Cummins also, until recently, had the ECU programmed by default to have the injectors wide open below certain temperatures until the engine reached a suitable idle. The large rev followed by hunting for a small amount of time is fairly typical behavior.

All 12V's are mechanically injected, these have p-pumps from factory or have had one swapped in. This pump has static timing, and doesn't flow full fuel at start up (emissions regulations). I'm also guessing they have their timing advanced and big injectors, both of which don't help cold starts.

I have 2 trucks for this and other reasons. I don't have power to my diesel for a block heater and don't want the wear and tear on it for short drives or cold weather.

Cummins engines seem to start like shit even when chilly.. I’d consider cold the teens or below, I’ve seen these things struggle in 35-50 degree weather. I have a Deutz diesel with a grid heater that doesn’t work and it lights instantly down to 15f or so then it take a few seconds of cranking.

These trucks all have p-pumps, the VE ,VP , or CR trucks start up much nicer. These guys also have advanced the static timing, making start up harder. Big injectors don't help much either.

@jackson meads P7100 Bosch injection pump. They are used from 94-98, they are the couple thousand HP trucks you see. Those are heavily compromised for daily driving and have static timing.
