In this episode, my guest is Dr. Michael Eisenberg, MD, a urologist and professor specializing in male sexual function and fertility at Stanford University. Based on his clinical work and research, he is considered a top world expert on male sexual and reproductive health. We discuss testosterone levels and what really impacts them, testosterone therapy, sperm quality and counts, penile and testicular health and function, pelvic floor and prostate and urinary tract health, erectile function and dysfunction and the various causes and treatments for common male sexual, hormonal and reproductive challenges.     Could you please consider featuring an episode on the Vagus nerve with a leading expert as your guest? It's intriguing how practices like yoga, meditation, cold water can impact the Vagus nerve, and there seems to be a lack of in-depth podcasts exploring this topic. As a side note, I happen to have a Vagus nerve stimulation unit, and it has been a valuable tool for me.

    Hello Andrew and the Huberman Lab team 💙 I’ve been an avid follower of the podcast from early days. But this is the first time I take the opportunity to leave a comment here. First of all I want to say a huge thank you for the work you do to share this broad wealth of knowledge with the general public. I can’t say enough how grateful I am for this podcast has had a direct positive impact on the quality of my life. I wish Andrew and everyone on the team good health and happiness. My question is not related to this episode per se. Unfortunately early this year I went through burnout at work. It was a difficult and scary time for me. Many episodes from this podcast have supported me in my process of recovery. On this path I have come across the Neurointegration Institute founded by Katerina Lengold. This institute has developed a program for NeuroBalance training aimed at combating chronic work stress and preventing employee burnout. I was wondering if Andrew, you may be familiar with their work? I would really appreciate if it would be possible for the Huberman Lab team to share their view on the work of the institute and perhaps other resources related to burnout? Thank you!     @@gustavrus Hello Gustav, I am really sorry to read that you are going through this difficult time. If anything i. The first place I can say put all your effort to restore sleep, rest as much as you feel is needed. Then I would really recommend you speak to a mental health professional. That’s what I did. Seek help from friends or family who you trust have the capacity to help. Also understanding the processes and mechanisms of the brain and body under stress was very helpful in giving me a rational reason not to put blame on myself.

    Would love more prolactin discussion. Particularly regarding high end of normal range of prolactin, e.g. 20-30 ng/ml. I'm at about 20ng/ml while taking daily Adderall (which I would believe would lower prolactin). From what I understand, tumors will put you in the 100+ ng/ml area. What effect does elevated (but still "normal") prolactin levels have on the body over time, that is distinct from the effects of low dopamine?

    Thanks for talking about testicular cancer. I was diagnosed with TC six years ago and was so confused how this could happen. Luckily, my oncologist and my urologist said that if there was a type of cancer to get this is the one you want. Funny. Survival rates for TC are quite good. The problem is there’s almost 0 information out there for men in terms of recognizing abnormalities. Conversations like yours help to normalize this topic.

    Made an appointment with Dr. Eisenberg for earlier this year after reading his website and credentials. After waiting months to see him for some concerns, I was instead evaluated by one of his residents which seemed scripted at best. Dr. Eisenberg did come in at the end to say he met me, but that was it contrary to the post visit report stating he was in the room supervising the entire time. They did send me across the street for a blood test, but I never heard from their office again after the results came out. Frustrated, I found another urologist who did a proper evaluation where I learned I have an enlarged prostate, urinary retention problems, borderline low testosterone, clinically deficient DHEA and not one but two asymptomatic inguinal hernias which required surgery. Maybe I'm just not wealthy enough to be treated by Stanford. They did have valet parking and nice live music in the lobby after all.

    I really appreciated this episode podcast topic. I have gained a lot of insight from this episode alone. I sincerely hope that this episode's topic(s) do become much more widely discussed amongst various types of individuals, experts, and maybe evens institutes. As yes, male infertility is just as important as the female infertility epidemic issue(s).

    Regarding pelvic floor: if you're physically active; playing sports, weight lifting, etc, you're unlikely to have a weak PF muscle.

    I had a hiatal hernia years ago, you will be at risk for getting another hernia on the opposite side. I would give yourself time to heal, and work on unilateral movements training very light

    This is a question for your memory episode (which was a long time back) In using memory from long term memory there are two components, memory encoding and memory retrieval. In most of your episodes there was (I believe and think) only focus on memory encoding but not enough on memory retrieval. The retrieval process is more important for memory because if you can't retrieve it or have access to it, you can't use your encoded memory. So please do an episode on memory retrieval and look at the works of Robert Bjork,his lab and others work on memory retrieval. According to Robert Bjork memory retrieval is a probabilistic phenomenon. Obviously you need to have the memory encoded in the first place to retrieve it, so efficient memory encoding is essential but later down the line, memory retrieval becomes more important

    Hey Dr Huberman! Regarding the at-home semen analyses, the lab provides you with a centrifuge and instructions so that you can test it on the spot, no mail in required. Supposedly shows volume and concentration. I've just ordered a kit to test this from Labcorp

    Bad idea, why would you cut off a part of your body? I get that people do it to chase pleasure after already having kids or if they don't plan them, but health > pleasure every day of the week, if something is not wrong don't fix it, your semen is suppost to carry your sperm for a reason even if you already had kids.

