Lens replacement is the best. My dad had lens replacement 8 years ago, and he has the eyesight of an eagle.

I had LASIK last Friday and very happy now , I have 20/10 vision. I know that there is a possibility to wear off as we age and it's fine to me, nothing is permanent on earth. But whether it's will just going to be 10-20 years, it's still very worth it for me. I am so happy!

I had a Viivty lens implant and family have told me about it, which doesn’t bother me at all but good to know as my doctor didn’t go over that.

for anyone going through cataract surgery, I would advise to be careful and check the fees for potential post op care (consultations & LASIK) as the clinic i’m seeing don’t cover this in the surgery fee.

Your videos are absolutely wonderful - Thank you very much - I have Panoptix lens and required an enhancement. I went from a Plus 8 with astigmatism to 20/20

I had lasik 27 years ago, resulting in 20/15 vision. I can still read perfectly fine as a matter of fact some would consider what I can read as micro print. My problem became that I just got glasses so I can clear up my now blurry far vision. The Dr. told me its barely a writable prescription. So I still recommend lasik and am considering getting it done one more time.

It's been over 10 years since I've had Lasik. My far vision has also degraded quite a bit blurry while watching tv. One thing I'm unsure on is now I can put on my glasses that I had prior to Lasik and get rid of the blurriness and colors seem much brighter. Anyone else have similar problems?

I’ve had the panoptix for about 3 months and they are great. I know I’m going to need a tweak to optimize my vision, however, the lenses are great. I don’t even mind the halo affect at night time driving as a seems to diffuse the glare. Just thought I’d let you know that I think your video’s have been great and very informative.

Thank you Dr. for another very informative video. I haven't heard much about chromatic aberation issue and contrast sentivitivy issue with the Pantoptix lens. Would you mind to provide some information about that? How strong are those negative effects? Are there any pictures visualising the effects? I know I can withstand halo and glare but I'm not sure if I can tolerate chromatic aberation or contrast sensitivity loss. I'm a programmer so I spend a lot of time in front of monitor. Also no problem for me to use glasses for mid or near sight in indoor but in case of sports glasses are a bit problematic.

My experience is that he's greatly underselling tbe Panoptyx lenses. I'm 69 and got the lens implants 3 months ago. I see so well now that I test at 20/16 for near vision which is amazing. I haven't seen this well since I was 10 years old. With a bright enough light I can see almost anything up close! I do show as 20/25 to 20/30 at distance so I may get additional Lasik to correct it.

Also, is the new SmartSurface laser technology better than LASIK? Thanks.

where do you practice Dr. Wong. I would love to schedule a consultation with you. I'd Lasik 6 years ago and my eyes sight are changing rapidly. I can't no longer reading small print on television less than 10 feet away. I can't even read a small prints on the medicine bottle any more. You're right about the Lasik, I don't have the focus and sharpness of an objects in close up. I have the mid and far range. I'd love to have your inputs on my eyes.

Thank you Dr. Wong for the helpful information. I am 62 and chose the Pan Optics Tri focal lens for my cataract lens exchange. The distance, medium and most of the close range vision was inferior to my non-operative eye with regard to focus. That lens has been exchanged for a Vivity lens with the same results: poor focus with no improvement over weeks. I am scheduled for the 2nd eye procedure in about 3 weeks. I don't want to keep exchanging IOL's for something new. I have never had a LASIK procedure. Can I "dial-in" the focus with that after these Vivity lenses have "settled-in"? Thank you for any help. Thank you for a very informative video, Dr Wong. What if I am getting close to age 45 (42) with myopia and astigmatism (with no near vision issues). Would LLR be preferred?

LASIK surgery twice here at age 47. Could have had a third they said if needed. I had really thick corneas. Reading of vision 20/10 in both eyes. Age now 62 and still 20/10 in both eyes. I do wear a reading glasses in low lighted areas other wise best thing I’ve ever done. I so afraid if I have to have cataract surgery I’ll loose my quality vision I have now. But I think the PanOptix I would be happy with if measurements agree.

@@lisaedmonds6294 Thank you for your question. The answer is that the Panoptix lens is both a multifocal and a trifocal lens!! It is a type of multifocal IOL, but generally provides a broader range of vision without glasses than previous multifocals. Hope that answers your question.

