High Blood pressure and the White Coat Syndrome

In this video, I discuss how we can work out who is more at risk of the consequences of high blood pressure and therefore warrants aggressive blood pressure medications and I also discuss white coat hypertension

    I’m a Critical Care nurse and I absolutely agree with you! It’s sad that many patients are being treated for the numbers rather than the symptoms, and they come back with more complications due to side effects of medications...     I agree. When the cuff starts squeezing my arm, I panic. Why? Because as a little child, the doctor sewed up my split lip while my mother had to hold me down by my arms. This was done without deadening. Otherwise, without the cuff, it’s usually fine. This doctor is so smart and wise.

    They have these new cuffs when they check the blood pressure I have Narcolepsy Cataplexy.It reads differently after using the right cup

    When I go to the doctor I want to ask about the enlarged heart .and. Blood pressure because I really don't know what is really going on I had the flu. They took an x-ray on it showed enlarged heart so may be asking wrong question just asking an don't know what is really going on.

    I lost my hearing in my left ear due to BP med, diuretics, I was passing to much sodium in my urine so I was hospitalized due to server low sodium levels.

    @@reneharris1441 Get the book Miracle Medicine Foods by Rex Adams and read the chapter on the heart, etc., especially the entries on enlarged heart. And ask your doc about that after.

    So the process of dealing with a white coat syndrome patient has never made sense to me. They take my BP when I arrive and it's high. Then they say, try and relax and we'll try again in a few minutes. In my mind the stakes are now HIGHER so my BP stays elevated. Then they say, come back in a few days and we'll try again. SO now the stakes are even higher and so is my BP! I keep telling them, you'll never get an accurate reading as long as I'm in this damn hospital. Luckily, I have a monitor at home that gives me a realistic daily reading.

    Yep, I can't even take at home. Now I get anxious just thinking about taking it. At this point even the thought of taking it makes me anxious. I have no doubt what causes this. But at a loss to figure out what to do about it. All I think of is stakes. It's kind of cyclical.

    I am exactly this way.. my BP doesn’t lower until I get away from the dr. Within 1/2 hour my BP is normal about. At the Dr. my BP goes as high as 200/100 .. it’s crazy

    I think I should thank you for saving my life. I have serious white coat syndrome, I believe by nurses over the years scaring me about blood pressure readings, which were normal to start and I began to be freaked out by the cuff. I am 80 years old, in perfect health, free of any high pressure damage- as determined from eye scans to kidney tests to anything. All is perfect. Well my doctor kept trying to get me on meds. I got a very excellent monitor and began checking myself at home, As my phobia began to ease my readings got lower and lower until I'm always getting a resting number between 128/83 to 123/79. Well my doc still wated me to get medicated but I refused and she threw a little tantrum over it. And I found your site and read some criticism of the new big pharma reccomendations and I'm not taking anything. Thanks so much.

    I’m 68 and have the same problem…I will do anything to stay away from meds,because the money and the worst thing the side effects…as a former employee of a nursing home and watching all the meds they used to take,never see a big benefit besides the ones for pain,stay safe and healthy😉

    I have never taken a HB pill. Four years ago I woke up with high BP 180/110 (most likely too much salt from eating sushi all week. It took 10 days with a no-salt diet and losing some weight to get it back down below 140/80. I modified my diet and lifestyle for 30 days and got it all the way down to 110/68. I did 30 days with Plant-Based Whole Foods with No Salt, No Oil, No Sugar, No Alcohol. Glad I figured it out on my own. I know a doctor would have medicated me. This doctor is the real deal.

    Every time when I measure my blood pressure I feel how my pulse increases immediately and I become anxious because I am afraid of the numbers

    I'm like this too. As I've gotten older the numbers have gotten higher and it drives me crazy. I'm always running scared from the BP cuffs now, even at the pharmacy machine ones. lol

    I've suffered with this problem for years, Domlika - even when I'd take my own blood pressure - the numbers were elevated - not as high as when I'd have it checked by the doctor but still high enough. I learned to relax over time, though, and now, I get normal readings pretty much all the time. You will too. Just stick with it! 😊

    Just keep taking it once or twice a week, and do it when you're watching a show you love. Don't pay too much attention to the machine and just let it take the reading while you watch your show. Also, take a few slow, deep breaths with your stomach, not chest. Take it three times and just log the numbers and let it go. Don't think about it much. BP is something that is labile. That means it goes up and down. You will have good days and bad days. Over time, as you keep taking it, you will notice that it will go on a downslope as you're less and less stressed about it. What happened to you happened to me in the beginning too. It's brought on by fear and anxiety, and it's a vicious cycle. You're controlling your salt and processed foods, right? And exercising and doing all the necessary things? All my best.