    “We don’t have any data” always used conveniently for both sides of the coin. I think we can logically deduce that alcohol or cannabis is not positive for fertility, especially if someone is struggling. Why take the risk? The data is clear on sleep disruption and changes in sleep architecture. Even with a single drink and poor night of sleep. Respect to Huberman for calling it out for what it is.

    As person on spectrum I experienced a ton of rejection then something changed in my late 20s. Women were suddenly were interested. So I had my first kid 34. So older age and increased chance of autism makes sense. In my case as autism is genetic.

    Thank you for the help and advice that you offer. I have noted that not much consideration has been given to SHBG as a cause for lower Free T. It would be great if more information could offered on how to lower this in males with higher than average levels, when combined with an also high total testosterone.

    Thanks for the heads up about the other source of information. However, my comment was mainly for the purpose of highlighting that TRT is not the be all and end all treatment. Taking a simple supplement like 10mg boron daily has been shown to significantly reduce SHBG in both males and females, thereby increasing Free T and improving function and aesthetic without the need for TRT in many men. Simply continually focusing on exogenous testosterone and therapies similar to this (even whilst highlighting side effects) to those that are less able to research, aligns their thoughts that pharmacology and synthetic compounds are the only way to improve.

    Very interesting topic. Just for penile length observation study, “volunteering to be measure “ would not be a confounding factor? Cause more confident individuals with their penile length would be more likely to participate the study?

    Dr. Huberman, first of all thank you for all of the useful information you provide! I have a question about AG1 that I hope you can answer- why are there such megadozes of some of the vitamins and minerals included in the blend? For example: 467% of the reference intake of an average adult of Vitamin C? 553% of the reference intake of an average adult of Vitamin E? 917% of the reference intake of an average adult of Vitamin B12? 1,100% of the reference intake of an average adult of Biotin? All of these found in 12g. of AG1 meaning in just 1 scoop... Why woud anyone need such high doses of these vitamins? Thanks in advance!

    I was just wondering about the SAME. This are dosages very high even for people who consume 0 amounts vegetables and fruits and in my opinion these amounts can be toxic. Dr. Eric Berg multivitamine mix has much better proportions of micronutrients with dosages that allow you to still have a normal healthy diet without worrying to exceed the safe limits.

    Me and all my friends tried getting a psychotic episode by smoking cannabis everyday for 15+ years. Not one made it. I do believe you and that they happen, it’s just interesting that it’s so rare.

    That doctor talking to Andrew about post finasteride syndrome. He is going off of lab work lol. Do you know how many men who are dealing with this and ones that have committed? Suicide have been told information just like this so-called doctor about lab work. It's doctors like him. You push to the side and you have to keep going until you get the doctors who are forthright and willing and very very intelligent and look deeper than into lab work.. thank you Andrew for taking this very serious because post finasteride is really affecting a lot of men and the number is growing. So thank you! Andrew, I know that you seem very genuine in helping

    I have to say I am disappointed that a physician would tell patients any amount of alcohol consumption is safe. Just give the accurate data, stop letting your personal preferences or need to be liked get in the way of caring for your patient. If their blood pressure is too high because of lifestyle maybe address that before prescribing as well. I did not hear one comment on improving cardiovascular health, though Dr. Huberman did seem to want to go there, over all the medications mentioned. IMHO

    Thank you for raising awerness about Post-Finasteride Syndrome. It is a debilitating, chronic condition with serious physical, neurological and sexual symptoms. More research is desperately needed to understand the condition better and to identify potential treatments.

    @@mc7231 Thousands of case reports online on Reddit, X, propeciahelp etc., official patient organizations formed independently from each other in the US, Australia, Germany, the UK, France and Italy, a black-box label warning in France that resulted in the producer organon pulling the product from the market in France, countless urologists stating publically that they would never prescribe Finasteride to healthy man in their 20s and 30s and 99 suicides officially linked to Finasteride in the VigiCase database of the World Health Organization - but hey seems like PFS is not real and people just made it up for fun 🙄🤦‍♂️ To answer your point: well my hair loss stopped even after quitting Finasteride, but I couldn't really care f* less because of all the debilitating symptoms I have with PFS

    Hello Professor Huberman, I was wondering how come in both the latest episode of sexual health and specifically in this one, the issue of premature ejaculation has not been treated. ED has been discussed lastly lately, however PE was not, even though is very common across males and mainstream doctors usually do not threat this properly or simply suggest drug as a form of intervention instead of studying any underlying (anatomical and/or psychological) causes. Would you do an episode or a deep dive on this topic in the future? Thanks in advance.

    There is no such thing as premature ejaculation, ejaculation is suppost to happen as fast as possible, the purpose of sex was never for pleasure, only procreation and when 2 being are procreating they should want it to happen as fast as possible, and so it naturally does, imagine you're a caveman 10000 BC and you're trying to procreate with a cavewoman and it takes you half an hour to ejaculate, you would get killed by so many predators in that time frame if it took you that long, people have ruined the concept of sex by chasing pleasure so people think there is such a thing as "PE", hope this clears things up.

