Neurosurgeon here. I’d like to add some corrections: I‘m sorry but you got the epidural space wrong. The bleeding is between the bone and the periosteal dura. The venous bleeding sides are incorrectly drawn. The cause ist basically 100% traumatic. There can be epidural empyema but not hematoma in infectious cases. Coagulopathy causes subdural, subarachnoid or intraparenchymal hematomas not epidural. Avms do not cause EDH but DAVF could technically (never seen it tough). The reason for the lucid interval is the accumulation of blood during a conscious phase after initial brief LOC , there is no venous shunting. Spinal EDH can cause paraplegia not only cauda equina syndrome. I have often seen parapareses or plegia but never cauda syndrome in EDH although possible. Your herniation explanations focus too much on arteries, it is basically a problem of tissue and capillaries. It is a matter of volume: 1. there is the subfalcine, then there comes the transtentorial herniation. Forget about a second epidural. Duret hematomas are an intraparenchymal bleed of the brain stem with 100% mortality rate and have no real association to the basilar artery. Burr hole is the wrong answer, EDH is coagulated, you won’t get it out via burr hole. MMA embolization is great to stop an active small bleeding but if there is already compression it won’t help you. And they use coils or glue, never gel. Otherwise fine video.

I suffered a TBI (specifically an epidural hematoma) when I was 13, and had an emergency craniotomy after I coded. It showed me my passion for medicine, and I now work in a hospital and hopefully will attend medical school in the next year or so. This was a fantastic video, easily digestible info. The anatomy part of this has a little mistake. Epidural hematomas don't bleed between the periostal and meningeal layer of the Dura mater, that would be the Sinus durae matris. It bleeds between the bone and the periostial layer :)

My sister was hit by a Toyota Tundra inside of a crosswalk at low speed the driver stopped immediately and offered assistance. She was rushed to the hospital via ambulance. She is in an induced coma with severe TBI and skull fracture. They can't control the ICP. I can't understand how she is so injured.
