    I bought your TRT cream ebook. I thought an important bit of information was missing. There is no mention at all about washing the scrotum before application. This might seem like common sense to do but washing twice a day seems a little excessive. But maybe necessary… How often is it necessary to wash the scrotum. Is once a day enough or is it necessary before each application (twice a day). And is it ok to apply the cream when slightly damp or must you be completely dry?

    It is not some other factor in injections causing elevated hematocrit. It’s the testosterone. But specifically, it’s the testosterone that stays around longer, without troughing, via using a long ester. Prop will not do this (as much as a long ester). Same with cream, because it peaks in the serum at 2.9 or so hours, may stay peaked for 1-2 hours, but ultimately troughs. That’s the explanation.

  • @keithnicholsmd6896

    @keithnicholsmd6896

    All methods of delivery increase hematocrit. It's what testosterone does with adequate levels. It is dose dependent and serum level dependent. Higher serum level trough concentrations, and not duration of treatment predict its development. The cream cause it's just as much secondary erythrocytosis as Injections when the cream was applied twice daily and optimal levels are achieved. There's only about a 23% -25% Drop from Peak levels 12 hours later. Nobody in this country has more experience with the cream and if you do your history, you'll find that I was the one that made popular scrotal application of high concentration, testosterone cream. Moral of the story is that when the cream is applied twice daily, there is no significant trough

  • @CortexLabsChannel

    @@keithnicholsmd6896 right. No one is disputing that. And cream doesn’t have to be applied twice daily for everyone, and if the right amount is applied in the morning, it’ll take guys through the entire day, feeling good, with a trough, which will prevent hematological parameters from being problematic, while still keeping them therapeutic and physiologic.

  • @keithnicholsmd6896

    @keithnicholsmd6896

    @@CortexLabsChannel even with once a day application, you're going to get an increase in hemoglobin and hematocrit that does not differ significantly from shorter acting esters like testosterone cypionate. So the cream, whether it be once daily or twice daily is going to cause a secondary erythrocytosis in most men when they have optimal levels. And I will be doing a lecture on this very channel in the next couple of weeks on the secondary erythrocytosis from testosterone, and it is not problematic and it has never caused harm in 85 years of using testosterone

  • @CortexLabsChannel

    @CortexLabsChannel

    @@keithnicholsmd6896 yes, but with a trough, because the ultimate determinant of hematocrit issues is the free test at trough, you have less of a concern, and less side effects relating to HCT creeping up (head pressure, SOB, headaches, lethargy). To make a blanket statement that high HCT is not a problem, is both myopic, and dangerous. I guess all of those highly qualified MDs suggesting therapeutic phlebotomies are just.. making stuff up. And all those on the forums, with symptoms when HCT gets up above range, are also making things up. Just like the guys with Estradiol that goes far above range. Sure thing, “doc.”