Dr Wong - are you familiar with the new Vivity IOL? Love to see a video on that. Also, would it be possible to do two different lenses in your eyes? For example the PanOptix in one eye and the Vivity in the other? From my reading, it appears to be much less susceptible to halo and light degredadation, albeit with less near correction. Thanks!

@@androidchannel309 no real halos with either. My PanOptix is more for near vision, the Vivity is wonderful for distance. The lens is like brand new shiny LASIK. I’m really thrilled that I chose this combination, no reading glasses and perfect distance.

I am 31. I have diabetes type 2, that has been uncontrolled on and off for the past 10 years. I am about to have bariatric surgery to help with my diabetes. I am considering the lasik that doesn't create a flap. I have really bad astigmatism im both eyes. What should i consider in my decision? Thanks

Thank you Doctor, your channel is very educational. You've inspired me to be spectical independent I have to have catarat surgery for my rt eye soon. Right now I have to choose between LAL and trifocal iol (Acreva trinova toric or Pan optix toric.) I can afford for both and have access to both. I never had prior surgery to my eyes and they are healthy(WFL). My concern is the vision quality of both lenses. What are the pros and cons?

In the US, the LAL is a monofocal (fixed-focus) lens. I'd recommend a lens that delivers a variable focus. The panoptix is excellent. I have no experience with the trinova lens. Sounds like you are outside the US.

If I can get the LAL with the EDOF + monovision outside the US, would it be a better option than the Pan Optix toric IOL? The LAL is free from positive and negative dysphotopsia with a quite predictable outcome. If I could get to 20/20 with both eyes combined, would the vision quality as good as the Pan Optix Toric with a variable focus?

This is a diffractive lens ? Aspheric ? Hence inbuilt blur and loss of contrast sensitivity due to splitting the light so no night driving, disphotpia, haloes etc are intrinsic still to this lens.

I had PRK on my left eye and custom view on my right. My right eye is still good. But my left eye has gone back some. I had PRK cause I had metal in my eye 3 weeks prior to my surgery. And the Dr suggested PRK. Is it possible to get lasik again on my left eye???

We have chosen to not offer the light adjustable lens at this time as we believe that other lens technologies offer better range of vision (far/mid/near) than the light adjustable lens.

Dr. Wong, thank you so much for your series on IOLs. I am scheduled to have cataract surgery in August and based on your videos, along with my surgeon, I have selected the PanOptix lens. However, with the FDA's approval of Alcon's IQ Vivity IOL, should I consider delaying my procedure until the Vivity IOL is launched in the US? My current condition is not critical and was wondering if the added benefits of the Vivity IOL was worth the wait.

I have not used the vivity iol.

I'm getting the Vivity refractive lenses instead of the fresnel/diffractive PanOptix lenses. I think Vivity was introduced around September 2020 after after this video was uploaded (July 2020 ). Scheduling surgery has been a pain, probably due to covid backlog of cataract patients.

Can the panoptix lens surgery help with night blindness? I'm considering laser surgery already but looking into other aspects. I'm 53 years old now

I had mine 12 years back. No problems with vision.

Do you have a video on PRK/ASA? I don’t qualify for Lasik. My cornea is too thin. Do any of the family members that you’ve done Lasik on have issues with bad dry eye or halos? I couldn't do LASIK so I had PRK, I'm on my 3rd touch up on top of a corneal transplant waiting on the 4th. I do have Keratoconus tho.

I've had cataract surgery- and retinal re-attachment and the good eye is , he said, a very little Fractional bit off my prescription and wants to touch it up with LASIK-- my retinalr repair guy said DO NOT allow him to do that - even if it is FREE-- what gives??? My GOOD eye- the one that only had a minor cataract- has no other issues.. and that is the one with the Panoptix lens- and needs a little help-- so he said LASIK-- My surgeion that did the retinal reattachment said NO. (he's a PIONEER- in some of the surgery techniques.. my retinal detachment was 5 year old and no other surgerion would touch it- but I'm beginning to see through it first time in YeARS.. getting slightly better evey day).. I tend to believe HIM-- but he won't say WHY he told me NOT to allow my other doc to do the lasik!!!! Any ideas??

If you have had good success with monovision in the past (if you had natural monovision or used contact lenses to successfully create monovision and you liked it) then you would likely like monovision with lens implants. On the other hand, if you have never tried monovision, then the overall success rate at adapting to monovision with cataract surgery is only 50%.