    Yeah, well, people aren't cavemen anymore, and most people find sexual intercourse beneficial in many ways. Spiritually, psychologically, and as a way to bond. 99.9% of the time people have sex it's not to procreate, so no, I don't think your reply helped the original poster at all.

    @@dave9547 i know it ultimately didn't help, but i wanted to make him realize that PE is not a problem, instead he is making it a problem by setting a goal that his body was naturally not programmed for, so of course when his goal is out of the reach of his natural abilities he won't be able to accomplish it naturally which is a method he was looking for, he can only do it by using whatever drugs doctors give him (which of course he shouldn't do) or by learning how to have an orgasm without ejaculating, even tho i don't recommend it from personal experience (because i don't think it's healthy), the best approach is to just not have sex outside of procreation, that's when life is the best for both genders, funny enough life is the best both spiritually and psychologically when there is no sex and the person saves their sexual energy completely, also from personal experience of course, may be different for others but i doubt it.

    When it comes to treatment relating to blood flow. I hear radio ads claiming ultrasound treatments have shown to restore damaged blood vessels. Are there any truth to these claims?     Being a health education graduate (much easier than MD), i can’t imagine the amount of information Dr. Eisenberg is privy to. With that being said, there is no one answer fits all concerning health because so many factors (genetic, environmental, behavioral) impact health so due to these intersections, his responses were valid and reasonable.

Can u look more into PSSD (Post-SSRI sexual dysfunction)? ts very scary and isuess that happens are not only sexual they are also neurological. Please its not taken seriously and the whole community with this syndrome is very belittled in medical areas and suffering. Imagine no pleasure from love, food and music☹️ terrible life.     Andrew, do you know any way of emulating the cognition-enhancing effects of nicotine without using it? I started when I was 14, gradually moved on to high nicotine snus for 15ish years and now completely quit after tapering to 4mg lozenges for a year. I feel horrible at times and incredibly slow and agitated, it worries me a lot and caffeine does not help, I could try bupropion but I only have xl tablets.

    The numbers just don't stack up. Side effects are extremely low, about 1% higher than placebo on the biggest studies. They have also done studies where one group was told about side effects and one wasn't and sure enough the group that was told about the side effects got them. There are millions of men on finasteride, and some small fraction of these men have psychological issues that finasteride is just an easy scapegoat for, especially if you are stressing and worrying about them.

    @@brandonkappes6695 all the actual data shows very low incidence of side effects, only slightly (1% )higher than placebo and cessation of side effects once medication is stopped. Think about it, the PFS symptoms are general depression and mental health symptoms , millions of people are on finasteride, some portion of theme will be experiencing depression, ED, anxiety these are things that effect young men, especially young balding men. Now these people have found each other on message boards are banding together to blame a drug they happen to have been taking for their problems, and in the process makings their problem worse because they are convinced they have poisoned themselves. I'd love to see how much of an overlap there is between PFS fearmongers and flat earthers.

    I am amazed about the lack on concise and on point answers given by Michael Eisenberg. Andrew Huberman asked a bunch of good question and all he got were elusive responses except one: when Andrew asked about advil and other drugs and their effects on the male sexual health, function & fertility Eisenberg was so decisive on his answer said that they do not affect the matter on the discussed subject but all other answers to Huberman's question were something like we do no have enough data, we do not know for sure, bla bla bla etc. Scetchy doctor in my opinion.

    Heat to the testes was discussed, particularly that it should be avoided, but only situations such as using the suana or riding a bike was discussed. The classic question came to mind but was not discussed - boxers or briefs? Would have loved to have heard a urologist's perspective. Or is there not enough data to support one over the other in terms of sperm and testosterone production, so it wasn't discussed? Or mayhe it just really not that huge of a contributing factor?

I have personally lost two marriages to this disorder and sometimes feel hopeless about finding a man who is NOT addicted to porn or other forms of sexual acting out.     Thanks for speaking about PFS. I’m a 23 year old patient myself and have suffered from a horrendous suite of neurological and physical symptoms. I’d love to hear more about it from you, especially about the science that has been done on this so far and what needs to be done in the future. It’s also important people know some of the most devastating symptoms can be non-sexual, like anhedonia, anxiety, joint pain, arrhythmias, tinnitus, etc.

    Same here man, all were warned about are sexual side effects when this drug can destroy your life in every aspect, I was a healthy 25 year old who took this poison for a month and I’m basically disabled, this drug as pretty much turned me into an 80 year old man overnight..

    I had no side effects from taking Finasteride for like 6 months now. Maybe some rare genetic variation makes dependency on DHT for daily functioning instead of testosterone? All the symptoms seems similar to ones of low testosterone.

    So it's well studied that heat will negatively impact sperm, but is this a concern if one isn't planning on having a kid in the foreseeable future? In this case, if it's just the current reservoir of sperm that's being damaged, I would assume it's okay to be in the hot tub / sauna without a cold pack around your scrotum? Or will chronic heat exposure stunt spermatogenesis in the long run?