    Kudos to you sir. Possibly the most informative clip I have seen on YT. I was put on BP meds because my Bp was around 200 at the doctors office. I began, after suffering terrible side effect for more than a year, to monitor daily at home while weaning of the drug. It has now been completely out of my system for 2 weeks and is averaging 138/75.

    After I was diagnosed with high blood pressure, when the doctor would come into the room and wrap that cuff around my arm, my numbers would go through the roof. Luckily for me, my doctor was aware of my anxiety and recommended that I get my own monitor. I kept a journal of all my readings, and my doctor adjusted my medications according to this, so yes - when Dr. Gupta talks about WHITECOAT ANXIETY, I know all about it. 👍

    Yes, I can relate to this, automatically BP goes up, doctor has increased my dosage,but I'll definitely be taking my own readings in the security of my home

    Anything medical freaks me out big time!!! 4 weeks ago was 243/147. Doctor scared and terrified me to the point I felt all giddy and faint. Doctor was ordering me to be admitted to hospital, anyway survived that stress. Walked out in complete haze, thinking, I m gonna sort this out myself. I was loaned a monitor, last reading was 191/121, its dropping, and yes, the thought, and the monitor freaks me out! I get hyper anyway, making changes to my lifestyle are helping me regain my health. Putting it bluntly, probably had this for a very long time, logically thinking I will be strong, and get it under control without pills - which I cannot take, as I have severe anxiety with anything medical

    You clearly have high blood pressure exacerbated by white coat syndrome. A normal blood pressure wouldn't spike to 243/147 from white coat syndrome alone. If you're not going to take medications, you need to change your diet and get more exercise immediately.

    ​@@excalibur9768 if you have an anxiety and panic attack disorder you absolutely can get those numbers with a bad panic attack. At home I'm 130/90 tops.....in a doctors office I'm 210/120 easy

    I like your theory of educating patients instead of treating with pills--unfortunately educating patients does not bring in money for the medical establishment. That why we as patients do well to educate ourselves--there is so much information out there that we can avail ourselves to and be educated about health matters! It is doctors like you that help us understand health matters and educate us.

    Love this video. I'm 28 years old with White Coat Syndrome and when I'm severely anxious my readings have been anywhere at 144 to 150. When I calm down my readings are at 125. It's crazy because most of the time the medical professional is always trying to tell me I have high BP, which makes me even more anxious. I've only had one person tell me that it's from anxiety, but it was because they took multiple readings and noticed that my BP was fluctuating. They asked me if I was anxious and I said yes. Gave me a BP medication tablet and waited a while for the next reading, it was low, waited again for next reading...it was high again. She said okay, this is definitly anxiety. Of course when I'm not anxious readings have been normal in the past 🤷🏽‍♀️

    I was checked into USC teaching hospital in Los Angeles for a week because 2 years worth of Drs couldn't put heart palpitations and premenopause together. Now when I see a BP cuff coming at me I panic.

    I wish this man was my cardiologist. So much good sense. But - Twenty-four hour monitoring also makes my blood pressure go through the roof, because it still seems to count as a 'medical environment'. The readings I take as a daily routine are the most consistent, and not at all worrying.

    This is so good! I went to the doctor in 1999 for a cough. My BP was high and I was put on RX. Never asked what I ate or did for exercise. When the new BP guidelines came out she doubled my dose. My now former doctor insisted on changing my BP RX because my BP was up at one appointment. I had brought readings showing my BP was good. I gave her a peace of my mind and walked out.

    On the white coat effect. My last BP readings a few months back at the surgery were pretty high, around 165/95. I think I’m socially phobic to start with. (I have no friends). But recently I’ve lost a lot of weight and walked an avge of 5 miles a day so wondered if it might have made some difference. I dreaded going to the surgery to be disappointed so today I got my BP monitor out of the cupboard. I hadn’t used it for so long (as I’d grown wary of the probable results) the batteries had crusted up and needed replacing. I nervously tried it a few times and was hitting about 155/75. Very disappointing. But it was very cold in the house and I was very hungry so I turned the heating on and ate and sat down again later, feeling a bit less apprehensive. I did 11 readings each spaced by a few minutes rest. The avge of those was 129/68 and had got progressively better as I went. The two lowest readings were 118 (over 67 and 68) and highest was 142/74. The point of this is that getting myself used to the process by repetition seemed to normalise it. My growing confidence/relief as better results emerged seemed to affect the result. I should add that the readings were lower when I realised DIDN’T have to breathe deeply or abdominally or otherwise try and control my level of relaxation.

