms/hands and I've always thought it must be so hard to deal with that. It's already frustrating enough sometimes for both me and others in my life to wait for me to type what I need to say. I think a lot of my communication would just be too much effort to bother for everyone involved if I had to use an even slower method like eye pointing. The way that this could not only make communicating important needs easier for people, but also make communicating easy enough to make it more feasible to just have a casual chat just makes me so happy. The people who dedicate their time and skills to creating ways to make life better for people with disabilities are truly wonderful human beings

It does also give me time to consider a little more what I actually want to say and how I want to word things, and also helps me to weed out who genuinely cares to listen to what I have to say and who doesn't because people who aren't genuinely invested in it don't tend to have the patience to wait while I type or pause a conversation for long enough to let me contribute to it

I am a bit of a scatter brain myself and might forget stuff but if we are having a full blown conversation, I could not see myself looking away from you or the monitor. I pay lots of attention to people when they speak but I've seen people that converse differently still listen and be engaged no matter the many barriers, like language. So I truly wonder, who would be rude in such a situation? Why? Well, at least you found a positive way of seeing things. I do have to learn from you, one of my very own fears is being forgetten/forget people. But like I said, a bit of a scatterbrain so I might end up with dementia. Some of the people in my life have the excuse of being neurodivergent (as I also am myself) so patience is harder for them, but some are also just kind of disrespectful and not all that nice. It gets to me less now than it used to. I mostly understand where their attitudes are coming from which helps me not to take it personally even when it's frustrating to be cut off from being able to communicate. I can certainly understand the fear of forgetting or being forgotten. I have a pretty appalling memory too. A couple of my relatives have suffered from dementia and it's such a cruel condition, the way it strips away so much of who a person is. I think the worst part must be when you're still with it enough to know your mental ability is declining and to be aware of how you are likely to end up. It must be terrifying. I hope that things don't end up going that way for you. Blessings to you also

It takes time for things to improve to trickle down the cost, we're only now seeing this one in its early life so obviously most won't be able to afford it. For example, back in the day (earlier than you may think), video chat was exclusive to rich people, now anyone can access it even the working class. It took over half a century, but we got there. TVs, Phones, Refrigerators, Microwaves, medicine, treatments, you name it, it always started where only the rich can afford. Unless you're in a pandemic... suddenly everyone matters, but the rich still got it first.

Most modern medicine was or is expensive before it becomes accessible. That doesn't make the tech bad, it just exposes flaws in the systems we use to provide it. Do you also call proesthetics and other mobility aids crap? How about glasses, eye surgeries, hearing aids, or cochlear implants? All of these aids were once inaccessible. Some of them still are to varying degrees. But continued development of them and improvement to varying systems that create and provide them have helped close the gap. It is foolish to think the same isn't possible for this tech if people continue to work on it.

    @@pop4843 My brother as well sadly has worsened his condition this last year. Right after he got Covid his epilepsy had a sudden increase in frequency and he started taking pills for it (before we managed with valium when needed). He always had a hard time expressing emotions in a "simple" way, but after that his impulse control got through the roof... He started screaming (as he already did, but not as) loudly and constantly, expressing great anger or surreal laughter alternatively with no logic, or just trying to say things but with no control or without clarity. Both by himself, with me and my family, or with other people... He was another person before and I miss that. I wonder how an experiment like this could go with him, what words could be expressed. To have a proof he can communicate with me. I wouldn't care if the first words he expressed towards me were insults, I'd take them lol. I don't believe so much in the sentence "I cannot imagine how hard it can be". All I can say is that we both (you with your brother and I with mine) know how mentally and phisically taxing all of this can be.

    My brother had ALS. He never lost his ability to speak, but he used the eye-tracking software to do work (emails/documents) and also created music with it (He had lost his ability to play piano). His music was played at his funeral in 2020, and then at our father's funeral this year. It was so amazing!

A video game can help with being active, or think on strategies, there are some that use hand language And you know.. The world needs games that blind people could play!     My brother had a stroke this year at just 30 years old. Mentally he is fine and he retained all his cognitive functions, but he has little to no control over his muscles. The most he can do at the moment is shake his head slightly and move his wrists a short distance. This video gives me hope that one day technology and AI will be able to help him regain a somewhat normal life.

    Omg. This was beautiful. I couldn't imagine having such young children and not being able to be the parent that they need. Watching them grow up has probably been heartbreaking.     You definitely can do it, I believe in you! My life was changed by modern medicine and some good doctors (*). I'm sure you can be one too. It's sorta funny, I had some pretty bad doctors and was on some _bad_ meds for a while, so I actually read so much about bipolar d/o & ADHD and various meds that I ended up deciding most of the changes (and a doctor listened). That research made me realize how cool the human body is, and now I'm applying to grad schools in biotech. (*) - I mean mood stabilizing meds. If I was born in '80 instead of '96 I would likely not be alive by my own hand.

My 97 year old dad is 97 and has dementia, and I cannot communicate with hi anymore, and it is very discouraging. These young scientists are breaking barriers, and advancing speaking aids to a phenomenal level. They are going to change so many lives for the better. What a gift to give people. Thank you guys.

    Five years have passed since my mother suffered a stroke following a poorly executed surgery in Mexico. She didn't live long after that, and I wish this technology had existed back then to hear her final words. I applaud these brilliant individuals for the development of this technology, as they are not only restoring speech to these individuals but also happiness.

    This is absolutely amazing. Ann and her husband are both so strong to have made it through so much, and the researchers who've worked to give Ann her voice back should be given all the recognition and funding they need to continue advancing the technology so that more people like Ann will be able to actually talk to their loved ones again.

    As a wire that was connected to her skull i think they did a wonderful job
