Epiretinal membrane, ERM / Macular pucker, film on retina - A State of Sight

Ғылым және технология

An epiretinal membrane (ERM) is a thin abnormal layer on the surface of the retina in the back of the eye. This can cause distorted and blurred central vision as well as swelling and thickening of the retina.
Watch this episode of A State of Sight with Isaac Porter, MD to learn more about ERM and the possible treatment options.

Very informative. I was just seen by a retinal specialist. Mine is not bad enough yet for surgery. But he provided me with an Amstel Grid to check my vision daily. If the distortion increases, we will meet then. If not, every six months.

Thank you Isaac - when the vitreous gel is removed why does this then cause a cataract to form in that eye? Is the entire vitreous gel removed or just the part not sticking to the retina? If one already has a partial detatched vitreous gel, would that likely have been the cause of an existing cataract? Can an epiretinal membrane peel off naturally in time and perhaps the peel disintegrate also?

I am seeing a small blur spot in my central vision in left eye, through which I am not able to read far from that eye only, otherwise I'm all okay with both eyes, does this require surgery?

Question: Does an epiretinal membrane ever get misdiagnosed or mimic the symptoms of wet macular degeneration? Background information regarding my situation. 38 year old male, normal 20/20 vision, with no medical issues prior. Including eye sight. Back in late December of 2017, I developed bulging in my central vision almost over the course of 1 day. Curved straight lines and distortion to central vision(eg faces, words). The preceding night prior to vision loss I was at a staff Xmas party. It was a late night and lots of partying/alcohol. The next morning was very hungover and had a lot of heavy dry heaving. Went out and while getting breakfast did have a tough time reading menu above. Did not make too much and sloughed it off as a "bad hangover: The following day while driving, noticed I couldn't make out street names on signs. It wasn't enough to send alarms, but after supper while watching tv, I noticed the black bars on the top and bottom of widescreen image were warped. At that time I discovered it was specific to my right eye. After consulting a family member, we made the decision to go to ER, to rule out a potential retinal tear. ER was basic eye assessment and seen by ophthalmologist early next morning(roughly 48hrs from first onset). Vision was 20/20 in L eye, and degraded to 20/70 in R eye. The next morning vision was more effected in binocular vision now(faces, words, central detail). Fluorescein angiography was done and referred to eye clinic same day to be further assessed by 2nd ophthalmologist. OCT scan was done in clinic that day and basically was offered Avastin injection as treatment(Xylea available but not covered until 65yrs old). No concrete Dx was given at this time but specialist basically said it would be treated as atypical wet macular degeneration(to R eye). I thought it was odd at the time due to my relatively young age but trusted the specialists assessment. On a side note, it is very difficult to get much info from the doctor due to how many patients they see in a day and clinic is almost overwhelmed by number of patients to see in a day. Within about 2-3 days vision had improved significantly, both in binocular vision and also when I isolated R eye. Was seen and examined by optometrist roughly 3 weeks later and vision was 20/20 in L eye, and 20/25 in R eye. I was seen in eye clinic next week(4 weeks from onset) and another injection was given. There was a dip in my vision acuity for a few days but restored after about a week. I could still notice slight curving on telephone polls and straight lines. I continued to be followed in eye clinic by ophthalmologist roughly Q6 weeks, receiving 3 more Avastin injections(5 total) to R eye. This would be July 2018. Vision has remained stable since the 2nd injection and returned to 20/15 vision in both eyes gradually. I do notice a very slight distortion at the current time(Jan 2020) and on Amsler grid, very slight curving of a few boxes in the lower R quadrant. Very small floaters(4-5) did become present 2 months after onset. So where I stand today, I am followed by ophthalmologist every 6 months for observation as long as it remains stable. Last injection I received would be July 2018(roughly 18 months ago). Just curious about your thoughts, as I stated, it is very difficult to consult with specialist as I live in an area where he is the only one available. It's always be a concern of mine with the initial loose diagnosis of atypical wet macular degeneration, when I factored in my young age, rapid onset, and following the events of heavy dry heaving, and the presence of floaters. It could be a coincidence, but after researching and coming across epiretinal membranes, many of the symptoms appear similar and would align with the events leading up to better. Any thoughts?

I have recently been dx'd with this in both eyes. The left side is more so. I think of it like drapes or curtains. The film is rather like a sheer drape.

What causes scarring on the retina. A long line that distorts vision. Especially with other eye closed is when I noticed it. The retina specialist didn't give me a definitive answer as to what caused it.

At times, it can be hard to determine the cause of a retinal scar. An old injury or old inflammation could leave a scar. Other times, we are not sure what caused the scar.

I was diagnosed with a retinal pucker 10 year before I did not go for the suggested surgery. Now I have developed a cataract in both eyes and surgery for pucker. I have no problem with pucker. Should I go for or only cataract?

Please have them taken out. You I promise do not feel and pain only the Dr working on them. They do one at a time and I'm truly telling you it doesn't hurt if you are asking that? Have it done. I have crappy eyes from my mom...she did nothing back in those days. I'm 73 and I just don't trust Drs especially the ones here in Logan UT only good Dr I've found in my back surgery Dr who after I had already two done my same surgeon big mistake. Dr Felix is the BEST. He fixed the mess this Dr Vernon did a mess and I walked after 8 years without a cane or walker right after surgery...so have your eye surgery not sure about these problems afterwards and only sleeping face down?? For months yikes

*An epiretinal membrane can distort vision* Epiretinal membrane (ERM) is a thin film on the surface of the retina that can cause decreased vision and distortion. This is a common problem in older patients, but it may not cause trouble in many instances. When an ERM worsens, it can distort the retina, cause wrinkling and swelling of the retina, and may need surgery to restore clear vision. This specialized operation is performed by vitreoretinal surgeons. Watch this episode of A State of Sight to learn more: kzread.info/dash/bejne/pHx2rdayedbXdMo.html #retina #vision #AStateofSight

Isaac Porter, MD Doc I'm only 19 yrs old and today i just found out that i have this ERM on my right eye.. they told me that i should leave it for now because my vision is 20/40 with glasses.. and that i just should take the surgery when my vision in the right eye got worst.. can you please give me some advice on what should i do? because i really need a good vision in my line of job.

Kenneth Sibug I always advise my patients to seek a second opinion when they are unsure about their eyes. If an ERM is affecting vision, it may take surgery to improve vision. However, this surgery generally causes a cataract to form and can require more surgery in the future. You may want to check with a good retina specialist in your area to learn more. Thanks for the question!