Anyone knows? I wear glasses for distance -2 and for reading +1,5, Cylinder 0.75 so I have three problems at once. Lasik aye surgery can fix problem when I need distance and reading glasses in one surgery? Or Lasik surgery can fix only one problem so they fix long distance but reading problem will stay as it was or will be worse? Please let me know. Thanks

Is anisometry (2.5D between the eyes, -3 and -5,5, no astigmatism) a contraindication to the implantation of a trifocal panoptix lens?

If an individual has anisometropia after receiving a panoptix lens and that anisometropia is bothersome and not correctable with glasses, then the two solutions would be: 1 wear a contact lens on the other eye to reduce the anisometropia or have surgery on the other eye to reduce the anisometropia.

Oh yea it happen to elderly people because like my mother got older and now she has like white hairs and it’s just like old people it get blurry like she call it the 老花 but she doesn’t want surgery again so she’s keeping her glasses 👓

IMHO the age for lens replacement is 40+. Below 40, LASIK is usually the best choice.

Thanks for sharing this info. I currently have an eye issue where my left eye drifts/feels as if to lose focus on something and feels like its moving away from the target which I'm currently seeing. This only happens when I'm looking at stuff on my phone or computer. When I'm outdoors away from those gadgets, it does not happen as frequent (probably only when I'm zoning out due to energy level). Does this sound like something Lasik or Panoptix could solve or could this be an illness? I can still see (although not totally 20/20 vision) if I command my left eye to move back in sync with the right eye, but when the left eye drifts that is when I get this issue.

I had blurred vision and I had the surgery in 01SEP22 now it’s 01JAN23 and my vision is blurred some days when I wake up and this last half day. I will talk to my Optometrist. I just turned 40 so it’s a little frustrating. Most of the time it’s normal. Any ideas doctor?

After about 4 years I went back and he did a laser surgery to clean the lens as it started to show some signs of cloud / cataract. I have had cataract surgery with the standard lens, I was told because I had prior lasik surgery it would not work to have a multifocal lens. My vision is not even corrected to 20/20. Very disappointed. Would I be a candidate for the Panotic lens for better vision?

Your Dr. stinks! I'm pretty sure switching lenses with replacement surgery has much higher risk than a first surgery because of scar tissue...it is a last resort. But you should be able to fix the crappy outcome with Lasik....you will always need reading glasses though. Good luck.

I'm going for cataract surgery in 4 days. I'm so scared. I don't want to go but I'm told if I don't I will end up going blind. So, guess I gotta let the doc do it. Just wish I was more confident. I'm so scared.

Our center has used the panoptix in only 1-2 eyes. These two eyes saw well. We have used the symfony in about 200 eyes successfully. We know that the symfony is very compatible with post-RK eyes. We have a much more limited experience with the panoptix in RK eyes.

Dr Wong, went in for cataract preliminary exam. Doctor said that because of my previous lasik surgery (15 years ago), he would not recommend the Panoptix multifocal - something about aberrations resulting from the lasik (even though he can't see them) combined with abberatins from the Panoptix would not give me the best vision. Can you please comment on this. Thank You

Agreed , if your lasik corrections was more than +2.0 diopters. ? Odds are you are asking for trouble to your cornea,= abrasions, keratoconus.etc is that right ? dr.Wong

I had lasik and had good vision for about 2 years. Then I had to get glasses again. For everything. Close and far. totally blurred.

@@shankhan547 hey it’s gone okay. It’s been 7 months and I think my vision is still good. I can still watch my anime with subtitles and see the screen and no issues with driving. I am also 8 months pregnant now (didn’t know I was pregnant when I had the Lasix done) so my vision might change.

THANK YOU… I can’t express how much I appreciate this video. I had Lasik done in 2004 (18 years ago). The day I had Lasik, I experienced horrific pain. I went to the top rated doctor in Dallas, TX (at the time). I knew many people who went to him too with zero problems. I also saw 10+ people get the procedure done the same day as me and they were completely fine. For some reason, my eyes were burning and watering uncontrollably for 24’ish hours. They said I just had an allergic reaction, which seemed odd, but 2-3 days later I was fine. I was VERY worried for the first 24 hours something went wrong with my surgery, but they assured me everything was fine and some people just have that reaction. After I recovered, my vision was AMAZING. I felt like I had super hero vision. I LOVED IT. Over time I have noticed my vision getting worse. I just turned 44 and today for the first time I tried reading glasses and was AMAZED how much they helped. My eyes have been bothering me for some time, but I have procrastinated looking into glasses. I don’t know if I want to jump into another procedure, so I really appreciate the honest info presented here. Based on this info, I don’t want to do anything YET, but I’m sure I will in the future. For now, I have to figure out how to incorporate some grandpa glasses into my life, lol. I hate the way they look, I don’t want to carry them around, but putting on a relatives reading glasses today was such a life changing difference, I know I need them. If you can let me know about a permanent solution for this new grey hair nightmare that has also started, that would be great! It isn’t coming in grey… it’s white! Everywhere 😂🤦🏻‍♂️ I’m not ready to look like Santa Claus. 🎅🏻🎅🏻🎅🏻

