    @@wardaddy9489 I also had this surgery a year and a half ago. I have a little bit of numbness & tightness still but it isnt noticeable at all unless I think about it. :)

    @@wardaddy9489 Soo I had jaw surgery + genioplasty so its hard to tell you which effects were from which,,, it really doesn't feel rigid however there's just a little bit of tightness left (like... if you ever work out your forearms, you know how they get a little bit tight? Like that but not as noticeable). The benefits have been life changing, it truly truly has some amazing impacts , my face looks so much better and I'd have it done again in a flash :)

    THANK YOU! I had a consult with a plastic surgeon today about chin implants and I had this concern, and he disregarded it. I now know i'm more of a candidate for sliding genioplasty.

    My mum wont let me review getting this as she was offered it 40 years ago and they put death as a risk. I use fillers to augment my jaw but want a permanent fix.

    Could you graft fat onto the mentolabial fold in order to make the chin implant look more natural?

    Yes, you can use fat or fillers or sometimes just doing the correct chin surgery will help the fold

    Sir, I sent you some pics of my face a couple months ago and you said that I'm only a candidate for genioplasty (same situation as this man in the video). Thank you very much for telling this information because otherwise it would look not perfect. I'm now ready for surgery, already visited my doctor and he knows what I need. But I just wanted to ask you how dangerous this procedure actually is? I understand that mostly it depends on surgeon's experience and skills but still would like to know how you rate risks of it. Is it the same as the orthognathic surgery? My bite is almost fine so I don't want to get orthognathic surgery because I heard a lot of bad things about it. My doctor said that genioplasty doesn't effect bite at all so its not so dangerous and recovery time of it is faster. Also my lower lip is located behind the top lip and doc said that it will also improve my lower lip and will level it with the top. Is it true? Sorry for so many questions but you're the only doctor that I can ask besides mine and you're not interested in selling me anything so your words are very valuable to me. Thank you!

    It frequently improve the mentalis function, which you would think it would make worse. Each patient is variable, but overall I would say yes. Dr. J

    How in the world, would sliding your chin down and forward make a difference to your jawline? Wouldn't it be the same

    It changes your chin and profile, so it does change the anterior jawline, but not the posterior. Dr. J :)

    Would he not need a chin shield genioplasty in order to softn the labiomental fold whilst bring the chin forward with the sliding genio?

    The fold frequently improves enough with lengthening but if it is very deep, I add the center portion of a silicone implant to bolster the fold.

    Are there patients which don't have "enough" chin to thereby do a genioplasty? I'm wanting to get rid of a very uncomfortable chin implant that's left me with a severely stiff lower lip area.

    Some patients need their jaw and chin moved. If you chim implant hurts, get it out.

    It takes about 1 hour and 20 minutes with general anesthesia.

    Pretty sure this is what i have. I just had chin lipo last month but still no jawline. It seems my chin is receedong and my lower lip is slightly behind my upper lip, making me look like a duck. Will alising genioplasty also help even the lower and upper lip by moving the lower lip forward?

    Obviously I can't comment on your exact situation because I've not seen you. But a genia plastic can reposition the chin and the lower lip depending on factors.

    I had a sliding genioplasty done and ended up with a deep labiomental fold anyways. Anything I can do about this? I’m only 6 weeks post op, my surgeon told me to wait 3 months before talking revision but will my fold really get any better with more time?

    I would tell my patient to wait. You can always get filler in the fold

    @@jniamtuiii Yeah I mean that's an option but it's not permanent. Looking at my chin I feel as if I should've had some vetical adjustment as well, which would've opened up the labiomental fold. I remember the first surgeon I had a consultation told me he could use some sort of bone to make sure I didn't end up with a deep labiomental fold. Do you know anything about that? Is that a potential option as well?

    @@Blue_velvet266 honestly pretty disappointed in my surgery. My chin is still recessed even after a 7 mm advancement and my labiomental fold never got better. Sure I look better than before when my chin was recessed but I’d still really like to have my surgery redone. I went for a cheaper surgeon who actually worked with Niamtu and said he’d done the surgery dozens of times but still ended up with a disappointing result.

    @@Blue_velvet266 make sure if you get a surgeon they listen to what you want. My surgeon said he couldn’t tell me how many mm I needed my chin moved forward until the surgery but I made sure I told him to go with the larger advancement. After my surgery was over he told me he only had the option to move my chin 7mm or 9mm for some reason but opted for the shorter option thus my chin is still recessed.

    Sir, I did watch all your videos about chin implants! Thanks for all the information you share with us!!! I have the same situation as a patient in this video. I'm a male and have a deep mentolabial fold. I need to move my chin forward to almost 10 mm and also would like to increase it in a vertical way as well (at least a little bit), because its pretty small in all dimensions. I cannot do genioplasty due to health problems, my plastic surgeon says that they can use fat grafting in my mentolabial fold so it will look good and place an individual implant in such a way that it will increase both the horizontal and slightly vertical projections. Tell me pls is that possible? I really have no other choice :(

    Personally not something I would try as it can be permanent. Personally I would try chin filler and filler in ML fold first. You can reverse that.

    Permanent? I thought you can put out implant if you don't like. Doc said fillers won't do much since its almost 1 cm recession. Or you mean fat grafting is permanent? They said sometimes you need to do fat grafting 2-3 times because not all fat can take root the first time in the place you need. Thank you!

    Dr. Niamtu, what's the largest advancement that you've done and what's the largest advancement that you (personally) could do? Also, concerning larger advancements (assuming you perform them), do you have experience using bone grafts from the iliac crest?

    I have advanced up to 15 mm but routinely do 5-10 mm advancements and generally don't use bone grafts unless it is very large advancement.

    A properly done genioplasty will help the crease. Filler can be injected.

    What if the patient is having a Class II malocclusion (overjet+deepbite), a convex profile, a small mandible with a deficient chin? Genioplasty or BSSO, which will get the best result in this case?

    hi dr joe. there has been a new implant out designed by doctor eppley which is a vertical lengthening one. what is your opinion on using them instead of doing a genioplasty?