    Im so glad you mentioned the reverse white coat syndrome. I have been sent to a & e before but then when I'm in a hospital it comes right down. I also get extrordinarily high readings at home like off the chart eye wateringly high but again gone to hospital and it's reasonably ok. I think I'm going to schedule a chat with you my case is very very complicated. You are the most logical person I've ever heard talking about blood pressure

    I wish more doctors were like you Dr Gupta , most doctors nowadays don’t educate patients well ,they just write a prescription with barely any explanation as to the patients problems - it’s very sad that there is no more real doctor person relationship- thank you for all your informative videos this helps so may people including myself

    Dr. Gupta, you are a GODSEND! I wish all doctors has your compassion and kindness. I also wish they would tell patients the diagnosis like you do. I was diagnosed with HBP many years ago, and at the time, I was not sure I really had it. Of course, they put me on meds...and I have NEVER felt the same. I get almost lightheaded at times. I have had all kinds of tests on my heart, and they have never found anything. I hate being on meds. I had a scare a few years back, and realized I had to change my life, so I began eating healthier and exercising daily. I have lost 65 lbs but still on meds. Anyway...keep the fantastic videos coming.

    ME TOO!!! I even get that way at home if I try to take it. I take it and it will be a little high, and wait about 5 minutes and take it again, and it ALWAYS comes down to close to normal.

    I went to orthopedic Dr and first appt no bp check on 2nd appt surprised me wth bp check 115/76. Next week went to GP obsessed with my bp, it was 150/100. Second check was 136/93 , she panicked and increased med. I can't take it. After 3 days, often near 90/60 and want to sleep.

    SUBSCRIBE. This is brilliant. My mom is 60 and has been a hiker, runner, active life style enthusiast her whole life. AND she’s been diagnosed with high bp (with no damage) since she was 16 and on and off bp meds that I watched her suffer from side effects on my whole life. I too live an active lifestyle with a healthy weight and no health problems but from watching her as a kid, I freak out every time my bp is taken at the doctor and they always hold me there until it’s down (causing more stress). I go home and take it and the numbers are PERFECTLY HEALTHY. This anxiety about getting my bp taken while I’m panicking and having all these doctors come in to threaten me with medication and a permanent diagnoses until I meditate myself out of panic mode has now kept me from doctors for years. Wish I could find someone like you.

    Both parents had this condition, I have it as well. I practice Tai chi, yoga, meditation, walking several miles a week. I lived under heavy construction in my building, for a year, my blood pressure was high, I didn't want to take a beta-blocker, because I was dealing with the reality of my environment. Your talks make sense and have a calming effect. I take hydrochloride 25 mg. Daily. Cholesterol numbers could be better but improve by watching my diet.

    I really appreciate videos such as this, where the focus of treatment is not directed to only numbers. For me, I have developed the white coat syndrome, which is new to me, and began a year ago. It began when what I believe was experiencing my first ever panic attack at 34 years of age. I have never experienced anything like this and because of it, I went to the closest urgent care because I wasn't sure what it was at the time. Even being an EMT, I have come across patients who are having panic attacks, but it never occurred this is what I might have had. I had the urgent care staff perform BP workup, including orthostatic checks, and glucose readings, and an EKG. From that day forward for four months, I went to the ER three times in one month for weird feelings (I prefer not to make this comment too lengthy) and each time my BP was around 160 to 170/85 to 95, with pulse rates between 100 to 135 at those times. I went to my primary care doc who also got a reading of 150/80 something, one time, then 140/ 80 something. Then I went to a cardiologist, who after seeing me only one time and getting a reading of around 145/80 something decided to put me on a beta blocker. Even after having eco cardiogram done, and showing a very healthy heart. I then went to an optometrist for visual issues I was having randomly. I specifically asked him to check for High blood pressure, and he confirmed I do not have it (this is I believe what you are talking about when they view the tiny vessels in the back of the eye). This gave me some relief, however, because of the first few doctor visits and my primary care doc and cardiologist being so focused on treating the numbers, I have developed bad anxiety anytime I go visit him. My readings at home using a wrist cuff, have been in the low 120s systolic and low 70s diastolic. I am very active, eat healthily, and I am not overweight. I had so much blood work done, that all came back normal, which again supports your outlook that when everything else is normal, there is little reason to worry or treat something that is not already normal to the person and that there are so many other factors. In conclusion, your insight was great, and further confirms my thinking, in order to help me reduce my health-related anxiety I now suffer with.

