Dr. Friedman's take on the “Consensus” Guidelines on Cushing’s disease

Although Dr. Friedman is at the forefront of Cushing’s Disease, he was not invited to be part of the Pituitary Society Consensus Guidelines on Cushing’s Disease published in Lancet Diabetes and Endocrinology in 2021, many of his ideas on Cushing’s Disease that he has been advocating for years were included in the recent guidelines. In this informative webinar, Dr. Friedman will discuss
1) The use of imaging for the diagnosis of Cushing’s Disease
2) The need for multiple testing to diagnose episodic Cushing’s Disease
3) The importance of UFC and salivary cortisol testing
4) The use of medication trial prior to surgery
5) The use of ketoconazole for the medication trial and longer-term treatment
6) Dr. Friedman will also discuss new Cushing’s medications.

    Thank you for this. So would not suppressing the Dex test with a 7.56 point more towards adrenal or pituitary? Also what would it mean if someone has plenty of high 24hr UFC’s & didn’t suppress with a 7.56, but no high LNS? ACTH ranging from 29-76.

    I was adrenal and cyclical. Even more frustrating for me was the too frequent occurrence of having a consistently low plasma cortisol, while the saliva cortisol (with serum and saliva cortisol being conducted at the same time) was usually high, but values fluctuated. My highest saliva cortisol was 2557 nmol/litre while serum cortisol was low ( roughly 40 nmol/litre). I was very sick. The best thing I did was get both adrenals removed.

    So if on Ketoconazole and a male, can you take testosterone to make up for the drop in those levels?