Intraocular lenses become dislodged due to scarring of the lens capsule, trauma, lens ligament atrophy, and laser treatment for secondary cataracts. Secondary cataracts (opacity of the posterior capsule of the lens) sooner or later develop in almost all patients who have undergone lens removal.

In my case I never had an eye surgery before but looking forward to do lasik but when I viewed this video something tells me that I should go for a LLR instead. by the way, I'm in my 30's now and looking forward for longer best results here in the Philippines.

Hey! Just saw your comment. I'm in the PH too, 39 yrs old, and never had any eye surgery before. Been wearing glasses for my 400+/- and 375+/- eye grades since HS and planning to have LASIK as well. Have you gone through an eye surgery already? Have you heard about the Shinigawa clinic?

Can I make an appointment with you for the Lasik surgery? And do you offer SMILE option?

Yes, certainly can cam get myopia again, after Lasik. Continued close work causes it again.

If and when better options become available in the future, would we be able to change out the Panoptix? How difficult is it to change an IOC?

It is possible to exchange one IOL for another if the capsular bag is intact.

@@beeg2point0 If you've had a yag capsulotomy to the eye, then IOL exchange becomes more complex.

To replace healthy lens with artificial ones is the same when you go to dentist to fix filling and being told that you need to extract the whole natural tooth and replace it with artificial one. Insane stuff

How much does it cost😔

I had LASIK when I was 36, I'm 52 now and just had recent eye exam, need glasses to see better, especially driving. Hate wearing glasses and don't think qualify for 2nd Lasik due to thin corneas. Had no idea about Implants just for clear vision, I thought implants were only for cataracts. My doctor offers PanOptix, should I get an evaluation?

Have you seen the presentation by Prof.Venkatesh - Dr Suvira which discusses the trade offs of having a lens replacement to address shortsightedness, cataracts, etc? Even though the vision is corrected for distance, the overall vision quality is substantially degraded as the optical implant does a poor job at providing good contrast. If you want to watch it is titled "The MTF-(Modulation Transfer Function) and its role in assessing visual performance of premium IOLS."

So I am 48 and can't focus up close and my distance is still good. . What do you recommend? I have never had any eyes surgerys. And if I get lasik now how long will it last?

We recommend premium lens replacement instead of LASIK for vision correction after age 45.

Can you discuss IOL exchange after YAG when somehow PanOptix turned out to land wrong or be wrong RX?

IOL exchange (IOLX) before Yag capsulotomy is fairly straight-forward. IOLX after yag capsulotomy may be more challenging and may limit the choice of IOL type that can be used to replace the explanted IOL.

How much it will cost

My grandpa and grandma both have eye issues. My grandpas surgeries went great. My grandmas caused her to literally go blind. I just have heard horror stories from people who’ve gotten lasik. And I also know how the same doctor who may have been amazing for one patient might screw up another. I just am scared of it messing up and ending up with worse eyesight or in pain. I am 24 with 20/150 eyesight. I wanna get it fixed but it’s just something I’m not sure if I trust.

Girl, once he said after surgery I still may need glasses, you lost me lol. It’s not so much that glasses bother me. It’s that I have to rely on an external device to see. I just want to be able to see clearly on my own. :(

My Dr actually sold me on Panoptic actually said “ u won’t need glasses” well I was shocked at the price we r retired so limited income but once you hear no more glasses! I pushed the financial obstacle aside. Now I have panoptic in my left eye went in for my day after check up the same Dr said u might need readers…… I was absolutely crushed. I feel I made a lifelong expense decision too hastily wo more information Canceled my other eye surgery going to a different Dr for another opinion bc I lost all faith in the Dr who sold me the “ no glasses” lens.