    Thank you so much!!!! You’re the ONLY doctor that makes perfect sense. The trouble is, once you’ve been put on medication you become scared by your doctor to go off it. My doctor won’t even reduce it in small increments, even though there are no signs of low level damage at all. My readings are 107/65 at home. It’s ridiculous, but where do you go for help. They’re all on the same bandwagon. Ugh!!!

    Thank you so much for your great info. This has given me a different outlook on my blood pressure. Stress is another factor that adds to the problem. I have fatty liver, leaky gut, psoriasis and Bell’s palsy. This all has added to my stress level. I won’t fight taking my blood pressure pill now. Thanks

    RN here. I have “white coat syndrome”. Years ago during a doctors visit, I allowed a doctor prescribe BP medicine. Big mistake! It bottomed out my BP and I had to be rushed to ER. IN THE ER I was fortunate to get a doctor who told me to throw the pills in the trash and he spent a few moments educating me about my BP and how many people are taking hypertensive meds who shouldn’t be taking them! I will never take them again no matter what. I’m 72 now and I’m fine!!

    Can you please share what the ER doctor teach you. I have very normal Bp when I am relaxed. However, can get pretty high at doctors and when anxious. I cannot get rid of the thought that I have hypertension.

    Yes thank you I have same issue and I am a CHHA my primary care put me on BP pills I had weakness in my legs cold feet never ever have I had that until I was put on BP. Other thing they put in my medical chart and was never told about they labeled me with anxiety disorder I’m like wth I’m not being treat for that and I’m only anxious when I go to the drs

    So useful as usual! And clearly explained. I am so weary of being medicated by population and my major risk factors than by specific info they know about me and my body. I have yet to have a doc ask me if I know what the general range of my BP over my lifetime, so I can understand why it may be of concern. Re risk of stroke: they advise more and more do this or that because of risk of stroke, but when I ask why the risk of stroke the response is again explained in terms of a population risk factor.

    I have been studying this for YEARS and you are the very first doctor that can really "sort" the whole process for me! I have had "higher" blood pressure(average 145/70) with some freak high numbers of 160/90 and some very LOW readings of 120/70. White coat syndrome has been a problem for years because every doc I see freaks out about my number. I anticipate this and immediately am higher. This always go down after leaving their office. I did do meds, had terrible side effects and have since abandoned them. My other risk factors are virtually zero and the doctors admit that but still pressure me to take meds. I now have printed the study you gave in another video to take with me about the NNT--THANK YOU SO MUCH! You have NO idea how relieved I feel after listening to you!

    Thanks for sharing this. I have an extreme anxiety when going for a doctor visit. I can measure my blood pressure at home and it’ll read fine 119/70 or max 120/78. At the doctor’s office, it’ll read 190/100 or higher. Not sure how to control this :(. I posted the above two years ago, but below is a new update. Because of my severe white coat syndrome or extreme anxiety, my doctor decided to refer me to a cardiologist just in case. The cardiologist did an echocardiogram to make sure my Heart is well. Thanks God! He found nothing. They finally rule this as an anxiety. They’re finally listening to me.

    Thank you so much for this video. Too many people are put on BP meds much too much! I have high BP but not consistently - only in a medical setting or a very stressful situation. The rest of the time my bp is within normal range. Thank goodness I have a very wise doctor who has never put me on meds for my BP! I’m 74 and doing fine.

    Thank you for explaining High Blood Pressure readings, it really makes a lot of sense to point to the actual individual. I suffer with HBP but live normally without any obvious low level issues. I was diagnosed with thin eye arteries or veins to my retina. However no effect on my vision. I do get white coat syndrome always. The DR. Usually does take a reading later in my examination which is lower. I take two prescriptions for the HBP which don’t seem to make much change taking the second pill. So you seem to be right on. However I don’t understand how to be measured for low level effects as my eye doctor did. By the way I am Italian born living in this country since the age of 6 years. I am now 67 years old. I appreciate your commentary it has opened my eyes

    Great information. Interesting some of my family have had a history of HTN and have taken the medication and have still had strokes.     Anxiety is main cause for me. Tried everything, deep breathing, trying to relax, etc etc, but as soon as that cuff tightens up my heart speeds up and I can feel my BP climbing.

    My BP numbers definitely go up when i am anxious. On some occasions, when i have gone to my doctor, my readings were better than they'd been at home because i felt relaxed there. Lucky me :).

    This video was so helpful! I have developed really bad white coat syndrome especially as I've gotten older. The moment I step into my Dr's office I feel my heart rate begin to rise and the anxiety begins. I've tried time and time again to control it and I simply can't. I'll tell the nurse everytime you're not going to get accurate reading and she looks at me a little crazy and says well let's just see what it is. Today it read 176/96 which is crazy high. I just looked at her and said I told ya! At home I get normal readings. I just wish more medical professionals would listen instead if making assumptions just to hurry patients along.     Same here. I have the exact problem. My heart rate goes up like crazy as soon as it's time for the b/p reading. It's gotten higher and higher under those conditions. Higher than yours. Meanwhile, I monitor it at home, take several readings a day and they are mostly in the 110/79 to 121/74 area, sometimes a little higher but never too high.

    Excellent Doc, my BP at home is always normal, pulse around 50, I'm a runner. Go to the Doc's office and it's always high. So I tell him the situation, and he says I want you to bring in your BP machine to make sure it works currently, so I do, and he says your just fine, no meds.     I did the 24 hr ambulatory monitoring (at my request - after watching these videos) and the averages were in the normal range. I was totally stressed out thru the whole process and had fluctuations with high readings. The doctor who had only seen me once 6 months ago ( reading was high, as usual) wanted to medicate immediately. When I asked her ( on a telemedicine conference with no reading) about the average numbers from the monitoring, she said the averages meant nothing. The length of time that the numbers were high was more important. She has never mentioned checking for low level damage. I have since changed doctors. Hopefully, I can get assessed and be reassured about my blood pressure. This is the biggest stressor in my life! Thankfully at 67 yrs old, I am vegan, work out regularly and use a host of natural methods to stay healthy! Thank you, doctor for your simple yet profound breakdown of high blood pressure and it’s treatment!

    You were wise to switch drs, Cheryl. She definitely would financially benefit from putting you on drugs for life based on a condition you obviously don’t have, which may give you awful side effects and create more issues. My dr understands I get anxiety in a medical setting bc of a bad experience I had with a bad dr years ago. He shared he’s even a bit of a hypochondriac who gets stressed about his health which is pretty funny. He’s cool with the at home monitoring as being accurate and said if I was truly hypertensive, I would never be able to get the good range readings ever. The readings can fluctuate 45 points for me, but we realize the high numbers only show up at the dr when that cuff is squeezing my arm and my heart starts pounding. It reminds me of a constricting snake and obviously we don’t walk around in daily life feeling that way very often.

    Let's face it - blood pressure is relative to each person, so ALL blood pressure meds are unnecessary. Worrying about it will CERTAINLY raise your blood pressure!

    This is a brilliant, detailed analysis. Most American doctors don't even measure the blood pressure themselves, passing it off to flunkies, much less look for the signs of peripheral blood vessel damage. Apparently it takes too much time and individual attention for them to bother. Just get a number and stick 'em on an expensive lifetime of side effects from medication. Particularly helpful is the division into three categories of types and risk. THANK YOU.

    Great explanation. Thresholds for real hypertension are different for people. Look for presence of low level damage before reaching for the prescription pad.

    This guy is great. Unbelievable that Europe has its BP tier at 140/90. That’s mine give or take 10 for each number and yet My docs prescribed 3 different BP meds. I stopped taking the 2 additional ones that were given on top my original 1 and my BP is right around 130-140 / 90 ish. I just need to eat healthier and work out more. Thanks for your help!

    I am so freaked out when I go to any clinic because I don't want medicine. I recently changed my diet drastically a month ago. I know my blood pressure is up because of the visit. I am on asthma meds and you know that elevates my blood pressure. After listening to you I am going to schedule an eye exam. I am very curious now. My liver was check a year ago and that was just fine. Thank you for explaining this.
